Abstract

The study explores the development of a Foot Soak Device to improve the sleep quality of pregnant women in their third trimester. Pregnant women often experience discomfort such as leg edema, back pain, and sleep disturbances due to increased fetal weight, shortness of breath, and fetal movements. Poor sleep quality can negatively impact health, weaken the immune system, and lead to complications like low birth weight in newborns. Existing foot soak devices lack key features such as temperature regulation, heating, and massage, prompting researchers to innovate a new foot soak device tailored to pregnant women. The research follows a Research and Development (R&D) method. It includes two stages: the first involves analysis and product development based on feedback from five third-trimester pregnant women, while the second stage tests the product’s efficacy through a quasi-experimental design with a one group pretest-posttest involving 20 pregnant women, using purposive sampling. Results from the first stage highlighted the need for a foot soak with temperature control, a heater, and massage functionality. The developed device incorporated these features, and 90% of participants found the foot soaks comfortable. In the second phase, the analysis showed a significant effect of the foot soak on improving sleep quality (p ≤ 0.05). The study concludes that midwives could implement this foot soak device as part of midwifery care to enhance the quality of life for pregnant women in the third trimester.

Abstract

Appendicitis is an inflammation of the appendix or appendix that can occur in anyone, causing abdominal pain. Risk factors for appendicitis include age, gender, diet, and family history. This is due to poor diet at that age. Treatment for appendicitis can be done by surgery. Appendectomy surgery is performed by appendectomy, which is a surgical procedure to remove the inflamed appendix. Normal surgical wound healing is influenced by various factors, namely coagulation, immune system disorders, nutrition, chronic diseases, medications, and suturing techniques. This study was conducted to determine what factors play a role in the wound healing process in post-appendectomy patients. To determine the factors that play a role in the healing of post-appendectomy patients at the Haji General Hospital in Medan in 2021-2023. Descriptive research with a retrospective design that takes data through medical records from 2021-2023. It was found that post-appendectomy patients were more in early adolescence (12-16 years) and late adulthood (36-45 years), with male gender who had a normoweight nutritional status with dry wound conditions with a high number of leukocytes (leukocytosis) and often occurred in the acute appendicitis type. Post-appendectomy patients at the Haji Medan General Hospital in 2021-2023 were mostly found in early adolescence (12-16 years) and late adulthood (36-45 years), male gender, nutritional status with the normoweight category, had dry wound conditions, with a high number of leukocytes (leukocytosis), and patients suffering from acute appendicitis.

Abstract

Sepsis and severe sepsis are the leading causes of death in critically ill patients admitted to intensive care units (ICUs) in the United States. The national economic burden for sepsis in Indonesia at 100,000 patients is estimated to reach USD 130 million. Currently, screening for patients who may have sepsis has also been used frequently. The assessment can be done with a qSOFA score measurement (quick SOFA) or rapid measurement. This study used a retrospective with secondary data from medical records of patients treated in the intensive care unit of Jogja Hospital from January through December 2023. The subjects of 62 patients suffering from sepsis were obtained, with 25 patients (40.3%) at low-risk qSOFA values and 37 (59.7%) at high-risk qSOFA values. The results of this study stated that there was no relationship between the patient's qSOFA score when admitted to the hospital and the mortality of sepsis patients in the intensive care unit of the Jogja Hospital (p>0.07). Thus, the results of this study provide important insights for clinical practitioners in evaluating and planning the treatment of sepsis patients, considering that qSOFA, although useful as an initial screening tool, should not be used as the sole factor in clinical decision-making regarding the management of sepsis patients.

Abstract

Musculoskeletal Disorders (MSD) are a common issue among bank employees that can significantly impact their health and productivity. This research adopts a scoping review methodology, focusing on international journals related to the Analysis of Factors Associated with Bank Employees' Musculoskeletal Disorders (MSDs). Data from various studies reveal that the body areas most frequently affected by MSDs in the banking sector are the neck, lower back, and shoulders. Factors such as non-ergonomic working postures, improper computer usage, job-related stress, repetitive tasks, and insufficient breaks play pivotal roles in elevating the risk of MSDs. In efforts to mitigate these risks, the implementation of ergonomic training stress management, as well as a proactive prevention and regular monitoring approach within the banking work environment, becomes imperative. These findings offer insights into the elevated risk of MSDs in bank workers, providing a foundation for initiatives to enhance their well-being and work quality.

Abstract

Community mental health services face ongoing challenges including fragmented care, difficulties evaluating outcomes, barriers to standardized support and educational tools, and stigmatizing attitudes. Digital technologies hold promises to help address gaps, but currently lack integrated platforms connecting services. Objective: To review literature on digital mental health interventions and examine their implications for a proposed cross-service digital platform in community mental health. Methods: A literature search identified peer-reviewed studies published between 2022-2024 evaluating digital tools in community or related mental health contexts. Relevant findings were synthesized regarding standardized platforms, remote psychological therapies, digitally enabled peer support, and technologies for stigma reduction. Results: Research supports digital platforms can help standardize functions, collect uniform outcomes data, and facilitate continuous learning. Smartphone apps show the potential to deliver effective remote therapies when combined with clinician support. Preliminary studies demonstrate digital peer support may mitigate loneliness and social connection platforms can positively shape attitudes. Conclusions: A thoughtfully designed shared digital platform incorporating validated tools, networked services, peer connection and stigma-reduction technologies may address fragmentation and empower community mental health clients. However, further research validating models is still needed given existing gaps. Equity, privacy, change management and adaptability must also be considered in future development and testing.

Abstract

This narrative review explores the risks and management considerations of vaginal delivery in individuals with high-grade myopia, a condition associated with potential complications like retinal detachment and choroidal neovascularization. Utilizing 24 references, the review follows a modified rapid literature review model as outlined by Tjoa and Sutanto (2024). Eleven core studies out of an initial 234, including case reports and retrospective studies, were analyzed in the final qualitative assessment. The findings suggest that high-grade myopia, by itself, generally does not require cesarean section (C-section), and vaginal delivery is considered safe for most patients, provided there is regular ophthalmological monitoring throughout pregnancy. There is no strong evidence supporting a universal recommendation for C-section solely due to high myopia, except in the presence of severe complications, such as choroidal neovascularization. The review emphasizes individualized care, with delivery mode decisions tailored to each patient's specific ocular risk factors. Routine C-sections without additional complicating factors are not justified, underscoring the need for regular ophthalmological evaluations to ensure optimal maternal and fetal outcomes. Due to the limitations of existing studies, further in-depth research is recommended to confirm these findings and refine clinical guidelines.
