Abstract

The causes of pneumonia include various microorganisms such as bacteria, viruses, parasites, and fungi. This condition can also be triggered by exposure to chemical substances or physical damage to lung tissue. The purpose of this study was to determine which antibiotic is more cost-effective based on therapeutic outcomes and more efficient costs between Levofloxacin and Ceftriaxone in adult hospitalized pneumonia patients who are BPJS Class III participants at RSM Ahmad Dahlan Hospital, Kediri City, in 2024. This study used a retrospective design, by tracing and analyzing patient data from previous periods through the collection of medical record data and hospital financial data. The sample was taken using a purposive sampling technique with a total of 40 patients. Data analysis was conducted using the Average Cost Effectiveness Ratio (ACER) method. The variables analyzed included therapeutic effectiveness and direct medical costs incurred during treatment. The results of the study showed that the effectiveness of therapy in hospitalized pneumonia patients at RSM Ahmad Dahlan in 2024 reached 100%, both in the Ceftriaxone therapy group consisting of 20 patients and in the Levofloxacin group consisting of 20 patients. Based on the ACER calculation results, the most cost-effective therapy in the management of pneumonia was the use of the antibiotic Ceftriaxone with a cost of Rp25,554, which is lower than therapy using the antibiotic Levofloxacin, which has an ACER value of Rp30,914.

Abstract

Type 2 diabetes mellitus (T2DM) not only causes metabolic disorders but also triggers chronic inflammation and hypercoagulation that contribute to changes in the physical properties of blood. Neutrophil Lymphocyte Ratio (NLR) as an inflammatory marker and Activated Partial Thromboplastin Time (APTT) as a coagulation parameter have the potential to reflect hemorheological disturbances that can be explained through Poiseuille's law. This study aimed to analyze the correlation between NLR and APTT and interpret it from a biophysical perspective. A cross-sectional study involved 21 T2DM patients at Mojowarno Christian Hospital, Jombang. NLR examination used a Mindray BC-5380 hematology analyzer and APTT used a CoaDATA 501 coagulometer. Results showed mean NLR of 3.43 (33% increased) and APTT of 30.40 seconds (95% normal). Spearman Rank correlation test showed no significant relationship between NLR and APTT (r=0.272; p=0.234). Although not statistically correlated, biophysically both parameters independently contribute to increased blood viscosity which, according to Poiseuille's law, will decrease blood flow rate and increase the risk of vascular complications. Direct viscosity measurement is needed for empirical confirmation. This study underscores the importance of a multidisciplinary approach in understanding the pathophysiology of T2DM.

Abstract

Paper soap is an innovation of solid soap in the form of thin sheets that is practical for travelling, but excessive brittleness remains a production chalenge. This study compare the effect of five types of plasticizers (glycerin, propylene glycol, polyethylene glycol (PEG-400), sorbitol, and sucrose) at five concentration levels (0%, 5%, 10%, 15%, and 20%) on the physical quality of paper soap based on SNI 2048:202 standars. The experimental study used a 5x5 factorial design with MANOVA analysis to test flexibility, pH, foam stability, and dissolution time. Glycerin 20% showed optimal results: high flexibility (14,67-fold), foam stability of 91,56%, dissolution time of 105 seconds, and pH 9,06. Sorbitol 15-20% can be an alternative with a higher pH, but moderate flexibility. PEG-400 and sucroese showed low flexibility despite accelerating dissolution. Glycerin 20% is the optimal formulation as it provides the best balance between flexibility, foam stability, and dissolution. The selection of plasticizer and their concentration must be done simultaneously to obtain optimal paper soap quality.

Abstract

Heavy metal exposure, such as lead (Pb), poses a severe threat to human health due to its toxic nature, which can trigger vital organ damage, particularly in the kidneys. The 'silver human' group is at high risk of Pb exposure due to the routine application of metallic paint over their entire bodies. This research aimed to evaluate the correlation between blood lead concentrations and serum creatinine levels as an indicator of kidney function among silver humans in Kediri City. This observational study, utilizing a cross-sectional design, involved six silver human subjects selected through total sampling. Pb analysis was performed using Atomic Absorption Spectrophotometry (AAS), while creatinine levels were assessed using standard laboratory methods. All respondents (100%) exhibited Pb levels within the normal range, with an average of 7.000 µg/dL. However, regarding the creatinine parameter, 50% of the respondents were found to have levels exceeding the normal threshold. Pearson statistical analysis indicated a p-value of 0.785 (p > 0.05). Consequently, no significant correlation was observed between blood lead levels and creatinine concentrations among the research subjects in Kediri City.

Abstract

Substance abuse can disrupt glucose metabolism and increase the risk of undetected hyperglycemia, particularly in vulnerable populations such as inmates. This study aimed to describe fasting blood glucose levels and metabolic characteristics among substance-abusing inmates. A descriptive cross-sectional study was conducted involving 80 inmates selected by total sampling. Data included fasting blood glucose, body mass index, and blood pressure. Descriptive statistical analysis was performed, including mean, median, standard deviation, range, and proportion of hyperglycemia. The mean fasting blood glucose level was 121.3 mg/dL (median 117 mg/dL; SD 22.77 mg/dL; range 84–204 mg/dL). Hyperglycemia was observed in 42.5% of respondents. A proportion of subjects were also classified as obese and hypertensive. Substance-abusing inmates demonstrate a considerable burden of hyperglycemia, highlighting the importance of routine metabolic screening in correctional settings. Abstract

Diabetes Mellitus (DM) is a chronic metabolic disorder characterized by a steadily increasing prevalence both globally and nationally. Patients with DM are highly susceptible to various infections, with Urinary Tract Infection (UTI) being one of the most frequent complications. The hyperglycemic state in DM patients leads to glycosuria, a condition where the presence of glucose in the urine creates an ideal environment for the proliferation of pathogenic microorganisms. Microbiologically, Gram-negative bacteria are the primary etiology of UTIs in diabetic patients. Bacteria such as Escherichia coli, Klebsiella pneumoniae, and Proteus mirabilis possess specific virulence factors that facilitate adherence to the urinary tract epithelium. The objective of this research was to identify Gram-negative bacteria and determine their percentage distribution among Diabetes Mellitus patients at Public Health Center X in Kediri. This research employed a descriptive research method with an accidental sampling technique, utilizing 14 urine specimens from DM patients. The results revealed the presence of Klebsiella sp. (36%), Escherichia coli (50%), and Salmonella sp. (14%) in the urine of the subjects. Based on these findings, it can be concluded that urinary tract infections in Diabetes Mellitus patients at Public Health Center X, Kediri, are caused by several types of pathogenic bacteria, with the highest prevalence being Escherichia coli, followed by Klebsiella sp., Salmonella sp., and Proteus sp. Early detection and management of bacterial infections are essential to prevent further complications in patients with Diabetes Mellitus

Abstract

Kesehatan vagina pada ibu hamil sangat bergantung pada keseimbangan flora normal yang berfungsi sebagai sistem pertahanan tubuh. Gangguan pada keseimbangan mikroorganisme ini dapat memicu infeksi yang menjadi salah satu faktor utama penyebab komplikasi kehamilan. Keputihan menjadi tanda awal adanya infeksi mikroorganisme di dalam vagina. Keputihan yang bersifat patologis dan dibiarkan terlalu lama dapat menyebabkan komplikasi berbahaya bagi ibu dan janin. Penelitian ini bertujuan untuk mengetahui gambaran jumlah bakteri Staphylococcus pada kasus keputihan ibu hamil trimester III. Bakteri Staphylococcus diisolasi dari spesimen swab vagina dan ditumbuhkan pada media Manitol Salt Agar (MSA). Koloni yang tumbuh diidentifikasi dan dihitung jumlah bakterinya berdasarkan metode Standard Plate Count. Teknik sampling dilakukan dengan accidental sampling dan diperoleh sampel sebanyak 16 spesimen swab vagina ibu hamil trimester III. Hasil penelitian menunjukkan bakteri Staphylococcus yang ditemukan yaitu, spesies Staphylococcus epidermidis sebanyak 7 spesimen (44%), dan Staphylococcus aureus sebanyak 9 spesimen (56%). Jumlah rerata koloni bakteri Staphylococcus epidermidis yaitu 4,4x105 CFU/ml, sedangkan Staphylococcus aureus 4,8x107 CFU/ml. Hasil penelitian ini dapat memberikan informasi tentang gambaran jumlah bakteri Staphyloococcus pada kasus keputihan ibu hamil trimester III, sehingga mendukung ibu hamil untuk selalu menjaga kebersihan vagina agar terhindar dari berbagai komplikasi selama kehamilan Kata kunci: Bakteri Staphylococcus, Keputihan, Swab vagina, Ibu hamil.

Abstract

Latar Belakang: Chronic low back pain (CLBP) adalah salah satu penyebab utama kecacatan di seluruh dunia dan seringkali mengakibatkan keterbatasan fungsional, penurunan kualitas hidup, dan penurunan produktivitas kerja. Transcutaneous Electrical Nerve Stimulation (TENS) adalah modalitas elektroterapi non-invasif yang umum digunakan dalam fisioterapi untuk mengurangi nyeri pada pasien dengan CLBP. Namun, bukti mengenai efektivitasnya dalam hasil rehabilitasi masih belum konsisten. Tujuan: Tinjauan sistematis ini bertujuan untuk mengevaluasi efektivitas Transcutaneous Electrical Nerve Stimulation (TENS) dalam rehabilitasi pasien dengan nyeri punggung bawah kronis. Metode: Studi ini menggunakan desain tinjauan sistematis berdasarkan pedoman PRISMA. Pencarian literatur dilakukan menggunakan Publish or Perish melalui basis data Google Scholar. Sebanyak 100 artikel diidentifikasi, diikuti analisis open access dengan Mendeley. 7 studi memenuhi kriteria inklusi dan dimasukkan dalam tinjauan akhir. Dengan kriteria inklusi uji randomize control trials dan tinjauan sistematis yang diterbitkan dalam lima tahun terakhir. Sintesis data dilakukan untuk menganalisis efek TENS pada intensitas nyeri, kemampuan fungsional, dan hasil rehabilitasi pada pasien CLBP. Hasil: Sebagian besar studi menunjukkan bahwa TENS efektif mengurangi intensitas nyeri dan meningkatkan hasil fungsional pada pasien dengan Chronic low back pain (CLBP), terutama bila dikombinasikan dengan latihan terapeutik. TENS juga menunjukkan efek analgesik jangka pendek yang positif dan meningkatkan partisipasi pasien selama program rehabilitasi. Kesimpulan: TENS tampaknya merupakan modalitas yang efektif dan aman untuk mengurangi nyeri dan mendukung rehabilitasi pada pasien dengan nyeri punggung bawah kronis, meskipun diperlukan studi berkualitas tinggi lebih lanjut untuk menetapkan protokol rehabilitasi yang terstandarisasi.

Abstract

Kali Petukangan is a small river stream located on Jalan Rawa Gelam, Cakung District, East Jakarta. This river is frequently used as a disposal site for both domestic and industrial waste, largely due to its proximity to the Pulogadung industrial area. This study aims to evaluate the water quality of Kali Petukangan by analyzing several physical and chemical parameters. The research process includes determining sampling points, collecting and transporting samples, conducting laboratory tests, and analyzing the data. The study employs a laboratory experimental design with a descriptive observational approach and a sequential explanatory method. The test results indicate that the average water temperature is 26°C, pH ranges from 7.59 to 8.30, color varies from 3.0 to 57.0 Pt-Co, conductivity ranges between 643 and 763 µmhos/cm, total dissolved solids (TDS) are between 313.3 and 453.3 mg/L, total suspended solids (TSS) range from 47.5 to 73 mg/L, dissolved oxygen (DO) ranges from 0.39 to 2.75 mg/L, and biological oxygen demand (BOD) is between 16.55 and 27.50 mg/L. Based on these results, the water quality of Kali Petukangan is classified as moderately polluted.
