Abstract

Background : Acute central nervous system infection can be rapidly progressive, causing death or permanent damage in a short period of time. The diagnosis and knowledge of possible etiological agent is critically important. We present a case of a 15-month-old girl with generalized seizures and ecchymosis suspected for meningococcal infection but there was no proof of the agent of infection.Case report : A 15-month-old girl was admitted with fever of 39,4°C, generalized seizures, and petechiae after suffered high fever and cough in previous day. This was her first episode of seizures. She has no past history of growth and developmental disorder. She became rapidly deteriorated after few hours being hospitalized in Kariadi. Brain CT-scan was not done due to her conditions. She had anemia, normal white blood count, monocytosis, thrombocytopenia (4000/mm3), low CD4 count (99 cell/mm3), hipoalbuminemia (1.4 g/dl), increased lactate (2.5 mmol/L) and procalcitonin (>200.00 ng/ml). Cerebrospinal fluid analysis revealed normal cells count and glucose but increased protein level (375.2 mg/dL). Meningococcal infection was suspected due to clinical appearance, and ceftriaxone was given. Bacteriological and fungal culture of CSF and blood showed no growth. Despite of cardiorespiratory support given, she died after 4 days of admission.Discussion : Features of seizure and general ecchymosis do not belong exclusively to meningococcal infection. There are other possible etiological agent such as Haemophylus influenza, Streptococcal infection and viral infection. Shortage of available diagnostic tools such as Polymerase Chain Reaction (PCR) made it difficult to confirm the etiological agent (Sains Medika, 4(2):204-209).Question Remarks : What agent which you think cause the infection?

Abstract

Introduction: Injection of intraperitoneal gentamicin at a dose of 60 mg/kg BW intra-peritoneal in rats for 7 days has been shown to trigger renal tubular degeneration and increase in high sensitivity C-reactive protein (hs-CRP) level. The use of purslane (Portulaca oleracea) to reduce hs-CRP levels and total score of renal tubular degeneration has not been reported.Objective: to determine the effect of purslane ethanol extract on hs-CRP levels and renal tubular degeneration score in gentamicin induced-renal damage in rats.Methods: This was a study using posttest only control group design. Twentyfive male Wistar rats aged 8-12 weeks, weighed 150-200 grams were allocated into 5 groups: normal (Nor-G), without treatment; negative (Neg-G), gentamicin (60 mg/kg BW), intraperitoneally; P-200; P-300 ; P-400 (gentamicin plus purslane extract at a dose of 200, 300, and 400 respectively) for 7 days. The hs-CRP was evaluated using the ELISA method. Total score of renal tubular degeneration was evaluated using modified score of Sarjadi.Results: there was a significant difference in mean hsCRP level and total score of renal tubular degeneration between groups (p <0.05). Posthoc analysis showed that hsCRP level and total score group of P-200, P-300 and P-400 were significantly lower than those of Neg-G, p <0.05. Meanwhile, the levels of hsCRP and total score of renal tubular degeneration in P-200, P-300, P-400, and Nor-G groups were not significantly different (p> 0.05).Conclusion: the administration of purslane ethanol extract at doses of 200, 300, and 400 mg/kg BW for 7 days improve hs-CRP level and total score of tubular degeneration similar to normal.

Abstract

Background: Various diseases depend on individual’s immunity. Many of which nowadays have not had the effective and formal therapy that makes experts researches the best and the most effective way for it; the use of Propolis as immunomodulator is one of them. This research aims for knowing the effect of propolis towards peritoneal macrophage phagocytic index on mice.Design and Method: This research is an experiment with post test study design randomized control group design. This study used mice that were divided into four groups randomly. The first group / Group-I were for negative control (standard feed and aquadest); The second group/Group-II were fed standard-feed, aquadest, and propolis at a dose of 1.25 mg/kgBM; the third group/group-III were fed standard-feed, water, and propolis at a dose of 2.5 mg/KgBM; The fourth group/Group-IV were fed standard feed, water, and propolis at a dose of 5 mg/KgBM. Treatment where conducted for 3 days.Result: The average macrophage phagocytic index, were at the highest level of it (7.82 1.63) while the lowest one were the first group 3.43 0.13. The Kruskall Wallis result stated that there is index difference among various groups with p 0.002 (p < 0.05).Conclusion: Propolis effected on mice peritoneal macrophage phagocytosis index (Sains Medika, 3(2):121-128).

Abstract

Background: Bacterial skin infection is the third most common health problem in Indonesia resulting from poor personal and environmental hygiene. The most common bacterial skin infection is the pyoderma. This study aims to find out relation between personal hygiene and environmental with the incidence of pyoderma in RSI Sultan Agung Semarang.Design and Methods: The study type was analytic observational with case control design. The sample consisted of 30 respondents RSI patients Sultan Agung for case group and 30 persons as control group is the neighbors of patients who have similar characteristics and are not suffering pyoderma. The data used are secondary data from medical records and primary data from questionnaires filled out by respondents, then the data were analyzed with chi-square and to determine the correlation power there was used a contingency coefficient test.Results: It was found that the good and the bad individual hygene for the case group were 3.3% and 66.7% respectively, while for the control group the good and bad individual hygene were 80.0% and 20.0% respectively. Chi-square test resulted in p=0,000 with contingency coefisien of 0.426. It was found that the good and the bad environmental hygene for the case group were 56.7% and 43.3% respectively, while for the control group the good and bad environmental hygene were 83.3% and 16.3% respectively. Chi-square test resulted in p=0,024 with contingency coefisien of 0.27.Conclusion: There was a significant correlation between the personal and environmental hygiene and poderma insicence at the RSI Sultan Agung with a moderate relationship between individual hygiene and weak relationship between environmental hygene (Sains Medika, 3(1):24-30).

Abstract

Introduction: inflammation has been shown to play a major role in the pathogenesis of cancer. Inflammatory process activates the immune system through pro-inflammatory mediators and subsequent triggers transformation into malignant cells. Some tumors or cancers has been associated with chronic infections, such as hepatitis B and C viruses (hepatocellular carcinoma), human papilloma virus (cervical cancer), Helicobacter pylori (gastric cancer and lymphoma), and prostatitis (prostate cancer). A considerable study have investigated the benefits of aspirin for the prevention and treatment of cancer or tumors. Objectives: This paper aims to describe the relationship between inflammation and cancer incidence, so that use of aspirin as an anti-inflammatory agent is a rational choice in the treatment and prevention of cancer. Conclusion: Aspirin potential for chemoprevention of various types of cancer. Considering the high risk of side effects of aspirin, aspirin is not intended as a routine therapy to prevent the occurrence of cancer.

Abstract

Introduction : One of the free radicals that can damage facial skin is coming from UV radiation of the sun. Free radicals are the main factors that can accelerate the early aging process. The administration of antioxidants are believed to increase immune system of the body especially to prevent skin aging.Objectives: to determine the antioxidant activity of the corn silk extract and cream formulation. Methods: Corn silk were extracted by maceration using 80% ethanol for 5 days. Determination of antioxidant activity was conducted using 2,2-diphenil 1-pichylhydazyl (DPPH). Optimization of the preparation formula using expert design by varying the concentration of tween 80 and spam 80. The evaluation of cream preparation included homogeneity, organoleptic, pH, adhesion, dispersive power and power irritation to the skin.Results: IC50 corn silk extract were obtained 30,03 ppm with AAI(antioxidant activity index) values of 3,33 ppm. From the results of evaluation of 8 formula indicates a homogenous cream preparation, evaluation of the organoleptic have a brownish-white color and does not become rancid, pH 6.5, adhesion range of 0.12 to 0.5 seconds, dispersive power from 5.1 to 7.3 cm and power irritation of the skin that show the results slightly irritating. Formula optimum dosage creams containing extracts of corn silk 0.0315%; propylene glycol 2.1%; 4.5% tween 80; Span 80 5.5%; sorbitol 20%; 5% stearic acid; VCO 20%; citric acid 0.7%; ascorbic acid 0.06%; Triethanolamine 2.45%; methyl paraben 0.25%; propyl paraben 0.15% and add distilled water 100ml.Conclusion: Corn silk extract (Zea mays L) has a very strong antioxidant activity and for the optimum cream formulation has comparison between emulsifier tween 80 and span 80 is 4.5% to 5.5%.

Abstract

Introduction: Ipomoea batatas L (LIP) contains antioxidants beta-carotene, vitamin C and vitamin E. Cigarette smoke containing ROS has been proven to decrease sperm quality.Objective: this study aimed to confirm that orange sweet potato extract increases sperm consentration, motility, and viability in mice exposed to cigarette smoke.Methods: In this experimental study with post test only control group design, 30 male mice were divided into 6 groups. Normal group (Nor-G), no intervention, served as the normal control group. Negative control group (Neg-G) was only exposed to cigarette smoke. Positive control group was only treated with IPL 16mg/ml. IPL-15, IPL-16, and IPL-17 group were treated with IPL 15, 16, and 17mg/ml and exposed to cigarette smoke, respectively.Results: Post Hoc LSD analysis showed that means of sperm concentration and viability in IPL-15, IPL-16, and IPL-17 group were significantly higher compared to that of Neg-G (p < 0.05) and were lower compared to that of Nor-G and Pos-G (p < 0.05). Mann Whitney analysis indicated that the persentage of sperm motility in IPL-15, IPL-16, and IPL-17, were significant higher compared to that of Neg-G, p < 0.05. Whilst compared to that of Nor-G and Pos-G the persentage of sperm motility in IPL-15, IPL-16, and IPL-17, there was no significant differences, p > 0.05.Conclusion: treatment of IPL capable of increasing sperm concentration, motility, and viability in male mice were exposed to cigarrette smoke.

Abstract

Introduction: A periodical database is important including for skin cancer. Periodical registration is needed to acknowledge changes in pattern and frequencies of skin lesion. Objective: The purpose of this study was to describe the pattern and the frequency of skin lesion in RSUD Kariadi.Method: A cross-sectional study was conducted through analysis of the medical records of patients diagnosed skin lesion in the pathology labolatory of RSUD Kariadi between 2008 and 2009. The variables were secondary data including age, gender, specimen area, dan histopathology diagnosis. Data was choosen by consecutive sampling from 381 medical records of skin tissues examined at laboratorium of pathology anatomy of Dr. Kariadi general hospital during 2008-2009.Result: 381 cases were recorded comprising of 246 (65%) neoplastic and 135 (35%) non neoplastic lesion. 120 patients presented with skin cancer, and 126 with benign skin lesion. Most malignancy was observed among female patients (62.5%) on age catagory of 15-39 (65%). The most common lesion was basal cell carcinoma (48.3%) followed by squamous cell carcinoma (33.3%), malignant melanoma (10%), skin appendix carcinoma (2.5%), other malignancies (4.9%).Conclusion: the most common malignancies in Dr. Kariadi general hospital before 2008 was similar to data from 13 laboratory of pathology anatomy in Indonesia, which is squamous cell carcinoma.

Abstract

Introduction: Good implementation of hand hygiene and the availability of cheap, affordable, and effective hand rub can prevent healthcare-associated infection. WHO has issued hand rub formulations that are easily self-produced, effective, and affordable. Objective: This study compares the difference between handrub’s effectiveness stored in National Hospital Diponegoro University ER, HCU, and storeroom for 2 and 10 weeks.Methods: This study was an experimental study with pretest posttest randomized group design. The effectiveness of hand rub measured by prEN12054 in cfu/ml. Results: There was no significant difference between efficacy of WHO formula A and B (p=0.458). In the Mann-Whitney test there was no difference between the effectiveness of WHO A and B formulas before storage (p = 0.567), after being stored for two weeks (p=1.000), and ten weeks (p=0.539). In the Kruskal-Wallis test, there was no difference in the effectiveness of the WHO A formula in three sites at weeks 0, 2 and 10 (p = 0.275, 0.584, 0.116), there was no difference in the effectiveness of the WHO B formula in three sites at week 0, the 2nd and 10th (p = 0.289.; p = 0.584, p = 1.000).Conclusion: No significant differences were found on the effectiveness of the WHO A and B formulas. There was no significant difference in the effectiveness of WHO A or B formulas stored in three sites. There were no significant differences in the effectiveness of WHO A or B formulas, before and after being stored for two and ten weeks in three sites.

Abstract

Provide financial guarantees for service delivery (Jampersal), the government established the program with the Regulation of the Minister of Health Minister Regulation No. 2562/ Menkes/ Per/XII/2011 on Technical Guidelines for Delivery Guarantee. This study was conducted to determine the factors that affect the implementation Permenkes 2562/Menkes/Per/XII/2011 Number of Technical Guidance Delivery Guarantee in Semarang Independent Midwives Practice. The method approach used in this study is a qualitative sociological research juridical, Specifications. This is a descriptive research study. Technical factors such as health facilities are factors that affect the implementation of the labor warranty. Delivery Guarantee service is good and the maximum is influenced by many factors such as health workers. Health personnel especially midwives are the main factors in service delivery assurance. Provisions of medicines in service delivery assurance in Independent Practice Midwife Semarang are still having problems. Jampersal huge cost on hospitals and midwifery practices vary independently. Juridical factors, among others, have made the law of warranty service delivery as a midwife practice internal self- regulation, which includes service procedures, assignment of medical personnel. The scope of midwifery includes antenatal care, maternity (high risk), childbirth, newborns, family planning, and management of complications in obstetrics.
