Abstract

The palm oil mill industry has many risk to occupational safety and health. There are still many work accidents that occur resulting in losses for the company. Therefore, it is necessary to do a risk analysis wit importance index that assesses risk by considering the frequency index and sevarity index. Risk assessment is carried out by experts namely mill manager, mill assistants, and foreman. Then the risk identification is done based on the cause and effect of the risk using the bow tie methode. Risk assessment indicators use risk indicators from palm oil mills. From 54 risk indicators there are 3 significant risk, namely employees exposed to hot iron (stew operation), employees inhaled / exposed to chemicals (wtp operation), noise ( engine room operation). And there are 2 high risks, namely employees exposed steam to hot steam (stew operation), employees exposed to fire bursts (boiler operation). The causes of significant and high risks are leaking pipes, opening the stew doors when high pressure, stew doors are not tightly closed, employee negligence, employees do not obey work instructions, high air pressure, lack of noise absorbent, material, there are worn, engine parts. The impact of significant and high risk are burns to the skin, material loss, shortness of breath, skin irritation, communication, disorders, damage to hearing. Control of sginificant and high risk using a complete PPE, providing evacuation tools.

Abstract

Process Safety is a branch of the science of safety that focuses on special accident control and characteristic of the process industry. Process Safety focuses on preventing the acute release of energy or substance in harmful quantities and limiting the magnitude and consequences of the discharge event. Process Safety can be achieved by reducing hazards and risks incurred at a wide acceptable level by the organization and / or society (Marshall and Ruhemann (IIPS), 2007). One such release is the Loss of Primary Containment (LOPC) which according to the American Petroleum Institute Recommended Practice (API RP) 754 is the release of flammable, toxic, corrosive, reactive, thermal, and pressure material from primary containment which includes: tanks, pressure vessels, pipes, trucks, rail cars, or equipment as primary containers or used for processing with the potential to cause damage. In the American Petroleum Institute Recommended Practice (API RP) 750 elements of Assuring The Quality and Mechanical Integrity of Critical Equipment that pressure vessels, machines and pipes shall be designed, manufactured, installed and maintained or inspected in accordance with required requirements. One form of corporate risk management is to conduct inspections as one of the Preventive Safeguards in Process Safety to prevent Loss of Primary Containment (LOPC). Abstract

Musculoskeletal Disorder is an occupational disease, often resulting in abnormalities of muscles, tendons, bones, joints, peripheral blood vessels, or peripheral nerves. Musculoskeletal Disorders occur when the work posture is not ergonomic, if the work posture is not ergonomic then the worker will easily experience complaints of Musculoskeletal Disorders (MSDs). For this reason, researchers conducted an analysis in this study to find out the results of the analysis of work postures that caused complaints of Musculoskeletal Disorders. This study uses a qualitative approach. Researchers took all workers in the Cleaning Intermediate Bulk Container PT. CPS totaling 6 people to be examined. The research instruments used were REBA Tools, Nordic Body Map Questionnaire, in-depth interviews, digital cameras, and arc rulers. data collection procedures such as observation, Nordic Body Map Questionnaire, documentation, field notes (Posture Study), and interviews. The method used is the REBA (Rapid Entire Body Assessment) method in the PT.CPS workshop area. Based on research that has been conducted on 6 respondents who work as Cleaning Intermediate Bulk Container at PT. CPS results and conclusions are obtained, namely, the work posture carried out by Cleaning Intermediate Bulk Container workers in the PT. CPS should be investigated further and repaired immediately. This job has a high-risk work factor, so it is necessary to hold innovations from the equipment aspect and the worker's health aspect so that they can feel comfortable at work and avoid musculoskeletal complaints. Abstract

Risk assessment was carried out at the medical waste combustion process facility using Incinerator. The method used in this research is HAZOP. This research is descriptive qualitative or observation research. The results of this study provide an overview of the risk level of the process at the Incinerator facility, where the highest risk level is in the process of Rotary Kiln Chamber 1, Chamber 2, Heat Exchanger, Water scrubber, Cyclone, Induce draft Fan, Sampling Point, and Exhaust stack. The level of risk is derived from probability and severity. In addition, recommendations are produced as materials for companies to make design improvements so that identified risks can be minimized. Abstract

Medical emergencies at school are temporary relief and treatment efforts on victims at school before being taken to hospitals, health centers, or health clinics for better help from doctors or paramedics. The purpose of the study was to design a medical emergency at Kamboja public elementary school 017. This study is a quantic study with triangulation techniques and is analyzed descriptively. The data used is primary data from interviews and observations, secondary data derived from the company. Results of research for the design of medical emergencies include creating SOPs, creating response time emergencies, simulating the application of medical emergency responses, and evaluating the completeness of medical facilities. First aid training in accidents (first aid) or basic first aid as a provision of knowledge and ability in case of an emergency in school. Conclusions were made the implementation of emergency response SOPs, the conformity of medical facilities in accordance with PERMEN No.15 / MEN / VIII / 2008, providing first aid training in accidents, and making response time emergency. Advice for public primary school 017 is to provide accident relief training on all staff and teachers in schools on a scale and make cooperation with medical authorities, health centers, clinics, and/or nearby hospitals for follow-up in the event of an emergency. Abstract

Development is basically a process of change that leads to better results, among others, through the process of economic growth. The process of economic development is carried out through economic activities in various sectors, including the industrial sector which requires business management and among which requires the support of Occupational Safety and Health (K3). Construction work is work that involves engineering consultants as front-end engineering and design planners as well as detailed engineering design contractors as executors as well as supervisory consultants. Construction work is a solid job of activities with a high enough level of risk such as heavy lifting work, working at height and limited space work the effects of the work in the event of an accident include damage to the equipment used, damage to the environment around the project and loss of workers' lives and the latter effect is called fatality as a whole these effects will affect the schedule of the project. project elesaian as well as swelling construction costs. Based on data from the Social Security Organizing Agency (BPJS) Employment 2015, the number of work accidents reached 105,182 cases of the total, 38% of the total was accidents of workers falling from a height. Abstract

Hospital XYZ of Balikpapan City, built-in 1949 by an oil company from the Netherlands established over a land area of 35,577.28 m₂. XYZ Hospital has 19 outpatient rooms, 17 inpatient rooms (150 beds), and several other facilities. This research used descriptive qualitative research with the number of respondents 5 people, the consideration used to determine the sample in this research is cleaning service officer (separation, packing, and transportation of infectious waste), medical waste burner (storage and burning of infectious solid waste) and head of HSE XYZ Hospital Balikpapan, this is done because the three parts are very closely related to infectious solid waste in the hospital. The results of field, observations showed a positive response to researchers in conducting field observations relating to the effectiveness of handling infectious solid waste in hospital XYZ. Based on research in the hospital there are conditions of waste management (segregation, packaging, storage, and standard of TPS building) 89% according to the rules and 11% are not in accordance with the rules, because there is an accumulation of B3 waste in TPS waste infectious waste, and for research hospital incinerator condition XYZ there are 80% suitable conditions and 20% unsuitable conditions, ie were not reaching the temperature at the maximum condition that is 1000 ° C which combustion only at condition 300 - 400 ° C which causes the condition of infectious waste not to ashes. Abstract

An emergency is an event or series of events that threaten and disrupt people's lives and livelihoods caused, either by natural factors and/or non-natural factors as well as human factors, resulting in human casualties, environmental damage, property losses, and psychological impacts. Fire control is all efforts to prevent fires by various efforts to control each manifestation of energy, procurement of fire protection facilities and rescue facilities and the establishment of emergency response organizations to eradicate fires. In this study, the author discusses the application of fire prevention at PT Pelindo IV Balikpapan. This study uses a descriptive research method with a qualitative approach where the author uses a checklist sheet regarding the suitability of fire prevention and interview guidelines to conduct interviews with sources who are considered to be the most knowledgeable about fire prevention at PT Pelindo IV Balikpapan. The results of the study found that the suitability of the emergency response system was 61.11% and life-saving facilities was 85.71% with an average value of suitability for fire prevention at PT Pelindo IV Balikpapan of 73.41% with the "enough" category. Abstract

The quality of electrical installations is highly dependent on the implementation and application of standard electrical installation regulations, namely PUIL in 2011 and other regulations of Law number 30 of 2009 concerning supporting electricity. Electrical installations including safety, protection, and equipment must be properly maintained. Due to the age factor, the installation will experience wear, aging or damage that will interfere with the installation. Therefore, the installation must be periodically checked and repaired parts that are worn, damaged or aging must be replaced. This type of research uses a qualitative descriptive method. Based on the results of this study, there is a feasibility of installation in the PT. Honda Hopefully Jaya Prima Balikpapan, 46.6% were declared eligible and 53.3% were declared ineligible. Electrical installations in terms of installation equipment, there are 46.6% of the 15 installations that meet the 2011 PUIL eligibility standards, while from the electrical installation safety there are 80% of the 15 installations that meet the 2011 PUIL eligibility standards. % of 15 installations that meet the 2011 PUIL eligibility standards. Installations in terms of insulation resistance meet 100% of the 15 feasibility installations mentioned in the 2011 PUIL standards. Judging from the grounding or grounding resistance, there are 93.3% of the 15 installations that are declared eligible according to with PUIL 2011. Abstract

Unsafe action is a failure to follow the requirements and correct work procedures, thereby increasing the likelihood of accidents. As many as 80%-85% of work accidents are caused by negligence or human error. The number of work accidents at PT. Pelindo IV Balikpapan is categorized as zero accident. However, the description of work behavior at PT. Pelindo IV Balikpapan is still far from the safe category. Use of tools that are not in accordance with regulations, not using appropriate PPE and other violations that can still be encountered in the daily work process. This study aims to determine whether the factors of knowledge and training can influence unsafe action on workers at PT. Pelindo IV Balikpapan. This type of research is using a quantitative approach, namely research related to scores because data collection uses a questionnaire as the main tool for collecting data. The type of research used is obserbasional which is cross sectional with primary and secondary data collection methods. The population is all workers who are in the field as many as 33 workers.
