Abstract

Sepsis is a major cause of morbidity and mortality among children worldwide, particularly in low- and middle-income countries. Early identification and continuous monitoring of organ dysfunction are critical components in paediatric sepsis management. The Paediatric Sequential Organ Failure Assessment (pSOFA) score is used to evaluate organ dysfunction and predict the risk of mortality. The aim of the study was to investigate the association between an increase in pSOFA score (∆pSOFA) and mortality among paediatric sepsis patients. This retrospective cohort study included paediatric patients aged 1 month to 18 years diagnosed with sepsis and treated in the PICU from January to December 2023. Receiver Operating Characteristic (ROC) curve analysis was performed to evaluate the predictive value of the pSOFA score for mortality. A total of 87 patients were included in the analysis, with 36 deaths recorded within the first 7 days. Patients with ∆pSOFA ≥1 had a six-fold higher risk of mortality (p<0.001; 95% CI: 3.26–11.02). The day-1 pSOFA score showed poor predictive ability (AUC=0.50), whereas the day-3 score demonstrated good predictive performance (AUC=0.85). An increase in the pSOFA score is significantly associated with a higher risk of mortality in paediatric sepsis patients.

Abstract

Sepsis neonatorum is a serious blood infection in newborns, mainly affecting premature or low birth weight infants. Early detection is tough, but new research suggests platelet indices could help diagnose it faster. The objective of this study is to analyze the diagnostic value of platelet indices (platelet count, MPV, and PDW) as early markers in suspected neonatal sepsis compared with the gold standard (blood culture). This was an observational analytical study with a retrospective cohort design conducted at the Adam Malik Hospital. A total of 115 suspected neonatal sepsis samples were enrolled. Data collection included demographic characteristics, platelet count, mean platelet volume (MPV), and platelet distribution width (PDW). The data were obtained from secondary sources in medical records and entered into the SPSS software for analysis. Statistical analysis was performed using receiver operator curve (ROC) to assess the diagnostic value of platelet indices (platelet, MPV, and PDW). The study involved 115 newborns suspected of neonatal sepsis, with 46.1% male and 53.9% female. Most were delivered via cesarean section and diagnosed with early onset sepsis. Common clinical signs included respiratory distress and distension. Key risk factors included prolonged rupture of membranes and low birth weight. Blood culture revealed Klebsiella pneumonia as the most common pathogen. Platelet indices showed varied sensitivity and specificity as predictors of sepsis. Diagnostic tests had low sensitivity but varied specificity for detecting sepsis with platelet count, MPV, and PDW.

Abstract

Neonatal sepsis remains a significant cause of morbidity and mortality, yet diagnosis is limited by nonspecific symptoms and delayed blood culture results. This study evaluated the diagnostic accuracy of procalcitonin (PCT), C-reactive protein (CRP), and mean platelet volume (MPV) as early biomarkers of neonatal sepsis. A retrospective cohort study was conducted in the Neonatology Unit of H. Adam Malik General Hospital, Medan, from January to December 2024. The sampling technique used is consecutive sampling. Of 346 neonates screened, 121 met the inclusion criteria and had concurrent PCT, CRP, MPV, and blood culture results. Clinical and laboratory data were extracted from electronic medical records. Diagnostic performance was assessed using sensitivity, specificity, predictive values, likelihood ratios, and ROC analysis. Among 121 neonates, 31.4% had positive blood cultures. MPV showed no significant difference between culture-positive and culture-negative groups (p = 0.104) and demonstrated limited discriminatory value (AUC 0.592). CRP was significantly associated with culture results (p = 0.037), showing high specificity (94.7%) but low sensitivity (30.0%) at ≥1 mg/L (AUC 0.624). PCT demonstrated the strongest performance, with significantly higher levels in culture-positive neonates (p = 0.022), sensitivity of 64.5%, specificity of 69.0%, and an AUC of 0.672. Diagnostic accuracy was 66.7% for PCT, 85.1% for CRP, and 55.4% for MPV. PCT provided the most balanced diagnostic utility for neonatal sepsis, while CRP served as a highly specific rule-in marker. MPV showed limited usefulness. Although these biomarkers contribute to early assessment, they are insufficient as standalone diagnostic tools and should be integrated with clinical evaluation and microbiological testing.
