Abstract

Cervical cancer is the second most common cancer among women in the world. Persistent infection with high risk human papillomavirus (HPV) is one of the necessary causes of cervical cancer development. However, host genetic factors may also play a role in cervical cancer carcinogenesis. Methylenetetrahydrofolate reductase enzyme, encoded by the MTHFR gene, regulates folate metabolism which is important for genetic expression and stability. Single nucleotide polymorphism (SNP) C677T in MTHFR gene may produce a thermo-labile enzyme, resulting in a reduced enzyme activity. The aim of this study was to explore the SNP C677T of MTHFR gene and the susceptibility to cervical cancer among cancer patients visiting Dr. Hasan Sadikin General Hospital, Bandung, Indonesia. This descriptive quantitative study involved cervical cancer patients recruited in 2010 and their control group. Genomic DNA was extracted from patients’ blood. MTHFR C677T genotype was performed using BeadXpress Reader Illumina® and some samples were re-genotyped for confirmation using conventional PCR-RFLP. The distribution of MTHFR C677T genotype in cervical cancer patients was 71.6%, 25.4%, and 3%, and 44%, 36%, and 20% in control group for CC, CT, and TT, respectively. This yielded a statistical significant difference of CC vs CT+TT (p 0.014 with OR 3.22 and CI 95% 1.24 – 8.33). Taken together, this result indicates that T allele has a protective effect against cervical cancer development. Further studies to confirm this effect in bigger population is warranted. Infeksi persisten oleh human papillomavirus (HPV) tipe risiko tinggi merupakan salah satu penyebab utama kanker serviks. Selain itu, factor genetic juga turut berperan dalam proses perkembangan kanker serviks. Enzim methylenetetrahydrofolate reductase yang disandi oleh gen MTHFR berfungsi meregulasi metabolism folat. Polimorfisme C677T pada gen MTHFR dapat membuat produksi enzim menjadi termolabil sehingga terjadi penurunan aktivitas. Distribusi folat yang abnormal dapat mengganggu proses metilasi, sintesis, dan perbaikan DNA yang dikaitkan dengan perkembangan kanker serviks. Tujuan penelitian ini adalah mengetahui distribusi polimorfisme C677T gen MTHFR pada pasien kanker serviks di Rumah Sakit Umum Dr. Hasan Sadikin Bandung, Indonesia. Penelitian deskriptif kuantitatif ini melibatkan pasien kanker serviks yang diinklusi tahun 2010 dan sebagai control adalah wanita yang datang untuk pemeriksaan PAPsmear. DNA genomic diisolasi dari darah pasien dan dihibridisasi dengan menggunakan system BeadXpress Reader Illumina® untuk menentukan jenis genotipenya. Genotipe beberapa sampel dikonfirmasi dengan metode PCR-RFLP. Hasil distribusi polimorfisme C677T gen MTHFR dengan genotipe CC, CT, dan TT pada pasien kanker serviks adalah 71,6%, 25,4%, dan 3% dan pada kontrol adalah 44%, 36%, and 20%. Hasil ini menunjukkan perbedaan yang signifikan antara pasien kanker serviks dan kontrolnya, dengan genotipe CC vs CT+TT menunjukkan nilai p=0,014 (OR 3.22 dan IK 95% 1,24–8,33). Simpulan, alel T menunjukkan efek yang protektif pada perkembangan kanker serviks. Abstract

BACKGROUND Basic knowledge of sexual and reproductive health is important to be introduced during the premarital period, to reduce high-risk sexual behavior among adolescents, as well as to increase the correct attitude in their future marriage. This study explored the knowledge of HIV/AIDS, sexually transmitted diseases (STDs) and anemia among unmarried adolescents. METHODS This was a cross-sectional analytical study on secondary data from the Indonesian Demographic and Health Population Survey 2017, initially conducted by the National Population and Family Planning Board (BKKBN). Knowledge of HIV/AIDS, STDs and anemia among unmarried adolescents aged 15-24 years in West Java Province and Jakarta, the capital city of Indonesia, was compared and analyzed using the chi-square test. RESULTS In total, 5,389 weighted data were retrieved. In general, adolescents in Jakarta had better knowledge of all diseases compared to those in West Java (p<0.001). Knowledge of anemia and STDs was poor compared to that of HIV/AIDS which was moderate to good (p<0.001). Interestingly, although the adolescents had a middle-low wealth index (43%), the majority (>90%) had their own mobile phone with almost daily internet access, however most of their information sources on health education were from their school period. CONCLUSION Premarital education in schools is imperative to enhance the knowledge of STDs and anemia. Moreover, poor knowledge of anemia is a challenge for introducing the topic of thalassemia major, an inherited autosomal recessive type of anemia due to hemoglobinopathy. Since Indonesia is harboring 6-10% of thalassemia carriers, integrative premarital education is needed for better family planning.

Abstract

Background and purpose: Indonesia is a tropical country, located through the equator line which is abundantly exposed with sun. However, vitamin D deficiency among population in Indonesia is common. To increase optimal vitamin D intake, knowledge on vitamin D and the role of sun exposure early in the adolescence period is imperative. This study aims to explore the knowledge of high school students about the sources and benefits of vitamin D. Methods: This was a quantitative analytical study with a cross-sectional design using survey method, conducted in 2019. A public madrasah high school in Bandung, Indonesia, was selected using convenience sampling. A validated questionnaire was distributed to students aged 16-18 years old, containing of two parts about sources of vitamin D (10 questions) and its benefits (4 questions). Results: In total, 198 high school students participated in the study, majoring in Natural Sciences (55.6%) and Social Sciences (44.4%), of whom 76.3% were females. The proportion of students who answered correctly about vitamin D was 72.8%, whereas 61.9% had correct answers about the sources of vitamin D and 83.8% about its benefits. Female students had significantly higher score compared to males (p<0.001). Interestingly, the Natural Science students had no significant higher score compared to Social Science students (p=0.227). Conclusion: The knowledge about the source of vitamin D and its benefit among female high school students is significantly better than males, thus, special attention to male students is needed. There is a trend, although not significant, that the knowledge of students majoring in the Natural Sciences is higher compared to the Social Sciences. This study suggests the need of an integrative health education and outdoor activities, as well as a better nutrition program, focusing especially on male students and the Social Sciences class.Keywords: knowledge, vitamin D, health education, sun.

Abstract

Toll-like receptor 8 (TLR-8) is known as part of intracellular signaling transduction for bacterial phagocytosis. Mycobacterium tuberculosis (Mtb) is intracellular pathogenic bacteria that is recognized by this receptor, and genetic variation of TLR-8 might alter susceptibility of the host towards pulmonary tuberculosis (PTB). This study aimed to determine whether TLR-8 gene polymorphisms were associated to PTB in Kupang, Indonesia. This case-control study compared demographic and clinical data between 115 PTB patients and 115 controls, then two TLR-8 single nucleotide polymorphisms (rs3764880 and rs3788935) were explored using the GoldenGate® Genotyping for VeraCode® / BeadXpress Illumina®. There is no significant difference between sex distribution of patient vs control groups. The polymorphisms (rs3764880 and rs3788935) are in Hardy-Weinberg Equilibrium in this population (p > 0.05). The distribution of major vs minor genotypes and alleles of TLR-8 polymorphisms in PTB patients were as followed: rs3764880 (GG vs GA vs AA, 50.0% vs 21.4% vs 28.6% ; G vs A, 60.9% vs 39.1% ) and rs3788935 (GG vs GA vs AA, 53.0% vs 21.7% vs 25.3%; G vs A, 62.9% vs 37.1%). Neither genotypes nor alleles were associated with PTB in this population (P > 0.05). Besides, when the analyses were stratified by gender, none of the alleles of polymorphism in both genders were associated with PTB cases. None of the TLR-8 polymorphisms have associated the risk of developing PTB in Kupang, East Nusa Tenggara population (as opposed to other studies in different ethnic groups). These might reflect the diversity of genetic polymorphisms in eastern Indonesia populations, suggesting different genetic backgrounds with western part of Indonesia.

Abstract

As the third-highest country with tuberculosis (TB) incidence worldwide in 2020, Indonesia has increasing TB cases resistant to various anti-TB therapy or multidrug-resistant (MDR)-TB, and ranked fifth for its high incidence reported in Global Tuberculosis Report 2020. Moreover, extrapulmonary TB (EPTB) is rising, and data studies on EPTB with MDR-TB in Indonesia are scarce. This study aimed to explore the proportion of extrapulmonary MDR-TB among TB cases in Dr. Hasan Sadikin General Hospital Bandung, West Java, Indonesia. A descriptive retrospective and cross-sectional study design were conducted, retrieving medical records from all suspect MDR-TB adult patients examined by GeneXpert®, at Dr. Hasan Sadikin General Hospital Bandung, West Java period 2012–2021. Those with EPTB were further analyzed, and the demographic data was collected as well as clinical history, behavioral history, sites of extrapulmonary MDR-TB, and drug resistance. Of a total 7,013 TB cases, 1,900 (27.1%) were MDR-TB cases, of whom 0.08% (n6) were extrapulmonary MDR-TB cases and 0.16% (n11) were combined with PTB. The main characteristics of cases with extrapulmonary MDR-TB were median age 27-year-old (range 25–34), male gender (64.7%), underweight BMI (84.6%), and predominantly were primary cases (35.3%). The anti-TB drug resistance in MDR-TB were pre-XDR-TB (11.7%), XDR-TB (5.6%), MDR-TB (42%), and RR-TB (40.7%). Although the proportion of extrapulmonary MDR-TB among all TB cases is small (0.2%), this disease can't be ignored and has a great potential to be explored. Most of them are rifampicin-resistant. Further studies need to include a larger population to have more overview of MDR-TB with EPTB.

Abstract

An injury to the skin that disrupts the soft tissue may form a wound. The healing process in response to injury is a dynamic and well-regulated process of cellular, humoral, and molecular mechanisms that consists of four partly overlapping phases: hemostasis, inflammation, proliferation, and remodeling. An impaired wound-healing process may cause a formation of an abnormal scar and chronic wounds, leading to a reduced life quality. Therefore, it needs an optimal prevention strategy. Many modalities have been claimed to accelerate wound healing. The trend of using natural products is increasing in most Southeast Asian countries due to their biodiversity. Nowadays, studies on natural compounds are increasing to accelerate wound healing. Moringa oleifera Lam. is a high-value plant that each part of it has a high nutritional value as well as a great range of medicinal uses, including anti-inflammatory, antimicrobial, antioxidant, and wound healing properties. In this review, we have explored the M. oleifera that are very rich in vitamins, minerals, fatty acids, and phytochemical compounds like quercetin, kaempferol, and vicenin-2, that play a role in the wound healing process. Moreover, these compounds may enhance the healing of wounds with pathological conditions such as diabetes, immunocompromised and persistent infection.

Abstract

TGF-β and VEGF are vital in cell proliferation and regeneration, as evidenced in processes like wound healing. The leaves of Moringa oleifera Lam exhibit anti-inflammatory and cell regenerative properties that may facilitate the transition from the inflammatory to the proliferative phase, enhancing wound repair. This research sought to discern the potential of orally administered moringa leaf extract in augmenting systemic wound healing, focusing on TGF-β and VEGF serum as in vivo molecular markers. This research was conducted at the Animal Laboratory of the Faculty of Medicine, Universitas Padjadjaran, and the Laboratory of Molecular Genetics, Universitas Padjadjaran, from January to March 2022. We divided thirty Swiss Webster mice into two categories: healthy and wound-treated. Wounded mice received 100 mg/kgBW Na CMC as a negative control, 500 mg/kgBW zinc syrup as a positive control, and M. oleifera leaves ethanol extract (MOLE) in doses of 280 and 560 mg/kgBW orally from day 3 to day 6. Wound progression was documented and measured on days 0, -1, -3, and -6. Day-6 blood samples were obtained, and TGF-β and VEGF serum levels were gauged using ELISA. Results from day 6 revealed that wound coverage in the 280 and 560 mg/kgBW MOLE groups was 13.76±5% and 13.38±4%, respectively. These percentages notably surpassed that of the negative control group (p=0.005). However, the TGF-β and VEGF serum levels in the MOLE-treated groups did not differ significantly from the negative control (p=0.081 and p=0.149, respectively). Thus, the study concludes that while MOLE expedites wound closure, it does so without the systemic involvement of TGF-β and VEGF in vivo.

Abstract

Selama kehamilan, kebutuhan nutrisi meningkat dibandingkan dengan wanita yang tidak hamil. Makanan bergizi selama hamil meliputi asupan energi, protein, vitamin dan mineral. Namun di negara-negara berpenghasilan rendah-menengah. Pemenuhan kebutuhan asupan makanan ibu hamil menjadi tantangan yang menyebabkan terjadinya defisiensi mikronutrien sehingga diperlukan suplementasi mikronutrien. Tinjauan pustaka ini bertujuan untuk mendeskripsikan pengaruh suplementasi mikronutrien terhadap dampak pada kehamilan. Artikel ini disusun menggunakan Preferred Reporting Items for Systematic Review and Meta Analysis (PRISMA). Database diambil dari Science Direct, PubMed, Cochrane, dan SAGE Journal dengan kata kunci “Micronutrient during Pregnancy” OR “Micronutrient Supplementation of Pregnancy” OR “Micronutrient of Pregnant Women” OR “Supplementation of Pregnancy”. Sepuluh artikel dianalisis mengenai desain tujuan, hasil serta kesimpulan. Hasil tinjauan pustaka ini menjelaskan bahwa suplementasi mikronutrien untuk ibu hamil yang penting yaitu zat besi (Fe), asam folat, kalsium (Ca), magnesium (Mg), tembaga (Mg), seng (Zn) dan vitamin A, vitamin B, vitamin C, vitamin D. Suplementasi ini dapat dikonsumsi sesuai apa yang dibutuhkan tubuh. Dampak konsumsi suplementasi selama hamil yaitu dapat mencegah terjadinya anemia, gizi buruk, dan preeklamsia/eklampsia pada ibu hamil, bayi lahir pendek, prematur, berat badan lahir rendah dan kematian.

Abstract

Introduction: Iron deficiency anemia in developing countries is common. However, knowledge on the etiology of anemia among young adults needs to be further explored, including hemoglobinopathy, since Indonesia is located in the thalassemia belt, harboring 6-10% thalassemia carriers. The study aimed to explore the knowledge about anemia among young adults in Jakarta and the province of West Java. Methods: A descriptive analytical study design using secondary data from the National Demographic and Health Survey 2017 was employed. A questionnaire was distributed to unmarried young adults (n=5389) aged 15-24, including questions about anemia. Knowledge was further compared between places, genders, and age groups. Result: Only 70% of young adults have heard about anemia. However, the knowledge was poor. The definition of anemia was lack of blood (48%), and only 5% stated it was due to iron deficiency. The etiology of anemia was lack of vegetables (21%) and meat (18%) consumption. As a treatment, only 9% mentioned the iron tablets. None mentioned thalassemia carrier as an alternative cause of anemia. Conclusion: Young adults in Jakarta and West Java are poorly informed about anemia. Socialization to increase knowledge about anemia is imperative. Thalassemia carriers need to be introduced among unmarried young adults for better family planning in the future.

Abstract

ABSTRAK Pasien lupus eritematosus sistemik (LES) memiliki risiko lebih tinggi terhadap infeksi tuberkulosis (TB) akibat penggunaan obat lupus dan penyakitnya. Di Indonesia, prevalensi TB aktif pada pasien LES mencapai 10,2%, jauh lebih tinggi dibandingkan populasi umum. Kurangnya pengetahuan pasien mengenai TB dapat menyebabkan keterlambatan diagnosis dan pengobatan yang optimal. Tujuan: Melakukan program edukasi Tuberkulosis pada pasien lupus yang diberi nama TULUS dalam meningkatkan pemahaman pasien LES tentang risiko dan pencegahan TB dan mengevaluasi efektivitas program edukasi tersebut. Kami melaksanakan kegiatan edukasi berbasis penyuluhan secara langsung dengan media pendukung berupa poster dan flyer. Pengetahuan pasien diukur dengan pre-test dan post-test. Kegiatan ini dilakukan di Poli Reumatologi, Rumah Sakit Dr. Hasan Sadikin Bandung. Hasil test dianalisis menggunakan uji Wilcoxon Signed Ranks untuk mengukur pengetahuan pasien. Sebanyak 21 pasien LES berpartisipasi dalam kegiatan edukasi TULUS. Terdapat peningkatan yang signifikan pada median skor pengetahuan dari 7 pada pre-test menjadi 9 pada post-test (p=0,001). Program edukasi TULUS terbukti efektif dalam meningkatkan pengetahuan pasien LES mengenai TB. Kegiatan lanjutan perlu dilakukan untuk menilai retensi pengetahuan tentang TB pada pasien LES. Kata Kunci: Lupus Eritematosus Sistemik, Tuberkulosis, Edukasi Kesehatan, Pencegahan TB.  ABSTRACT Systemic lupus erythematosus (SLE) patients have a higher risk of tuberculosis (TB) infection, due to the use of lupus drugs and the disease activity. In Indonesia, the prevalence of active TB in SLE patients has reached 10.2%, much higher than the general population. Lack of patient knowledge about TB can cause delays in diagnosis and optimal treatment. To evaluate the effectiveness of the Tuberculosis Education in Lupus Patients called TULUS education program in increasing the knowledge of SLE patients about TB risks and prevention. We carried out direct education methods with supporting media in the form of posters and flyers. Patient knowledge was measured before and after the education. The activity was conducted at the Rheumatology Clinic, Dr. Hasan Sadikin General Hospital, Bandung. The pre-test and post-test results were analyzed using the Wilcoxon Signed Ranks test to measure patient knowledge. A total of 21 SLE patients participated in the TULUS education. There was a significant increase in the median knowledge score from 7 on the pre-test to 9 on the post-test (p=0.001). The TULUS education program has proven effective in increasing SLE patients' knowledge about TB. Further activities are needed to assess retention of knowledge about TB in SLE patients. Keywords: Systemic Lupus Erythematosus, Tuberculosis, Health Promotion
