Abstract

Introduction: Aseptor LTCM (long-term contraceptive method) is still not sufficient because the community’s knowledge is still minimal about the advantages and disadvantages, limitation of trained personnel, and existing installation. One of the LTCM is the intrauterine device contraceptive method, which is a long-term method that is effective when giving a child space. Objective: To explore the opinions of mother acceptors of family planning who have less interest in choosing the intrauterine device (IUD) contraceptive method Method: This research uses a qualitative and exploratory descriptive method. The data collection technique is non-probability sampling. The total sample consists of 10 people. Result and Discussion: The results from the 10 respondents, who clarified their knowledge, perceptions, and opinions about the Intrauterine Device (IUD) contraceptive method, show that while the respondents are generally familiar with the IUD method, some have negative perceptions about it. These perceptions are often influenced by family members and colleagues. Additionally, some respondents have negative views of the IUD method because of information they have heard in the community, which often causes fear. As a result, they have no intention or desire to use the IUD method. Therefore, health professionals need to provide accurate and clear guidance to mothers so they can fully understand the IUD method. Conclusion: It is concluded that mothers who accept family planning are aware of the advantages and disadvantages of the IUD method. This is why some mothers have negative perceptions and ideas about the IUD method.

Abstract

Introduction: Stockouts of vital medicines remain a critical issue in hospitals, particularly when drug availability drops to zero in pharmacy facilities. Although supplies may still exist elsewhere, it often takes up to a week for replenishment, potentially jeopardizing patient care. The "vital" category includes life-saving drugs whose unavailability is unacceptable. In Timor-Leste, stockout rates reported by the Ministry of Health were 20% in 2020, 22% in 2021, 19% in 2022, and rose sharply to 43% in 2023. Objective: This study aims to identify the factors contributing to stockouts of vital medicines at the Guido Valadares National Hospital (HNGV) Pharmacy Facility. Method: One of the methods used in this research is a quantitative analytical approach with a cross-sectional study design. The sampling technique employed is probability sampling, which approximates the total sample. Data analysis was conducted using the SPSS program to perform a Spearman Rank Correlation. Result and Discussion: Results revealed that human resources (p = 0.00, r = 0.416), medicine management (p = 0.00, r = 0.472), and patient volume (p = 0.00, r = 0.600) significantly influence stockouts. Conclusion: To eliminate the problem of stockouts in Timor-Leste, health professionals must pay attention to the factors influencing stockouts. Additionally, professional medication control services should plan medication purchases before supplies run out.

Abstract

Introduction: The repeated use of anticoagulants like heparin and antiplatelets such as clopidogrel in individuals with vascular thrombosis necessitates an evaluation of the nephrotoxicity risk. Objective: This study seeks to assess the impacts of heparin and clopidogrel on renal function. Method: This cohort retrospective design examined data from 120 individuals with vascular thrombosis who were administered heparin (n=60) or clopidogrel (n=60). Renal parameters (serum creatinine, eGFR) were evaluated prior to and following therapy. The statistical analysis employed a paired t-test and multivariate logistic regression, utilising a 95% confidence interval. Results and Discussion: The heparin cohort had a notable elevation in serum creatinine (1.8±0.4 vs 1.2±0.3 mg/dL; p=0.02) and a reduction in eGFR (45±12 vs 68±15 mL/min/1.73m²; p=0.01) in comparison to the clopidogrel group. Therapy exceeding 7 days elevated the incidence of renal impairment by 2.3-fold (OR: 2.3; 95% CI: 1.4-3.8) in the heparin cohort. The nephrotoxic impact of heparin is much greater in patients with cardiovascular comorbidities (p=0.04). Conclusion: Heparin presents a greater risk of inducing renal dysfunction than clopidogrel, particularly with prolonged usage and in individuals with concomitant conditions. Rigorous oversight of renal function and evaluation of alternate treatments are advised.

Abstract

Introduction: Tuberculosis (TB) remains a major global infectious disease, influenced by social determinants such as poverty. Community-based interventions (CBIs) are crucial for prevention, as they address biological and economic factors, enhance early detection, and engage communities in identifying, preventing, and managing TB. Strategies like active case finding (ACF) and community-based TB preventive treatment (TPT) reduce transmission, improve diagnosis in children, overcome transport barriers, and increase patient acceptance. Method: This systematic review examines CBIs in developed and developing countries, focusing on TB incidence and treatment adherence. Data were obtained from PubMed, Scopus, Web of Science, and Google Scholar. Results and Discussion: Proactive case finding accelerates diagnosis and treatment, reducing transmission and improving outcomes. In Uganda, multidrug-resistant TB patients favored community-based care with transport support. Fluoroquinolone-based TPT was safe, reduced adverse events, and lowered TB cases. Conclusion: CBIs are effective in preventing and treating TB, lowering prevalence rates. Key risk factors include diabetes, smoking, and low BMI. For maximum impact, community health workers require recognition, training, fair compensation, and integration into national health systems.
