Abstract

The difference in rates between the real costs and the INA-CBG package rates is one of the causes of the decline in service revenues at the hospital, one of which is in Dr. Moewardi as a type A hospital in the Surakarta area. In Tribulan 1 2020, there were 2680 differences in negative claims. One of the cases that caused a high negative difference was the obgyn case with Cesarean Section action, namely 73 files which caused a negative difference of IDR-361,971,358. The accuracy of the diagnostic code can affect and cause differences in the INA-CBGs rates. The purpose of this study was to analyze the accuracy of the Cbgs Ina code the cause of negative claims in Dr. Moewardi. The method in this research is descriptive research with a cross sectional approach, namely examining the data directly at the time of the study. The study population was the BPJS claim file in the 1st quarter of 2020 which had a negative difference of 2680 files. The sample in this study used a sampling technique with a purposive sampling method with the criteria for the inclusion of a claim file that had a negative difference between obgyn cases and Cesarean Section action with the code Ina Cbgs (O-6-11-I, O-6-11-II, O- 6-11-III) and negative claim file exclusion criteria with obgyn cases not using Cesarean Section measures and negative claims files with non-obgyn cases. The number of samples was 73 files. Analysis of the accuracy of the diagnosis code and action in this Cesarean Section case is interpreted as a percentage. The results showed that the correct code was 71.23% compared to the incorrect percentage of 28.77%. The highest category of inaccuracy in the diagnosis code was due to the secondary diagnosis that was not coded by 42.86%, and the lowest was wrong in determining the primary diagnosis by 4.76%. Of the 28.76% (21 documents) that were not fast, only 2 files differed between hospital fees and researchers which caused a negative difference of Rp. 2,559,500. Conclusion Based on the results of the study, the accuracy of the obsgyn case diagnosis code by section action was greater than the incorrect one. Not all of the incorrect code causes the difference in the amount of the difference in negative claims.

Abstract

Based on a survey at the hospital, outpatient visits with coronary heart disease had increased from the previous year, namely 50 patients (2017) and 120 patients (2018). This study aims to determine the relationship between patient characteristics and re-visit of outpatients with coronary heart disease. The type of research used in this research is quantitative research, the sample used is 120 patients. Data were collected by means of a questionnaire and then analyzed by using the Chi-Square test. The results showed that the highest age was 58-67 years (39.9%), the highest gender was male (73.3%), the highest payment method was general patients (63.3%), the highest patient origin was patients came of their own accord (63.3%) and complications of the disease, there were complications that existed in the amount of 72.1%. Chi-Square test results were age (p = 0.037), gender (p = 0.0001), method of payment (p = 0.0001), patient origin (p = 0.0001) and disease complications (p = 0.0001) there is a connection with a repeat visit. The most related logistic regression test results were the method of payment and the origin of the patient with a value of p = 0.0001. It is known that the characteristics of age, sex, method of payment, patient origin and disease complications are associated with repeat visits, while the most related ones are payment methods and patient origins. Suggestions that can be conveyed are to be able to maintain superior services that have been in great demand by patients for treatment.

Abstract

The development of information systems is currently experiencing very rapid development which we now recognize as the era of digitalization, one of which is in the field of health services. Primary healthcare is a type of medical health service that provides health services to individuals at the first level health service. The problem that most often arises is related to the registration system which is still done manually by recording in the registration book, thereby allowing the risk of errors occurring and the process of searching for visit data taking a long time. This research aims to improve better service for visits so it is necessary a good, systematic and integrated data handling and processing in a developing service system, even at the level of independent health service practice. This research method uses Field Observation, Library Research, Interviews, Study Research Literature (Research Literature Study), System Design, System Implementation and Testing. Application system from syntax errors and functionally produces functions as expected. The health service system application (Good Care) has been tested and can be used so that it can increase the effectiveness and efficiency of health services at the independent doctor's practice at Clinic of Wijaya Kusuma.
