Abstract

Improvements in cataract surgery techniques and intraocular lens (IOL) calculations have raised patient expectations for precise visual outcomes. Refractive surprise occurs when the post-operative refractive outcome differs from the intended target, leading to unexpected visual outcomes. Secondary piggyback IOL implantation and IOL exchange are techniques to correct the refractive surprise. We presented some case series of refractive surprise after cataract surgery. Case I was A 55-year-old woman presented with the complaint of blurred vision in her left eye after cataract surgery ten months before. The visual acuity was 0.125 in the right eye (RE) and 0.05 in the left eye (LE). The best-corrected visual acuity of the left eye was 0.63 with a refraction of S+6.00 C-0.50 × 20°. She was diagnosed with hyperopic surprise in the left eye and pseudophakia in both eyes. The piggyback intraocular lens implantation procedure was chosen to correct the refractive surprise. Case II was A 72-year-old man presented with the complaint of blurred vision in his right eye after cataract surgery five years before. The visual acuity was 1/60 in his right eye and 0.16 in his left eye. The best-corrected visual acuity of the right eye was 0.4 with a refraction of S-14.00 C-1.50 × 125°. He was diagnosed with myopic surprise in his RE. The IOL exchange to scleral fixation with the Yamane technique implantation procedure was chosen to correct the refractive surprise. Piggyback IOL implantation and IOL exchange are proven, safe, and effective methods for correcting refractive surprises following cataract surgery.

Abstract

Background: Caesarean section (CS) is a surgical procedure performed when vaginal delivery is not possible due to medical or non-medical indications. The increasing prevalence of CS worldwide has raised concerns regarding maternal and neonatal health. Objectives: This study aims to identify maternal factors contributing to the increased incidence of CS at Sitti Khadijah 1 Maternity Hospital, Makassar, for the period of January 1 – December 31, 2023. This research provides a unique contribution by analyzing CS prevalence and risk factors in a private Islamic maternity hospital, a setting underrepresented in national data. Furthermore, quantitative evaluation using multivariate analysis was conducted to determine the most dominant risk factors, supporting evidence-based prioritization in clinical decision-making and antenatal care planning in similar urban Indonesian contexts.Methods: A cross-sectional study was conducted using secondary data from medical records of mothers who delivered at Sitti Khadijah 1 Maternity Hospital in 2023. Maternal age, history of CS, premature rupture of membranes (PROM), gestational age, and preeclampsia were analyzed. Statistical tests included Chi-Square and multivariate analysis.Results: Of 341 deliveries, 300 (88.0%) were CS cases. Significant associations were found between CS and history of CS (p=0.004), preeclampsia (p=0.000), and PROM (p=0.000), while maternal age (p=0.751) and gestational age (p=0.183) were not significant. Mothers with a previous CS had a 99.0% likelihood of undergoing repeat CS. Preeclampsia strongly predicted CS, with 94.8% of cases delivered surgically. PROM was also a determinant, with 96.3% undergoing CS. Multivariate analysis confirmed history of CS as the strongest predictor (OR=19.689; 95% CI 2.589–149.737; p=0.004).Conclusion: History of CS, preeclampsia, and PROM are significant factors associated with CS. The risk of CS is significantly associated highest with history of CS. These findings highlight the need for improved maternal healthcare strategies to manage high-risk pregnancies and reduce unnecessary CS procedures.

Abstract

Verruca vulgaris (VV), a cutaneous disorder associated with human papillomavirus (HPV), presents as a benign proliferation of keratinocytes. Verruca vulgaris therapy with destructive methods such as trichloroacetic acid (TCA) and electrocautery is the most commonly used therapeutic modality. The study aims to assess the therapeutic efficacy and clinical benefit of trichloroacetic acid (TCA) and electrosurgery in managing VV by synthesising evidence from clinical studies or case series. Systematic review adhered to the guidelines outlined in the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA). To ensure the collection of accurate and relevant information, literature searches were conducted across PubMed, Google Scholar and Cochrane Library covering publications from 2014 to 2023. Twelve studies were included, comprising eight articles on TCA and four articles on electrosurgery. Trichloroacetic acid achieved a maximum cure rate of 93.3% while electrosurgery reached 100%. During six-month follow-up period, TCA showed no recurrence (0%), whereas electrosurgery had a recurrence rate of 21,9%. Electrocautery has more advantages than TCA with a higher cure rate. Even though it has a higher cure rate, electrocautery has a higher recurrence rate with more severe side effects than TCA.

Abstract

Fibrosarcoma in adults is a rare malignant tumor originating from fibroblasts, accounting for 3% of all sarcomas in adults. We report the case of a 22-year-old man with a large, solitary, persistent mass on the right sole, accompanied by swelling and pain, as well as a mixture of blood and necrotic tissue. There was no history of trauma. Initial X-ray examination showed a soft tissue mass without periosteal reaction or bone damage. Ultrasonography confirmed the presence of a solid, slightly heterogeneous mass within the plantar fascia of the foot. Subsequently, Magnetic Resonance Imaging (MRI) showed an irregularly shaped solid mass with irregular margins, without a capsule, containing small areas of necrotic tissue, measuring approximately 7.4 x 4.4 x 4.3 cm in the right plantar fascia. The mass appeared to spread to the surrounding muscles without damaging the adjacent bones, with no evidence of neurovascular compression. Histopathological examination revealed highly proliferative fibroblastic cells arranged in a herringbone pattern. Fibroblastic cells with spindle-shaped nuclei, enlarged, coarse chromatin, and eosinophilic cytoplasm were present within the collagenous mass. The patient underwent tumor resection with clean margins of 1–2 cm, followed by chemotherapy, and is scheduled for radiotherapy. It is essential to perform appropriate radiological examinations and to correlate clinical, laboratory, and histopathological findings to diagnose fibrosarcoma and plan the best course of treatment.

Abstract

Background: Hypertension is a risk factor for morbidity and mortality from cardiovascular disease. Left ventricular hypertrophy (LVH) is the main target organ due to hypertension, which is a predictor of the development of cardiovascular diseases such as heart failure. Echocardiography is the gold standard examination for determining heart geometry, and concentric LVH is often found in hypertensive patients. Electrocardiography (ECG) is a widely available, low-cost, and rapid modality for diagnosing LVH in healthcare facilities. The Peguero–Lo Presti (PLP) criteria in ECG examination have better sensitivity and specificity compared to other criteria. Objectives: This study aims to determine the diagnostic value of PLP ECG to diagnose concentric LVH.Methods: This research is an analytical observational study with a cross-sectional approach carried out at the inpatient installation of Dr. M. Djamil General Hospital Padang for 6 months, starting from April to September 2024. The study subjects who met the inclusion and exclusion criteria were 90 samples selected by consecutive sampling. An ECG examination was conducted using PLP criteria and echocardiography with the M-mode method to assess left ventricle (LV) geometry. Data were analysed using a 2x2 table to obtain sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and diagnostic accuracy.Results: From 90 samples, LVH geometry was found concentric in 65.50%, eccentric 4.40%, concentric remodelling 11.10%, and normal 18.90%. In the diagnostic test, PLP criteria had 79.66% sensitivity, 90.32% specificity, 94.00% PPV, 70.00% NPV, and 83.33% accuracy to diagnose concentric LVH in hypertensive patients.Conclusion: Peguero Lo-Presti criteria on ECG can be used for screening and early diagnosis of concentric LVH in hypertensive patients.

Abstract

The majority of dives go off without any problems, but there are physiological changes brought on by pressure fluctuations in the underwater environment that might cause serious harm. Decompression sickness (DCS) is a condition resulting from the formation of nitrogen bubbles in body tissues due to rapid decompression after a dive, and one of the severe manifestations of DCS is spinal cord DCS. Although it is uncommon, it is a neurological diving emergency that can cause permanent impairment. The gold standard treatment for all DCS, including spinal cord DCS, is hyperbaric oxygen (HBO) therapy. In this article, we present a case report of a 43-year-old male with spinal cord DCS with a high MEDSUBHYP score, but who refused HBO therapy, which led to long-term morbidity and residual neurological deficits. The patient had received non-steroidal anti-inflammatory drugs (NSAIDs) and methylprednisolone as adjunctive therapy, but they provided no benefit. This article discusses the clinical presentation of spinal cord DCS, its challenging diagnosis, and several factors that predicted poor prognosis in patients with spinal cord DCS. We also highlight the logistical barrier to getting HBO therapy in Indonesia, which may be one of the main reasons the patient refused to be referred to another facility.

Abstract

Background: Mental health problems, particularly depression, among adolescents in a community have become a growing public health concern globally. Early preventive efforts are essential to reduce the burden of depression. Educational interventions in the community are one of the recommended strategies.Objectives: This study aimed to determine the effectiveness of mental health education in increasing adolescents’ knowledge of depression prevention in a community. Methods: This study employed a pre-experimental one-group pretest-posttest design involving 60 adolescents selected through purposive sampling in a rural community setting. The data were collected using a validated knowledge questionnaire on depression prevention and analysed using paired-sample t-tests in SPSS version 28. Results: The findings revealed a significant increase in the mean knowledge score after the intervention (pretest mean = 11.82 ± 2.41; posttest mean = 16.45 ± 1.89; p < 0.001). These results indicate that the mental health education intervention effectively improves adolescents’ knowledge of depression prevention.Conclusion: Mental health education can be an effective strategy in improving adolescent knowledge of depression prevention in community settings. This finding supports the importance of community-based educational programs as part of sustainable mental health promotion aligned with the Sustainable Development Goals.

Abstract

Background: Endotracheal intubation in pediatric patients undergoing elective surgery can trigger hemodynamic response, including increased blood pressure and heart rate, due to sympathetic stimulation. Premedication is required to attenuate these responses. However, the comparative effectiveness of gabapentin and lorazepam remains to be investigated.Objectives: This study aims to evaluate and compare the effects of gabapentin and lorazepam premedication on blood pressure and heart rate during tracheal intubation in pediatric patients.Methods: This randomized controlled clinical trial involved 64 pediatric patients undergoing elective surgery. They were divided into two groups: the gabapentin group (15 mg/kg) and the lorazepam group (0.025 mg/kg). Hemodynamic parameters were measured before and after intubation. Results: The study results showed that the gabapentin group had lower systolic blood pressure, diastolic blood pressure, and mean arterial pressure (MAP) compared to the lorazepam group after intubation (p<0.05). Gabapentin was also more effective in maintaining blood pressure stability than lorazepam, while both groups showed comparable heart rate parameters (p > 0.05).Conclusion: Gabapentin premedication is more effective in suppressing the surge in blood pressure caused by intubation than lorazepam, while lorazepam is more significant in reducing heart rate. Overall, gabapentin may be a more optimal premedication option for maintaining hemodynamic stability in pediatric patients undergoing elective surgery.

Abstract

Background: Type 2 Diabetes Mellitus (T2DM) is known to affect multiorgan systems, including auditory function, with metabolic dysregulation playing a central role. Leptin and ghrelin, as metabolic hormones, are involved in energy homeostasis and neuroprotection, and may be influenced by fasting behavior. Objectives: This study aimed to explore the associations between fasting practices, serum leptin and ghrelin levels, and hearing function in adults with T2DM.Methods: A cross-sectional study was conducted involving 86 adults with T2DM, categorized into fasting (n=46) and non-fasting (n=40) groups. Serum leptin and ghrelin levels were measured using ELISA after an overnight fast. Hearing thresholds were assessed with pure-tone audiometry at 500, 1000, 2000, and 4000 Hz. Hearing loss was defined as a mean threshold >25 dB. Data were analyzed using Mann–Whitney U tests, Chi-Square tests, and logistic regression.Results: The fasting group had significantly lower leptin (32.85 ± 31.03 ng/mL vs. 40.89 ± 23.40 ng/mL, p=0.012) and ghrelin levels (5.48 ± 3.37 ng/mL vs. 7.32 ± 3.32 ng/mL, p<0.001). Hearing thresholds were slightly lower in the fasting group (20.80 ± 8.10 dB vs. 23.32 ± 11.13 dB), though the difference was not statistically significant (p=0.192). However, the frequency of hearing loss was significantly lower in fasting participants (21.7%) than in non-fasting participants (37.5%, p=0.015). Logistic regression revealed that higher leptin levels increased the risk of hearing loss (OR = 1.023, 95% CI: 1.004 - 1.042, p = 0.015), whereas fasting status was protective (OR = 0.429, 95% CI: 0.197 - 0.935, p = 0.033). Ghrelin, age, and sex were not significant predictors.Conclusion: Fasting may be associated with more favorable metabolic profiles and a lower frequency of hearing loss in individuals with T2DM. These findings suggest a potential protective role of structured fasting practices in auditory health, warranting further longitudinal studies to explore causality and underlying mechanisms.

Abstract

Evidence from the past five years has highlighted increasing interest in pharmacological adjuncts at improving transurethral resection of the the prostate (TURP). Both randomized controlled trials (RCTs) and observational research have investigated the role of carbazochrome sodium sulfonate (CSS), with varying results. This systematic review aimed to evaluate the effectiveness of CSS in transurethral resection of the prostate (TURP) for benign prostatic hyperplasia (BPH). The review was conducted in accordance with the Prefrred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. A comprehensive search of Embase, PubMed/MEDLINE, Scopus, Web of Science, and the Cochrane Library was performed. Eligible studies included cohort studies, case-control methodologies, and RCT investigations related to CSS in TURP procedures and reporting outcomes such as blood loss and complications. Five RCTs comprising a total of 550 participants met the inclusion criteria. The findings indicated that CSS was associated with a significant reduction in intraoperative bleeding compared with tranexamic acid (TXA). Across all studies, CSS exhibited a favorable safety profile, with no severe adverse effects reported. CSS appears to be a promising advancement in managing perioperative bleeding during TURP procedures for BPH. Its hemostatic efficacy and safety profile suggest potential benefits for surgical outcomes and patient recovery. Nevertheless, further high-quality, large-scale studies are warranted to establish standardized treatment protocols and to clarify optimal role of CSS in contemporary urological practice.
