Abstract

Cholelithiasis occurs due to the formation of stones in the gallbladder that block the flow of bile, causing right upper abdominal pain, nausea, vomiting, and fever. This case study adds new data that warm compress therapy is effective in significantly reducing pain levels in cholelithiasis patients with complications of acute kidney injury and liver insufficiency aged 59 years. The method used in this study was the Numerical Pain Rating Scale (NPRS) to assess the level of pain, and observation, interview, physical examination, and hospital medical records were used. The NPRS rating scale is a scale of 0 to 10. The number 0 means no pain while the number 10 indicates maximum pain. The application of warm compress therapy was carried out by placing a cloth soaked in 38°C water on the patient's right upper abdominal area, with a duration of implementation of around 15 to 20 minutes for 4 days. The results showed a significant decrease in pain levels after warm compress therapy. Before the intervention, the pain felt was 7/10, and after the intervention, the pain decreased by 4/10 on the fourth day of the intervention. The role of nurses in non-pharmacological pain management, particularly warm compress therapy, includes roles as nursing care providers, educators, and patient safety advocates. However, this study has limitations in the use of cloth as a compressed medium because the cloth is not able to maintain a stable hot temperature during the intervention. As a result, the temperature received by the patient's body may differ from the initial temperature of the compress. Thus, it is recommended that further researchers use special bags, such as hot water bags, for warm compress therapy so that the hot temperature can be maintained more stably and effectively.

Abstract

Background: Hypertension is a leading cause of cardiovascular disease and premature death worldwide. Despite the growing use of multidisciplinary self-care interventions to support patient-centred hypertension management, evidence on their effectiveness in improving quality of life remains fragmented. This scoping review aims to map and synthesise the available evidence on the effectiveness of self-care–based interventions in improving the quality of life of patients with hypertension. Methods: This scoping review followed PRISMA-ScR guidelines, the PCC framework, and the JBI-refined Arksey & O’Malley methodology. Literature searches were conducted in PubMed, ScienceDirect, Scopus, and EBSCOhost (2021–2026). Eligible studies included experimental studies and systematic reviews involving treated hypertensive patients. After independent PRISMA-based screening by four reviewers, 13 articles were included. Data were appraised using JBI tools and synthesised through thematic analysis to classify intervention models by lead professionals, delivery modes, and key self-care components. Results: From 1,328 identified records, the literature spans a broad international scope; however, it remains dominated by studies originating in High-Income Countries (HICs) or those using advanced digital health infrastructures. In contrast, evidence on the specific impact of these interventions on quality of life in Low- and Middle-Income Countries (LMICs), including Indonesia, remains relatively limited. The synthesis results identified three main categories of self-care-based intervention models: (1) professional-led models (facilitated by nurses, pharmacists, physicians, or community health workers and multidisciplinary collaborative care models), (2) digital health delivery modes (including mHealth, tele-nursing, and hybrid models), and (3) structured self-care components (focusing on self-monitoring, medication management, lifestyle modification, and psychosocial support). Although these models have proven effective in significantly lowering systolic and diastolic blood pressure while enhancing patient self-efficacy and quality of life, a research gap remains regarding the lack of direct comparative analysis between models in resource-limited settings and the systematic evaluation of long-term psychosocial outcomes. Conclusion: Nurse-led, multidisciplinary, and technology-supported interventions can improve blood pressure control and quality of life in hypertension. Future research should evaluate their effectiveness and efficiency across different settings, particularly in Indonesia.

Abstract

Background: Coronary artery disease (CAD) remains a major cause of morbidity and mortality worldwide, requiring long-term self-management to prevent recurrent cardiovascular events. Telehealth has emerged as a promising approach to support self-management through remote monitoring, education, lifestyle modification, and patient–provider communication. This scoping review aimed to map and synthesise the available evidence on telehealth-supported self-management interventions and their outcomes among adults with CAD. Methods: This scoping review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews (PRISMA-ScR) guidelines and the Arksey and O’Malley framework refined by the Joanna Briggs Institute (JBI). Guided by the Population–Concept–Context (PCC) framework, literature searches were conducted in PubMed, ScienceDirect, Scopus, and EBSCOhost for studies published between 2016 and 2026. Four reviewers independently screened titles, abstracts, and full texts against predefined eligibility criteria. Data were charted using a structured extraction form, and methodological quality was appraised using JBI critical appraisal tools. Findings were synthesised using descriptive evidence mapping and narrative descriptive analysis. Results: Following the screening and eligibility assessment, 17 studies met the inclusion criteria and were included in the final synthesis. The included studies described a wide range of telehealth-supported self-management interventions, including mobile health applications, web-based platforms, telemonitoring systems, wearable devices, text messaging programs, cardiac telerehabilitation, and artificial intelligence-assisted technologies. Narrative descriptive synthesis showed that interventions commonly incorporated self-management components, including physical activity promotion, medication adherence support, dietary modification, smoking cessation, symptom monitoring, and psychosocial support. Across studies, reported outcomes included improvements in self-efficacy, health literacy, physical activity, medication adherence, quality of life, cardiovascular risk-factor management, and selected clinical indicators. Common intervention characteristics included personalized feedback, remote monitoring, nurse-led support, and multidisciplinary collaboration. The evidence also highlighted gaps related to long-term sustainability, implementation in resource-limited settings, digital equity, and adaptation across diverse healthcare contexts. Conclusion: This scoping review mapped the current evidence on telehealth-supported self-management for adults with CAD and demonstrated substantial diversity in intervention modalities, self-management strategies, and reported outcomes. The findings highlight important knowledge gaps related to implementation, sustainability, and equity, particularly in low-resource settings. Future research should prioritize implementation-focused studies, culturally adaptable telehealth models, and long-term evaluations to support the integration of telehealth into secondary prevention and CAD management

Abstract

Background: Assessing cancer-related patient-reported outcomes is rare in Indonesia. A prior study identified the need for cancer management at the clinic level to overcome low screening, late diagnosis, late treatment, and awareness of cancer. Purpose: This study aimed to describe cancer patients’ self-reports of the severity of symptoms and how it correlates to their characteristics in West Java Province, Indonesia. Methods: A cross-sectional survey of patients with cancer was conducted. As many as 227 patients (response rate 90.8%) were recruited from RSHS Hospital and nearby shelters using purposive sampling technique. Data were collected via respondent’s characteristics questionnaire and the Integrated Palliative Care Outcomes Scale. Data were analysed using bivariate and multivariate chi square tests. Results: A total of 227 questionnaires were received and analysed (response rate 90.8%). The participants were Sundanese (84%), married (81.5%), 36–55 years old (63.9%), housewives (51.5%), and had primary-level education (57.5%).  These patients had been diagnosed between 6 and 12 months (35.7%), with a mean of 1.69 years (SD 2.7), and most patients were without comorbidities (85%). The mean distance to the hospital was 149.3 km (SD=271.7). The most prominent physical symptom experienced was pain (72.2%), while the most prominent emotional concern was family anxiety (45.4%). More than 50% of participants reported severe symptoms. Further statistics show that there is a correlation between performance and pain scale level with severity of patient-reported outcomes (p-value 0,001;α 0,05). Conclusions: The results suggest that patients reported severe outcomes in physical and psychological. Patients with severe physical showed significantly higher pain levels. Systematic assessment of patient-reported outcomes via inexpensive measures can be a key starting point for the management of patients’ symptoms, especially in countries where palliative care services are less organised. Introducing systematic assessment of reported outcomes tools in local practice can help staff perform assessments with a low budget.  Nurse-led interventions can be implemented to provide holistic care for physical and psychological symptoms.
