Abstract

Background: Based on UNAIDS data from 2019, Southeast Asia has 3.8 million of the world's HIV-positive people. In Asia, Indonesia has the fifth-highest risk of HIV/AIDS infection. According to the Madiun City Population Control and Family Planning Health Service, there will be 148 more HIV/AIDS cases in 2022, indicating a continued increase in cases. Analyzing the health issues with the HIV/AIDS Program at the Madiun City Population Control and Family Planning Health Service is the goal of this study. Method: This research is a descriptive study conducted during July–August 2023. Determining problem priorities using the Hanlon method and identifying the root causes of the problem. Primary data collection was carried out using the interview method, or in-depth interviews. Results: The results of determining problem priorities show that PLWHA who are new to ARV have not met the target of 95% and are based on the number of cases of PLWHA who are new to ARV from 2021, namely 29 cases and in 2022 there has been an increase of 43 cases. Identification of the root cause of the problem was carried out through interviews or in-depth interviews, that there were several factors that caused the occurrence of new PLWHA on ARVs, namely the low awareness of PLWHA to start treatment, the attitude of PLWHA who did not accept their HIV status so they did not want to start treatment and the lack of support from family/peer friends/ This is due to the lack of knowledge and information related to HIV/AIDS. Conclusion: This requires strengthening the mentoring program for PLWHA by providing assistance not only to PLWHA who have started treatment but all those who have been found to be immediately accompanied by NGOs, as well as involving local community leaders to promote anti-stigma.

Abstract

Firefighters are particularly vulnerable to several work-related illnesses and injuries, including toxic gas poisoning, burns, respiratory distress, and bodily injury. Therefore, an important first step in implementing successful prevention tactics is to identify possible K3 hazards precisely and accurately. To prevent industrial accidents that endanger workers due to hazards arising in the Medan City Fire and Rescue Department, the purpose of this study is to identify hazards and evaluate risks using the HIRA method analysis. The research was conducted using qualitative methodology. Three data collection methods were used: documentation, in-depth interviews, and observation. The informants in this study amounted to 4 people consisting of 2 key informants and 2 main informants. Based on the results of hazard identification and risk assessment, there are 19 accident risks spread across 5 activities in the Medan City Fire and Rescue Service. Based on the results of the study, it was concluded that 10 potential hazards fall into the meaningful category, 4 potential hazards fall into the medium-risk category, 4 potential hazards fall into the low-risk category, and high-risk categories as many as 1 potential hazard. Based on these conclusions, suggestions were submitted to the Medan City Fire and Rescue Service to increase the provision of personal protective equipment (PPE) such as gloves, helmets, heat-resistant shirts, and pants, and SCBA was an anticipation of the proposal submitted to the Medan City Fire and Rescue Service. Medan City Fire and Rescue Officers are also required to always comply with established work rules to reduce risks and avoid work accidents.

Abstract

Tuberculosis (TB) is an infectious disease caused by the bacterium Mycobacterium tuberculosis and is one of the unresolved health problems in Lamongan District. The purpose of this study was to analyze the problems in the Child Tuberculosis Prevention and Control Program in Lamongan Regency. This study was a descriptive observational study conducted at the Lamongan District Health Office from January 02 to January 27, 2023. Problem identification was conducted using brainstorming techniques and then prioritizing the problems using the CARL method (Capability, Accessability, Readiness, and Leverage). Furthermore, identification of the causes of problems that have been prioritized is carried out with a fish bone diagram using a process approach. Based on the root causes of the problems that have been identified, recommendations are given to the health department to overcome the problems found. Suboptimal cross-program/sector coordination with a CARL value of 505.440 became the top priority health problem in the child tuberculosis control program. Therefore, it is necessary to optimize the discovery of child TB suspects and education through integration with other activities, improving the competence of field officers, ensuring budget availability, data management, good recording and reporting.

Abstract

Dementia is a condition characterized by significant impairment in at least one domain of cognition, including executive function, complex attention, language, learning, memory, perceptual-motor, or social cognition. Risk factors for vascular dementia include smoking, hyperlipidemia, hypertension, diabetes, atrial fibrillation, obesity, and lack of exercise. This study aims to examine the relationship between metabolic disorders and dementia in the elderly. The design of this research was cross-sectional at the Hana Nursing Home in January 2024, with inclusion criteria in the form of an elderly group of at least 60 years. The independent variables in this study were blood pressure, hemoglobin, hematocrit, body mass index, cholesterol, uric acid, and random blood sugar. The dependent variable in this study includes the Mini-Mental State Examination (MMSE) score. This study included 61 respondents who met the inclusion criteria. The results of the multiple linear regression statistical test (Backward LR) with 6 stages stated that body mass index (p-value: 0.001; B: 0.281) and hematocrit (p-value: 0.011; B: 0.528) played a significant role in decreasing the value MMSE. The conclusion of this study is that body mass index, and hematocrit are related to dementia. This research opens up insight into the importance of controlling body weight and body composition, especially in the elderly group to prevent decline in cognitive function in old age.

Abstract

One of the tasks of humans in the industrial world is moving and transporting materials at great risk of triggering Musculoskeletal Disorders (MSDs). One production area with the potential for occupational disease hazards is the Assembly and Decoration area, which is a place for mixing colors. This area has a fairly high potential for occupational diseases in the form of MSDs because everyday workers always use manual product handling or what is known as lifting for 8 hours, hence workers often experience complaints of MSDs on the back, waist, shoulders, and arms. This research is a descriptive quantitative research type with analytical observational methods. This research is cross-sectional. The population in this study was 30 workers in the assembly and decoration area of PT. X. Measurement of MSDs among workers in the Assembly and Decoration areas using the Nordic Body Map (NBM) questionnaire and the RULA method. The results of measurements using the Nordic Body Map (NBM) questionnaire show that workers in the Assembly and Decoration sections have the potential to experience MSDs problems. Apart from that, based on the results of RULA measurements on Assembly and Decoration workers, a total score of 5 was obtained. So it can be concluded that this work is in the medium category and action is needed soon.

Abstract

The proportion of MDR TB in Jayapura Regency in 2022 is 2.32% and the figure is higher than MDR TB in Papua Province, which is 151 cases. Treatment of MDR TB is more complex than drug-sensitive TB The purpose of this study was to determine the factors associated with the incidence of multidrug resistant tuberculosis (MDR TB) in patients with Pulmonary TB in the Health Office of Jayapura District, Papua Province. The type of case control study on the population of tuberculosis patients in the Jayapura Regency Health Office was 1,162 patients and samples were 27 cases and 54 controls. Data were obtained using medical record data and analyzed using chi square and logistic binary regression.  The results showed that factors that were significantly associated with the incidence of MDR TB in patients with Pulmonary TB were side effects (p-value 0.000; OR = 9.1; CI95% (3.07 – 26.93), medication adherence (p-value 0.000; OR = 136; CI95% (25.45-724.06), duration of treatment (p-value 0.000; OR = 7.273; CI95% (2,546-20,773), treatment history in patients with Pulmonary TB in the Department (p-value 0.000; OR = 4.2; CI95% (8.801-21.991) and history of comorbidities (p-value 0.000; OR = 19.6; CI95% (5.79-66.34). Adherence to medication was the dominant factor in the incidence of MDR TB (p-value 0.000; OR = 36; CI95% (25,545 – 724,068). Conclusion: Adherence to taking medication is the dominant risk factor for MDR so it is recommended that in addition to PMO from Puskesmas need assistance from family. Studies have shown that TB patients who adhere to treatment are more likely to recover quickly and are no longer susceptible to other diseases than patients who do not adhere to treatment.

Abstract

Health human resources (SDMK) are indispensable in improving the degree of public health in a country. The determination of human resource needs is carried out using the Anjab & ABK needs plan (Position Analysis and Workload Analysis). However, in its implementation, several obstacles hinder the effectiveness of the Anjab & ABK needs assessment at the Medan City Health Office. The purpose of the study was to analyze SWOT on the RENBUT ANJAB & ABK Barriers in the Medan city health office. The type of research used in this study is descriptive research using a qualitative approach. This research uses a case study design. Data collection techniques in this study were carried out by observation and in-depth interviews. The subjects of the study were selected purposively. The key informants in this study were the head of the Head of Health Human Resources, the Needs Plan Program Holder at the Medan City Health Office, 2 Health Workers, and 2 Communities. The data is then analyzed narratively to elaborate the discussion and conclusions. The results showed that the obstacles in RENBUT ANJAB   & ABK in the Medan City Health Office were in Needs Planning In the Medan City Health office, the obstacle that existed was the lack of a team in preparing the plan, causing officers holding the renbut program to be overwhelmed in inputting additional data. Pegwai agencies become obstacles in inputting RENBUT data due to a lack of sensitivity in inputting scheduled data so that data results are more skewed in wants rather than needs. Irregular employee mutations also cause data to be inappropriate so that the agency will experience worker vacancies.
