Abstract

Stroke is one of the leading causes of death worldwide and causes a high financial burden. Malnutrition in stroke patients is associated with worse clinical outcomes. However, until now there is no universally agreed gold standard for establishing a diagnosis of malnutrition. The Controlling Nutritional Status (CONUT) score has been reported to have good sensitivity and specificity in detecting malnutrition in hospitals. In addition, the CONUT score has also been shown to have good ability in predicting clinical outcomes of stroke patients. Implementing CONUT Score in clinical practice may provide management consideration.

Abstract

Parkinson's disease (PD) is a rapidly growing neurological disorder with significant health implications, especially in low- and middle-income countries (LMICs) like Indonesia. This review analyzed the challenges and opportunities for managing PD in Indonesian primary care. The prevalence of PD in Indonesia has surged by 143% since 1990, reaching 89.91 per 100,000 people in 2019, which is likely underestimated owing to healthcare system limitations. Primary care is critical for early PD detection, although diagnostic accuracy is a concern. Enhancing the primary care capacity for early identification is essential through specialized training, telemedicine, and PD-specific assessment tools. Continuous monitoring of medication adherence, treatment efficacy, and symptom progression is vital for its effective management. Integrating palliative care principles and supporting caregivers is crucial for providing comprehensive care. This review recommends a structured approach to PD management in Indonesia, incorporating it into chronic disease programs, improving diagnostic tools, and developing caregiver support systems to address the growing PD burden and optimize patient outcomes in resource-limited settings.

Abstract

Hemodialysis (HD) is vital for managing patients with end-stage chronic kidney disease, ensuring their quality of life. Hospital X in Banyumas District, a private Type C hospital, has adopted single-use dialyzers to enhance safety and minimize infection risks. This study employs the Internal-External (IE) and SWOT matrices to develop strategies for improving the hospital's single-use HD services. Data were gathered through interviews, questionnaires, and hospital records, focusing on internal and external factors affecting HD services. The IE matrix indicates that the hospital is positioned for aggressive growth, suggesting strategies like market penetration, product development, and supply chain integration. The SWOT analysis identifies several strengths, such as BPJS support and a strategic location, while addressing weaknesses like limited promotional activities and dependency on vendors. Opportunities include government health policies and technological advancements, counterbalanced by threats such as economic instability and increased competition. Key recommendations include strengthening internal coordination, leveraging BPJS support, optimizing promotional strategies, and addressing vendor dependency through diversification. These strategies aim to enhance efficiency, attract more patients, and ensure competitive advantage. By aligning internal strengths with external opportunities, Hospital X can sustainably grow its HD services and improve overall healthcare delivery.

Abstract

Inflammation is a body defense process against various potentially damaging causes. Excessive inflammation in the vascular can cause an autoimmune process in vasculitis disease. Vasculitis can cause damage to the tissues and organs that are vascularized. So far, the heterogeneous pathophysiology and clinical manifestations of the disease have been well understood. Although therapy with glucocorticoids is effective for disease remission, complications that often occur earlier in patients can fail the therapy. Therefore, a more effective method for accurate diagnosis is needed as a patient management approach. A more detailed and interrelated understanding of the disease will further encourage the improvement of the quality of the medical procedures so that it is expected to improve the patient’s prognosis. This literature review aimed to explain detail started from the prevalence of vasculitis cases, followed by the underlying risk factors and the development of effective diagnostic and management flows to date. The method of this literature review was searching for articles from various databases in PubMed, Google Scholar, and the Google search engine. Based on various literature obtained, each classification of vasculitis has unique characteristics including the type of inflammation that occurs and the onset of the disease that differs for each age group. The diagnostic flow still starts from anamnesis to the use of imaging and laboratory examinations and the application of criteria for a definitive diagnosis. Further management is differentiated based on the type of vasculitis, treatment phase, and response to therapy. Further studies are needed, especially in determining the reference values ​​of laboratory examinations used for diagnosis, determining the dose of therapy, and efforts to treat complications that occur. Vasculitis remains a heterogeneous autoimmune disease that requires further attention in efforts to establish a diagnosis and apply the most appropriate management to the patient’s condition.

Abstract

Background: Breast cancer is one of the most common malignancies in women. In Indonesia, most cases are diagnosed at an advanced stage, which makes therapy success more challenging. Neoadjuvant chemotherapy is a commonly used initial approach to shrink tumor size before surgical intervention. However, not all patients show a good response to this therapy. One important mechanism in cancer progression is angiogenesis, which is regulated by growth factors such as VEGF-B. VEGF-B plays a role in blood vessel formation and is associated with more aggressive tumor characteristics. Therefore, VEGF-B expression has the potential to be used as a predictive biomarker for chemotherapy response. Methods: This study used a prospective cross-sectional design involving 30 patients with locally advanced breast cancer at RSUP Dr. Hasan Sadikin, Bandung. All patients received three cycles of neoadjuvant chemotherapy with the CAF regimen (cyclophosphamide, doxorubicin, and 5-fluorouracil), followed by modified radical mastectomy. VEGF-B expression was assessed through immunohistochemical examination of biopsy samples. Chemotherapy response was evaluated based on clinical changes in tumor size and classified as complete response, partial response, or no response. Data analysis was performed using Spearman's test, chi-square, and multivariate logistic regression. Results: The average age of the patients was 47.9 years. Two patients (5%) showed a complete response, 20 patients (50%) had a partial response, and 18 patients (45%) did not respond. Most patients had high tumor grade (52.5%). Moderate VEGF-B expression was found in 60% of patients, and strong expression in 40%. There was a significant relationship between age (p = 0.023), tumor grade (p = 0.027), and VEGF-B immunoexpression (p = 0.026) with chemotherapy response. Logistic regression analysis showed that VEGF-B expression (OR = 0.16; 95% CI: 0.03–0.82) and age (OR = 3.33; 95% CI: 1.30–8.60) were significant predictors of therapy success. Conclusion: High VEGF-B expression is associated with a decreased response to neoadjuvant chemotherapy. In addition, younger age also showed a tendency toward a lower response. VEGF-B may be considered a potential predictive biomarker in the early evaluation of locally advanced breast cancer patients to improve therapy personalization and treatment effectiveness.

Abstract

Background: The global increase in ultra-processed food (UPF) consumption has raised significant public health concerns due to its association with cardiovascular disease (CVD). UPFs are typically high in sodium, saturated fats, added sugars, and artificial additives, which contribute to metabolic disturbances and increase CVD risk. This scoping review aims to synthesize the current evidence on the relationship between UPF consumption and CVD risk in adults. Methods: A systematic search was conducted using PubMed and Google Scholar for studies published between 2019 and 2024. Eligible studies included observational research assessing UPF consumption and its impact on cardiovascular health. A total of 172 articles were initially identified, and after applying inclusion criteria, nine relevant studies were selected for review. Results: The findings consistently indicate that high UPF consumption is linked to an increased risk of hypertension, obesity, dyslipidemia, and elevated triglyceride levels—key contributors to CVD. Several studies also highlight a significant association between frequent UPF intake and higher rates of cardiovascular mortality, particularly among high-risk populations. Conversely, diets rich in minimally processed foods, such as whole grains, fruits, and vegetables, demonstrate protective effects against CVD. Conclusions: There is a strong positive correlation between UPF consumption and heightened CVD risk. Public health interventions should focus on reducing UPF intake while promoting healthier dietary patterns to mitigate cardiovascular disease burden. Further longitudinal studies are needed to explore the long-term effects of UPF consumption on cardiovascular health

Abstract

Human Immunodeficiency Virus (HIV) is an infectious agent that disrupts the immune system by destroying CD4+ T lymphocytes, thereby triggering a decline in immune function and the development of Acquired Immunodeficiency Syndrome (AIDS). HIV employs a complex infection mechanism, including binding to host cell receptors, integration of genetic material, and modulation of the immune system to evade detection. The immune response to HIV involves the innate immune system, such as the activation of macrophages and dendritic cells through pattern recognition receptors, as well as the adaptive immune system through the activity of cytotoxic T cells and antibody production by B cells. However, HIV utilizes various strategies to evade immune responses, including rapid mutation and down-regulation of immune molecule expression. Antiretroviral therapy (ART) is the primary strategy for inhibiting HIV replication and maintaining immune system function, and it has been proven effective in reducing the morbidity and mortality associated with HIV infection, although a curative therapy has yet to be discovered.

Abstract

Background: Cervical cancer is one of the leading causes of death in women, which can be prevented through early detection. However, the low level of knowledge and interest of Women of Reproductive Age (WUS) towards the IVA Test examination is a challenge in prevention efforts. This study highlights the novelty of using animated video-based educational media in increasing interest in the IVA Test examination in remote areas, which has rarely been studied. Research Objective: To analyze the effect of educational videos on increasing knowledge and interest of WUS in conducting the IVA Test examination in Muara Benangaq Village, West Kutai Regency. Method: Quantitative research with a pre-experimental one group pre-test-post-test design. The sample consisted of 48 WUS selected through a purposive sampling method. Data analysis used the Wilcoxon Rank test. Results: Before the intervention, a low level of knowledge was found in 38 respondents (79%), while after the intervention it increased to 32 respondents (67%) in the good category. Initial interest in the IVA Test was low in 33 respondents (69%), and increased to 38 respondents (79%) after the intervention. Statistical analysis showed a significant increase (p-value 0.000 for knowledge, p-value 0.021 for interest). Conclusion: Animated educational videos have been proven to be effective in increasing WUS knowledge and interest in the IVA Test, especially in areas with limited access to information. The results of this study can be a recommendation for multimedia-based health education strategies to support cervical cancer prevention programs.

Abstract

Background: Diabetes mellitus is a chronic disease characterized by increased blood sugar levels (hyperglycemia) and glucose intolerance caused by the pancreas not producing enough insulin or the body being unable to use the insulin produced effectively. Objective: to determine the effectiveness of yoga therapy education on knowledge and attitudes in T2DM patients in Takalar Regency. Method: This research uses an experimental approach with a one-group pre-post test design. The number of respondents was 30 using the Accidental Sampling sampling method. The statistical test used is the paired sample test with ρ<0.05. Results: In this study, pre-intervention knowledge results were obtained with a mean of 5.47, and mean post-intervention knowledge increased to 8.47 with a value of ρ=0.001, while pre-intervention attitudes had a mean of 20.20, while post-intervention attitudes the mean increased to 26.83, with a value of ρ=0.001. Conclusion: there is an influence of yoga therapy education on the knowledge and attitudes of T2DM patients in Takalar Regency.

Abstract

Hypertension is a condition where blood pressure is higher than normal, and one of its causes is increased inflammation and oxidative stress. Moringa leaves contain high levels of antioxidants, such as flavonoids, which can help control blood pressure as an effort to prevent complications from hypertension. The study aimed to determine the effect of moringa leaf tea consumption on blood pressure in hypertension sufferers among Prolanis participants at the Sidabowa Primary Clinic, Patikraja District, Banyumas Regency. An analytical observational study with a case-control design, which was selected through total sampling. Data analysis was performed using the Independent T Test. The respondents of this study numbered 41 individuals. The results of this study showed the significance values of the blood pressure changes between the two groups: systolic blood pressure 0.146 (p>0.05) and diastolic blood pressure 0.258 (p>0.05), which means there was no effect of moringa leaf tea consumption on blood pressure in hypertension sufferers among Prolanis participants. Consumption of moringa tea did not change the blood pressure of hypertensive patients in prolanis participants of Sidabowa Primary Outpatient Clinic, Patikraja District, Banyumas Regency after consuming moringa tea for 14 days. However, the decrease in systolic blood pressure was greater in the case group.
