Abstract

The success of labour is influenced by optimal labour progress during the active stage, characterised by accelerated cervical dilation. The active stage of labour is the longest phase and a critical period determining the success of a normal delivery. If labour progresses more slowly than expected, it can potentially lead to complications for the mother and baby. The purpose of this study was to analyse the effect of palm acupressure on the duration of the active stage of labour. The research method used a true experiment with a two-group pre-test post-test control design. Fifty-eight primigravida subjects giving birth at RSPAL Dr Ramelan in Surabaya were divided into two groups. The intervention group received palm acupressure, while the control group received deep breathing relaxation. Labour duration was monitored using a partograph. The independent t-test showed a p-value of 0.041, indicating a difference in labour duration between the two groups. The average difference in labour duration was 1 hour and 43 minutes. In conclusion, palm acupressure affects the duration of the active stage of labour. This intervention has the potential to prevent prolongation of the first stage and can be used as an alternative to strengthen uterine contractions in cases of labour with contraction disorders

Abstract

Third-trimester primigravida women often experience anxiety due to physical and psychological changes before childbirth. A preliminary survey at Afiyah Clinic, Gedangan, Sidoarjo, found that 30 out of 37 third-trimester primigravida women (81.08 per cent) experienced anxiety. One non-pharmacological intervention that may reduce anxiety is the combination of a Maternity Pillow Dual Wedge and prenatal yoga. To determine the anxiety levels of third-trimester primigravida women before and after the administration of a Maternity Pillow Dual Wedge combined with prenatal yoga at Afiyah Clinic, Gedangan, Sidoarjo. This study employed a quasi-experimental Two-Group Pretest–Posttest Control Design. The sample consisted of 30 respondents, divided into intervention and control groups of 15 each, selected through purposive sampling. Anxiety levels were measured using the PRAQ-R2 questionnaire. Data were analysed using the Paired Sample T-Test and Independent Sample T-Test. The Paired Sample T-Test showed a p-value of 0.000, indicating that the combination of a Maternity Pillow Dual Wedge and prenatal yoga significantly reduced anxiety levels among third-trimester primigravida women. The Independent Sample T-Test yielded a p-value of 0.457, indicating no significant difference between the intervention and control groups. The combination of a Maternity Pillow Dual Wedge and prenatal yoga was effective in reducing anxiety among third-trimester primigravida women and has the potential to be applied as a non-pharmacological intervention in midwifery care.

Abstract

Gangguan tidur merupakan salah satu keluhan yang dapat muncul pada primigravida ketika memasuki trimester III. Kondisi tersebut berkaitan dengan perubahan fisiologis, biomekanik, hormonal, dan psikologis selama kehamilan. Berbagai perubahan ini dapat menurunkan kenyamanan ibu dan pada akhirnya memengaruhi aktivitas sehari-hari. Penelitian ini dilakukan untuk menilai perbedaan efektivitas antara penggunaan Maternity Pillow Dual Wedge yang disertai Prenatal Yoga dan Prenatal Yoga tanpa tambahan bantal terhadap kualitas tidur primigravida trimester III. Penelitian memakai rancangan quasi-experimental dengan pola two-group Pretest-Posttest. Sebanyak 30 primigravida dipilih melalui total sampling dan dibagi menjadi dua kelompok, masing-masing 15 responden. Intervensi berlangsung selama 14 hari dan kualitas tidur diukur menggunakan Pittsburgh Sleep Quality Index (PSQI). Analisis menggunakan Wilcoxon Signed Rank, paired t-test, dan Mann–Whitney. Pada kelompok kombinasi, 13 responden (86,7%) semula berada pada kategori gangguan tidur sedang dan setelah intervensi jumlah yang sama berada pada kategori ringan. Rerata skor PSQI turun dari 11,80 menjadi 5,00 (p = 0,001). Pada kelompok Prenatal Yoga, rerata skor berubah dari 11,00 menjadi 9,26 (p = 0,000). Uji antar kelompok menunjukkan p = 0,001. Dengan demikian, kedua intervensi menunjukkan perubahan kualitas tidur, tetapi perbaikan pada kelompok yang menggunakan kombinasi Maternity Pillow Dual Wedge dan Prenatal Yoga lebih besar. Pendekatan kombinasi tersebut dapat dipertimbangkan sebagai salah satu intervensi nonfarmakologis dalam asuhan kebidanan untuk membantu meningkatkan kualitas tidur primigravida trimester III.
