Abstract

Abstrak: RSIA Dian Pertiwi Karanganyar merupakan Rumah sakit khusus tipe C di Kabupaten Karanganyar, yang menangani pasien dengan kehamilan dan persalinan risiko tinggi, keadaan ini berakibat pada meningkatnya operasi mayor kebidanan emergensi dan meningkatkan komplikasi dari tindakan tersebut. Peningkatan komplikasi tindakan operasi mayor kebidanan emergensi berbanding lurus dengan peningkatan permasalahan hukum yang akan dihadapi oleh tenaga kesehatan. Timbulnya permasalahan hukum akibat komplikasi tindakan operasi mayor kebidanan emergensi tersebut mengisyaratkan diperlukannya perlindungan hukum terhadap tenaga kesehatan yang melakukan layanan operasi emergensi tersebut. Peraturan perundangan yang berlaku, perijinan Rumah Sakit, peraturan internal Rumah Sakit, persyaratan perijinan tenaga kesehatan merupakan piranti yang disediakan untuk melakukan pelindungan hukum bagi tenaga kesehatan. Penelitian dengan metode yuridis sosiologis, deskriptif analitis dan metode pengumpulan data dengan studi lapangan yaitu wawancara dan studi kepustakaan telah dilakukan di RSIA Dian Pertiwi terhadap tim operasi kebidanan tahun 2022 untuk mengetahui sejauh mana pelindungan hukum terhadap tim operasi tersebut telah dijalankan. Penelitian ini bertujuan untuk memperoleh data dan gambaran secara lengkap tentang pelaksanaan pelindungan hukum bagi tenaga kesehatan di RSIA Dian Pertiwi. Hasil penelitian bahwa pengaturan perlindungan hukum meliputi segi norma hukum dan bentuk atau tehnis pengaturan hukum bagi tenaga kesehatan telah terpenuhi dengan baik demikian juga pelaksanaan dari pelindungan hukum terhadap tenaga kesehatan yang meliputi pihak terkait, bentuk pelindungan hukum dan ruang lingkup pelaksanaan pelindungan hukum. Satu kasus fatal akibat atonia uteri yang terjadi paska operasi perlu dicermati Manajemen Rumah Sakit dan Tim Operasi, untuk mencegah kasus fatal terulang kembali.Abstract: RSIA Dian Pertiwi is a specially hospital in Karanganyar District , referral center a high risk for pregnancies and deliveries, including a many babies and childs who finding health problems likes infection, malnutrition and others. This condition actually increases an obstetrian emergency operation like cesarian section, automatically increase the complication about this and increase the legal health problem for health workers Arise legal problems consequence complication of obstetrician emergency operation, induce legal health problems for health workers who prepared patient for obstetrian emergency operation. Law regulations, internal hospital regulation, letter of license for health workers can proctect them too legal medical problems In 2022 our studies on yudiris sosiology method, spesification descriptive and analisis data with interview and literature studies aim to collecting data and put the desription perfectly about implementation legal protection for find out legal protection for health workers who done in mayor obstetrician operation room Result about this studies are regulation of legal protection including the norm of law and tehnic of law regulation about health workers against the risk of obstetrician emergency operations in RSIA Dian Pertiwi can be fulfilled, similar about implementation legal regulation including legal protection, scope of law regulation and the related institution are fulfilled too. There is one fatality case cause atonia uteri occur after this emergency operation, can finishing with discussion between hospital management and the pasien until achieved agreement. At least the fatality case make our operation tim and health workers introspection how this moment can occuring and make the changging too avoid the same insident.

Abstract

BACKGROUND: Thalassemia patients with repeated transfusions (multi-transfusion) are often at risk of experiencing early osteoporosis. Several studies have demonstrated that osteoporosis in these individuals was associated with altered bone remodeling, characterized by decreased insulin-like growth factor-1 (IGF-1) and serum calcium ions, as well as increased serum β-Cross Laps (β-CTx) levels. Despite the prevalence of this condition, there is limited literature on the relationship between IGF-1 levels and calcium ions with β-CTx. Therefore, this study was conducted to examine the relationship between IGF-1 and calcium ions levels with serum β-CTx in multi-transfused β-thalassemia major patients.METHODS: A cross sectional study involved 29 thalassemia patients with multiple transfusions, aged 2-18 years, that were selected from the electronic medical records. Calcium ion levels were examined using ion selective electrode method. Subsequently, IGF-1 and serum β-CTx levels were examined by enzyme-linked immunosorbent assay (ELISA), and the data were analyzed using the Pearson correlation test.RESULTS: The results showed that the mean serum IGF-1 levels, calcium ion, and β-CTx were 20.11±20.763 ng/mL, 1.26±0.07 mmol/L, and 9330.40±1696.76 ng/mL, respectively. Statistical analysis showed a significant relationship between the levels of IGF-1 (r=-0.573; p=0.001) and calcium ion (r=-0.373; p=0.046) with serum β-CTx. A moderate negative relationship was found between IGF-1 levels and β-CTx, while calcium ion levels and β-CTx showed a weak negative relationship.CONCLUSION: A moderate negative correlation between IGF-1 and serum β-CTx, and a weak negative correlation between calcium ion and serum β-CTx suggest that IGF-1 and calcium ions may serve as potential indicators of bone turnover and osteoporosis risk in multi-transfused β-thalassemia major patients, underscoring their potential role in routine clinical evaluations.KEYWORDS: miR-200a expression, NO level, early-onset preeclampsia, late-onset preeclampsia

Abstract

This erratum is to explain a typographical error in Table 2 occurred in the published issue of Indonesian Journal of Clinical Pathology and Medical Laboratory Volume 31, Number 2 in March 2025, pages 136. The error was in writing the Fibrinogen level which should be a period, not a comma and D-dimer level in the table, which should be a comma, not a period.

Abstract

This study explores the integration of telemedicine into the Indonesian healthcare system, focusing on the economic and management strategies that optimize service delivery. It investigates the current state of telemedicine in Indonesia, identifying key challenges and opportunities within this emerging field. The research employs a mixed-methods approach, combining quantitative data analysis with qualitative case studies, to provide a comprehensive overview of telemedicine's impact on healthcare efficiency and accessibility. The findings reveal that telemedicine offers significant potential for enhancing healthcare services in Indonesia, particularly in remote and underserved areas. However, it also highlights the need for robust economic models and effective management practices to ensure sustainable implementation. The study discusses the implications of telemedicine for healthcare policy and management, suggesting strategic approaches for its integration into the national healthcare system. It emphasizes the importance of stakeholder collaboration, infrastructure development, and policy support in realizing the full benefits of telemedicine. The research contributes to the growing body of knowledge on telemedicine in developing countries, offering valuable insights for healthcare practitioners, policymakers, and researchers. This study underscores the transformative potential of telemedicine in reshaping healthcare delivery in Indonesia, advocating for strategic investments and policy reforms to harness its full potential.

Abstract

Background: Obesity considered as a low-grade inflammation. Increased body fat has known to trigger inflammation. Platelet profile is a number of platelet-related parameters that can predict inflammation consisting of: platelet count (PLT), Platelet Larger Cell Ratio (P-LCR), Mean Platelet Volume (MPV). Differences in platelet profiles (PLT, P-LCR, MPV) in women with and without central obesity have only been investigated in a few studies.Objective: To prove differences in platelet profiles in women with and without central obesity.Materials and Methods: A cross-sectional observational study was conducted on 88 women with and without central obesity in RSND during July-September 2021. Data included age, abdominal circumference, hip circumference, PLT, P-LCR, and MPV. PLT, P-LCR, and MPV were measured using Sysmex XS-500i instrument. Statistical analysis was using Mann-Whitney test.Results: Mean of women PLT with and without central obesity were 338.72±71.09x103/uL and 309.09±44.36x103/uL. Difference platelet levels in women with and without central obesity was p=0.022. Median MPV of women with and without central obesity were 10.5 (8.5-11.8)fL and 9.7 (8.5-11.6)fL. Difference MPV values in women with and without central obesity was p=0.000. Median P-LCR of women with and without central obesity were 28.2 (12.3-44.3)% and 21.5 (15.2-37.1)%, respectively. Difference P-LCR value in women with and without central obesity was p=0.002.Conclusion: Platelet profiles (PLT, MPV, P-LCR) can be used as a marker of chronic low-grade inflammation in women with central obesity.Keywords : central obesity; MPV; PLT; P-LCR

Abstract

The global Pandemic that has emerged has led to big challenges for the world health system, showing limitations in infrastructure, response, and international coordination. Inequality in the distribution of health resources and access to maintenance, along with obstacles in communication and coordination of inter-country policies, shows the urgent need for evaluating and improving global health management strategies. This condition encourages the need for in-depth analysis of strategies that can be implemented in dealing with pandemics, including strengthening the health system and international cooperation. This study aims to identify and analyze various strategies in global health management to deal with pandemics that emerge, focusing on threats, challenges, and opportunities for international cooperation. This study uses a qualitative approach, relies on data from relevant studies and studies, and is followed by in-depth analysis to integrate findings. The results of this study indicate that the current global health system faces various challenges in dealing with the Pandemic, which include infrastructure readiness, the effectiveness of surveillance systems and early detection, as well as the need for stronger international cooperation. There are also challenges in coordinating between countries, political and economic influences, as well as obstacles in communication and dissemination of information. This study found that international cooperation, especially in research and medical development, sharing resources, and coordination by international organizations, is the key to tackling the Pandemic. Building the capacity of the health system in developing countries is also an important aspect of global strategy.
