Abstract

Chronic kidney disease is a growing health concern, particularly among the elderly population. Anemia therapy in chronic kidney disease  patients typically involves the use of epoetin alfa, either alone or in combination with folic acid. However, differences in patient profiles and the alignment between actual treatment costs and INA-CBGs package tariffs remain a challenge in healthcare delivery. This study aimed to analyze the disparity between real costs and INA-CBGs tariffs for hemodialysis therapy. A retrospective cohort design was employed from the health provider’s perspective, analyzing direct medical costs of 73 chronic kidney disease  patients undergoing hemodialysis at RSUD Arifin Achmad Pekanbaru between January and March 2025. Data were collected using data collection forms, medical records, and financial reports, and analyzed using univariate methods. The majority of patients were aged ≥45 years (70%) and predominantly male (56.16%), indicating higher prevalence of chronic kidney disease  in older adults. The study found that the actual costs of both therapies were substantially higher than the corresponding INA-CBGs package tariffs, highlighting a significant gap between real treatment costs and standardized reimbursement rates.

Abstract

Stunting is a chronic nutritional problem that affects not only children’s physical growth but also their cognitive development, including concentration, memory, and future academic readiness. Cognitive development in stunted children is influenced by multiple factors, including infection history and parental stimulation. This study aimed to analyze the relationship between infection history and stimulation and cognitive development among stunted children. This study employed an observational analytic design with a cross-sectional approach. It was conducted in the working area of Tamamaung Public Health Center, Makassar, from October to November 2025. A total of 40 stunted children aged 12–42 months were recruited using total sampling. Data were collected through structured interviews with parents using validated and reliable questionnaires. Data were analyzed using univariate analysis and bivariate analysis with the Chi-square test at a significance level of 0.05. Most children were aged 12–24 months (40.0%) and female (72.5%). Moderate infection history was reported in 50.0% of respondents, while high and moderate levels of stimulation were each found in 42.5% of children. More than half of the children (52.5%) demonstrated appropriate cognitive development. Bivariate analysis showed that infection history was not significantly associated with cognitive development in stunted children (p=0.316), whereas stimulation was significantly associated with cognitive development (p=0.017). Children who received higher stimulation were more likely to demonstrate age-appropriate cognitive development than those who received inadequate stimulation. Infection history was not significantly associated with cognitive development among stunted children, whereas stimulation showed a significant relationship. Consistent parental stimulation should be considered an essential strategy to optimize cognitive development in children with stunting.

Abstract

Bronchopneumonia remains a common respiratory condition that frequently leads to impaired breathing patterns in pediatric patients. Dyspnea that is not properly managed may lead to hypoxia and increased respiratory workload. Simple, safe, and evidence-based non-pharmacological interventions are needed to alleviate dyspnea symptoms. One such intervention is handheld fan therapy, which works by stimulating trigeminal mechanoreceptors to improve breathing comfort and support physiological adaptation. This study aimed to implement and evaluate the effectiveness of handheld fan therapy based on the Roy Adaptation Model in managing impaired breathing patterns among patients with bronchopneumonia. This study employed a descriptive case study design. The subjects consisted of three pediatric patients diagnosed with bronchopneumonia who met the inclusion criteria. Data were collected through nursing assessment, clinical observation, physical examination, and medical record documentation. The handheld fan therapy intervention was administered twice daily for three consecutive days, with each session lasting 5–10 minutes. Nursing care implementation followed the Indonesian Nursing Diagnosis Standards (SDKI), Nursing Outcome Standards (SLKI), and Nursing Intervention Standards (SIKI). Data were analyzed descriptively to evaluate adaptive response changes in patients. The findings indicated that all patients presented with primary complaints of dyspnea and cough with a nursing diagnosis of impaired breathing pattern. Following three days of intervention, improvements in respiratory status were observed, including decreased respiratory rate, reduced dyspnea levels, decreased cough frequency, improved oxygen saturation, and increased patient comfort. Handheld fan therapy based on the Roy Adaptation Model is an effective non-pharmacological nursing intervention for reducing dyspnea and improving breathing patterns in patients with bronchopneumonia. This intervention has the potential to be integrated into evidence-based nursing practice for respiratory care management.

Abstract

Choking can cause serious respiratory disorders and even death if not treated immediately. Choking Management simulation is and effective method to handle choking situations. In Giri Peni Subdistrict, 7 out of 10 mothers in Giri Peni Wates have not received information about chokingmanagement using this method. Choking incidents often occur while eating or playing, and many deaths result from lack of knowledge about first aid. This study aims to determine teh effectiveness of Choking Management simulation on the knowledge of Family Welfare Empowerment (PKK) mothers regarding first aid for choking. This study used a quasy eksperiment design with a one group pretest-posttest approach and purposive sampling, involving 30 respondents. Data was collected from November 2-9, 2025 using an expert-validated questionnaire, and analyzed with Wilcoxon test. This study showed a significant increase in knowledge scores, with an average of 16,63 at pretest and 17,40 at posttest (Z value of 0,001 < 0,05). Choking Management simulatiom is beneficial for improving mother's knowledge and skills in providing first aid to choking victims.

Abstract

Hypertensive emergency is an emergency condition characterized by a significant increase in blood pressure accompanied by target organ damage, one of which is decreased consciousness. This condition can cause various nursing problems, including ineffective peripheral perfusion due to hemodynamic disorders and increased vascular resistance. This case study aims to analyze the clinical practice of nursing with the innovative intervention of Hand and Foot Massage with lemongrass oil in reducing blood pressure in hypertensive emergency patients with decreased consciousness in the ICU of Aji Muhammad Parikesit Tenggarong Regional Hospital. The method used is a case study with a comprehensive nursing care approach to client Mr. C who is treated in the ICU. The nursing process includes assessment, determination of nursing diagnosis, planning, implementation, and evaluation. The innovative intervention of Hand and Foot Massage with lemongrass oil was given for three consecutive days with a duration of 15–20 minutes per session, accompanied by blood pressure measurements before and after the intervention. After the innovative intervention of Hand and Foot Massage with lemongrass oil showed a decrease in systolic blood pressure with an average value of 15.67 mmHg and diastolic with an average value of 11 mmHg. In conclusion, the innovative hand and foot massage with lemongrass oil effectively lowers blood pressure. Therefore, healthcare workers can implement this innovative hand and foot massage intervention with lemongrass oil to lower blood pressure in hypertensive emergencies.

Abstract

Non-hemorrhagic stroke patients often experience limited mobility and prolonged bed rest during hospitalization, increasing the risk of developing pressure ulcers due to continuous pressure on bony prominences. This condition can be exacerbated by other factors such as sensory impairment, skin moisture, and inadequate nutritional status. Pressure ulcers are a common complication in immobilized patients and can lead to increased length of hospital stay, increased risk of infection, and higher healthcare costs. Therefore, preventive measures are needed through appropriate nursing practices, one of which is skin care using a skin barrier to maintain skin integrity. Petroleum jelly is known to have occlusive properties that can maintain skin moisture and protect it from friction and irritation. This analysis of nursing clinical practice aims to analyze the application of skin care using petroleum jelly as a skin barrier to prevent the risk of pressure ulcers in non-hemorrhagic stroke patients in the Elang III Ward of Aji Muhammad Parikesit General Hospital, Tenggarong. The method used was clinical practice analysis with a comprehensive nursing care approach that includes assessment, establishing a nursing diagnosis, planning, implementation, and evaluation. The results of the analysis showed that routine application of petroleum jelly to high-risk areas can maintain skin integrity, reduce excess moisture, and reduce the potential for pressure ulcers. The conclusion of this study shows that the use of petroleum jelly as a skin barrier is a simple, safe, and effective nursing intervention in preventing the risk of pressure ulcers in non-hemorrhagic stroke patients and has the potential to improve the quality of nursing care.

Abstract

The pregnancy process requires special care because it relates to the health of the mother and fetus. Various complaints and discomforts often arise during pregnancy, which can interfere with the activities of pregnant women. The purpose of this study was to identify factors related to the quality of sleep in pregnant women in their third trimester. This study was an analytical survey using a cross-sectional approach conducted at TPMB ZIANDTA in Ogan Komering Ulu Regency from July to December 2025. The population of this study consisted of 60 pregnant women in their third trimester. The sampling technique used was non-random sampling with a purposive sampling method, resulting in 35 respondents. From the Chi-Square test results, ρ Value of 0.007 (< 0.05) was obtained, meaning that there is a relationship between the mother's age and the sleep quality of pregnant women in their third trimester. ρ Value of 0.011 (< 0.05) was obtained, meaning that there is a relationship between prenatal exercise and the sleep quality of pregnant women in their third trimester.  ρ Value of 0.003 (< 0.05) was obtained indicating a relationship between anemia and sleep quality in pregnant women in their third trimester, and ρ value of 0.011 (< 0.05) indicating a relationship between BMI and sleep quality in pregnant women in their third trimester. It is recommended that health workers improve the health status of mothers so that the sleep quality of pregnant women is more adequate.

Abstract

The issue of inadequate breast milk (ASI) production among lactating mothers is a primary obstacle to the practice of exclusive breastfeeding for six months. “Insufficient breast milk production may stem from stress, exhaustion, inadequate nursing stimulation, or insufficient lactation-supportive nutrition. This study sought to assess the impact of katuk leaf (Sauropus androgynus) decoction on breast milk production in breastfeeding mothers residing in Kurungan Nyawa I Village, Buay Madang District, East OKU Regency. The intervention is providing 150 ml of Katuk leaf decoction everyday for seven consecutive days. This study employed a quasi-experimental design utilizing a pretest-posttest methodology without a control group. The sample comprised 17 lactating mothers. The Shapiro-Wilk test was employed to assess data normality, and the data were analyzed using the Paired Sample T-Test. The results indicated a substantial impact of Katuk leaf decoction on enhancing breast milk production, with a p-value of 0.000 (p<0.05). The mean breast milk volume prior to treatment was 76.47 ml, which rose to 122.12 ml post-intervention, reflecting an average increase of 45.53 ml. This notable enhancement indicates that katuk leaf decoction positively influences breast milk production. Breastfeeding women with insufficient milk supply are advised to take katuk leaf decoction as a natural remedy to enhance lactation.

Abstract

Anxiety in third trimester pregnant women is a common condition and can impact the labor process and the well-being of the mother and fetus. One non-pharmacological method that can be used to reduce anxiety levels is hypnobirthing. This study aims to determine the effect of hypnobirthing on the anxiety levels of third trimester pregnant women at TPMB Ziandta in 2025. This type of research is quantitative with a pre-experimental design using a one-group pretest–posttest approach. The population in this study were all third trimester pregnant women who underwent pregnancy check-ups at TPMB Ziandta, with a sample of 31 respondents taken using an accidental sampling technique. Data collection was carried out using an anxiety level questionnaire. Data were analyzed using a T-Test statistical test. The results showed that there was an effect of hypnobirthing on the anxiety levels of third trimester pregnant women with a p value = 0.000 (<0.05). The conclusion of this study is that hypnobirthing has a significant effect on reducing the anxiety levels of pregnant women in the third trimester at TPMB Ziandta in 2025. It is hoped that hypnobirthing can be implemented as a non-pharmacological intervention in antenatal care to help pregnant women manage anxiety before childbirth.
