Abstract

Introduction: Midwives are essential healthcare providers for maternal and neonatal health, particularly in normal deliveries. Their competence directly affects the health outcomes of both mothers and newborns. This study aimed to identify the barriers and facilitators influencing midwife competence in providing normal delivery care in Community Health Centers (CHCs) in Gasan Gadang, Padang Pariaman, Indonesia. Methods: A cross-sectional study was conducted among 31 midwives in CHCs in Gasan Gadang, Padang Pariaman, Indonesia. Data were collected using questionnaires assessing midwife competence, workload, work environment, and disciplinary adherence. Descriptive statistics, inferential analysis (Chi-square), and multivariate analysis were performed using SPSS v.16. Results: 61.3% of midwives demonstrated good competence in normal delivery care. A significant association was found between workload and competence (p=0.001). No significant association was observed between the work environment and competence (p=0.149) or between disciplinary adherence and competence (p=0.567). Multivariate analysis revealed that workload had the most significant impact on midwife competence. Conclusion: Workload is a significant factor influencing midwife competence in normal delivery care. Interventions aimed at optimizing workload, such as adequate staffing and resource allocation, are crucial for ensuring competent care and positive health outcomes.

Abstract

Maternal anxiety during the childbirth process is a common psychological condition that can potentially increase the risk of complications, prolong labor, and reduce the likelihood of a positive birthing experience. High anxiety levels in mothers may interfere with the effectiveness of labor, impact maternal and fetal well-being, and lead to increased medical interventions. One of the most important support systems during labor is the presence and active involvement of the husband. The husband’s role is not only limited to being present but also includes providing emotional, physical, and motivational support to the mother. Recognizing the importance of this role, this community service program was designed to enhance husbands’ understanding and skills in supporting their wives during childbirth, thereby helping to reduce maternal anxiety. The program consisted of several stages, including the delivery of structured health education sessions on maternal needs during labor, demonstrations of effective communication techniques, simulations of support during different stages of childbirth, and interactive discussions to address questions and share experiences. Through these activities, participants were encouraged to build empathy, strengthen their knowledge about labor, and develop the confidence to assist their wives effectively. The results of the program indicated a noticeable improvement in the husbands’ awareness of the importance of their role, as well as increased confidence in providing both emotional reassurance and physical assistance. Participants reported feeling better prepared to accompany their wives, manage unexpected situations, and foster a more supportive environment during childbirth. This initiative highlights the significant impact that targeted education and training for husbands can have on maternal psychological well-being, ultimately contributing to safer and more positive birth experiences.

Abstract

Psychological preparation One thing that mothers must prepare before giving birth is to avoid panic and fear and be calm, so that pregnant women can go through the birthing period well and be more prepared and ask for support from the people closest to them. Mothers who are not ready to face childbirth are one of the factors causing maternal mortality (MMR). Many efforts have been made to support efforts to reduce MMR, one of which is by preparing for childbirth starting during pregnancy. From the initial survey at the service location, namely Poskeskel Pakan Kurai Bukitinggi, several problems were found, especially the problems of pregnant women, one of which was the problem of dealing with maternal psychology regarding the physical and mental readiness of mothers in preparing for childbirth in the third trimester of pregnant women from 11 pregnant women who were accompanied by health cadres. Of the 6 pregnant women, in discussions with pregnant women, several problems were raised, one of which was the problem of pregnant women's lack of knowledge about how to prepare for the birthing process. At the activity stage, there will be introduction, education provided directly to pregnant women, guidance and outreach to pregnant women. Evaluation is carried out by providing questions and answers. The results of this activity are the provision of education and the existence of leaflets which are directly read by pregnant women. There is an increase in mothers' knowledge about pregnancy preparation which includes dealing with maternal psychology regarding physical and mental readiness in preparing for childbirth. With increased knowledge, a positive attitude will be formed from pregnant women to prepare for childbirth.

Abstract

Antenatal care aims to see how the fetus grows and develops and aims to improve the welfare of the mother and fetus. WHO's efforts to reduce the number of high-risk pregnancies include routine examinations for all pregnant women, as well as follow-up examinations for mothers with moderate comorbidities and complications, obstetric and neonatal examinations for women with severe comorbidities and complications. Yeoh et al, (2016) stated that as many as 26% of women with high-risk pregnancies never had an Antenatal Care examination or visit. while 80% of pregnant women experience no complications, and carry out regular Antenatal Care visits according to the recommended re-visit schedule. Pregnant women who do not regularly carry out prenatal checks will have a risk of experiencing pregnancy complications such as chorioamnionitis, placental abruption and a 10x greater risk of giving birth to a baby with a low birth weight, a 12x greater risk of fetal death and a 10x greater risk of maternal death
