Abstract

Background: Developing herbal medicines for antibacterial activity has been challenging in Indonesia. Of the alternative antimicrobials, areca nut (Areca catechu L) has antimicrobial activity since it contains flavonoids, tannins, phenols, alkaloids, saponins, terpenoids, and glycosides. The use of antibiotics in underdeveloped and developing countries is still irrational, and bacterial infection in Indonesia is still a problem to resolve, so searching for new herbal medicines is very important. Staphylococcus aureus is the most common normal flora that causes infection. The two types of Staphylococcus aureus are Methicillin-resistant Staphylococcus aureus (MRSA) and Methicillin-sensitive Staphylococcus aureus (MSSA). The study aimed to determine the efficacy of the antibacterial activity of areca nut ethanol extract against MRSA and MSSA isolates. Methods: The research was conducted at the Biomedical Laboratory, Faculty of Medicine and Health Sciences, University of Jambi. Young and old areca nut ethanol extracts were macerated using 96% ethanol. The antibacterial activity test of the ethanol extract was carried out using the Kirby-Bauer disc diffusion method against the isolates of MRSA (clinical isolates) and MSSA (SA 25923/K+, SA 29213/K-). Results: The results showed that the phytochemicals of young areca nut extracts contained alkaloids, flavonoids, phenols, and steroids, and old areca nut extracts contained alkaloids, flavonoids, and phenols. The MRSA isolates did not show the activity of the two areca nut ethanol extracts and were also resistant to Ceftadizime, Gentamicin, Ceftriaxone, and Cefoxitin. The MSSA isolates showed strong activity against old areca nut ethanol extract, moderate activity against young areca nut ethanol extract, and resistance to gentamicin. Conclusion: MRSA bacteria are resistant to the ethanol extract of young and old areca nuts, while the ethanol extract of old areca nuts has strong antibacterial activity against MSSA. Abstract

Background : The incidence of prehypertension in adults has a threefold risk of developing hypertension and triggering heart disease when compared to other ages population. This study aimed to correlate the intake of advanced glycation end products (AGEs) with the incidence of prehypertension in adults in Kota Jambi. Method : A cross-sectional study using the correlation method with a sample of 134 people aged 21 – 59 years who came for treatment at the Tanjung Pinang Public Health Center, Kota Jambi and sample selected by purposive sampling. Data collected by using a questionnaire and analyzed by Spearman rank. Results : a strong relationship between AGEs intake and prehypertension (p = 0.000; r: 0.725) Conclusion AGEs intake has a correlation with the incidence of prehypertension in adults.

Abstract

Background: Fenofibrate is an antihyperlipidemic belonging to BCS class II, practically insoluble in water and has high lipophilicity, so it is proven that the inhibitor of the rate of absorption of fenofibrate from the digestive tract is slow to dissolve. This study aims to increase the solubility of fenofibrate by forming nanocrystals. Method: Formation of nanocrystals using the wet milling method Result: The fenofibrate nanocrystals produced a particle size of 775.966 nm, a zeta potential of -21.302 mV, and a solubility of 5.977 μg/mL. Conclusion : The optimum formula with tween 80 components, grinding speed and grinding time in the manufacture of fenofibrate nanocrystals using the Factorial Design method obtained 0.2% tween 80, grinding speed of 500 rpm and grinding time of 1 hour. The critical parameter test results for the optimum formula obtained a particle size of 775.966 nm, a zeta potential of -21.302 mV and a solubility of 5.977 µg/mL. The DSC test showed that there was a difference in the melting point peaks and a decrease in crystal intensity between pure fenofibrate and fenofibrate nanocrystals. The FTIR test showed no difference in functional groups between pure fenofibrate and fenofibrate nanocrystals. The XRD test showed that there was a difference in peak crystal intensity between pure fenofibrate and fenofibrate nanocrystals. The SEM analysis shows that there are differences in shape and magnification used between pure fenofibrate and fenofibrate nanocrystals.

Abstract

Background: Based on patient safety incident data, the Ministry of Health, it is known that incident cases that occurred in the Special Region of Yogyakarta Province were 13.8%. Based on Patient safety’s policy in Indonesia namely PMK No. 11 of 2017 concerning patient safety states that every health service facility is required to have an information system that is used to improve patient safety, including the health center. The purpose of this study was to evaluate the use of the patient safety incident information system at the Gondomanan Health Center by using the Health Matrix Networks. Methode: This study uses a qualitative descriptive method with a case study design. Data collection used interview guidelines with 5 informants, namely the head of the health center, the patient safety team (1 doctor, 1 midwife, 1 nurse) and the dental polyclinic service coordinator (1 poly nurse). The sampling technique used purposive sampling. Result: There are 6 Health Matrix Network (HMN) tools that are used to measure the evaluation of patient safety information systems, namely information system resources, information system indicators, data sources, data management, CIS information products, and dissemination and use of SIK information. In terms of resource attributes or variables (policies, human resources, facilities and infrastructure and finance), health indicators, data sources, information products and the use of IKP reporting information are in accordance with the patient safety incident reporting guideline policy, and PMK No. 11 of 2017. Meanwhile, the attributes of data management and information products in the patient safety reporting information system have not been implemented optimally. Conclusion: The information system for reporting patient safety incidents at the Gondomanan Health Center requires improvement in procuring special SOPs for internal reporting of patient safety incidents which are part of the data management attributes.

Abstract

ABSTRACT Background: Chronic wounds are a challenge for wound care professionals and a global economic burden that has increased morbidity and mortality in patients worldwide. Purpose: This study aims to determine the efficacy of wound cleanser with guava leaf extract (Psidium guajava Linn) and betel leaf extract (Piper betle L) 25% on the inhibition of bacterial growth in chronic wounds. Methods: The research conducted was laboratory experimental in nature. Guava leaf extract (Psidium guajava Linn) and betel leaf extract (Piper betle L) were formulated in the form of liquid soap with a concentration of 25% then evaluation of the physical preparations, allergy tests and bacterial tests were carried out in the form of the inhibition of liquid soap against Pseudomonas aeruginosa bacteria, Staphylococcus aureus, Escherichia coli in chronic wounds, Bacillus subtilis, Bacillus cereus, Staphylococcus epidermidis and Yeast Candida Albicans. Data collection is carried out after passing the ethical test. Results: This proves that liquid soap preparations of ethanol extract of guava leaves (Psidium guajava Linn) and betel leaf extract (Piper betle L) have medium, strong and very strong inhibition against Pseudomonas aeruginosa (12.28 mm), Staphylococcus aureus (19.48 mm), Escherichia coli (13.94 mm), Bacillus subtilis (27.58 mm), Bacillus cereus (11.11 mm), Staphylococcus epidermidis (14.44 mm) and category of respond medium inhibition on Yeast Candida Albicans (8.13 mm). Conclusion: Based on the results of the study, it can be concluded that the formulation of liquid soap preparations of ethanol extract of guava leaves (Psidium guajava Linn) and betel leaf extract (Piper betle L) with a concentration of 25% fulfills the quality requirements for liquid soap preparations. The results of this liquid soap antibacterial test had a strong response in inhibiting the growth of the bacteria Pseudomonas aeruginosa, Staphylococcus aureus, Escherichia coli, Bacillus cereus, Staphylococcus epidermidis. Abstract

Background : Health workers are a group with a very high risk of exposure during the treatment process of covid-19 patients. Many nursing staff have mental health disorders, as they not only bear the overload, high risk of infection, and fatigue. Many factors are the cause of anxiety experienced by health workers during the Covid-19 pandemic such as age, gender, education, employment, and marital status. The purpose of the study was to find out the factors that influence nurse anxiety in handling covid-19. Method : Design survey research with correlational study type using cross sectional time approach. The samples in this study were all nurses who treated covid-19 patients at who were 53 nurses with total sampling techniques. Results : Research instrument using Zung Self Rating Anxiety Scale questionnaire with data analysis using spearman-rank. The results showed nurse anxiety in handling Covid-19 mostly in the mild category (60.4%). Conclusion : There is a relationship between age(p value: 0,0001) and working period(p value: 0,002) factors with nurse anxiety in handling Covid-19 (p value < 0.05). There is no relationship of gender factors, education, and marital status with nurse anxiety in handling Covid-19 (p value > 0.05).

Abstract

Background Fishermen's work accidents are a problem of work safety in the world, including in Indonesia. Sometimes the weather is extreme, increasing the risk of accidents and death for fishermen. (Objective) The research objective was to determine the relationship between knowledge, attitudes, PPE, and machine availability tools for fishermen in Tanah Choose Village, Banyuasin District, Banyuasin Regency in 2022. Method: This research is a quantitative study with a cross-sectional design. This research method is interview and observation. The results of the survey were analyzed invariably and bivariately using the Chi-Square statistical test. Results 56.2% of respondents experienced work accidents, 20.8% of respondents had good knowledge, 52.1% of respondents with a positive attitude, 50% of respondents used complete PPE, and 33.3% of respondents used machine safety devices. Bivariant results show that there is no relationship between knowledge and work accidents (value = 0.614), there is no relationship between attitude and work accidents (p-value = 0.687), there is no relationship between PPE and work accidents (p-value = 1.000), there is no relationship between the availability of machine safety devices and work accidents (p-value = 1,000) Conclusion: It is hoped that fishermen will improve their attitude, use complete PPE when working, and use safety devices on every work machine and equipment.

Abstract

ABSTRACT Background: Fetal distress is one of the causes of fetal death in the world, according to the World Health Organization (WHO) in 2015, data for the last 5 years amounted to 637,000 deaths caused by fetal distress. The incidence in Indonesia is that 34.7% of 100,000 live births experience fetal distress. Fetal distress is an emergency case of pregnancy in the fetus. Fetal distresss causes fetal hypoxia so that the fetus requires immediate oxygen supply. Objective: to determine the characteristics of the respondents, fetal heart rate before and after administration of oxygen, the effect of giving oxygen to in-partum patients with fetal distress at Panti Nugroho Hospital, Purbalingga. Method: using a pre-experimental design type of one group pretest posttest. The research sample was pregnant women in childbirth with fetal distress by accidental sampling technique, namely 30 respondents. Observation sheet containing mother's identity, mother's pregnancy status, gestational age and FHR value before and after administration of oxygen. Data analysis used bivariate analysis, namely the Wilcoxon test. Results: 20% showed an improvement in fetal heart rate of 10-20 beats/minute, 46% had an improvement in fetal heart rate of 20-30 beats/minute, while 33.3% had an improvement in fetal heart rate of more than 30 beats/minute. There is an effect of giving 4 liters/minute of oxygen to inpartu patients with fetal distress with a p value of 0.00 (p>0.05). Conclusion: Administration of nasal cannula oxygen 4 liters/minute can affect the fetal heart rate. Key words: labor, administration of oxygen, fetal distress

Abstract

Background : Vitamin D deficiency is a common health problem in the world, even in tropical countries such as Indonesia. The Lack of vitamin D intake from food or supplements can increase the risk of having vitamin D deficiency. The association between vitamin D and sleep quality has been proven by the discovery of vitamin D receptors in the brain and brainstem, which are the sleep centers of the body. Vitamin D also takes part in the process of light transduction and melatonin production, that affects circadian rhythm and sleep quality. Poor sleep quality often causes health problems which will affect the quality of life, work productivity, and increases the risk of death. The purpose of this study is to determine the association between vitamin D intake and sleep quality index within the 17-35 year age group.Method : This study is an observational analytic study using a cross-sectional approach. Data was collected using consecutive sampling methods. Vitamin D intake was measured by the Vitamin D Estimation Only - Food Frequency Questionnaire (VIDEO-FFQ) and sleep quality index was measured by the Pittsburgh Sleep Quality Index (PSQI). Data analysis is proceeded using the chi-square method.Results : The total of respondents were 119. 68,1% of the respondents had inadequate vitamin D intake and 80.4% of the respondents had poor sleep quality index. The analysis shows that there was no significant association between vitamin D intake and sleep quality index (p value = 0.325). Conclusion : Most of the respondents had inadequate vitamin intake and poor sleep quality index. There was no significant association between vitamin D intake and sleep quality index within the 17-35 year age group. Abstract

Complications that are often experienced by patients with chronic kidney failure undergoing hemodialysis are sleep disturbances. Sleep disturbances are experienced by at least 50-80% of patients undergoing hemodialysis > 12 months. The purpose of this study was to determine the relationship between duration of hemodialysis therapy and insomnia in patients with chronic kidney failure at Arifin Achmad Hospital, Riau Province. The research method is a cross sectional approach. The population of this study were all 58 patients undergoing hemodialysis at Arifin Achmad Hospital. The sampling method was purposive sampling. The measuring instrument used in this research is the Insomia Severity Index (ISI) questionnaire. Statistical test using Chi Square test. Results Most of the levels of mild insomnia were 26 respondents (44.8%), moderate insomnia was 21 respondents (36.2%), not clinically significant as many as 8 respondents (13.8%), and severe insomnia was 3 respondents (5.2%). The results of statistical tests obtained p value of 0.487 > (0.05). It can be concluded that there is no relationship between undergoing hemodialysis therapy with insomnia in patients with chronic kidney failure at Arifin Achmad Hospital, Riau Province. Suggestions for further research that can be expected to examine the factors associated with insomnia in patients with chronic kidney failure.
