Abstract

Background: Broken bones can cause soft tissue edema, bleeding in the muscles and joints, tendon rupture, damage to nerve tissue and damage to the vasculer and surgery are ways that are carried out to restore the movement function of extremitas and the impact that occurs due to these actions is pain. Acute pain is still found in postoperative non-pathological fracture patients who have received analgesic ketorolac.Subjects and Method: This was a quasy experiment study conducted at Dr. Soetomo Hospital, Surabaya, East Java. A sample of 46 patients post orthopedic surgery was selected for study by purposive sampling. The dependent variable was pain. The independent variables were range of motion and deep breathing exercises. Pain was measured by visual analog scale (VAS). The data were analyzed by t-test.Results: After treatment, pain level in the inter­vention group (mean = 2.43; SD = 1.41) was lower than the control group (mean= 3.48; SD= 1.38) with p= 0.014.Discussion: Combination of range of motion and deep breathing relaxation therapy effective to reduce pain among post orthopedic surgery patients.Correspondence: Hery Sasongko. Abstract

Background: Every five years, the American Heart Association (AHA) releases new guidelines on CPR. Previous guidelines have focused on Airway-Breathing-Circulation (ABC) advocacy. However, in 2010 guidelines for CPR recommended changes in the sequence of BLS steps to CAB (chest compression, airway, and breathing) for adults, children and infants (excluding newborns). The purpose of this review is to explain the reasons and provide scientific evidence about the results of CAB actions compared to ABC.Subjects and Method: Systematic reviews were done by searching the database through PubMed, Google Scholar, and Science Direct. Key words for this review include: AHA Guideline AND CPR 2010, CAB in CPR, CAB, and RCT (Randomized Controlled Trial) guidelines and why does ABC turn into CAB? The inclusion criteria are systematic reviews, clinical reviews and guidelines.Results: The results are in the form of 3 review articles and 1 RCT study. The basic reasons for changing ABC to CAB are (1) The most common case of cardiac arrest in adults, and the initial element of critical BLS (basic life support) is chest compression and early defibrillation by changing sequentially to CAB, chest compression will start faster; (2) Most victims of cardiac arrest outside the hospital do not get CPR observers, this is an obstacle to opening the airway and exhaling; and (3) Chest compression provides vital blood flow to the heart and brain. One RCT study showed that the time to do the first resuscitation with the CAB technique (mean = 25; SD = 10) was faster than the ABC technique (mean = 32; SD = 12) and statistically significant (p = 0.002).Conclusion: Chest compression is the most important aspect of heart attack management because airway maneuver takes a long time. Abstract

Background: Quadriceps tendon ruptures are uncommon injury that affects middle-aged women, predominantly it affects middle aged men. However, the bilateral quadriceps tendon rupture is extremely rare in the absence of systemic disease.Subjects and Method: We reported a case of bilateral quadriceps tendon rupture in a healthy 65 year old female. She felt pain in both knees since 4 months before entering the hospital. Local physical examination on both knees reveal the loss of normal contour of rectus femoris, tenderness at superior pole patella the ruptur site, and palpable defect within 2 cm of superior pole patella. Patient unable to extend knee against resistance, and unable to perform straight leg raise. The patient was carried out a surgical repair with consent.Results: After the surgery both knees were immobilized with knee extension splint for 6 weeks. Patient evaluation revealed no pain and progressive recovery of muscular strength after four months and could be able to walk and returned to normal activity.Conclusion: Surgery is needed to repair quadriceps rupture.Keywords: Quadriceps tendon, quadriceps rupture, repair tendon.Correspondence: Pamudji Utomo. Abstract

Background: Arterial blood pressure is an easily get variable, including systolic blood pressure, diastolic blood pressure, mean arterial pressure (MAP) and pulse pressure (PP). Although the variable blood pressure has clinical importance in many cardiovascular diseases, the variable that has become the best predictor in clinical practice cannot be determined. The purpose of this study is to determine the prognostic value of PP and MAP at the start of hospitalization in patients with acute myocardial infarction (IMA).Subjects and Method: This was a retrospective cohort study conducted at cardiovascular intensive care unit, Dr. Moewardi Hospital, Surakarta, Central Java. A sample of 150 IMA patients from July 2013 to December 2013 was selected for this study. Blood pressure was measured the first time the patient arrives at the emergency unit to determine the value of PP and MAP. Then the patient was observed by looking at mortality during hospitalization as a final result. Basic characteristics data were analyzed using logistic regression and displayed in quartile form. The Kaplan-Meier curve was used to see mortality in each quartile. Cox proportional regression analysis was used to see the magnitude of the risk of variable PP, MAP and other variables on mortality.Results: The highest mortality was found in the first quartile group of PP (OR= 1.81; p<0.001) and MAP (OR= 1.69; p<0.001). The Kaplan-Meier curve showed the first quartile of the two groups had the lowest survival, while the third highest quartile (p <0.001). The results of the cox analysis showed a decrease in mortality risk of 0.49 per increase in PP by 10 mmHg (95% CI= 0.07 to 1.00; p= 0.044) and 0.31 each increase in MAP by 10 mmHg (95% CI= 0.09 to 0.53; p= 0.003).Conclusions: Blood pressure measurement can produce two variables as predictors of mortality in IMA patients, namely PP and MAP. Abstract

Background: Various bacteria have been identified as the agents causing bloodstream infection. Multidrug resistant (MDR) strains, such as Gram-negative pathogens producing extended-spectrum beta-lactamases (ESBL) and Methicillin-resistant S.aureus (MRSA) are potential bacterial agents causing bloodstream infection in the hospital setting. The increase of these MDR strains in bloodstream infection is alarming since there are limited antibiotic choices for treatment. This study aimed to investigate the agents causing bloodstream infection and to investigate their antibiotics susceptibility profiles. Subjects and Method: Samples were venous blood collected from patients admitted to hospital from January 2014 to December 2016. Bacteria were grown in-automated BACTEC® blood culture (BD, New Jersey, USA). The identification of MRSA strain was conducted using oxacillin susceptibility test. The bacterial identification and identification of ESBL-producing bacteria was conducted using colorimetric assay using Vitek2®system (bioMérieux).Results: The antibiotic profiles of all bacteria isolated from venous blood samples were analysed. The most bacteria isolated from the blood stream were K. pneumonia, A. baumannii, E. coli, P. Aeruginosa, E. cloacae, S. marcennes, S. haemolyticus, S. hominis, S.epidermidis, and S. aureus. Conclusions: The resistant profiles of bacteria isolated from blood stream showed that there was a high incidence of MDR bacteria, including potential ESBL-producing bacteria, carbapenemase-producing bacteria and MRSA strains.Keywords: Bloodstream Infection, Staphylococcus aureus, Extended Spectrum Betalactamase, Antibiotic Resistance.Correspondence: Betty Suryawati. Abstract

Background: Melorheostosis is a rare sclerosing bone dysplasia characterized by its classic radiographic feature of flowing/dripping candle wax. It usually affects one limb, more often the lower extremity, and rarely the axial skeleton. The etiology remains unknown and various theories have been proposed to explain the pathogenesis of this disease. It often has an insidious onset in early adult life. Symptoms include pain, oedema, and limitation of joint movement.Case presentation: A patient aged 25 year old male complaint of pain on the left lower extremity for the last 3 years. There is muscle atrophy at left extremity, bowing of the thigh, awelling at the foot. There is palpable mass at the left bottom, solid consistency and fixed. There are also limitation ROM of hip, knee and ankle. From the radiography examination showed inhomogen thickening of the periosteal cortical and endosteal, dripping candle wax appearance, and sclerotic lesion at soft tissue.Conculsion: X-ray is a sufficient method for diagnosis of melorheostosis.Other imaging techniques are essential for decision about therapeutic intervention (CT or MRI). Laboratory findings are usually in physiological range. Abstract

Background: Gout is an antioxidant, the final product that is produced from the metabolism/ breakdown of purines, but if in excessive amounts in the blood, it will experience crystallization and can cause gout. Giving cinnamon extract to male white rats with hyperuricemia has the effect of reducing uric acid levels, because the extract of cinnamon contains Polyphenols namely Sinamldehid which is high enough (68.65%) to be a source of antioxidant compounds which can inhibit Xanthine Oxidase Enzine. This study aimed to analyze the therapy of dekokta cinnamon with honey to reduce uric acid for people with gout.Subjects and Method: This was a quasi experiment with pre and post-test design. The study was conducted in Gambirsari Community Health Center, Kadipiro Subdistrict, Mojosongo, Surakarta, Central Java, in May 2018. A sample of of 50 people with gout sufferers were selected by quota sampling and divided into 2 groups. The treatment group of 25 people received cinnamon decoction with honey with a concentration of 15%. The therapy was carried out for 1 week with a dose of once a day. In the control group, Allopurinol 100 mg was given for 1 week, with a dose of once a day. The data were analyzed by Wilcoxon and Mann-Whitney est.Results: Uric acid in decoction cinnamon with honey group was lower than in control group and it was statistically significant (p= 0.023).Conclusion: Decoction cinnamon with honey can reduce uric acid level among gout patients.Keywords: cinnamon, decoction, honey, goutCorrespondence: Yeti Nurhayati. Abstract

Background: Long-standing knee osteoarthritis also is a common cause of severe bone deficiency (especially proximal tibia). Total knee arthroplasty is indicated in patient with knee osteoarthritis who did not able to perform activity daily living. This study aimed to evaluate the functional knee score of the patient with knee osteoarthritis who performed total knee arthroplasty with screw augmentation.Subjects and Method: This was an analytic observational study conducted at Soeharso Orthopaedic Hospital, Surakarta, Central Java, from January to December 2016. A sample of 30 patients with knee osteoarthritis with tibial defect who performed total knee arthroplasty with screw augmentation was selected for this study. The dependent variable was functional knee score. The independent variable was knee arthroplasty with screw augmentation. Knee function was measured by knee society score. The difference of knee function score between before and after total knee arthroplasty with screw augmentation was analyzed by t test.Results: There were 30 patients (25 females and 5 males) with age between 54 to 73 years old. Abstract

Background: Baby massage is the oldest and most popular touch therapy known to mankind, which is also the art of health care and treatment that has been practiced for centuries. This study aims to determine the effect of infant massage on bonding attachments between mother and baby and the quality of infant sleep.Subjects and Method: This was experimental study with Randomized Controlled Trial (RCT) design conducted in five health centers in Ponorogo Regency, East Java in March to April 2019. The total sample was 120 mothers and infants aged 3 to 6 months divided between massage groups (treatment) and not massage (control) based on The dependent variable of this study was bonding attachment and sleep quality. The independent variable in this study was infant massage. Data collection was using questionnaires and checklists. Abstract

Background: Stroke risk factors disparities in race-ethnic. Bataknese is the most stroke rates compared to non-Bataknese at H. Adam Malik General Hospital Medan. This may be due to genetics and lifestyles that have a link to increase stroke risk factors in Bataknese. Bataknese has a characteristic lifestyle that are more eating, typical foods also contain lots of cholesterol, and have a habit of drinking traditional alcoholic beverages.Subjects and Method: This was a cross-sectional study selected by consecutive sampling tech­niques, clinically proven stroke patients and computed tomography (CT) scan studied at H. Adam Malik General Hospital Medan from June to November 2018. This study aimed to know the difference of stroke risk factor between stroke patient of Bataknese and non-Bataknese.Results: This study recruited 70 samples consisting of 45 Bataknese and 25 non-Bataknese. There were no significant differences in risk factors for hypertension, dyslipidemia, heart disease, dia­betes mellitus, smoking, and obesity among stroke patients in the Bataknese and non-Bataknese. But the distribution of hypertension was higher in Bataknese (50%) compared non-Bataknese (30%) (PR=0.67, 95%CI= 0.19 to 2.40, p= o.755). Dislipidemia was higher in Bataknese (38.6%) compared non-Bataknese (18.6%) (PR=1.39, 95% CI= 0.52 to 3.71, p=0.692). Heart disease was higher in Bataknese (17.1%) compared non-Bataknese (8.6%) (PR=1.15, 95% CI= 0.37 to 3.57, p=1.000).  Diabetes mellitus was higher in Bataknese (17.1%) compared non-Bataknese (10%) (PR=1.10, 95% CI= 0.31 to 2.87, p=1.000). Smoke was higher in Bataknese (27.1%) compared non-Bataknese (14.3%)(PR= 1.59, 95% CI= 0.38 to 6.61, p=1.000). Obesity was higher in Bataknese (11.4%) compared non-Bataknese (4.3%) (PR=1.59, 95% CI= 0.38 to 6.61, p=0.735). There was significant differences in risk factors for alcohol consumption between stroke patients in the Bataknese and non-Bataknese, where alcohol consumption was higher in the Bataknese (15.7%) compared non-Bataknese (1.4%) (PR=7.77, 95% CI= 0.94 to 64.22, p=0.045).Conclusion: A significant difference in stroke risk factors between stroke patients in Bataknese and non-Bataknese was alcoholic consumption.Keywords: stroke, risk factor, Bataknese, Non-BatakneseCorespondence: Laura Panca Susila Tambunan.
