Abstract

Background: The perception of adults about penis size and sexual satisfaction is crucial to enjoying all sorts of sexual relationships or intimacy. However, communication about how penis size influences sexual satisfaction has been consciously kept under the lid, leading to a series of worries such as prostitution, sexual deviations, infidelity, nagging, domestic violence and suicidal ideations. Aim: The study investigated the perception of adults (25 years and above) about penis size and sexual satisfaction. Also, the authors examined the perception of adults’ marital status and gender about penis size and sexual satisfaction. Methods: Using a descriptive survey research design, data were gathered from 221 respondents who were 25 years old and above. A simple random sampling technique was used to select adults for the study. The self-developed research instrument tagged “ PSSSQ” was used for data collection. The validity and reliability (r=0.68) of the research instrument were ensured by the researchers. Results: The findings revealed that moderate penis size is chiefly preferred for sexual satisfaction by adults. Conclusion: Given any penis size to get sexual satisfaction, each male or female involved in any sexual relationship, should mutually commit to the effective usage of any penis size that can enhance sexual gratification.

Abstract

Latar belakang: Diabetes mellitus tipe 2 (DMT2) adalah penyakit gangguan metabolik yang di tandai oleh kenaikan gula darah akibat penurunan sekresi insulin oleh sel beta pankreas dan atau gangguan fungsi insulin (resistensi insulin). Tujuan penelitian: Penelitian ini bertujuan untuk mengetahui hubungan physical activity berbasis health belief model dengan sikap dalam mengontrol kadar gula darah pada pasien DMT2 di wilayah kerja Puskesmas Polongbangkeng Utara Kabupaten Takalar. Metode penelitian: Desain penelitian yang di gunakan deskriptif analitik dengan pendekatan cross-sectional studi. Dengan jumlah sampel sebanyak 50 responden. Dengan tehnik pengambilan sampel purposive sampling. Hasil penelitian: Hasil analisis bivariat menunjukkan bahwa uji fisher’s exact test di peroleh nilai hitung p=0,000 lebih kecil dari nilai α=0,05. Dari analisis tersebut dapat di artikan bahwa Ha diterima dan Ho di tolak. Kesimpulan: ada hubungan physical activity berbasis health belief model dengan sikap dalam mengontrol kadar gula darah pada pasien diabetes mellitus tipe 2 di Kabupaten Takalar 2023.

Abstract

Latarbelakang: Congenital atrioventricular block (CAVB) merupakan kondisi langka, ditemukan pada sekitar 1 dari 20.000 kelahiran, sebagian besar disebabkan oleh autoantibodi maternal yang memasuki sirkulasi fetus melalui plasenta. Metode: Laporan kasus: seorang ibu hamil berusia 34 tahun, gravida 3 paritas 2 usia kehamilan 31 minggu dirujuk ke RSUD Koja dengan bradikardia janin. Pada pemeriksaan USG didapatkan CAVB derajat III. Pada USG, pasien mengalami polihidramnion dengan denyut jantung atrium 151 bpm dan denyut jantung ventrikel 67 bpm, direncanakan akan dirujuk ke RS Harapan Kita untuk penanganan lebih lanjut. Pemeriksaan fisik menunjukkan tanda-tanda takikardia dan deformitas Swanneck pada jari tangan, serta diduga rheumatoid artritis. Pada pemeriksaan laboratorium didapatkan hipertiroidisme subklinis dengan diabetes melitus gestasional. Pasien dikonsultasikan ke bagian penyakit dalam, direncanakan untuk pemeriksaan panel autoimun, namun hal ini terkendala oleh ketersediaan rumah sakit dan pendanaan pasien. Pasien ini tidak dapat dirujuk karena memutuskan untuk pulang atas permintaannya sendiri. Pasien datang ke RS Tipe C, dilakukan tindakan SC dengan indikasi fetal distress dan bayi meninggal pada hari kedua. Kasus ini menunjukkan beberapa kendala dalam diagnosis dan penatalaksanaan CAVB, seperti kendala ekonomi, budaya dan pendidikan dalam proses tersebut. Kesimpulan: Terdapat banyak tantangan dalam tatalaksana pasien hamil, termasuk dalam rujukan, diagnosis dan penatalaksanaan penyakit langka seperti CAVB banyak mengalami kendala yang menghambat upaya pelaksanaannya, sehingga perlu upaya bersama untuk memperbaikinya.

Abstract

Latar Belakang: Overactive bladder (OAB) merupakan sindrom yang mengganggu kualitas hidup, khususnya kualitas hidup wanita. Pilihan terapi farmakologis untuk OAB adalah agen antimuskarinik (80%), seperti solifenacin. Agen agonis beta-3 juga merupakan pilihan terapi farmakologis baru untuk OAB, seperti mirabegron. Inkontinensia urin urgensi merupakan gejala OAB yang paling mengganggu, sehingga perlu dilakukan penilaian apakah solifenacin dan mirabegron memberikan perbaikan gejala yang berbeda. Kasus: Seorang wanita berusia 50 tahun dengan diagnosis overactive bladder (OAB) dengan gejala urgensi, frekuensi berkemih 20 kali/hari, nokturia 3 – 4 kali/malam, dan inkontinensia urgensi tanpa tanda-tanda peradangan saluran kemih. Pembahasan: Pencarian literatur dilakukan melalui Google Scholar, Pubmed, Embase, clinicaltrials.gov dan Cochrane library. Ditemukan total 532 artikel dengan hanya dua artikel meta analisis yang dicocokkan dan dinilai. Validitas, kepentingan dan penerapan meta-analisis dianalisis menggunakan tinjauan sistematik alat Oxford CEBM. Meta analisis menunjukkan bahwa solifenacin dan mirabegron tidak memiliki perbedaan yang signifikan dalam hal jumlah rata-rata episode inkontinensia urgensi. Kesimpulan: Solifenacin dan Mirabegron mempunyai pengaruh yang sama terhadap jumlah episode inkontinensia urgensi pada pasien OAB.

Abstract

Exposure to blue light from digital devices such as smartphones, tablets, and computers has become a major concern for eye health. Blue light, with wavelengths between 400-500 nanometers, can penetrate deeper into the eye and potentially cause retinal damage. This article reviews various studies assessing the impact of blue light on eye health, including the risks of developing computer vision syndrome, myopia, and age-related macular degeneration (AMD). Although the use of blue-blocking lenses has been suggested as a preventive measure, its long-term effectiveness requires further investigation. Other strategies, such as screen time regulation and the use of blue light-reducing software, are also analyzed as part of efforts to mitigate the negative effects of blue light. The findings from this review indicate that while there is potential risk from blue light exposure, more research is needed to develop more effective preventive strategies and to understand its long-term impact on eye health.

Abstract

Background: Coronary Artery Disease (CAD) is a condition caused by the formation of blockages in the coronary blood vessels. The primary non-invasive procedure for CAD patients is Percutaneous Coronary Intervention (PCI). However, complications during PCI, such as unstable hemodynamic arrhythmias, can occur, possibly due to Abrupt Vessel Closure (AVC). This remains a major issue for PCI failure and necessitating Coronary Artery Bypass Grafting (CABG). Case: We report a 68-year-old male patient with CAD and total occlusion of the Right Coronary Artery (RCA) and total occlusion of the Left Circumflex (LCx) who was unable to undergo PCI at a previous hospital. Subsequently, re-catheterization at RSUP Dr. Sardjito, CAD3VD was diagnosed, leading to a planned CABG surgery. The patient’s clinical condition was relatively stable, though he had a low ejection fraction (41%). Induction, invasive monitoring placement, and intubation proceeded smoothly. CABG was performed with three grafts (LIMA-LAD, SVG-OM, SVG-PDA), and successful weaning was achieved with dobutamine support. The patient was in the ICU for 2 days for clinical and hemodynamic optimization before being transferred to the ICCU for further intensive care. Discussion: The main principle of anesthetic management in this case is to maintain a balance between myocardial oxygen supply and demand. Strict monitoring of hemodynamic changes during surgery is essential to guide necessary supportive therapy. Patients with low ejection fractions are at high risk for post-operative mortality and complications. Post-operative management in the ICU focuses on optimizing clinical condition and addressing any emerging potential issues. Conclusion: Surgery for patients with CABG requires complicated and complex anesthetic techniques. This operation requires collaboration and good communication between the surgeon and the anesthesiologist.

Abstract

Background: Indonesia ranks 6th highest with Diabetes mellitus (DM) in the world in 2019. There are complications occur due to DM type 2. To prevent this, fasting blood sugar (GDP) monitoring and patient diet by using pocketbooks as an educational compliance can be carried out. Objective: To identify the effect of using pocketbooks as an educational medium in monitoring dietary compliance and GDP levels of type 2 DM patients in Sumbang. Methodology: Quasi-Experimental method was used with pretest – posttest design on 100 DM Prolanis patients at Puskesmas Sumbang 1 and 2 which were divided into two groups. Both groups were given different education where the treatment group used a pocketbook while the control group was given oral education. Results: In the pretest-posttest data of GDP levels, there were differences in results before and after the intervention in the treatment group (Wilcoxon test, p = 0.000) while the control group gave no difference in results before and after the intervention (Wilcoxon test, p = 0.090). In the pretest – posttest diet adherence data, there were differences in results before and after the intervention in the treatment group (Paired T Test, p = 0.000) while the control group gave no difference in results before and after the intervention (Paired T Test, p = 0.074). The Mann Whitney test and the unpaired T test showed that the two variables had a significant difference (p = 0.000). Conclusion: there is an effect of pocketbook education intervention on GDP levels and dietary compliance of type 2 DM patients in Sumbang District, Banyumas Regency which reduces GDP levels and improves respondents' dietary compliance.

Abstract

Farming activities were one of the main occupations in Banyumas Regency. Pesticides were often used to exterminate pests that harmed agricultural yields, but their improper use could adversely affect body organs, including the liver. This research aimed to determine the relationship between types of pesticides based on their usage function and liver function in farmers in Sikapat Village, Sumbang District, Banyumas Regency. This study was an observational analytic study with a cross-sectional design. The study population consisted of farmers using pesticides in Sikapat Village, Sumbang District, Banyumas Regency, from March to October 2023. Farmers who were willing to participate in the study and were present during data collection were included as research subjects. The sample was taken using the purposive sampling method. Liver function was measured by examining AST and ALT levels, while pesticide use data were obtained using a questionnaire. Data analysis used the Chi-square test with Fisher's or Kolmogorov-Smirnov alternatives at a significance level of 0.05. Results showed that there was an increase in ALT and AST was only observed in the group of farmers using a combination of pesticides (p=0.259). The highest average ALT levels were reported in the use of combined pesticides (26.2 ± 12.9 U/L), followed by insecticides (21.4 ± 2.5 U/L), herbicides (19.4 ± 5.2 U/L), and fungicides (15.1 ± 1.1 U/L) (p=0.072). The highest average AST levels were reported in the use of combined pesticides (24.2 ± 5.3 U/L), followed by fungicides (21.8 ± 1.2 U/L), insecticides (21.7 ± 3.9 U/L), and herbicides (20.8 ± 2.6 U/L) (p=0.157). In conclusion, there was no relationship between the use of pesticide types and liver function in farmers in Sikapat Village, Sumbang District, Banyumas Regency.

Abstract

This research aims to determine and analyze the Community Satisfaction Index with Public Services at the Sentani City Health Center UPTD. This research is a type of descriptive research that uses Community Satisfaction Index (IKM) analysis. The population used in this research is all people who are receiving services at the Sentani City Community Health Center. The sample will be taken as many as 100 people from service users in April and May using a sampling technique, namely the Non-Probability sampling technique with accidental sampling type. Data collection techniques use observation, documentation and questionnaires. Processing and analyzing research data uses the Descriptive Statistical Analysis method. The results of research on the Community Satisfaction Index with Public Services at the UPTD Puskesmas Sentani Kota, based on the guidelines of the Regulation of the Minister for Empowerment of State Apparatus and Indonesian Bureaucratic Reform Number 14 of 2017 concerning guidelines for compiling Community Satisfaction Surveys have gone well. Where the results of the Community Satisfaction Index are obtained are seen from 9 indicators with IKM conversion value= ∑Weighted NRR × 25 (Conversion Value 3.271 × 25 = 81.79 with service quality "GOOD". Based on indicators of community satisfaction with services at the Sentani City Community Health Center, it was found that the highest indicator for having the highest IKM was Facilities and Infrastructure=3, 54 conversion value 88.50 "A" weight value in the "Very Good" category. Meanwhile, the lowest indicator value is Completion Time = 3.01 with a conversion value of 75.25 "C" weight value in the "Not Good" category.

Abstract

Anesthesia plays as an important role in the caesarean section procedure. The use of multimodal analgesia regimen has been considered as the gold standard of anesthesia for patients undergoing cesarean section. This study examines the comparison between the onset and duration of motoric and sensoric nerve blocks in patients undergoing cesarean section with spinal anesthesia using Bupivacaine Combination and Bupivacaine Only at Margono Soekarjo Hospital, Purwokerto. This research was a randomized controlled trial with double blind. Inclusion criteria in this study were patients aged 20-45 years, ASA physical status II and BMI between 18.5-35. Exclusion criteria in this study were patients with anatomical abnormalities or severe congenital abnormalities, contraindications to spinal anesthesia, history of allergy to the anesthetic agents used and unconsciousness patients. Subjects were divided into two groups, each group consists of 15 patients, Bupivacaine Combination (spinal anesthesia with Bupivacaine 7.5 mg + Fentanyl 25 mcg + Morphine 100 mcg) and Bupivacaine Only (spinal anesthesia with Bupivacaine 15 mg). Data were analyzed using Independent Sample T Test with a significance level of α=0.05. There were differences in onset of motoric block (p=0.00), duration of motoric block (p=0.00), onset of sensoric block (p=0.00) and duration of sensoric block (p=0.00) between Bupivacaine Combination and Bupivacaine Only groups. The Bupivacaine Combination group has a slower onset and shorter duration of anesthesia than the Bupivacaine Only group.
