Abstract

The occurrence of patient accidents is like an iceberg phenomenon. Medical errors remain a global issue in the healthcare sector. The culture of safety aims to create safer patient care, considering that Adverse Events can not only increase healthcare service costs but also bring hospitals into a blaming area. This study aims to describe the patient safety culture at Melati Perbaungan General Hospital. This descriptive study observes the implementation of patient safety culture in preventing adverse events in the inpatient department at Melati Perbaungan General Hospital using a total sampling technique with criteria including human resources in the inpatient department, such as executors, responsible persons, and department heads, totaling 65 people. The measurement of patient safety culture uses the Hospital Survey on Patient Safety Culture (HSPSC) questionnaire with 12 dimensions of patient safety culture. The results show that the majority of respondents' characteristics in terms of age and length of work greatly support the creation of a patient safety culture, as most staff are in their productive age (31-40 years) and have an optimal working period (1-5 years) in performing their duties. The implementation of the patient safety culture by staff at Melati Perbaungan General Hospital is in the Positive Response category at 77.9%. This indicates that the patient safety culture has been well implemented.

Abstract

Rendahnya literasi kegawatdaruratan kesehatan reproduksi pada wanita dewasa di era digital menjadi tantangan serius, terutama dengan maraknya informasi kesehatan yang belum terverifikasi di media sosial. Kegiatan pengabdian ini bertujuan mengevaluasi efektivitas edukasi kegawatdaruratan kesehatan reproduksi dalam meningkatkan pengetahuan wanita dewasa di era digital. Kegiatan menggunakan desain deskriptif kuantitatif dengan pendekatan one-group pretest–posttest, melibatkan 40 wanita dewasa berusia 45–60 tahun yang dipilih secara purposive sampling di Desa Salam Tani, Kecamatan Pancur Batu, Kabupaten Deli Serdang, Sumatera Utara pada tanggal 10 April 2026. Edukasi diberikan melalui ceramah interaktif, diskusi kelompok kecil, dan pemanfaatan media digital dengan materi meliputi konsep kesehatan reproduksi, perubahan reproduksi pada usia dewasa, kegawatdaruratan kesehatan reproduksi, serta cara menyikapi informasi kesehatan secara kritis di era digital. Pengetahuan diukur menggunakan kuesioner terstruktur 15 item dan dianalisis dengan uji Wilcoxon Signed Rank Test menggunakan SPSS versi 26 (α = 0,05). Hasil menunjukkan peningkatan pengetahuan yang signifikan (p = 0,001), dengan responden berpengetahuan tinggi meningkat dari 20% menjadi 50%. Edukasi terbukti efektif meningkatkan pengetahuan dan sikap kritis wanita dewasa.

Abstract

Female genital care is also known as vulva hygiene. Genital care (vulva hygiene) training for women aims to provide knowledge and skills in maintaining genital hygiene to prevent infection, maintain reproductive health, improve physical and psychological comfort, and prevent reproductive health emergencies in women. This study aims to analyze the effect of genital care training on adult women's skills in preventing reproductive health emergencies using a simulation/demonstration method of genital care (vulva hygiene). This study was a quantitative study using a one-group pretest-posttest design with a sample size of 40 adult women using a purposive sampling approach. Data were analyzed using a T-test. The results of this study showed a significant difference in skill levels between the pre- and post-simulation groups (p<0.005). Therefore, it is concluded that genital care (vulva hygiene) training using direct simulation can improve the ability to prevent reproductive health emergencies. Therefore, training using this simulation method is expected to be used for various types of learning activities: cognitive, affective, psychomotor, and interpersonal. Abstract

Diarrhea in children remains a public health problem that can lead to dehydration, impaired growth and development, and even death. One preventive effort against recurrent diarrhea is the implementation of Clean and Healthy Living Behavior (CHLB) within families. This study aims to describe the implementation of family nursing care in applying CHLB to prevent recurrent diarrhea in children. The research method used a case study approach through stages of family nursing care, including assessment, diagnosis, intervention, implementation, and evaluation. Instruments consisted of observation and interviews with families. The results showed an increase in family knowledge and attitudes after being provided with health education on CHLB, including handwashing with soap, consumption of clean water, maintaining food hygiene, and using sanitary latrines. The implementation of CHLB was effective in reducing the risk of recurrent diarrhea in children by promoting healthier family behaviors. The conclusion of this study is that family nursing care through CHLB implementation is effective in enhancing the prevention of recurrent diarrhea in children. It is recommended that CHLB education be developed as a routine program in family health services to reduce the incidence of recurrent diarrhea in children.

Abstract

The abuse of narcotics, psychotropic substances, and addictive substances (NAPZA) is a serious social and health problem in Indonesia. Many victims of drug abuse experience psychological disorders such as anxiety, depression, and loss of motivation to live. In the context of rehabilitation, spiritual counseling is an effective approach to assist patients in their recovery process, both psychologically and spiritually. This study aims to implement spiritual counseling for drug rehabilitation patients at Prof. Dr. M. Ildrem Mental Hospital, Medan. The method used was a case study with a qualitative approach with two drug rehabilitation patients. The counseling process was conducted through a spiritual approach that included strengthening faith, practicing religious practices, and providing emotional support based on religious values. The study results showed positive changes in patients' behavior, mindset, and coping mechanisms after receiving spiritual counseling intervention. This study concludes that the implementation of spiritual counseling can facilitate the recovery process and improve the overall quality of life of drug rehabilitation patients.

Abstract

Stroke is a clinical syndrome characterized by focal or global cerebral dysfunction lasting for 24 hours or more, which may lead to disability or death and is caused by spontaneous bleeding or inadequate blood supply to brain tissue. Motor aphasia is a condition involving damage to the entire cortex in the Broca's area. Motor aphasia manifests as verbal impairment accompanied by symptoms such as difficulty speaking, dysarthria, and visible increased effort when attempting to communicate. The AIUEO therapy is a treatment designed to improve speech so that it can be better understood by others by moving the tongue, lips, facial muscles, and pronouncing the sounds "AIUEO". The "AIUEO" therapy represents a standard pattern of phonetic symbols; thus, when pronouncing "AIUEO", the tongue, lips, and facial muscles move and facilitate speech recovery. The design of this study was descriptive with a case study approach involving two clients diagnosed with the same condition. Nursing interventions were provided by the researcher for three days for both cases with daily evaluations. Nursing actions performed over three days in cases 1 and 2 showed similar outcomes. In case 1, involving aphasia, Mrs. A's initial communication ability improved from a score of 9 to 15, while Mrs. D's score improved from 12 to 16. This speech therapy is administered to individuals experiencing communication disorders. Conclusion and suggestion: the implementation of nursing interventions over three days in both cases demonstrated consistent evaluation results. Clients are advised to perform the therapy regularly in order to achieve optimal outcomes.
