Abstract

Depression is a prevalent psychological comorbidity among patients with Diabetic Foot Ulcers (DFUs), significantly hindering wound healing and diminishing quality of life. This study aimed to analyze the influence of health literacy and patient perception of wound stage on depression, with resilience acting as a mediating variable. Employing a quantitative approach, the research utilized Partial Least Squares Structural Equation Modeling (PLS-SEM) for analysis. A sample of 102 DFU patients was recruited from wound care networks in the Central Java region. The findings revealed that health literacy significantly impacted resilience (F² = 0.238) and directly contributed to reduced depression levels (F² = 0.205). Patient perception of wound stage also demonstrated a moderate influence on resilience (F² = 0.203) and a strong influence on depression (F² = 0.500). Resilience itself exhibited a moderate effect on depression (F² = 0.213). Mediation analysis indicated that both health literacy and perception of wound stage indirectly affected depression through resilience, with a p-value < 0.05 for both mediation pathways. In conclusion, this study demonstrates that health literacy and the perception of wound condition significantly influence the level of depression in DFU patients, both directly and indirectly through the mediation of resilience. Enhancing health literacy and fostering a positive perception of the wound through psychosocial approaches can be crucial strategies in mitigating the risk of depression. Furthermore, interventions aimed at strengthening resilience should be integrated into DFU care programs to improve the mental well-being and overall quality of life of these patients.

Abstract

Magnetic Resonance Imaging (MRI) in pediatric patients presents significant challenges due to the difficulty children often experience in remaining still and cooperative during the procedure. Monitored Anesthesia Care (MAC) is frequently employed to address these challenges, ensuring both patient comfort and optimal imaging quality. This study aims to provide a detailed overview of the procedural protocol and potential adverse effects associated with the use of MAC in pediatric MRI examinations. A qualitative descriptive analysis was conducted based on a clinical case from the radiology department of a tertiary care hospital in Indonesia, supported by relevant literature review. Key procedural steps include pre-procedure assessment, laboratory testing, intravenous line placement, sedation administered and monitored by an anesthesiologist, continuous vital signs monitoring, airway management, and post-anesthesia recovery. Potential complications identified include allergic reactions to gadolinium-based contrast agents, local injection site bruising, hypothermia, airway obstruction, laryngospasm, post-sedation agitation, confusion, respiratory depression, and heightened anxiety due to the confined MRI environment. The report highlights the necessity of strict adherence to safety protocols, comprehensive patient preparation, and the availability of emergency equipment and trained personnel to mitigate risks. In conclusion, the use of MAC in pediatric MRI is a safe and effective strategy when performed with proper planning and multidisciplinary collaboration, enhancing diagnostic accuracy and patient safety.

Abstract

CT scanning is a medical imaging technique that allows detailed visualization of structures within the body using X-ray radiation. The pixel intensity in the image is interpreted in Hounsfield Units (HU), which measure the relative density of tissue compared to water. WW and WL are two important parameters in adjusting the visualization of CT scan images, allowing radiologists to optimize the display of various tissue structures. WW is the middle value of the selected HU range. This value determines the grayscale intensity that is the center of the image contrast range. WL in tissue with a certain density, such as distinguishing between brain tissue and lesions or edema. The purpose of this study is to optimize the use of parameters to improve image quality and diagnostic accuracy and determine the optimal WW and WL. This research supports secondary transformation in the health care system, in improving the quality of clinical services through innovation in diagnostic technology. This research is an experimental study. Direct measurements were made of variations in 10 window width values and 3 window level values. After data collection, three radiologists assessed the data using a questionnaire. A kappa test was performed to assess the agreement or similarity of observer perceptions in assessing the questionnaire. Data analysis was performed using the Kolmogorov-Sprinov test to determine the appropriate test. The normality test yielded a p-value > 0.05, indicating that the data were normally distributed. The Kappa test results of 0.82 with p <0.001 indicate that there is a very good level of agreement or agreement between observers in assessing the quality of CT Scan images. The Kolmogorov-Smirnov results show that the data distribution is normal with a p value = 0.120 (p > 0.05), thus fulfilling the requirements for parametric testing. The results of the Paired T-Test show differences in image quality between WW and WL. Window Width at 95 with Window Level 35 is proven to provide the most optimal image quality and produces the highest score (mean = 4.5). And shows better contrast between the area ofbleeding and normal brain tissue, so that bleeding can be clearly identified. The clarity of brain anatomy is also better seen compared to wider or narrower WW variations. In addition, the noise level is relatively low, so it does not interfere with the radiologist's interpretation in assessing the image.

Abstract

Anemia during pregnancy remains a major public health concern in Indonesia, with a prevalence of 37.1% reported in 2020. Despite ongoing iron supplementation programs, mild anemia continues to occur, potentially leading to maternal and neonatal complications. Mobile health applications offer innovative opportunities to enhance maternal knowledge and self-monitoring practices. This study aimed to develop and evaluate a prototype of a mobile-based anemia education application for pregnant women. A pre-experimental study involving 30 pregnant women aged 31–43 years at Puskesmas Ngesrep, Semarang, was conducted. Hemoglobin (Hb) levels were measured and participants were guided to use the application, which included features such as anemia information, Hb input, automatic anemia classification, and prevention tips. Results showed that 90% of respondents had normal Hb levels, while 10% experienced mild anemia. Feedback on the application was overwhelmingly positive: 100% of respondents agreed the application was easy to use, informative, visually appealing, and helpful in understanding anemia, with willingness to use it again. These findings align with previous studies emphasizing the role of mobile health tools in maternal education. The study concludes that the application has strong potential for further development, particularly in content enhancement and interactive features. The prototype reached Technology Readiness Level (TRL) 4–5, validating the system in a limited setting. This work contributes to advancing digital maternal health solutions in Indonesia. Abstract

This integrative review aims to synthesize the literature from 2015–2025 to analyze changes in practice and the effectiveness of digitally mediated gratitude interventions. This study uses an integrative review approach by synthesizing 21 selected studies that align with the PRISMA guidelines. The analysis focuses on digital gratitude expressions, evidence of effectiveness, and factors influencing online interventions. The review identifies three main findings: (1) The emergence of various digital gratitude expressions (e.g., personal journaling apps to social media posts). (2) Strong randomized controlled trial evidence indicates that digital gratitude interventions significantly improve subjective well-being, often through reducing negative thoughts. (3) Intervention effectiveness is influenced by design features such as guidance, task variation, and the integration of Artificial Intelligence (AI). This study proposes a reconceptualization of gratitude in the digital age as a multifaceted phenomenon, encompassing new dimensions such as performative, archived, and mediated gratitude. This conceptualization has important implications for positive psychology theory and the design of future digital interventions. Abstract

Anemia in adolescent girls is a global public health issue that affects nutritional status and future reproductive outcomes. Adolescents with anemia are at higher risk of delivering stunted offspring, making anemia prevention essential in breaking the intergenerational cycle of malnutrition. This study aimed to analyze factors associated with anemia among adolescent girls as an effort to prevent stunting. This cross-sectional study was conducted at SMPTQ Pangeran Diponegoro, Semarang, from August to December 2023. A total of 92 female students aged 10–15 years who met the inclusion criteria were selected using total sampling. Data were collected using questionnaires and physical measurements, including hemoglobin levels, BMI, MUAC, and dietary intake. Data were analyzed using chi-square tests (α = 0.05). The prevalence of anemia was 25%. Menstrual duration was significantly related to anemia (p = 0.041; RR = 2.188). Other factors (BMI, MUAC, breakfast habits) were not significant. Menstrual duration was significantly associated with anemia among adolescent girls. School-based anemia screening and nutrition education are recommended to prevent anemia and future stunting.
