Abstract

Type 2 diabetes mellitus (T2DM) patients are often required to adhere to oral antidiabetic drugs (OADs) prescribed by healthcare professionals for maintaining optimal blood glucose levels. Therefore, this study aims to assess adherence to OADs among outpatients with T2DM. A descriptive cross-sectional design was used, involving 32 participants selected through convenience sampling at Adam Malik General Hospital in Medan, North Sumatra, Indonesia. Data collection was conducted in July 2024, and descriptive statistics were used to analyze adherence levels and sociodemographic characteristics. The results showed that the majority of participants were under 60 years of age (68.8%), male (65.6%), had completed high school education (59.4%), were employed (71.9%), and had a monthly household income below the regional minimum wage (65.6%). In addition, more than 50% had been living with diabetes for five years or longer (53.1%), were covered by national health insurance (90.6%), resided in urban areas (62.5%), and had no diabetes-related complications (81.3%). The mean adherence score to OADs was 6.66 (SD ±1.20), with moderate adherence being the most common (56.3%), followed by high (28.1%) and low levels (15.6%). The results also showed that only a limited number of T2DM patients exhibit optimal adherence to their treatment regimens. This shows the need for healthcare providers and relevant stakeholders to develop targeted interventions and support systems aimed at enhancing medication adherence among diabetic patients.

Abstract

Nausea and vomiting is a complaint that often occurs in cancer patients undergoing chemotherapy. Even though he had received antiemetic therapy before giving chemotherapy, this complaint still appeared and even dominated the other complaints. So complementary therapies are needed to accompany the medical therapy provided to achieve holistic care in patient care. One complementary therapy that is very effective in reducing nausea and vomiting is acupressure. This study aims to conclude the importance of acupressure intervention in reducing complaints of nausea and vomiting in breast cancer patients. Searching for articles in this systematic literature review used the PRISMA method, which focuses on acupressure interventions in reducing nausea and vomiting. The search strategy used Boolean Search with keywords acupressure AND breast cancer OR breast cancer AND nausea and vomiting. The author selected the articles collected according to the determination of keywords in the inclusion and exclusion criteria. Six databases were searched from Proquest, ScienceDirect, CINAHL, Pubmed, Google Scholar, and SpringerLink sites from 2019-2023, and all articles were evaluated according to relevance and research quality. 15 articles have been analyzed according to characteristics, interventions, nausea and vomiting instruments used, and research objectives from 7 articles of international and 8 national reputations. This systematic literature review study significantly resulted in the conclusion that complementary acupressure therapy can reduce complaints of nausea and vomiting in breast cancer patients. Acupressure intervention can be used as evidence-based practice in improving holistic patient care

Abstract

Pengabdian pada masyarakat ini mengkaji pelaksanaan program pelatihan kesiapsiagaan bencana berbasis komunitas di Desa Marbun Toruan, dengan fokus pada peningkatan ketahanan lokal melalui latihan simulasi dan sesi edukasi. Program ini, yang melibatkan 60 warga setempat, terdiri dari fase pra-penilaian, intervensi edukasi, latihan simulasi, dan pasca-penilaian. Temuan menunjukkan bahwa peserta mengalami peningkatan signifikan dalam pengetahuan, sikap kesiapsiagaan, dan keterampilan praktis. Hasil utama meliputi peningkatan daya ingat pengetahuan, partisipasi aktif dalam latihan, dan kesadaran yang lebih tinggi tentang peran dan tanggapan kesiapsiagaan bencana. Hasil ini menunjukkan bahwa pelatihan bencana berbasis komunitas dapat secara signifikan meningkatkan kesiapan lokal, dengan implikasi untuk mengembangkan strategi intervensi yang disesuaikan di komunitas berisiko tinggi serupa Kata Kunci : Kesiapsiagaan bencana; ketahanan komunitas; pelatihan simulasi;manajemen darurat; mitigasi bencana

Abstract

Isolation is an unpleasant experience. Changes in the environment, physical isolation from loved ones and boredom cause negative effects that can deteriorate the health of confirmed COVID-19 patients. Explorasi of the experiences of COVID-19 confirmed patient from several research articles aims to help identify the quality of life of patient with confirmed COVID-19. Methods: PRISMA is used for evaluation of research results. SPIDER to formulate research question with the database used by PubMed, CINAHL, Proquest, and Google Scholar. JBI is used to extract analyze data. Results: 6 qualitative studies included in the review. The experiences of 108 COVID-19 confirmed patients with 32 findings resulted in 7 categories based on similarities in meaning and three themes that were synthesized from categories, namely the physiological response of patients with confirmed COVID-19, the psychological response of patients with confirmed COVID-19, and treatment in health fasilities. Conclusion: patients with a confirmed COVID-19 experience various responses while undergoing treatment in the COVID-19 isolation room. In response to this response, patients with confirmed COVID-19 apply different coping mechanisms. In this condition, social support is an important factor in supporting the recovery of patients with confirmed COVID-19. Providing services in the COVID-19 isolation ward needs to rely on and understand the natural condicions of patients with confirmed COVID-19 by developing appropriate intervention strategies.

Abstract

Background: Pneumonia, pleural effusion, and respiratory alkalosis are complex conditions that are often found in patients after surgical resection (PS Resection). These conditions can compromise respiratory function and increase the risk of serious complications, such as respiratory failure and death. Evidence-based maintenance approaches, including non-pharmacologic pursed-lipbreathing (PLB) therapy, have been shown to be effective in managing respiratory distress. Objectives: This study aimed to apply the application of PLB therapy in patients with these conditions. Methods: This study is a case study on a 23-year-old female patient at Adam Malik General Hospital. The patient had a history of PS Resection, susp. pneumonia, dextra pleural effusion, and respiratory alkalosis. OTB intervention was performed four times a day for three consecutive days. Data were collected through measurement of oxygen saturation and level of breathlessness before and after the intervention. Results: The results showed that OTB therapy was effective in increasing oxygen saturation and decreasing the patient's shortness of breath level. The patient's breathlessness level decreased from scale 7 (severe) to scale 1 (mild) within three days. The intervention worked by improving gas exchange and increasing respiratory ventilation efficiency. In addition, PLB provides additional benefits in the form of relaxation and increased activity tolerance. Conclusion: The conclusion of this study is that PLB therapy is a non-pharmacological method that is simple, effective, and can be applied to patients with complex respiratory disorders. This therapy can also teach patients and families to support home care. Further studies with larger samples and longer observation periods are needed to confirm these findings.

Abstract

Family participation plays a vital role in the success of diabetes self-management (DSM). In Indonesia, family-centered care is culturally embedded, yet limited research has explored how families experience and interpret their roles in supporting self-management among patients with type 2 diabetes mellitus (T2DM). This study aimed to explore family experiences in supporting diabetes self-management practices among patients with T2DM in Medan, Indonesia. A descriptive phenomenological approach was employed. Twelve family members who cared for T2DM patients at Chairuddin Panusunan Lubis Hospital were recruited using purposive sampling. Data were collected through in-depth interviews and field notes from February to May 2025. Colaizzi’s seven-step analysis method guided data analysis to extract significant statements and formulate themes. Credibility was ensured through member checking, dependability via team discussion, and confirmability through audit trails. Five interrelated themes emerged: (1) controlling diet, (2) arranging physical training (3) administering medications, (4) emotional support, and (5) utilization of health care facilities. These themes reflected how families act as emotional motivators, behavioral supervisors, and coordinators of care to strengthen patients’ adherence and self-efficacy. Family involvement was not limited to instrumental assistance but extended to emotional reinforcement and facilitation of healthcare access. Family engagement forms the backbone of effective DSM, integrating physical, emotional, and social support. Strengthening family-based interventions is essential to enhance adherence, glycemic control, and quality of life among patients with T2DM. Abstract

This study aims to explore the experience of nurse managers implementing strategies for preventing and controlling COVID-19 infection in hemodialysis units. The research uses qualitative methods with a descriptive phenomenological approach. Based on the results of the research analysis, 5 themes were found, namely: (1) Adapting to the COVID-19 PPI, (2) Managing the COVID-19 PPI, (3) Having obstacles in implementing the COVID-19 PPI, (4) Striving to increase the compliance of nurses, patients and visitors, (5) Have hopes for maximum implementation of PPI COVID-19. In conclusion, nurse managers, through leadership, preparedness and continuing to innovate, become a strategy for managing resources accompanied by motivation to form a solid nursing team in carrying out prevention and control of COVID-19 infections. Abstract

Type 2 diabetes mellitus (T2DM) is a group of metabolic diseases that occur when the body cannot effectively use the insulin it produces, resulting in hyperglycemia. T2DM mostly affects adults and is usually caused by age, overweight and inactivity. To manage diabetes appropriately or achieve adequate glycemic control, patients must perform diabetes self-care activities by organizing a proper diet, doing physical exercise, monitoring blood sugar levels, complying with prescribed medications, performing foot care, and quitting smoking. The purpose of this study was to determine the relationship of diabetes self-care activities with glycemic control in type 2 diabetes patients in the Padang Bulan Medan Health Center. This research method is quantitative non-experimental with correlation design. The majority of respondents carried out insufficient self-care activities (47.1%) with 79.3% uncontrolled glycemic control. The relationship between diabetes self-care activities and glycemic control was significant with p = 0.000 (<0.001) with a value of r = 0.643. Health center workers are expected to provide diabetes self-care education. Improving the implementation of diabetes self-care activities is aimed at preventing further complications and improving the quality of life of patients. Keywords: Diabetes.
