Abstract

Background and Objectives: People with chronic illness tend to report multiple and complex symptoms that decrease their ability to meet their needs and quality of life (QoL). As an approach to address people with chronic illness, palliative care is still shortly focused on people with chronic malignancies in Indonesia. Eventually, that paradigm makes the spectrum of palliative care interpreted narrowly only for cancer or end-of-life (EOL) patients. This study explores the experience and needs of patients diagnosed with non-malignant chronic illness towards their illness to identify the need for palliative care. This study used a qualitative method with a phenomenological approach. Material and Methods: A total of seven (7) participants were involved in semi-structured interviews that were previously selected based on screening using an adult palliative patient score developed by Cipto Mangunkusumo Hospital (RSCM), Jakarta. Only participants with a score above or equal to 4 (four) are selected to participate. All qualitative data is recorded, transcribed, and analyzed using Colaizzi's approach. Results: There are seven major interrelated themes emerging from this qualitative study that uniquely match with the illness trajectory of non-malignant chronic patients. The themes are: (1) negative feelings that cannot be described at the time of diagnosis, (2) ignorance of the disease process, (3) late realization that illness cannot be cured, (4) helplessness during the medication/treatment process, (5) shame of being a burden to family and surroundings, (6) limited access, (7) family and God as motivation and hope reinforcement to recover at the end of illness stage. Conclusions: People with non-malignant chronic complain of several challenges, which are not much different from those with cancer. Therefore, patients with non-malignant chronic illness (PWNMCI) also have a similar need for palliative care. A deeper and broader assessment of palliative care should also be implemented in patients with non-malignant chronic illness from the early diagnostic process.

Abstract

Breast cancer is the second most prevalent type of cancer and one of the leading causes of mortality. Efforts to reduce mortality rates can be achieved by empowering patients to manage symptoms caused by cancer and the side effects of treatment therapies. This study aimed to evaluate the effectiveness of empowerment education in increasing self-efficacy in breast cancer patients. The study employed a systematic review of eight articles sourced from various databases, including ProQuest, SpringerLink, ScienceDirect, Sage Journal, Google Scholar, and PubMed. A total of 8,693 articles were retrieved from seven databases. After screening based on inclusion criteria, 638 articles were deemed eligible. Based on the selection of titles, abstracts, and full texts, 22 articles were chosen, and after duplicate elimination, 10 articles were analyzed using the Joanna Briggs Institute (JBI) quality assessment tool. The analysis results indicated that the empowerment approach is effective in improving self-efficacy and quality of life in breast cancer patients. By utilizing empowerment education, healthcare professionals can provide holistic support to patients, improve therapeutic outcomes, and reduce breast cancer mortality rates. This study recommended the integration of empowerment education as an essential component of breast cancer management in various healthcare facilities.

Abstract

Background: Oral mucositis (OM) is a common and highly symptomatic complication of cancer therapy that affects the quality of life function. Chemotherapy and radiation are the most widely used interventions in cancer treatment. The prevalence of oral mucositis side effects among 294 head and neck cancer patients was 93.9%. Purpose: To analyze oral care management to overcome mucositis in cancer patients undergoing chemotherapy and radiation therapy. Method: Literature review study using Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and PICOS approach, P: cancer patients undergoing chemotherapy and radiation, I: oral care management, C: no specific treatment or other standard care, O: reducing severity and accelerating healing, S: randomized controlled trial (RCT). Literature search using keywords “management” AND "oral mucositis" OR “stomatitis” OR “oromucositis” OR “oromucositides” AND "cancer", “management” AND “mucositis” OR “oral mucositis” OR “mucosa inflammation” OR “stomatitis” AND “cancer treatment”. Results: Various methods have been used to treat oral mucositis in cancer patients undergoing chemotherapy and radiation. Although there are differences in the timing, method, and frequency of mouthwash use, most of the literature shows that mouthwash is effective in reducing oral mucositis in patients. Conclusion: Management of oral mucositis can be done to treat and prevent oral mucositis in cancer patients currently receiving chemo and radiation therapy.   Keywords: Cancer; Chemotherapy; Mucositis Oral; Radiotherapy.   Pendahuluan: Oral mucositis (OM) merupakan komplikasi umum dan sangat simtomatik dari terapi kanker yang memengaruhi fungsi kualitas hidup. Kemoterapi dan radiasi adalah intervensi paling banyak digunakan dalam pengobatan kanker. Prevalensi efek samping mukositis oral diantara 294 pasien kanker kepala dan leher adalah 93.9%. Tujuan: Untuk menganalisis management oral care mengatasi mucositis pada pasien kanker yang menjalani terapi kemo dan radiasi. Metode: Penelitian literature review menggunakan panduan Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) dan pendekatan PICOS, P: pasien kanker yang menjalani kemoterapi dan radiasi, I: manajemen perawatan oral, C: tidak ada perawatan khusus atau perawatan standar lainnya, O: mengurangi keparahan dan mempercepat penyembuhan, S: randomized control trial (RCT). Pencarian literatur menggunakan kata kunci “management” AND "oral mucositis" OR “stomatitis” OR “oromucositis” OR “oromucositides” AND "cancer", “management” AND “mucositis” OR “oral mucositis” OR “mucosa inflammation” OR “stomatitis” AND “cancer treatment”. Hasil: Berbagai metode telah digunakan untuk mengatasi mukositis oral pada pasien kanker yang menjalani kemoterapi dan radiasi. Meskipun terdapat perbedaan dalam waktu, cara, dan frekuensi penggunaan obat kumur, sebagian besar literatur menunjukkan bahwa obat kumur efektif dalam mengurangi mukositis oral pada pasien. Simpulan: Manajemen penanganan mukositis oral dapat dilakukan untuk mengatasi dan mencegah mukositis oral pada pasien kanker yang menjalani terapi kemo dan radiasi.   Kata Kunci: Kanker; Mukosisitis Oral; Radiasi; Terapi Kemo.

Abstract

Abstract Backgound: Nurse competence can be influenced by the empowerment of head nurse. Empowerment is how head nurse conditions the climate and work culture in the nursing ward that can improve nurse competences. The existing literature is still very varied, therefore a systematic review is needed to determine the specific empowerment carried out by the head nurse based on published scientific evidence.Purpose: The study was aimed to know how the empowerment of head nurse affected nursing staff competences in the hospitalMethods: Conducted a systematic review of the literature from 2013 to 2023. Data were collected from 6 databases (EMBASE, EBSCOHost, Sage Journals, ScienceDirect, Scopus, and PubMed), using the keywords “empowerment”, “empowerment of head nurses”, and “empowerment AND head nurses”. The literature study was limited to English-language, as well as literature that had abstracts and full text. The systematic review was organized using PRISMA. The quality of the articles was assessed using JBI critical appraisal tools.Results: There were 8 articles included in the review category. The results showed that there are four actions of the head nurse to empower the staff, including: 1) providing training to increase skills and knowledge, 2) providing autonomy, 3) being open, communicative and caring, and 4) being a shelter.Conclusion: The head nurse played an important role in managing the ward by setting policies to coordinate nursing staff tasks, and solve problems with the staff. Empowerment can improve the organization climate and culture to sharpen the quality of nursing care.Keywords: empowerment, head nurse, nursing staff, systematic review Abstrak Latar belakang: Kompetensi perawat dapat dipengaruhi oleh beberapa factor, salah satunya dalah empowerment dari kepala ruangan. Empowerment yang dimaksud dalam hal ini adalah bagaimana kepala ruangan mengkondsikan iklim dan budaya kerja di ruangan yang dapat meningkatkan kompetensi perawat. Literatur yang ada tentang topik ini masih sangat bervasiasi, oleh karenanya dibutuhkan sistematik review untuk mengetahui empowerment yang spesifik dilakukan kepala ruangan berdasarkan bukti-bukti ilmiah yang telah dipublikasikan.Tujuan: Untuk mengetahui bagaimana empowerment kepala ruangan dalam mempengaruhi kompetensi perawat pelaksana di rumah sakit.Metode: Melakukan tinjauan literatur secara sistematis dari tahun 2013 sampai dengan 2023. Data dikumpulkan dari 6 database (EMBASE, EBSCOHost, Sage Journals, ScienceDirect, Scopus, dan PubMed), dengan menggunakan kata kunci empowerment, empowerment of head nurse, dan empowerment AND head nurse. Studi literatur terbatas hanya pada literatur yang berbahasa Inggris, serta literatur yang memiliki abstrak dan full text. Sistematik review disusun dengan menggunakan PRISMA. Kualitas artikel yang dihasilkan dinilai dengan menggunakan JBI critical appraisal tools.Hasil: Terdapat 8 artikel yang masuk kedalam kategori review. Hasil review menunjukkan terdapat empat tindakan kepala ruangan untuk memberdayakan staf perawat, antara lain: 1) memberikan pelatihan untuk menambah skill dan pengetahuan, 2) memberikan otonomi, 3) bersikap terbuka, komunikatif dan caring, dan 4) menjadi tempat bernaung bagi perawat dalam bekerja.Kesimpulan: Kepala ruangan berperan penting dalam mengelola unit ruangan dengan menetapkan kebijakan untuk mengkordinasikan tugas dari staf perawat, dan bersama-sama dengan staf perawat memecahkan masalah. Empowerment yang dilakukan kepala ruangan terhadap staf perawat dapat meningkatkan iklim dan budaya organisasi untuk mempertajam kualitas asuhan pelayanan keperawatan.Kata Kunci: empowerment, kepala ruangan, perawat, systematic review

Abstract

Background: Cancer is a disease or part of cells in the body that grows abnormally in parts of the human body and can spread to other parts of the body. Cancer can occur in children and adults. Patient navigators serve as intermediaries between individuals and the healthcare team, assisting patients in addressing and overcoming obstacles that may hinder access to medical services. They offer personalized support throughout the continuum of care, guiding patients through the various stages of their treatment journey. Purpose: To determine the effect of navigators on cancer patient satisfaction. Method: A systematic review approach in line with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The author used several research article databases, namely PubMed, ProQuest, Science Direct and Sage. 3 articles met the criteria and were analyzed and criticized using the JBI instrument. Results: Patient navigator is an effective strategy to improve diagnosis time, reduce anxiety, and increase patient satisfaction. Conclusion: The role of patient navigator in the context of cancer care can increase patient satisfaction and self-efficacy.

Abstract

Background: Patients with lymphoma often experience various signs and symptoms that can significantly affect their quality of life. Pain and poor sleep quality are two main symptoms commonly experienced by lymphoma patients. The combination of pain and sleep disturbance can negatively impact patients' energy, mood, and ability to perform daily activities, thus requiring appropriate attention and management in the care of lymphoma patients. Objective: This study aims to apply Evidence Based Nursing Practice: Lymphoma Exercises in nursing care for patients with lymphoma using the Theory of Planned Behaviour approach. Method: This approach emphasizes the importance of attitudes, subjective norms, and perceived behavioral control in shaping nursing diagnoses. This framework can enhance understanding and improve patient care. The research method used is a quantitative experiment without a control group. Respondents completed questionnaires to measure pain using the Numerical Rating Scale (NRS), and to measure sleep quality using the Pittsburgh Sleep Quality Index (PSQI). Result: The results show that the intervention had a substantial positive impact on pain and sleep patterns of lymphoma patients. Recommendation: The reduction in pain and improvement in sleep quality can be explained through several potential mechanisms

Abstract

Cancer patients often experience a variety of signs and symptoms that can significantly impact their quality of life. This research aims to apply the Theory of Planned Behavior to nursing care for cancer patients. This approach emphasizes the importance of attitudes, subjective norms, and perceived behavioral control in forming nursing diagnoses. n the context of cancer patients, understanding the factors of the Theory of Planned Behavior, namely attitudes toward the behavior, subjective norms, and perceived behavioral control, can help identify effective interventions to improve medication adherence, the adoption of a healthy lifestyle, and participation in follow-up care. This framework can increase understanding and improve patient care. The research method used is a case study approach. The research results show that the application of the Theory of Planned Behavior allows nurses to provide holistic care involving various aspects of cancer patients' lives including physical, emotional, social, spiritual and cultural. By understanding the concept of the Theory of Planned Behavior, nurses can recognize that changing a patient's behavioral habits requires planning, where from an accepting attitude to becoming a habit of carrying out therapy requires a process and needs support from both family and health workers so that goals and outcomes can be achieved.

Abstract

Cakupan pemberian air susu ibu (ASI) eksklusif di Kota Pagar Alam, tahun 2011 sekitar 43% tergolong rendah. Sebaliknya, pemberian susu formula meningkat tiga kali lipat dari 10,3% menjadi 32,5%. Iklan susu formula telah menyentuh bidan swasta dan puskesmas melalui pendekatan produsen susu formula dan pemberian susu formula secara gratis kepada ibu menyusui. Penelitian yang bertujuan mengetahui determinan kegagalan praktik pemberian ASI eksklusif di Kota Pagar Alam Provinsi Sumatera Selatan ini menggunakan desain studi unmatching kasus kontrol. Populasi adalah seluruh ibu yang mempunyai bayi berusia 7-12 bulan. Penarikan sampel dilakukan dengan metode proportional random sampling. Variabel terikat praktik adalah pemberian ASI eksklusif, variabel bebas adalah pemberian sampel susu formula. Ibu yang mendapat sampel susu formula dan yang tidak mendapat dukungan tenaga kesehatan berisiko 3,67 dan 4,2 kali lebih besar untuk tidak memberikan ASI eksklusif. The coverage of exclusive breastfeeding in the City of Pagar Alam in 2011 was by 43%. Advertising of infant formula has reached privately practicing midwives or health centers. The approach from infant formula manufacturers to midwives in health centers is by providing free milk formula to nursing mothers to be distributed under the pretext of promotion. The objective of this study is to analyze the determinants of exclusive breastfeeding practice failures in the City of Pagar Alam of South Sumatra Province. The population study with an unmatched case-control design was conducted in the City of Pagar Alam. The population was all breastfeeding mothers who had babies in the city of Pagar Alam of South Sumatra Province. The research subjects are breastfeeding mothers who had babies aged 7 - 12 months who selected with proportional random sampling method. The variables of the study included the dependent variable, i.e, the practice of exclusive breastfeeding, the independent variable, i.e, promotion of free milk formula samples. The risk of not exclusively breast feeding amang mothers who had accepted formula milk samples and who absence of support posed by health workers is 3.67 and 4.20 times higher the mothers who had not accepted the formula milk sample and who absence of support posed by heath worker.
