Abstract

Latar Belakang: Neuropati perifer merupakan salah satu komplikasi mikrovaskuler dari Diabetes Mellitus (DM) yang terjadi pada bagian perifer dan menimbulkan kerusakan fungsi saraf. Kerusakan fungsi saraf tersebut dapat mengenai saraf sensorik, motorik, dan otonom. Tujuan: Tujuan penelitian ini adalah untuk mengetahui gambaran neuropati perifer di wilayah kerja Puskesmas Kedungmundu Semarang. Metode: Penelitian ini merupakan penelitian deskriptif kuantitatif dengan pendekatan cross sectional. Teknik sampling yang digunakan yaitu total population sampling. Pengambilan data dilakukan dengan lembar pemeriksaan neuropati perifer yang merupakan modifikasi dari Michigan Neuropathy Screening Instrument (MNSI) dan Michigan Diabetic Neuropathy Score (MDNS) terdiri atas 38 item. Analisis data yang digunakan yaitu analisis univariat. Hasil: Sebanyak 113 responden berpartisipasi dalam penelitian ini. Mayoritas responden berusia dewasa tengah (73.5%), sebagian besar perempuan (61.9%), telah menderita DM >5 tahun (50.4%), dan memiliki kadar GDS ?200 mg/dL (52.2%). Sebagian kecil responden memiliki riwayat amputasi dan riwayat Diabetic Foot Ulcer (DFU) (3.5% dan 5.3%). Mayoritas kerusakan fungsi saraf adalah kerusakan otonom baik kaki kanan maupun kaki kiri (89.9%;85%). Lebih banyak responden yang memiliki neuropati ringan (55.8%) daripada neuropati sedang ataupun berat (28.3%;9.7%). Kesimpulan: Hasil penelitian ini menunjukkan bahwa sebagian besar responden mengalami neuropati perifer. Oleh karena itu, diperlukan pemeriksaan dini untuk mencegah neuropati yang lebih berat Kata kunci: Diabetes Melitus, Neuropati Perifer

Abstract

Latar belakang: Pasien dengan diabetes harus memperhatikan manajemen gizi yang sangat penting untuk mengontrol status glikemik. Salah satu fokus pengelolaan gizi pada DM adalah untuk mencegah hiperglikemia yang mungkin terjadi. Setiap individu seharusnya mengikuti ketentuan diet yang berlaku. Namun, banyak masalah gizi muncul yang mencakup pengetahuan, kesadaran, kepatuhan, dan juga implementasinya. Dengan demikian, pemantauan gizi yang dikonsumsi oleh pasien dengan DM sangat penting untuk mempertahankan hasil metabolisme yang optimal.Tujuan: Tujuan dari penelitian ini adalah untuk mengidentifikasi asupan gizi pada pasien dengan DM.Metode: Penelitian ini adalah penelitian deskriptif dengan menggunakan metode survei untuk mengumpulkan keseluruhan data. Sejumlah sampel diambil secara consecutive pada pasien diabetes di satu Puskesmas di Kota Semarang. Sebanyak 93 responden berpartisipasi dalam penelitian ini. Kriteria inklusi meliputi pasien dengan DM yang berusia 18 – 70 tahun. Kuesioner recall 24 jam digunakan untuk mencatat dan mengamati asupan gizi dan pengambilan tiga kali tidak berturut-turut meliputi 2 hari hari kerja dan 1 hari pada akhir pekan. Analisis univariat dilakukan untuk mengintepretasikan data.Hasil: Hasil penelitian ini menunjukkan bahwa lebih banyak responden yang berpartisipasi dalam penelitian ini adalah perempuan daripada laki-laki (n=68, 62,4%; n=35, 37,6%), masing-masing. Sebagian besar responden (n=83, 89,2%) digolongkan sebagai tidak tepat dalam menerapkan asupan gizi.Simpulan: Masih banyak masyarakat yang menyandang DM belum tepat dalam menerapkan diet yang harus dikonsumsi. Pasien DM harus mematuhi perencanaan makan dengan jadwal, jumlah, dan jenis makanan yang tepat dengan komposisi karbohidrat, lemak, protein yang seimbang dan konsisten dengan jadwal makan. Pengkajian dan monitoring terkait dengan konsumsi nutrisi pada penyandang DM sangat diperlukan.

Abstract

Thalassemia is one of blood diseases that genetically inherited suffered by people worldwide, also in Indonesia. The number of people with thalassemia increases every year. Thalassemia could not be cured, however it can be prevented by early detection in premarital screening. The aim of this study was to describe knowledge of premarital couples regarding premarital screening thalassemia. This study was a quantitative research with a descriptive survey to premarital couples in KUA Tembalang, Semarang. The samples were determined purposively. The questionnaire of knowledge and attitude towards thalassemia and pre-marital screening was distributed to respondents, then analyzed descriptively. The result presented in the frequency and percentage. There were 71 couples participated in this study. The result showed that 75.4% of premarital couples have never heard about thalassemia. Only 46.5% of premarital couples have a good knowledge toward thalassemia. Moreover, 40.8% respondents know the right definition about screening. However, more premarital couples did not know that thalassemia could be prevented by premarital screening that they were. It is concluded that couples' knowledge about thalassemia and premarital screening thalassemia was still low. So, knowledge and understanding to thalassemia and early detection – premarital screening are important to ascertain individual status of thalassemia.

Abstract

Pasien kanker yang menjalani kemoterapi maupun radioterapi dapat mengalami masalah pada rongga mulut yaitu terjadinya mukositis. Mukositis menyebabkan ketidaknyaman pada mulut, ketidakmampuan untuk mentoleransi makanan atau cairan (disfagia) sampai akhirnya mengalami penurunan status gizi, mukositis juga dapat menimbulkan dampak pada fisik, psikologis, dan ekonomi. Oral hygiene saat menjalani kemoterapi maupun radiasi merupakan perawatan yang teraman dan termurah dalam menanggulangi mukositis. Tujuan penelitian ini adalah untuk melihat perbedaan derajat mukositis dengan oral hygiene menggunakan larutan kopi. Desain dalam penelitian ini menggunakan quasi experiment dengan rancangan penelitian pretest- posttest with control group design. Populasi pada penelitian ini adalah pasien yang menjalani terapi radiasi di poli Radioterapi RSUP dr. Kariadi Semarang. Penilaian derajat mukositis dinilai dari instrumen WHO. Data dianalisis dengan uji paired t test dan independence t-test untuk melihat pengaruh. Hasil penelitian menunjukkan rata-rata derajat mukositis sebelum diterapkan oral hygiene menggunakan kopi adalah 3 di kelompok intervensi dan 3 pada kelompok kontrol, setelah diterapkan oral hygiene dengan kopi nilai rata-rata mukositis menjadi 2 pada kelompok intervensi dan pada kelompok kontrol mempunyai nilai rata-rata untuk mukositis tetap berada pada derajat 3. Terdapat perbedaan yang signifikan pada derajat mukositis pasien kanker kepala leher yang menjalani radioterapi sebelum dan sesudah penerapan oral hygiene dengan kopi pada kelompok intervensi p<0.05 dan terdapat pengaruh oral hygiene dengan kopi terhadap derajat mukositis p<0.05.

Abstract

Diabetic Foot Ulcer (DFU) is one of the complications often experienced by patients with diabetes mellitus (DM). This is a serious problem that leads to disability, morbidity, and mortality among diabetic patients. However, in Indonesia, studies about DFU characteristics are very limited. This study aimed to describe DFU characteristics among patients with DM. A consecutive sampling involved patients who met inclusion criteria was performed in Tugurejo Hospital; Islamic Sultan Agung Hospital, and Dr. Moewardi Hospital. Their demographic characteristics, clinical condition, and wound appearances were noted and documented. Diabetic Foot Ulcer Assessment Scale (DFUAS) that consists of 11 characteristics was used to observe the wound features. The data were quantitatively analyzed to elucidate the result. A total of 73 patients participated in this study. It was revealed that 50.7% were females and 75.3% have been diagnosed with hyperglycemia. The average age of the respondents was 53.26 years. Observation of DFU characteristics showed that depth of wound was identified more in subcutaneous/ dermis to fatty tissue (40 patients; 54.8%) than in other layers. Moreover, our findings indicated that most of the respondents were identified at a severe level of the wound (55; 75.3%). It is concluded that DFU characteristics vary among patients with DM. Early identification and intensive surveillance are important to improve the management of DFU and to avoid lower limb amputation.

Abstract

Pain is an issue often experienced by cervical cancer patients. Some coping strategies to manage this issue have been conducted by patients. However, its success is influenced by some factors. This study aimed to predict factors influencing coping strategy of pain among cervical cancer patients. The Method is a correlational analytic study had involved cervical cancer patients staged 2 and 3 aged 25 – 65 years old with pain and chemotherapy. The Coping Strategies Questionnaire (CSQ) was used to assess coping strategy describing patient’s thought and feeling associated with pain. Multivariate regression analysis was used to analyse data. The result is Out of 92 respondents, 50% were categorised as late adulthood and 59.8% experience mild pain. Result showed that coping strategy is associated with education, age, marital state, leucocyte and pain scale (p value < 0.05). However, occupation, income, chemotherapy cycle, chemotherapy effects, haemoglobin, thrombocyte, urea and creatinine are not significantly related to coping strategy. Besides, multivariate regression analysis shows that the most influencing factor toward coping strategy is the variable of age. Age is the most influencing factor toward individual’s coping. For that reason, strengthening coping strategy of cervical cancer patients needs to maintain age factor. Keywords: Cervical Cancer; Pain Coping; Coping Strategy

Abstract

Background. Prediabetes is a condition when blood glucose levels are above normal but do not yet meet the criteria for diabetes mellitus. Many people have already been diagnosed with prediabetes. However, there is little data that shows the prevalence and characteristics of people with prediabetes.Objective. To identify the prevalence and demographic characteristics of adults with prediabetes in Semarang, Central Java, Indonesia.Method. A cross-sectional study conducted in 21 Puskesmas (public health centers) in Semarang which were randomly selected from a total of 37 Puskemas. Out of 123 respondents participating in this study, 105 respondents met the inclusion criteria of the study. Oral Glucose Test Tolerance (OGTT) was done after peripherally screening for random glucose levels. Univariate analysis was performed to calculate the frequency and percentage of each variable.Results. The result shows that the prevalence of adult respondents with prediabetes in this study accounted for 17.14%. This study also found that the respondents’ mean age was 38.57 ± 14.84. The majority of the respondents were women (66.7%). They have normal BMI, yet higher waist circumference (23.82 ± 4.48; 83.13 ± 11.34), respectively.Conclusion. Prediabetes is prevalent among adults in Semarang. Thus, to control the incidence of uncontrolled DM, screening needs to be done to identify individuals with prediabetes in order to prevent it from developing into DM. In addition, the application of healthy lifestyle interventions among adults at risk of diabetes also needs to be improved.

Abstract

Latar belakang: Asupan gizi merupakan salah satu faktor yang berperan penting dalam proses penyembuhan luka, khususnya Diabetic Foot Ulcer (DFU). Oleh karena itu, selain merawat kaki, pasien DFU sangat dianjurkan untuk memperhatikan diet dan gizi yang dikonsumsi.Tujuan: Penelitian ini bertujuan untuk mereview peran gizi pada proses penyembuhan DFU.Metode: Metode yang digunakan dalam penelitian ini adalah literature review yaitu mengkaji secara kritis temuan-temuan yang telah dipublikasikan sebelumnya. Artikel dari 3 database, yaitu SCOPUS, PubMed, dan Google Scholar yang sesuai dengan kriteria inklusi kemudian dianalisis untuk menjawab tujuan penelitian. Kriteria inklusi artikel yang disertakan dalan penelitian meliputi artikel yang telah dipublikasikan pada jurnal internasional dengan rentang publikasi dari Januari 2015 sampai Desember 2019, berupa Randomized Controlled Trial (RCT), dan  berbentuk full-text. Enam artikel dianalisis dan disintesis untuk mendapatkan hasil penelitian berdasarkan proses skrining, seleksi, dan pemilihan artikel.Hasil: Literature review ini menunjukkan bahwa gizi, baik dalam bentuk macronutrient (misalnya omega 3) maupun micronutrient (seng, magnesium, vitamin D, vitamin E, dan probiotik), berperan penting dalam proses penyembuhan luka DFU. Macro dan micronutrient ini berperan mulai dari proses modulasi proliferasi sel, metabolisme kolagen, serta sebagai biomarker fase inflamasi dan oksidasi. Asupan gizi yang tepat akan mengontrol dan menekan komplikasi DFU sehingga penyembuhan lukanya berjalan secara optimal. Omega-3, seng, magnesium, vitamin D, vitamin E, dan probiotik berperan besar dalam peningkatan produksi kolagen, sintesis protein, dan pembuangan bakteri serta sel - sel nekrotik.Simpulan: Baik macronutrient maupun micronutrient, keduanya dibutuhkan di dalam tubuh untuk mendukung proses penyembuhan luka DFU. Masing-masing komponen tersebut mempunyai peran penting dalam proses penyembuhan luka.

Abstract

Physical activity plays an important role in controlling blood glucose level and reducing the risk of cardiovascular diseases in patients with diabetes. COVID-19 pandemic causes movement restrictions that can affect physical activity in patients with diabetes. This review aimed to describe the physical activity of patients with diabetes during the COVID-19 pandemic. A scoping review of three journal databases including PubMed, SCOPUS, and ScienceDirect was conducted to locate articles that reported physical activities during the COVID-19 pandemic. Articles are selected according to inclusion and exclusion criteria based on the flowchart of the PRISMA-ScR. There were 11 articles that met the inclusion criteria. The results showed that during the COVID-19 pandemic there was a change in the level, type, and place of physical activity in patients with diabetes. The level of physical activity of patients varies with the majority of research showing a decrease in physical activity levels. While the type and place of physical activity turn into physical activity that can be done at home. This review recommended that facilitation should be implemented to increase the motivation and awareness to perform physical activities.
