Abstract

Cervical cancer in Indonesia in 2018 ranks second in cancer in women in Indonesia with an incidence rate of 348.809 cases with a mortality rate of nearly 60% of the incidence, namely 207.210 deaths. Deaths from cervical cancer are projected to continue to increase and are estimated to reach 12 million deaths by 2030 if not treated properly. The incidence of cervical cancer in Indonesia is estimated to have 180.000 new cases per year and the death rate is thought to reach 75% in the first year. This death is mainly associated with the majority of newly diagnosed patients who are already at an advanced stage (70% of cases) and are already at the terminal stage at the time of diagnosis.

Abstract

HPV is a sexually transmitted virus, and high-risk HPV DNA was found in 99.7%of cervical cancer specimens. Within 12 to 24 months of exposure to the virus,90% of HPV infections disappear or become inactive. However, infection with high-risk strains of HPV persist which then increases the risk of progression to cervicalcancer. The detection of precancerous lesions consists of various methods,including pap smear (conventional or liquid-base cytology / LBC), visualinspection of acetic acid (IVA), visual inspection of lugoliodine (VILI), and HPV DNAtest (genotyping / hybrid capture).

Abstract

WHO has created a global strategy to accelerate the elimination of cervical canceras a public health problem and a world burden. Cervical cancer is a disease thatcan be prevented and cured, as long as it is detected early and treated effectively.Cervical cancer is also a disease that reflects global injustice. The burden isgreatest in low- and middle-income countries, where access to public healthservices is limited and screening and treatment for the disease has not been widelyapplied. In 2018, nearly 90% of all deaths worldwide occurred in low and middleincome countries. Furthermore, the proportion of women with cervical cancer whodie from the disease is more than 60% in these countries, more than double thenumber in many high-income countries, which is only 30%.

Abstract

Screening is a public health intervention administered to a target population without symptoms. Screening is not performed to diagnose a disease, but to identify individuals with a higher likelihood of developing the disease itself or a precursor to the disease. Not all diseases are suitable for screening programs. The following criteria help determine whether a disease is suitable for a screening program: (1) The disease is bound to have serious consequences. (2) The disease must have a detectable preclinical and asymptomatic stage. (3) Treatment at the preclinical stage should influence the long-term course and prognosis of the disease being screened. (4) Care must be available and accessible to those who have a positive screening test. History, screening tests and treatment options for cervical pre-cancer meet these criteria.

Abstract

Vitamins are probably the best-selling dietary supplements in the United States, in which an estimated 35% of the population take multivitamin and mineral supplements. Data relating microconstituents to carcinogenesis are extremely limited, as explained in the first report. A striking feature of many of the compounds in the three groups mentioned above is their capacity to prevent or retard the occurrence of neoplasia. However, there is a paucity of data concerning the conditions under which inhibition occurs, the mechanisms of inhibition, and the precise impact of these microconstituents on humans. A literature review was conducted in the electronic databases PubMed and Google Scholar using the index terms "carcinogenesis" and "cancer" and "vitamin D" All types of studies were included for this study, such as randomized controlled trials, systematic reviews, literature reviews, and pilot studies published between 2010 and 2021. Articles which not written in English were excluded from the study. This search resulted in 10 papers. Many of the mechanisms proposed for vitamin D and cancer prevention have been studied only in the context of one tissue or one type of cancer and so further studies must be conducted to determine if these mechanisms can be generalized. For example, while higher serum (25(OH)D3) is associated with reduced cancer risk, and CYP27B1 is expressed in non-renal cells, there is no evidence that the low-level expression of CYP27B1 in tissues leads to meaningful local production of 1a,25(OH)2D3 or calcitriol, and that this mediates the protection from cancer provided by high vitamin D status. In conclusion, we suggest that vitamin D compounds to be employed in the treatment of, alone or in combination with other anticancer agents. solar UV-B radiation, acting through the production of vitamin D, is a risk reduction factor for over a dozen forms of cancer.

Abstract

Objective: To evaluate the clinical efficacy, operability, radicality, toxicity, and incidence of recurrences of neoadjuvant chemotherapy (NAC) followed by radical hysterectomy (RH) among patients with stage IIB cervical cancer. Method: This is an observational clinical study at Dr. Moh. Hoesin Hospital, Palembang. Data were analyzed from 27 patients who matched the inclusion criteria and underwent 3 cycles of neoadjuvant chemotherapy (NAC) with Paclitaxel (75 mg/m2) in combination with Cisplatin (50 mg/m2) and Docetaxel (75 mg/m2) combined with Carboplatin (300 mg/m2) according to AUC 6, followed by radical hysterectomy from January 2012 until December 2013. Result: The operability rate after NAC was 96.4%. Lymph node metastases were negative in 75% of patients, and we found bilateral lymph node metastases in 14.3% of patients. Parametric infiltrations were negative in 85.7% of the patients, and positive in 14.3% of patients. No vaginal infiltrations were found. As much as 89.3% of the patients did not experience any side effect, while anemia and thrombocytopenia were found in 10.8% of the patients. We found that 7.1% of patients had recurrences within 6 months interval. Conclusion: NAC followed by radical hysterectomy showed significant advantages for patients with stage IIB cervical cancer, with fewer side effects. However, long-term evaluation and a larger number of patients are required to confirm this result. Keywords: cervical cancer, neoadjuvant chemotherapy, radical hysterectomy

Abstract

Objective: To determine the association between BRCA1 gene Q356R (1186A=G) polymorphism and epithelial ovarian cancer incidence. Methods: This study is an observational analytic study with casecontrol study design. All patients diagnosed with epithelial ovarian cancer that were treated in the outpatient clinic and inpatient ward of the Department of Obstetrics and Gynecology, Dr. Mohammad Hoesin Hospital, Palembang who met the inclusion criteria were included in this study. DNA extraction was performed on blood samples, followed by PCR-RFLP process. Results: We obtained the genotype distribution of BRCA1 Q356R (1186A=G) polymorphisms to be QQ genotype (wild-type) on all 50 subjects in the case group (100%) and 50 control subjects (100%). Similarly, all BRCA1 alleles have the Q allele. The results of this study found no polymorphism of the BRCA1 Q356R (1186A=G) in the ovarian cancer and control groups. Conclusion: Polymorphism of BRCA1 gene Q356R (1186A=G) was not significantly associated with epithelial ovarian cancer incidence. Abstract

Abstract Objective: To determine the efficacy of neoadjuvant chemotherapy on changes of vitamin A serum in advanced cervical carcinoma patient. Method: A randomized clinical trial was performed in the Oncology Gynecology Polyclinic and Ward of Dr. Mohammad Hoesin General Hospital, Palembang from September 2019 to September 2020. There were 30 subjects of advanced cervical carcinoma. Vitamin A levels and tumor volume subjects were assessed before and 1 month after 3 cycles of NAC and analyzed using the Paired T Test and Wilcoxon test. The correlation between vitamin A levels and tumor volume was analyzed by using Spearman's Rho test. Data analysis was using SPSS version 22.0. Results: This study showed statistically not significant increase on vitamin A levels after 3 cycles of NAC chemotherapy (p=0.515). However, there was a significantly decrease in tumor volume after 3 cycles of NAC (p=0.000). In addition, there was a moderate negative correlation between tumor size and vitamin A (r=-0.475; p=0.008). Conclusion: It can be concluded that there was significantly decrease in tumor volume after 3 series NAC chemotherapy and the smaller tumor size, the higher level of vitamin A serum. Keywords: cervical cancer, neoadjuvant chemotherapy, randomized clinical trial, vitamin A, Abstrak Tujuan: Untuk mengetahui efektivitas kemoterapi neoadjuvan terhadap perubahan kadar serum vitamin A pada karsinoma servik stadium lanjut. Metode: Penelitian uji klinik tanpa pembanding telah dilakukan di Poliklinik dan Bangsal Onkologi Ginekologi RSUP Dr. Muhammad Hoesin Palembang sejak September 2019 hingga September 2020. Didapatkan 30 sampel karsinoma serviks stadium lanjut. Kadar vitamin A dan volume tumor dinilai sebelum dan 1 bulan setelah 3 siklus NAC dan dianalsis menggunakan uji Paired T Test dan Wilcoxon. Korelasi antara Kadar vitamin A dan volume tumor sampel dianalisis menggunakan uji Spearman Rho’s. Analisa data menggunakan SPSS versi 22.0. Hasil : Pada penelitian ini terdapat peningkatan tidak signifikan kadar vitamin A setelah kemoterapi (p = 0,515). Terdapat penurunan secara signifikan volume tumor sebelum dan sesudah kemoterapi NAC 3 seri (p = 0,000). Selain itu, didapatkan korelasi negatif sedang yang bermakna antara ukuran tumor dan kadar vitamin A ( r = -0,475 ; p = 0,008). Simpulan : Disimpulkan bahwa terdapat penurunan volume tumor setelah kemoterapi NAC 3 series dan semakin kecil ukuran tumor maka semakin tinggi kadar vitamin A dalam serum. Kata kunci: kanker serviks, kemoterapi neoadjuvan, uji klinik tanpa pembanding, vitamin A.
