Abstract

Background: Fatigue can reduce working performance, worker motivation, and work produc­tivity. Fatigue due to exposure to heat pressure which causes excessive sweating must be overcome through the provision of fluids that are right for workers. Coconut water contains electrolytes, so it is very good to replace lost body fluids during activities throughout the day. The purpose of this study is to analyze differences in work fatigue with interventions giving 4 liters of drinking water and giving 1 liter of young coconut water (Cocos nucifera L.) accompanied by giving 3 liters of drinking water.Subjects and Method: This study is a quasi-experimental design with a research design of one group pre and post-test design. The study population was the production of cassava chips factory workers with a total sample of 20 people. The sampling technique is purposive sampling. The dependent variable is work fatigue while the independent variable is giving 4 liters of drinking water and giving 1 liter of young coconut water (Cocosnucifera L) accompanied by giving 3 liters of drinking water. The type of data collected is primary data in the form of work fatigue (in milliseconds) measured using the Reaction Timer. Bivariate analysis was conducted using the paired T-test.Results: Fatigue after giving 1 liter of young coconut water and 3 liters of drinking water was lower (mean = 118.00; p = 0.001) than the group given 4 liters of drinking water (mean 164.94; p= 0.001).Conclusion: Work fatigue in the group given 1 liter of young coconut water and 3 liters of drinking water was lower than the group given 4 liters of drinking water.Keywords: fatigue, young coconut water, drinking water, heat pressureCorrespondence: Bona Rina Ria Rajagukguk. Abstract

Background: Periodontal diseases are prevalent both in developed and developing countries and affect about 20-50% of global population. Several risk factors such as smoking, poor oral hygiene, diabetes, medication, age, hereditary, and stress are related to periodontal diseases. The purpose of this study was to determine the effect of smoking on the risk of periodontal disease, using path analysis.Subjects and Method: A case-control study was carried out in Tasikmalaya community health center, West Java, from October to November 2018. A sample of 200 patients was selected by fixed disease sampling. The dependent variable was periodontal disease. The independent variables were age, knowledge on oral health, education, income, smoking behavior, oral and dental health behavior, plaque, and calculus. The data were obtained from medical record and questionnaire. The data were analyzed by path analysis.Results: The risk of periodontal disease directly increased by smoking behavior (b= 0.92; 95% CI= 0.09 to 1.75; p= 0.030), after controlling for the effect of calculus (b= 1.23; 95% CI= 0.40 to 2.07; p= 0.004) and age (b=1.63; 95% CI= 0.76 to 2.50; p<0.001). The risk of periodontal disease directly decreased by better knowledge on oral and dental health (b=-0.92; 95% CI= -1.72 to -0.12; p= 0.023) and high income (b= -1.47; 95% CI= -2.32 to -0.60; p<0.001). It was indirectly affected by plaque and education.Conclusion: The risk of periodontal disease directly increases by smoking behavior, after controlling for the effect of calculus and age. In addition, the risk of periodontal disease directly decreases by better knowledge on oral and dental health and high income. Abstract

Background: Percutaneous coronary intervention (PCI) is one of the most effective therapies in patients with coronary artery disease (CAD) after the attack. Cardiac rehabilitation (CR) makes an important contribution to secondary prevention of cardiac events. The World Health Organization defines CR as the sum of activities required to influence favorably the underlying cause of the disease, as well as to ensure the best possible physical, mental and social conditions, so that they may, by their own efforts, preserve or resume when lost as normal a place as possible in the life of the community. This study aimed to estimate the effects of CR on NT-proBNP and METs among patients with CAD after PCI.Subjects and Method: This was a cohort study carried out at Dr. Moewardi Hospital, Surakarta, Central Java, from November to February 2018. A sample of 18 patients with CAD after PCI was selected for this study. The dependent variables were NT-proBNP, METs, and quality of life. The independent variable was CR. The three outcome variables were measured before and after CR. The data before and after CR were compared and tested by t-test.Results: NT-proBNP level in CAD patients after PCI (Mean= 97.17 ng/ml) was lower than before PCI (Mean= 1,629.4 ng/ml), and it was statistically significant. METS value in CAD patients after PCI (Mean= 8.31 ml/kg) was higher than before PCI (Mean= 6.41 ml/kg), and it was statistically significant. The quality of life also increased significantly.Conclusion: Cardiac rehabilitation is effective to lower NT-proBNP level, as well as to increase METS value and quality of life of CAD patients.Keywords: coronary artery disease, cardiac rehabilitation, NT-proBNP, METs, quality of lifeCorrespondence: Noer Rachma. Abstract

Background: The pathogenesis of heart failure after myocardial infarction is associated with the pathogenesis of cardiac remodeling. Galectin-3 (Gal-3) has a role in the pathophysiology of cardiac remodeling after acute myocardial infarction. N-Acetylcysteine (NAC) can prevent inflammation, remodeling and left ventricular dysfunction, interstitial fibrosis, and improve survival. The purpose of this study was to examine the effect of oral N-Acetylcysteine on Galectin-3 in acute myocardial infarction patients.Subjects and Methods: This was an experimental study with pre and post, single-blind, and randomization methods. The study was conducted at Dr.Moewardi General Hospital Surakarta, Central Java, from June to August 2018. A sample of 29 acute myocardial infarction patients with ST-segment elevation who received fibrinolytic therapy was selected for this study. 14 patients as a control group received standard therapy and 15 patients as intervention group received oral NAC supplementary therapy 600 mg three times daily for three days. The dependent variable was Gal-3 levels. The independent variable was NAC supplementary therapy. The data were analyzed by Mann Whitney test.Results : Gal-3 levels in intervention group (mean= 8.95; SD=1.76) were lowered than the control group (mean= 11.42; SD= 3.76) and it was statistically significant (p= 0.026). Conclusion: Supplementary therapy of NAC 600 mg orally 3 times a day for 3 days can reduce levels of Gal-3 in patients with acute myocardial infarction who receive fibrinolytic therapy.Keywords: galectin-3, n-acetylcysteine, acute myocardial infarction Correspondence: Akhmad Jalaludinsyah. Abstract

Background: The prevalence of liver cirrhosis (SH) worldwide is at seventh place as the cause of death. Increased pressure in the portal vein Portal Hypertension (HP) causes esopha­geal varices and the more likely the patient to bleed. Endothelial dysfunction in cirrhosis of the liver causes a decrease in nitric oxide (NO) levels resulting in an imbalance between relaxation factors and portal vein contractions and increasing hs-CRP levels. Simvastatin can increase NO levels and improve endothelial dysfunction in patients with SH and can reduce levels of hs-CRP. This study aimed to determine the effect of simvastatin administration on nitric oxide and hs-CRP level in patients with liver cirrhosis.Subjects and Method: This was a randomized controlled trial conducted at Dr. Moewardi hospital, Surakarta, Central Java. A sample of 29 patients was divided into treatment and control groups. 14 patients in the treatment group were given simvastatin dose of 20 mg. 15 patients in the control group received a placebo. The dependent variable was a nitric oxide and hs-CRP levels. The inde­pendent variable was Simvastatin administration. Nitric oxide serum level was measured by an enzyme-linked immunosorbent assay (ELISA). Hs-CRP level was measured by the immune­chemilu­minescent method. The data were analyzed by t-test.Results: NO level after Simvastatin 20 mg administration (Mean= 7.93 µg/mL; SD= 1.78 µg/ mL) was higher than before (Mean= 5.14 µg/mL; SD= 2.25 µg/mL) and it was statistically significant (p = 0.001). Hs-CRP level after Simvastatin 20 mg administration (Mean= 2.04 mg/L; SD= 0.80 mg/L) was lowered than before (Mean= 7.82 mg/L; SD= 1.15 mg/L) and it was statistically significant (p= 0.001).Conclusion: Simvastatin administration increases NO serum levels and reduces the hs-CRP level in patients with liver cirrhosis.Keywords: nitric oxide, hs-CRP, simvastatin, liver cirrhosisCorrespondence: Budhi Hariyanto. Abstract

Background: Problems that inhibit breastfeeding in the first week include less breast milk production. Increasing breast milk production can be done by stimulating or massage the breast. Breast massage can be done by lactapuncture massage. This study aimed to examine the effect of lactapuncture massage on prolactin hormone levels and breast milk production among primiparous mothers.Subjects and Method: This was a quasy experiment study conducted at Dr. M. Ashari hospital, Pemalang, Central Java, Indonesia. A sample of 32 primiparous postpartum mothers was selected by simple random sampling. The dependent variable was breast milk production. The independent variables were lactapuncture massage for 7 consecutive days. The data were analyzed by t-test.Results: Prolactin hormone level in intervention group (mean= 313.80; SD= 45.54) was higher than in control group (mean= 292.09; SD= 97.87) but it was statistically non-significant (p= 0.428). Breast milk production in intervention group was higher (mean= 9.36; SD= 0.71) than in control group (mean= 7.39; SD= 0.23) and it was statistically significant (p<0.001).Conclusion: The lactapuncture massage on 2-3 times clockwise at each point for 7 consecutive days increasing prolactin hormone levels and on breast milk production.Keywords: lactapuncture massage, prolactin hormone, breast milk productionCorrespondence: Siti Patimah. Abstract

Background: Maintaining body mass index is one of the social problems, especially for young athletes. This study aimed to determine the relationship of body mass index (BMI) and age to muscular endurance in soccer athletes in Medan.Subjects and Method: This was a cross-sectional study soccer clubs in Medan, North Sumatera, Indonesia, in April 2018. A sample of 96 soccer players was selected for this study. The dependent variable was muscular endurance. The independent variables were BMI and age. Body weight was measured by weight scale. Body height was measured by microtoise. The other data were collected by questionnaire. Muscle endurance was assessed by 1 minute push up test. The data were analyzed by Person product moment.Results: BMI was positively correlated with muscle endurance but it was not statistically significant (r = 0.09; p = 0.390). Age was positively correlated with muscle endurance and it was statistically significant (r= 0.62; p= 0.001).Conclusion: BMI and age are positively correlated with muscle endurance among soccer players.Keywords: BMI, muscle endurance, soccer athleteCorrespondence: Dewi Maya Sarah. Abstract

Background: Giant cell tumor (GCT) of bone arising from aphalanx of finger is extremely rare. Although, the occurrence of GCT in the hands is a rare, it has been reported that GCT of the small bones (small-bone GCT) carries a higher risk of local recurrence and metastasis than conventional GCT.Subjects and Method: We report two cases of GCT arising from a phalanx of a hand finger. First patient was treated with wide excision of the tumor and fusion with fibular bone graft with the help of a K-wire. Second patient was treated by excision of tumour (Curretage) with allograft bonegraft for bone defect.Results: At their most recent follow-ups (4 months, respectively), both were recurrence free and had returned to their previous occupational and recreational activities.Conclusion: The effectiveness of these methods in the treatment and prevention of recurrence is still under discussion and there is no gold standard for treating GCT.Keywords: GCT, phalanx of finger, excision, illiac crest bonegraft, allograftCorrespondence: Pamudji Utomo. Abstract

Background: Uterine cervical carcinoma is the most gynecological malignancy in women worldwide. The most common type of cervical carcinoma histology is squamous cell carcinoma and has a better prognosis than adenocarcinoma. Neoadjuvant chemotherapy is the management of early-stage cervical carcinoma before radical hysterectomy is performed. In addition to clinical-pathological factors, biomolecular markers are now being developed and used as parameters for evaluating chemotherapy responses. Vascular Endothelial Growth Factor-C is a VEGF derivative that plays a role in the process of vasculogenesis, limphangiogenesis, and the formation of new lymphatic channels which play an important role in metastasis. This study aimed to examine the effect of Paxus-Carboplatin chemotherapy in decreasing VEGF-C in squamous cell cervical carcinoma tissue.Subjects and Method: This was a quasi experiment study conducted at Department of Obstetrics and Gynecology and Pathology Anatomy Laboratory of Dr. Moewardi Hospital/ Faculty of Medicine, Universitas Sebelas Maret. 25 tissue samples of squamous cell stage IB2-IIA2 cervical carcinoma immunohistochemistry VEGF-C expression was carried out before and after Paxus-Carboplatin chemotherapy was given. The data were analyzed by Wilcoxon test.Results: VEGF-C expression in squamous cell cervical carcinoma tissue after chemotherapy was lower (mean = 4.20) than before chemotherapy (mean = 6.16) and it was statistically significant (p<0.001).Conclusion: Neoadjuvant chemotherapy decreases VEGF-C in squamous cell cervical carcinoma tissue.Correspondence: Uswatun Khasanah Kartikasari. Abstract

Background: Distal femur fractures must always be considered for vascular disorders given the high risk. The incidence rate of femoral fractures is 1/10000 patients per year with the incidence rate of vascular injury around 0.10% -2.00%. Salter-Harris (SH) fracture classification is used in cases of fractures involving the growth plate (physeal plate) in children. Volkmann's ischemic contracture is caused by severe injury to the deep tissue and nerves. If tissue pressure is equal to or more than the perfusion pressure of the capillary, the microcirculation will be disrupted and ischemic occur. Normal myosit cells will be replaced by fibroblasts that tend to contract.Subjects and Method: A 10 years old boy had a traffic accident while riding a motorcycle 6 days before the patient came to the hospital. The patient could not walk, feeling pain and swelling on the left knee, the patient didn’t bring to the hospital immediately after the accident. On physical examination, there was angulation in the distal left femur, there was tenderness on palpation and unable to do dorsiflexion or moving the fingers of the left foot but still could feel touch stimu­la­tion on the pedis.Results: In our patient case with the diagnosis of left femoral artery occlusion et causa closed fracture distal femur (Salter-Harris type II) managed by surgery to release the femoral artery with internal fixation to the distal femur using Kirschner wire (minimally invasive) and massage to the femoral artery give optimal results.Conclusion: Surgery to release the femoral artery with internal fixation to the distal femur using Kirschner wire (minimally invasive) and massage to the femoral artery give an optimal result.Keywords: Salter-Harris fracture, femoral artery, Kirschner wire, Volkmann's ischaemic contractureCorrespondence: Pamudji Utomo.
