Abstract

Background: Population aging continues to intensify the demand for comprehensive geriatric nursing assessments, particularly in primary care and community health settings. In many contexts, these assessments are still performed manually, which can be time-consuming, prone to documentation errors, and difficult to integrate into ongoing care planning. Digital screening tools may help address these challenges; however, their effectiveness largely depends on usability and user experience among frontline nurses. Objective: This study aimed to evaluate the usability and user experience of the Geriatric Care Screening Tool, a digital application developed to support geriatric nursing assessments, from the perspective of nurse users. Methods: A usability and user experience evaluation study was conducted using a descriptive quantitative design within a user-centered development framework. Twenty nurses involved in geriatric nursing services participated in the evaluation. User needs were explored through a focus group discussion (FGD), followed by expert content validation of the application. Usability was assessed using the System Usability Scale (SUS), while user experience was evaluated using the User Experience Questionnaire (UEQ). Data were analyzed descriptively and interpreted using established benchmark standards. Results: The mean SUS score was 70.38, indicating marginally acceptable usability with a “Good” adjective rating. UEQ results showed positive user experience across all dimensions. The highest scores were observed in Clarity (1.75), Accuracy (1.20), and Stimulation (1.04). Lower scores were identified in Attractiveness (0.73), Efficiency (0.86), and Novelty (0.65), suggesting areas that require further improvement, particularly related to interface design, navigation speed, and innovative features. Conclusion: The Geriatric Care Screening Tool demonstrated acceptable usability and a generally positive user experience among nurses. While the application meets basic functional requirements for geriatric nursing assessment, further refinement—particularly in efficiency, visual design, and innovation—is needed to optimize its use in clinical and community practice.

Abstract

Background: The implementation of the Indonesian Nursing Diagnosis Standards (IDHS) in clinical practice continues to encounter obstacles, including limited time for assessment, documentation burden, and inconsistency in diagnostic interpretation among nurses. Advancements in artificial intelligence (AI) offer opportunities to design clinical decision support systems that may enhance the accuracy, uniformity, and efficiency of nursing diagnoses. Objective: This study aimed to explore nurses’ experiences, challenges, and expectations regarding the use of IDHS and to assess the initial feasibility of developing an AI-based system to support standardized nursing diagnoses. Methods: A descriptive qualitative design was employed using in-depth interviews with five nurses from diverse clinical settings, including emergency, critical care, inpatient, outpatient, and community services. Thematic analysis was conducted to identify perceptions and needs related to IDHS and AI. Content validity of the preliminary system design was evaluated by three expert validators using the Item-Level Content Validity Index (I-CVI) and the Scale-Level Content Validity Index/Average (S-CVI/Ave). Results: Five major themes emerged: (1) varied experiences in formulating nursing diagnoses, (2) inconsistent use of IDHS in practice, (3) challenges related to documentation and diagnostic interpretation, (4) positive attitudes toward AI integration in clinical workflows, and (5) the need for features such as automated diagnostic suggestions, clarification of clinical criteria, and integration with electronic medical records. Content validity testing demonstrated strong agreement among experts, with I-CVI values ranging from 0.80 to 1.00 and an S-CVI/Ave of 0.92. Conclusion: The study indicates that integrating AI into the IDHS-based diagnostic process holds substantial potential to improve diagnostic quality, standardization, and clinical efficiency. The content validity results support further development and prototype testing in subsequent research phases

Abstract

Background: Hypertension is referred to as “the silent killer” because it often does not show any symptoms, so people are unaware that they have it. This condition can increase the risk of various serious diseases such as aneurysms, heart failure, metabolic syndrome which can trigger diabetes mellitus, heart disease, stroke, dementia, and an increased risk of end-stage kidney disease. Dietary compliance is very important to avoid complications of hypertension, where self-efficacy is one of the main determinants of success in maintaining dietary compliance. Objective: This study focuses on determining the relationship between self-efficacy and diet compliance in patients with hypertension and comorbidities. Methods: This study is a quantitative study with a cross-sectional design. The sampling technique used was purposive sampling, where 120 respondents with hypertension and comorbidities at Dr. Moewardi General Hospital, Surakarta, were selected. The research instruments used were a self-efficacy questionnaire and a dietary compliance questionnaire. Results: Statistical test results using SPSS and Chi-Square analysis showed a significant relationship between self-efficacy levels and diet compliance in hypertensive patients with comorbidities (P-value <0.001). Most respondents had high self-efficacy and adhered to their diet, namely 59 respondents (93.7%), while 55 respondents (96.5%) of those with low self-efficacy were non-adherent. Conclusion: This study indicates that the higher a person's self-efficacy, the higher their confidence in adhering to a diet. Therefore, interventions aimed at increasing self-efficacy can be an effective strategy for improving diet compliance

Abstract

Background: Depression in elderly individuals with hypertension is a common but often underdiagnosed mental health issue that affects quality of life and treatment adherence. This study aimed to identify factors associated with depression in elderly patients with hypertension. Design : A descriptive correlational study with a cross-sectional design was conducted. Methods : This study involved 68 community-dwelling elderly patients with hypertension in a village of North Sulawesi selected through total sampling. Data were collected using structured questionnaires covering sociodemographic and clinical characteristics and the Geriatric Depression Scale-15 (GDS-15). Data were analyzed using the Mann-Whitney U test, Kruskal-Wallis H test, and Spearman’s rank correlation. Results : The mean depression score was 9.43 (SD = 1.79), with nearly half of participants experiencing moderate depression (48.5%). Only the duration of hypertension was significantly correlated with depression score (r = 0.275, p = 0.023). Conclusions : These findings highlight the need for routine depression screening among elderly hypertensive patients, particularly those with a longer history of illness. Clinical implications include adopting integrated care models that combine hypertension management with psychosocial support in primary care settings

Abstract

Background: Islamic boarding schools (pesantren) have distinctive educational settings, yet emergency preparedness education remains limited by conventional learning methods. Objective: This study aimed to develop and validate a three-dimensional (3D) hologram-based learning media on school safety zones and first aid for students in Islamic boarding schools. Methods: This research employed a Research and Development (R&D) approach using the 4D model, consisting of the Define, Design, and Develop stages. Validation was conducted by two material experts and two media experts using standardized assessment instruments covering aspects of content accuracy, visual quality, and practicality. Data were analyzed descriptively to determine the feasibility level of the developed media. Results: Validation by material experts yielded an average score of 93.33%, while media experts obtained 92.94%, both categorized as very feasible. These findings indicate that the 3D hologram-based learning media meets both substance and technical standards required for preparedness and first-aid education. Conclusion: The developed 3D hologram-based learning media was declared very feasible based on expert validation, making it a valid and appropriate educational tool for future implementation in nursing and health education programs.

Abstract

Background: Diabetic foot ulcers (DFUs) represent a major chronic complication of diabetes mellitus and are associated with infection, extended hospital stays, and an increased likelihood of lower-limb amputation. Individuals with diabetes remain at lifelong risk of DFUs, with recurrence frequently reported. Consistent and appropriate foot self-care is therefore considered a key preventive measure, particularly in community settings with limited access to specialized care. Objective: This study aimed to examine the relationship between foot self-care behaviour (FSCB) and the risk of developing diabetic foot ulcers among adults with type II diabetes living in rural communities. Methods: A cross-sectional study was conducted involving 100 adult outpatients with type II diabetes attending the Sukawati II Primary Health Care Centre, Gianyar, Bali. Participants were recruited using consecutive sampling. Data were collected through structured questionnaires that included demographic characteristics and foot self-care behaviour assessed using the Nottingham Assessment of Functional Foot Care (NAFF). The risk of diabetic foot ulcers was evaluated using Inlow’s 60-second diabetic foot screening tool, incorporating a 10 g monofilament test. Data analysis employed the Gamma correlation test to determine the association between variables. Results: The findings showed that 62% of participants demonstrated poor foot self-care behaviour. Additionally, 50% of respondents were identified as being at risk of developing DFUs. Statistical analysis revealed a strong and significant inverse relationship between foot self-care behaviour and DFU risk (p = 0.001; r = −0.613). Conclusion: Poor foot self-care behaviour is strongly associated with an increased risk of diabetic foot ulcers among individuals with type II diabetes in rural settings. These findings highlight the importance of strengthening foot care education and preventive interventions at the primary health care level

Abstract

Background: Children’s emotional reactions, such as fear or resistance, are strongly influenced by how parents manage their behavior during medical procedures. Effective behavioral management which means parenting practices in managing children’s behavior, plays a crucial role in helping children face medical procedures calmly and cooperatively. Various factors may affect implementation of behavioral management, including knowledge and experience. Objective: This study aimed to examine the association of parents’ knowledge and experience on the implementation of behavioral management in parenting. Methods: This study employed a quantitative, non-experimental design. The population consisted of parents whose children received health examinations at the Pediatric Outpatient Clinic, with a total of 70 participants selected using accidental sampling. Data were analyzed using ordinal logistic regression. Results: Respondents with poor knowledge had a 68% lower likelihood of demonstrating better behavior management compared with those with good knowledge (OR = 0.32; 95% CI: 0.10–0.96; p = 0.05), while moderate knowledge showed a similar but statistically non-significant trend (OR = 0.42; p = 0.09). Respondents with poor experience had a 70% lower likelihood of exhibiting better behavior management (OR = 0.30; 95% CI: 0.09–1.00; p = 0.05), whereas moderate experience was not significantly associated with behavior management (OR = 1.18; p = 0.73). Conclusion: Parents are expected to apply behavioral management strategies in clinical setting based on knowledge and experience to reduce fear and anxiety in children, such as preparatory communication, distraction techniques, play therapy and collaboration with health care staff.

Abstract

Background: Interprofessional Collaboration (IPC) enhances the quality and safety of healthcare by integrating the expertise of multiple health professionals; however, its implementation in hospital settings is often hindered by barriers such as ineffective communication, unclear professional roles, hierarchical dominance, and unsupportive workplace culture. Objective: This study aimed to analyze the relationship between demographic characteristics, work experience, and workplace culture and the implementation of IPC among nurses. Methods: A cross-sectional quantitative study design was employed to examine the association between demographic factors, work experience, and work culture with IPC implementation among nurses at a general hospital in Banten Province, Indonesia. Eighty nurses were recruited using stratified random sampling. Data were collected through structured questionnaires and analyzed using descriptive and inferential statistics with SPSS version 27.0. Results: IPC implementation was significantly associated with age (p = 0.048), education level (p = 0.047), work experience (p < 0.001), and work culture (p < 0.001). Work culture emerged as the most influential factor, with an odds ratio (OR) of 7.429 (95% CI: 2.703–20.419). Conclusion: Both individual demographic factors and organizational context influence nurses’ IPC implementation, with workplace culture being the most influential factor, highlighting the need to foster a collaborative work environment through interprofessional communication training, inclusive decision-making, and supportive institutional policies.

Abstract

Background: Type 2 diabetes mellitus is a chronic metabolic disorder characterized by persistent hyperglycemia due to impaired insulin secretion and/or insulin resistance. One of the most common long-term complications is impaired peripheral tissue perfusion of the lower extremities, which can progress to diabetic foot ulcers and significantly increase the risk of amputation. Consequently, identifying effective and accessible non-pharmacological interventions to improve peripheral circulation is crucial. Objective: This literature review aimed to evaluate the effectiveness of Buerger Allen Exercise (BAE) in improving peripheral tissue perfusion among patients with type 2 diabetes mellitus. Methods: A literature review was conducted using Google Scholar, PubMed, ScienceDirect, ProQuest, and Elsevier databases. Articles published between 2016 and 2024 were identified using the keywords “Buerger Allen Exercise,” “BAE,” “Type 2 Diabetes Mellitus,” “Peripheral Perfusion,” and “Ankle Brachial Index,” combined with Boolean operators “AND” and “OR.” Inclusion criteria comprised intervention studies published in English or Indonesian that reported outcomes related to peripheral perfusion. Results: Ten studies met the inclusion criteria. Overall, BAE was associated with significant improvements in peripheral perfusion, evidenced by increased Ankle Brachial Index values, improved capillary refill time, enhanced skin temperature and color, strengthened peripheral pulses, and reduced lower extremity pain. Several studies also reported greater benefits when BAE was combined with adjunctive interventions such as vitamin C supplementation, foot spa therapy, or music therapy. Conclusion: Buerger Allen Exercise is a low-cost, feasible, and evidence-supported non-pharmacological intervention that effectively enhances peripheral tissue perfusion in patients with type 2 diabetes mellitus and may help prevent vascular complications and improve patient well-being.

important role in maintaining maternal health. However, inappropriate activity levels may be associated with various adverse maternal outcomes. Previous studies have often focused on single health outcomes, limiting understanding of how physical activity relates to multiple maternal conditions within the same population. Objective: This study aimed to comprehensively examine the associations between physical activity levels and multiple maternal health outcomes, including constipation, low back pain (LBP), blood pressure, chronic energy deficiency (CED), and anemia, among pregnant women. Methods: This cross-sectional correlational study involved 251 pregnant women consecutively recruited from two community health centers. Physical activity was assessed using the Pregnancy Physical Activity Questionnaire (PPAQ), while constipation, LBP, blood pressure, CED, and anemia were measured using the Constipation Assessment Scale (CAS), Numeric Rating Scale (NRS), and a standardized checklist form. Data were analyzed using Chi-square and Fisher’s exact tests, with odds ratios (ORs) and 95% confidence intervals (CIs) calculated for significant associations. Results: Most respondents reported moderate levels of physical activity. Physical activity level was significantly associated with LBP severity (p = 0.020), with women engaging in vigorous activity showing higher odds of severe LBP compared with those performing sedentary to moderate activity (OR = 1.83; 95% CI: 0.94–3.54). No statistically significant associations were observed between physical activity level and constipation, blood pressure, CED, or anemia. Conclusion: Low back pain was the only maternal health outcome significantly associated with physical activity level in this study. These findings highlight the importance of antenatal care interventions that prioritize screening for LBP, assessment of physical workload patterns, and guidance on safe and balanced physical activity to prevent severe LBP during pregnancy.
