Abstract

Background: Juvenile Systemic Sclerosis (JSSc) is a rare chronic-autoimmune rheumatic disease leading to potential delay in diagnosis, especially in limited sources due to lack of knowledge and facility. This case report aimed to present a rare case of JSSc in limited sources of medical facilities. Case presentation: A 15-year-old girl was earlier suspected of having congenital heart disease due to the presentation of bluish fingers without any detectable clinical heart disease manifestation, including normal electrocardiography as well as echocardiography. The patient experienced pain and difficulty executing wrist endorotation, representing limited joint movement action. Further physical examinations showed pale, cold fingers and toes, bluish color in the peripheral represented sclerodactyly and arthritis. Skin showed multiple erythema-hypopigmentation papuls, in accordance with gottorn papule, gottorn sign. The antinuclear antibody titer was 1:320. Chest CT-Scan result was a fibrotic line at the inferior lobe of the left lung with the left diaphragm tenting due to a chronic inflammatory process. Skin biopsy shows an acanthotic epidermis, dermafibrocollagenous tissue with extensive fibrosis, and sclerosis with vascular proliferation. These findings meet JSSc PRES, ACR, and EULAR standard classification criteria, including 1 major and 4 minors. The treatment regimen was started with methylprednisolone 2 mg/kg/day, then tapering off once clinical manifestation alleviated, methotrexate 10 mg/BSA, and folic acid 1 mg a day. Long-term monitoring plans are also scheduled. Conclusion: Recognizing early symptoms of JSSc would provide a better outcome.

Abstract

Introduction : Statistic data consistently showed that adolescent is one of the vulnerable group to HIV/AIDS transmission, as it reflected by a persistently increase of HIV/AIDS infected patients from year to year. Level of knowledge of adolescents is the main component of HIV/AIDS transmission prevention program. The aim of this study was to assess effectiveness of social campaign to increase level of knowledge of HIV/AIDS among senior high school adolescents, and correlation between level of knowledge and negative attitude. Methods : 71 students from state senior high school in Mengwi district, Badung Regency, Bali, were taken by consecutive method. The data was taken by questionnaire, before and after the social campaign was given. Results : There were 50,7% students had good knowledge before the campaign was given, and 59,2% students still had negative attitude. 70,4% students agreed that adolescent is susceptible to HIV/AIDS. Proportion of adolescent with good knowledge after the campaign was given, increased significantly into 98,6% (p=0,000), and there was an increase of pretest score, from 57,23 (SD 20,4) and posttest score was 84 (IQR 8). There was correlation between level of knowledge and negative attitude ( spearman’s rho= -0,543, p=0,000). Conclusion : Social campaign effectively increase level of knowledge of adolescent, and level of knowledge is correlated with negative attitude of adolescent toward HIV/AIDS.

Abstract

Highlights: • HIV-infected children had lower capute scores than normal children. • HIV child age range 12-24 months almost suffer delayed speech, and were in suspect criteria was founded. Abstract: The cognitive, motor and language aspect of HIV-infected children is an important issue affecting their quality of life. The capute scale, divided into Cat and Clams scores, is commonly used to assess children's cognitive, motor, and language functions. This study assessed Cat and Clams' scores of HIV-infected children. We performed Cat and Clams assessment on 136 children consisting of 68 HIV-infected children and 68 normal children as control aged 0-36 months. The Capute scale examined both groups at the first meeting (first month), and for the rest six months, we evaluated the progress of cognitive, motor, and language development. In the first meeting, we found that HIV-infected children had significantly low capute scores than the control group. The sixth-month Capute score in HIV-infected children was also significantly lower than the control group. HIV-infected children had lower Capute scores than normal children.

Abstract

Background Sepsis remains a leading cause of pediatric intensive care unit (PICU) admissions in Indonesia. Serum ferritin is a potential biomarker for sepsis severity that requires validation. Objective To evaluate the association between serum ferritin and sepsis severity in children. Methods In this cross-sectional study, we consecutively enrolled children with sepsis (aged 1 month to 18 years) with lactate-excluded PELOD-2 scores of ≥7 admitted to the PICU of a tertiary referral hospital. Severe sepsis was defined as a PELOD-2 score of ≥11. Serum ferritin levels were measured at the time of PELOD-2 assessment. We determined the optimal ferritin cut-off point for severe sepsis by ROC analysis and performed bivariate and multivariate analyses to assess the association of ferritin level with sepsis severity. Results Among 74 subjects, 48.6% had severe sepsis. Serum ferritin concentration showed excellent discriminative power for severe vs. non-severe sepsis, with an area under curve (AUC) of 80% (95%CI 69.7-90.4%; P<0.001). The optimal serum ferritin cut-off level was 1,473.88 ng/mL, with a sensitivity of 80.6% and specificity of 78.9%. Severe sepsis was strongly correlated with elevated ferritin [bivariate prevalence ratio (PR) 3.8 (95%CI 2.0 to 8.2; P<0.001); adjusted PR 3.9 (95%CI 1.7 to 8.6; P=0.001)]. Conclusion Serum ferritin may serve as a biomarker for severe sepsis in children with good discriminative performance and may be useful for risk stratification of pediatric sepsis.

Abstract

Introduction: Limited awareness and diagnostic resources contribute to delayed recognition of primary immunodeficiency (PID). The Jeffrey Modell Foundation (JMF) 10 warning signs have been widely used for screening. However, their diagnostic performance has not been validated in Indonesia. Objective: To evaluate the diagnostic performance of the JMF 10 warning signs for early detection of PID among children with recurrent infections in Indonesia. Methods: This multicentre cross-sectional study was conducted across 10 hospitals in Jakarta involving 254 children aged 0–18 years who met the severe, persistent, unusual, and recurrent (SPUR) infection criteria. The diagnosis of PID was established based on clinical diagnosis by the European Society for Immunodeficiencies (ESID) Registry Working Definitions. Analysis was performed to determine the sensitivity, specificity, predictive values, and accuracy of the JMF 10 warning signs. Results: Diagnostic performance for PID improved with the number of warning signs, with overall accuracy increasing from 15% (1 sign) to 86.2% (≥5 signs). A total score of ≥3 signs was associated with higher occurrence of PID (PR 2.63; 95% CI 1.23–5.60; p = 0.008), providing an optimal balance of 75.7% sensitivity and 48.9% specificity. The two most specific indicators were recurrent severe sinus infections (99.5%) and ear infections (95%). All PID subjects required IV antibiotics. Conclusion: The JMF warning signs remain a valuable clinical screening tool for early detection of PID among Indonesian children. A threshold of three or more warning signs provides reasonable accuracy for screening and referral of PID in resource-limited settings.

Abstract

Background: Choledochal cyst is an uncommon but important cause of biliary pathology in the pediatric population which can lead to choledocholithiasis and recurrent pancreatitis. High clinical suspicion, comprehensive imaging, and multidisciplinary evaluation are essential for accurate diagnosis and treatment. Definitive management is achieved through surgical intervention. Case: A 7-year-old girl experienced recurrent postprandial right upper quadrant and epigastric pain for three years, with fluctuating pancreatic and hepatic enzymes. Initial ultrasound and magnetic resonance cholangiopancreatography (MRCP) revealed ductal dilatation with biliary sludge, while multislice computed tomography (MSCT) suggested autoimmune pancreatitis, leading to temporary steroid response. Symptoms persisted, and contrast-enhanced magnetic resonance imaging (MRI) demonstrated fusiform common bile duct dilatation with debris, confirming Todani type IC choledochal cyst and choledocholithiasis. She underwent laparoscopic cyst and gallbladder excision with Roux-en-Y hepaticojejunostomy. Intraoperative and histological findings verified sludge, gallstones, and chronic cholecystitis. Discussion: Choledochal cysts should be considered in pediatric recurrent pancreatitis, particularly with biliary obstruction. Advanced imaging and differential diagnosis are crucial. Surgical excision with biliary reconstruction is the treatment of choice. Conclusion: Early recognition and timely surgical intervention are essential to prevent long-term complications.
