Abstract

Perinatal mental health (PMH) disorders affect 15.6–37% of postpartum women, yet midwives report low confidence in detection and management due to inadequate training and protocols. This study evaluated midwives' readiness to handle postpartum PMH disorders in community health centers. A convergent parallel mixed-methods design was used in eight Mataram City health centers (December 2023–January 2024). Quantitatively, 102 purposively sampled midwives completed validated questionnaires on sociodemographics, screening practices, and PMH handling, analyzed via chi-square tests. Qualitatively, thematic analysis of in-depth interviews with six informants (managers, midwives, and mental health nurses) explored referral experiences using N-Vivo. Most midwives (86.27%) did not handle PMH appropriately. The implementation of PMH data assessment (37.25%), screening (37.26%), and classification (22.56%) was low. Handling PMH significance association with screening (p=0.001, OR 14.31), classification (p=0.001, OR 23.22), and data assessment (p=0.009, OR 9.43). Age, education, experience, and training were not associated. Themes emphasized referral collaboration with family consent, consultation with mental health nurses/doctors, and hospitalization for the most serious cases. Midwives are not yet sufficiently prepared to address the perinatal mental health (PMH) needs of postnatal women. Poor screening uptake is a consequence of the absence of protocols and time limitations, and this highlights requirements for training and standardized pathways. Referral dependence is compensatory but inconsistent, mirroring global appeals for inter-professional approaches. Targeted PMH training and referral protocols are crucial in order to assist the development of midwives' skills to provide prompt care and reduce maternal-infant risk. Interventions should be tested in longitudinal studies across a variety of Indonesian contexts.

Abstract

Indonesia's neonatal mortality rate stands at 12 deaths per 1,000 live births, driven by prematurity, asphyxia, and congenital anomalies. Inconsistent Essential Newborn Care (EENC) implementation fuels preventable first-24-hour deaths. This study aims to assess EENC knowledge and practices among Indonesian health professionals. A cross-sectional survey of 132 experienced providers (>3 years in delivery/perinatology) from hospitals, community health centers, and independent midwifery practices used a validated questionnaire via Google Forms. Descriptive statistics and chi-square tests identified associations (P<0.05). Overall, 66.4% showed poor EENC knowledge. Strengths included breastfeeding cues (99.2% correct attachment; 93.9% rooting), but deficits marked contraindications (34.8% for maternal hemorrhage >1,000 ml; 49.2% post-birth wheezing). Practices excelled in basics (93.2% recognized hypothermia/cord/skin-to-skin/breastfeeding/infection prevention; 90.2% upright positioning) yet faltered in sequencing: only 15.9% prioritized birth time announcement then thorough drying, 50.8% suctioned airways immediately, and 48.5% endorsed supine second-stage positioning. Workplace significantly affected knowledge (χ²=9.467, P=0.009): hospitals were the worst (79.1% poor), and independent midwifery practices were the best (33.3%). Age, education, experience, and training showed no association. Procedural sequencing and contraindication gaps stem from heavy workloads and patchy training. Skills-focused programs with supervision and supportive settings are essential to boost competency and decrease deaths. Observational studies should capture real behaviors and assess training/policy impacts on newborn outcomes.

Abstract

The perinatal period brings physical, emotional, and social changes for mothers, which heighten their risk of experiencing perinatal depression. In 2022, its prevalence reached 20% in low- to middle-income countries. Worldwide, postpartum depression is regarded as a significant public health issue that demands targeted interventions. This study aims to review the results of research on the mothers’ experiences with perinatal depression. It used scoping review that consisted stages of conducting a focusing review using the SPIDER framework (Sample size, Phenomenon of Interest, Study Design, Evaluation, and Research type); conducting literature searches using relevant databases, i.e., Pubmed, Wiley, ScienceDirect, ProQuest, Lexis Nexis and Sage Journals; Relevant studies were selected based on inclusion and exclusion criteria; conducting JBI critical appraisal to assess the quality of the literature; and conducting data extraction, analyzing and reporting the results. The PRISMA Flowchart, representing the Preferred Reporting Items for Systematic Reviews and Meta-Analyses, was utilized to formally delineate the process of the literature search. Twenty articles met the inclusion criteria and were included in the review. Through thematic analysis, key concepts were identified and organized into three themes: symptoms, causes, and treatment of perinatal depression. Perinatal depression is a significant condition that requires timely detection and appropriate treatment to safeguard the health of both mothers and their babies. This study highlights how maternal experiences influence mental health and recovery, and it supports the development of interventions that are more sensitive to emotional and cultural needs.

Abstract

Hypertension in pregnancy is currently the leading cause of maternal mortality. Self-care strategies, delivered via media adjusted to technological progress, accessibility, and pregnant women's needs, offer a practical means to manage this condition. represent a feasible option for the management of this condition. Because of its widespread use, WhatsApp could be used to improve self-care in hypertensive pregnant women. This study aims to determine the effect of WhatsApp on self-care in pregnant women with hypertension. A quasi-experimental pretest-posttest control group design. A total of 42 respondents were recruited by consecutive sampling from six community health centers, 21 respondents were in the intervention group and 21 respondents were in the control group. The outcomes were evaluated using the Hypertension Self-Care Profile (HBP-SCP) questionnaire, which was adapted for the Indonesian context and validated (r=0.517–0.957) and reliable (α=0.935) both before and after health education. Data analysis used nonparametric Wilcoxon matched-pairs and Mann-Whitney tests. In the intervention group, self-care scores improved significantly post-intervention (p<0.05), unlike the control group (p>0.05). The Mann-Whitney test confirmed a significant difference between groups (p<0.05).WhatsApp education boosted self-care scores, utilizing its accessibility and closed-group features for uniform information delivery. These findings advocate integrating such media into community health center protocols, bolstering health professional educational roles. Randomized trials on long-term adherence to WhatsApp-guided self-care protocols and studies comparing these to traditional in-person counseling recommended for future research.

Abstract

First aid competence among adolescents is a crucial component of community-based disaster risk reduction, particularly in rural areas with limited access to emergency healthcare services. This study aims to evaluate the effectiveness of the First Aid Guidelines (FAG) mobile application and an indigenous-language short films in improving first aid knowledge, attitudes, and skills among junior high school students in a rural Indonesian setting. A quasi-experimental study without a control group involved 120 students who were selected through simple random sampling. Data were collected using validated questionnaires and simulation-based assessments. Statistical analysis employed the Wilcoxon and the Mann-Whitney U tests. Both the FAG application and the short film significantly improved students' first aid knowledge (p=0.001), with no significant difference in effectiveness between them (p=0.955). The FAG application significantly enhanced students' attitudes (p=0.001), while the short film did not (p=0.207). Conversely, the short film significantly improved practical first aid skills (p=0.001), but the application did not (p = 0.115). There were no statistically significant differences found between the two media in terms of attitude (p=0.955) and skill improvement (p=1.000). The use of both the FAG application and a culturally adapted short film was effective in improving rural adolescents' first aid knowledge. The FAG application was more impactful in shaping positive attitudes, but thee short film demonstrated greater efficacy in improving practical skills. The use of application-based educational technology and short films in school health curricula is recommended to enhance first aid skills in adolescents.

Abstract

Preeclampsia remains one of the leading causes of maternal mortality in Indonesia. Prevention efforts through nutrition education based on local foods are considered effective in reducing the risk. This study aimed to develop, assess the feasibility of the module through expert validation and evaluate the effectiveness of the CAMPE (Cara Mencegah Preeklamsia-Prevent Preeclampsia through Nutritional Management) Module for high-risk pregnant women in Takalar Regency, South Sulawesi. This study employed a Research and Development (R&D) approach with a quasi-experimental effectiveness test using a pretest–posttest control group design, involving 140 pregnant women (70 in the intervention group and 70 in the control group). Data were analyzed using paired t-tests to assess within-group pretest–posttest differences and independent t-tests to compare outcomes between the intervention and control groups. Validation results indicated that the module was highly feasible (mean score: 4.64). After the intervention, knowledge increased from 60.75 to 85.60 and attitudes from 70.25 to 91.40 in the intervention group. Statistical tests showed a significant difference (p<0.000) compared to the control group. In conclusion, the CAMPE Module proved to be effective and significantly improved the knowledge and attitudes of high-risk pregnant women regarding preeclampsia prevention. This module is appropriate for use as a local food–based nutrition education tool in primary healthcare services. Future multicenter randomized studies with longer follow-up periods are needed to evaluate long-term effectiveness and clinical outcomes.

Abstract

Delayed management of obstetric complications increases maternal and neonatal mortality rates. The Modified Obstetric Early Warning System (MEOWS) facilitates early detection, determines safe delivery, and improves neonatal outcomes, especially when combined with obstetric factors. This study aims to analyze the effect of MEOWS implementation on the delivery mode and neonatal outcomes, both individually and in combination with obstetric factors. This study used an observational analytical method with a cross-sectional design. Data were collected from the medical records of 281 patients who gave birth at Bhayangkara Brimob Hospital in 2024. Sampling used probability sampling with proportional stratified random sampling. The independent variable was MEOWS, the dependent variables were delivery mode and neonatal outcomes, while obstetric factors were considered confounding variables. Bivariate analysis used chi-square and Fisher's exact tests. Multivariate analysis used binary logistic regression. Bivariate results: MEOWS was linked significantly to neonatal outcomes, but not with delivery mode. Multivariate analysis: MEOWS had no significant effect on delivery mode and neonatal outcomes (p>0.05). Delivery Mode was significantly associated with parity (p = 0.012; OR 2.073) and history of previous delivery (p < 0.001; OR = 4.858). Meanwhile, neonatal outcomes were significantly associated with gestational age and parity (p<0.05; OR 45.49 vs OR 0.171). The application of MEOWS combined with obstetric factors improves the prediction of complications, guides clinical decisions, reduces unnecessary interventions, and contributes to better outcomes. Further research is recommended to develop an integrated risk prediction model to improve the accuracy of assessment and enhance safety for both mothers and newborns.
