Abstract

Background: Advances in science and technology continue to develop rapidly, including in the use of the National Health Insurance (JKN) Mobile application. Mobile JKN is a digital innovation developed by BPJS Kesehatan. With this application, people can easily access comprehensive health services. Mobile JKN is a progressive step in ensuring equal access for everyone. In addition, users of this application can be more practical when forgetting to bring a physical card; Just open the application, the officer can view the card online. Objective: This study aims to explain the relationship between the use of the Mobile JKN application and the quality of health services. Research method: This study uses a quantitative method with an analytical survey approach and a cross-sectional design. The population in this study is all BPJS participants in the working area of the Pagar Merbau Health Center, Deli Serdang Regency. Samples were taken purposively, and primary and secondary data were collected for analysis. Results: The results showed that the majority of respondents (64.7%) used the Mobile JKN application, and the majority of respondents (70.4%) stated that the quality of health services they received was good. There was a significant relationship between the use of Mobile JKN and the quality of health services in the working area of the Pagar Merbau Health Center, Deli Serdang Regency (p=0.002). It is hoped that people in the Pagar Merbau Health Center area will make more use of the Mobile JKN application to get more effective and efficient services when receiving treatment. In addition, health workers are expected to continue to socialize about Mobile JKN on a regular basis, so that the public always remembers the importance of using this application to get optimal service.

Abstract

Background: Improper use of the foot can cause problems when walking and even endanger a person's health. The arch of the foot helps withstand impact pressure and provides stability when standing or walking. Flatfoot is a deformity related to the foot, such as medial longitudinal arch disorders of the foot, valgus deformity of the heel, and forefoot abduction. The exercise methods that can be given to overcome flatfoot are Towel toe curl exercise and tibialis posterior strengthening. Purpose: to improve foot function by strengthening the intrinsic muscles of the foot. While tibialis posterior strengthening exercise is to strengthen the tibialis posterior muscle, which is part of the extrinsic muscle. Research method: This type of research is quantitative with a one group pre-test and post-test design. Data analysis using paired sample t-test. The sample of this study were children at SD Negeri 104242 Lubuk Pakam who suffered from flatfoot with a total of 22 samples. The intervention given was towel toe curl and tibialis posterior strengthening 3 times a week for 4 weeks with a total of 12 actions. Before the dynamic balance intervention, respondents were measured first (pretest) with the Y Balance Test (YBT) and re-measured after the intervention (Posttest). Results and discussion: The results of the paired sample t-test obtained a significant value (p) of 0.000, greater than the alpha value (a) (0.000 <0.05), meaning that there is a significant effect of towel toe curl exercises and tibialis posterior strengthening on improving dynamic balance in flatfoot children. Conclusion: Based on the results of the study, it can be concluded that towel toe curl exercises and tibialis posterior strengthening can be one of the training methods that can be applied to overcome dynamic balance disorders in flatfoot sufferers

Abstract

Background: Transcranial Magnetic Stimulation (TMS) is a form of non-invasive brain stimulation with the capacity to modulate cortical excitability. Low-frequency TMS (1 Hz) inhibits regional brain activity and allows contralateral cortical excitability through modulation of interhemispheric inhibition. Until now, research on low-frequency TMS that shows its inhibition on regional brain and the possibility of contralateral cortical stimulation is still very limited and not widely known. The purpose of this study was to determine the inhibitory effect of Transcranial Magnetic Stimulation (TMS) in post-stroke patients. Methods: Literature review was conducted to identify all articles that discuss the use of low-frequency TMS to determine the inhibitory effect of post-stroke patients on non-lesion areas. Databases from Hindawi, PubMed, Science Direct, and Cochrane with research types of pilot studies and randomized controlled trials published in the last 10 years by entering the keywords “Transcranial Magnetic Stimulation”, “Inhibition”, and “Stroke”. The research was conducted by systematically collecting, processing, and analyzing data. Results: Of the 505 articles identified, then selected according to the title there were 57 articles, obtained 10 articles that had abstracts according to the study. After that, the selection of articles according to the inclusion and exclusion criteria by reading the full article, there were 4 studies selected for review. 4 articles were included in this study showing motor improvement, increased functional ability, and cognitive improvement in post-stroke patients. Conclusion: TMS has beneficial effects on motor recovery in post-stroke patients with lasting effects.

Abstract

Background: Patients in the intensive care unit are individuals who are in critical condition due to failure of function of one or more organs, with hemodynamic instability. They require special care and intensive monitoring, in the hope that their condition can improve through appropriate medical action (Setiyawan, 2016). The incidence of stroke is increasing every year, there are 11 million stroke cases in the world, especially in low-income countries, especially in South Asian states and it causes 4 million deaths every year, around 30% of them in Southeastern countries. In addition, stroke is one of the leading causes of death and disability in adults in the United States. Every year, nearly 795,000 people in the country experience a stroke, either new or recurrent, and this causes approximately 130,000 deaths annually. (WHO, 2016). Research Objective: To understand the effect of level 1 progressive mobilization on changes in oxygen saturation in patients with decreased consciousness. Methodology: This research uses quantitative methods. The approach is quasi-experimental with a utilitarian design, where the intervention is applied to one group, then observations are made on the independent variables. Research Results and Discussion: Before progressive mobilization measures were carried out, the level 1 oxygen saturation value in patients with impaired consciousness at Grandmed Lubuk Pakam Hospital, of the 24 respondents, 2 respondents had oxygen saturation of 92% (8.3%), 4 respondents 93% (16, 7%), 7 respondents 94% (29.2%), 7 respondents 95% (29.2%), 3 respondents 96% (12.5%) and 7 respondents had oxygen saturation 98% (4.2%) . Conclusion: The average O2 saturation value in patients with decreased consciousness before doing level 1 progressive exercise was 93.33, whereas after it increased it was 95.67.

Abstract

Following childbirth, moms will undergo emotional fluctuations characterized by mood swings, anxiety, dizziness, and feelings of despair. Impaired maternal understanding of infant care can lead to psychological issues that impact the mother's physical well-being, including disrupted sleep patterns and reduced appetite. Failure to promptly manage anxiety in postpartum mothers can lead to the development of baby blues syndrome and postpartum depression. This study employs a Quasi Experiment design with a pre-posttest not include a control group to quantitatively investigate the impact of combining facial acupressure therapy with lavender aromatherapy on anxiety reduction in postpartum mothers. The investigation was conducted in Medan Tuntungan District, located in Medan City. One hundred postpartum women constituted the sample in this study. The data collection process involved administering treatment (facial acupressure therapy with lavender aromatherapy), followed by observation and interviews conducted using the HARS instrument. The analysis conducted using the Wilcoxon Signed Ranks Test revealed a negative ranks value of 94. This indicates that out of the 100 postpartum women who participated in the study, 94 mothers reported a reduction in anxiety levels. The positive rankings index is 0, indicating that none of the 100 postpartum moms who participated in the study reported a rise in anxiety levels. The tie value of 6 indicates that among the 100 postpartum moms who participated in the study, 6 mothers did not show any significant difference in anxiety levels before and after undergoing a combination of Facial Acupressure Therapy with Lavender Aromatherapy. Moreover, it is established that the significance value at this stage of analysis is 0.000 or greater than ? (0.05). Therefore, it can be inferred that there is a significant impact of the combination of Facial Acupressure Therapy with Lavender Aromatherapy on the reduction of anxiety levels in postpartum mothers.

Abstract

Atherosclerosis occurs when plaque accumulates and hardens within the arterial walls, leading to blood vessel blockages and increasing the risk of coronary heart disease (CHD). The extracts of butterfly pea (Clitoria ternatea L.) and roselle (Hibiscus sabdariffa L.) contain flavonoids that possess strong antioxidant and anti-inflammatory properties. This study aims to assess the effects of the combined extract of butterfly pea and roselle (EKTR) on atherosclerosis biomarkers such as Vascular Cell Adhesion Molecules-1 (VCAM-1), Intercellular Adhesion Molecule-1 (ICAM-1), and Interleukin-6 (IL-6). The research employed a True Experimental Laboratory approach using a Post-Test Control Group Design. The subjects were 36 female Wistar rats (Rattus norvegicus L.), randomly divided into six groups through a Completely Randomized Design (CRD). Data were collected on the levels of VCAM-1, ICAM-1, and IL-6 in the serum of obese white rats, measured using an ELISA photometer. Data analysis was conducted using SPSS version 18, with statistical evaluation performed via one-way ANOVA and Tukey's post hoc test. The results showed that increasing doses of EKTR led to significant reductions in atherosclerosis biomarkers such as VCAM-1, ICAM-1, and IL-6 compared to the positive control and other groups. At a dose of 500 mg/kg BW, group F3 recorded IL-6 levels of 30.49 ± 1.60 ng/mL, VCAM-1 level of 29.42 ± 2.58 ng/mL, and ICAM-1 levels of 28.42 ± 1.72 ng/mL. In conclusion, butterfly pea and roselle, rich in flavonoids with potent antioxidant and anti-inflammatory properties, are effective as traditional remedies for reducing atherosclerosis biomarkers and for preventing and treating coronary heart disease (CHD) at an optimal dose of 500 mg/kg BW.

Abstract

Primigravida mothers are mothers who are experiencing pregnancy for the first time. Primigravida mothers are usually not ready to face pregnancy because it is something new to experience. The unpreparedness of primigravida mothers occurs both physically and psychologically. Anxiety is a feeling of discomfort manifested by restlessness, worry or fear. This is a manifestation of psychological and physiological factors. The third trimester is a time of alertness where at this time, pregnant women understand that their child can be born at any time. Mothers also feel impatient with the birth of the baby which makes mothers alert and vigilant. Anxiety during pregnancy can trigger changes in physical activity, sleep and nutrition. This can have an impact on fetal development and changes in the mother's mood. Yoga is a complementary therapy that can be an option to overcome anxiety in pregnant women, especially primigravida mothers in the third trimester. The design of this study was One Group Pretest and Posttest with the population being all primigravida pregnant women in the third trimester who checked their pregnancy at the Matahari Clinic. This study was tested on 17 samples taken using a purposive sampling technique. The results of the study showed that there was an average decrease in anxiety of 7.0000 and a p value of 0.000, which means that yoga exercises have an effect on reducing anxiety in pregnant women. So it is expected that midwives in the clinic are expected to apply yoga exercises to overcome anxiety in pregnant women, especially primigravida pregnant women in the third trimester.

Abstract

Anemia is a condition in which the number of red blood cells or the concentration of hemoglobin in them decreases from normal levels. This study aims to determine hemoglobin levels in postpartum mothers before giving green bean juice and hemoglobin levels in postpartum mothers after giving green bean juice. In conducting this research, the research used a quantitative research design with a one-group pretest-posttest design model, that is, before it was carried out, observations were made on the sample and after treatment, observations were made again. The data collection technique was carried out using a total sampling technique. Based on the research results, the probability of the test results listed in the table shows that the resulting probability is 0.000. This means that the probability < level of significance (? = 0.05) so that Ha is accepted and H0 is rejected. Thus, it can be stated that there is a significant difference between the hemoglobin levels in postpartum mothers with anemia before giving green bean juice (Pretest) and the hemoglobin levels in postpartum mothers with anemia after giving green bean juice (Posttest). The conclusion that can be drawn is that giving green bean juice (Phaseolus Radiates) affects the hemoglobin levels of postpartum mothers with anemia at the Pratama Cahaya Lubuk Pakam Clinic in 2024.

Abstract

Background: Myofascial Pain Syndrome is a common health problem where the main cause is repetitive injury. Various interventions can be performed to reduce pain and symptoms from MTrPs, one of which is dry needling (DN) and muscle energy technique (MET). Purpose: of this study is to determine the effect of DN, MET, and combination of DN+MET on pain at MTrPs m.upper trapezius. Research method quantitative three groups pre and post test design. Sample was taken using total sampling technique and total of 48 people with MTrPs m.upper trapezius obtained. Numeric Rating Scale (NRS) used to measure pain intensity. The statistical tests used are Wilcoxon signed ranked test and the Mann-Whitney U. Results and discussion: effect of DN intervention with P-Value= 0.000 (P<0.05), MET P-Value= 0.000 (P<0.05), Combination of DN+MET P-Value= 0.000 (P<0.05). Comparison of DN with MET P-Value= 0.001 (P<0.05), DN with combination of DN+MET P-Value= 0.030 (P<0.05), MET with a combination of DN+MET P-Value= 0.000 (P<0.05). Conclusion: based on statistical result DN, MET, and the combination of DN+MET had the same effect on reducing pain at MTrPs m.upper trapezius, where the DN+MET combination proved to be more effective and more recommended in the treatment of MTrPs m.upper trapezius. Abstract

Chronic kidney failure is a progressive and irreversible kidney dysfunction in which the body's ability to maintain metabolism, fluid and electrolyte balance is disrupted, resulting in uremia or azotemia. Hemodialysis is a form of treatment for patients with chronic kidney failure, in which the patient's blood is diverted out of the body through a dialyzer, which occurs through diffusion and ultrafiltration, after which the blood is returned to the patient's body. Several methods to maintain muscle strength and reduce fatigue scores, namely intradialytic exercise is an exercise program developed that can increase muscle strength, muscle endurance and physical activity. The general objective of this study was to determine the effectiveness of intradialytic exercise involvement interventions on muscle strength and fatigue scores in hemodialysis patients at GrandMed Lubuk Pakam Hospital. This research method is a quantitative study, using a quasy experiment with a one group pre-test post-test design. The population in this study were all patients with Chronic Kidney Failure who were recorded at the Medical Records Hospital Grandmed Lubuk Pakam from October 2021 to December 2022. With inclusion criteria: respondents experienced movement disorders, muscle cramps, and weakness. The number of samples was 22 respondents. The sampling technique used in this research is non-probability sampling, namely purposive sampling. The statistical test used is the Wilcoxon test. The Wilcoxon test results obtained significant results from the intradialytic exercise technique on muscle strength and fatigue scores with a p value of 0.00 each. So that it can be concluded that there is an effect of giving good intradialytic exercise interventions on muscle strength & fatigue scores in chronic kidney failure patients undergoing hemodialysis
