Abstract

Background: Adolescence is a transition period that is vulnerable to changes in emotions and behavior, including anxiety and smoking behavior. Indonesia is the 5th country with the highest smoking rate in the world, in 2023 the Indonesian health survey states that more than 70 million smoke.  In Lampung in 2022 there will be 87.01% smokers. Purpose: To find out the relationship between anxiety levels and smoking behavior among teenagers. Method: This type of research uses a quantitative research method with a cross-sectional approach, total sampling technique. The analysis in this study is univariate and bivariate analysis with the chi-square test and Spearman's rho. The population and sample in this study were all male students in grades X-XI at Senior High School Persada Bandar Lampung, totaling 49 students. Results: The level of anxiety of the majority was in the moderate category, namely 17 respondents (35%) and for smoking behavior, the majority were in the high category, 17 respondents (35%). After the chi-square test was carried out, the p-value was obtained at 0.000 <0.05 and the Spearman's rho test obtained a correlation coefficient value of 0.954, meaning that there was a relationship between the level of anxiety and smoking behavior and the correlation between the two variables was very strong. Conclusion: There is a significant and very strong relationship between anxiety levels and smoking behavior. It is hoped that schools can develop educational programs in the form of anxiety and smoking behavior prevention programs through group counseling, health education about the dangers of smoking and anxiety management training.   Keywords:  Adolescents; Anxiety; Smoking Behavior.

Abstract

Background: Indonesia has diverse disaster potential based on its location and Lampung Regency, Lampung Province, recorded 31 disasters from January 1 to August 2023. The latest data for 2024 shows three tornadoes and 15 earthquakes. Adolescents are a vulnerable community when disasters occur. This situation must be anticipated by providing educational videos on tsunami mitigation. Purpose: To determine psychological preparedness of adolescents for disasters. Method: This research was quantitative, using a pre-experimental method with a one-group prctest-posttest approach. The population in this study was 81 adolescents aged 10-19 sampling. Data analysis used univariate and bivariate methods using dependent t-tests. Results: The average knowledge of adolescents about tsunamis before being given educational digital video learming media was 8.3. The average knowledge of adolescents mean of 13.23. The results of data analysis indicate an effect of digital video leaning media on tsunamis on adolescents knowledge, with a mean of 4.700 and a p-value of 0.000<0.05. Conclusion: There is an effect of digital video learning media on adolescents knowledge in can provide health education about the dangers of tsunamis, through educational videos or animated disaster response simulation videos.   Keywords: Adolescents Knowledge; Digital Media Video; Psychological Preparedness; Tsunami.

Abstract

Background: Anxiety is one of the most frequently reported psychological problems among adolescents, particularly among Vocational High School students who are required to meet both academic and career readiness demands. Mindfulness has been widely recognized for its effectiveness in reducing anxiety across various populations; however, empirical studies focusing on Indonesian vocational education settings remain limited. Purpose: To evaluate the effect of mindfulness intervention on anxiety levels among students and to identify variations in effectiveness based on gender and baseline anxiety levels. Method: A quasi-experimental pre-test–post-test design with a control group was employed. Participants were selected through purposive sampling, and anxiety levels were measured using the Hamilton Anxiety Rating Scale (HARS). Statistical analysis was conducted using the Wilcoxon test and Two-Way ANOVA. Results: A significant decrease in anxiety levels was found in the intervention group (p < 0.001), whereas no significant change occurred in the control group. Effectiveness varied according to gender and initial anxiety level. Conclusion: Mindfulness intervention is effective in reducing anxiety among students and may yield greater benefits when tailored to individual characteristics.   Keywords: Adolescents; Anxiety; Mental Health; Mindfulness; Vocational Education.

Abstract

Background: Schizophrenia is a chronic mental disorder that significantly affects cognitive function and behavior, frequently characterized by prominent delusions in Paranoid Schizophrenia. Delusional nursing problems are marked by severe distortions of reality perception, which impair patients’ ability to think logically and interact appropriately with their environment. Purpose: To describe and evaluate the implementation of Reality Orientation Therapy (ROT) in addressing delusional nursing problems in a patient with Paranoid Schizophrenia. Method: This research employed an intensive descriptive case study conducted over four days to illustrate and assess the application of Reality Orientation Therapy in Mrs. Y at Liliy 4 Ward, Seorojo Hospital. The primary intervention focused on the consistent, non-confrontational application of ROT through orientation to person, place, and time, integrated with collaborative pharmacological management. Results: The findings demonstrated that Reality Orientation Therapy was substantially effective in correcting cognitive distortions and enhancing reality-based thinking abilities. This improvement was evidenced by significant clinical progress and increased scores in reality-oriented thought content, indicating that the delusional nursing problem experienced by Mrs. Y was successfully resolved. Conclusion: Reality Orientation Therapy can be considered an effective nursing intervention for reducing delusions and improving reality-based thinking in patients with Paranoid Schizophrenia when implemented consistently and collaboratively with pharmacological treatment.   Keywords: Delusions; Paranoid Schizophrenia; Reality Orientation Therapy (ROT).

Abstract

Background: One of the vital signs monitoring systems currently being developed in medical-surgical wards is the Early Warning Score (EWS). However, its implementation still needs to be evaluated, given the frequent occurrence of code blues and even deaths in the ward due to delays in recognizing deteriorating patient conditions. Purpose: To determine and analyze the quality of documentation regarding the implementation of EWS monitoring in patient setting. Method: A retrospective survey of patient medical records, supported by an assessment tool. Medical records were collected based on inclusion criteria, namely medical records from both medical and surgical wards. Results: The completeness of the EWS form was still quite low at 44%, with monitoring frequency accuracy at 12%, and total score calculation accuracy at 3.8%. Several important parameters that were incomplete and needed improvement included blood sugar (65%), which was the component with the lowest achievement; level of consciousness (84%), blood pressure (89%), temperature (90%), total score (89%), and escalation scale (88%), which was still suboptimal. Conclusion: The significant gap between the implementation of EWS monitoring and the accuracy of its documentation indicates that nurses' consistent documentation of EWS monitoring needs to be improved. This condition increases the risk of clinical deterioration, delayed intervention, serious complications, and even death. Inaccurate documentation also hinders interprofessional communication and increases the potential for clinical errors. Abstract

Background: Quality of Nursing Work Life (QNWL) plays an important role in maintaining nurses' performance in the quality of health services. If QNWL is low, this can increase job stress, thereby reducing the quality of care and nurses' performance. Purpose: To determine the relationship between quality of nursing work life and job stress among inpatient nurses. Method: A quantitative study. The design was analytical with cross sectional, the research sample consisted of 46 nursing inpatients at Arafah Islamic Hospital, Jambi City. Data was collected using a validated questionnaire. Data analysis included univariate analysis, normality test with Shapiro-wilk, and bivariate analysis with spearman rank correlation. Results: Based on the results of the Spearman rho statistical test, it shows that the overall sub-variables of quality of nursing work life and job stress have a significant relationship with a p-value (<0.05). Conclusion: It is important for nurses to maintain a balance in the quality of their work in healthcare services. A good quality of nursing work life will result in maximum improvement in work quality. The better the quality of nursing work life, the lower the level of job stress experienced. Abstract

Background: Despite telemedicine being a significant instrument for mental health management, psychological and social factors such as stigma and lack of desire frequently hinder patient engagement.  Although psychological methods demonstrate potential in addressing these disorders, comprehensive evidence remains limited as of May 2025.   Purpose: To delineate psychosocial strategies in telehealth that enhance patient engagement with mental health disorders, evaluate their impact, and identify challenges and research gaps. Method: This work employs a scoping review and article selection methodology in accordance with the PRISMA-ScR criteria, utilizing a literature search. Boolean approaches were employed to search PubMed, Scopus, PsycINFO, IEEE Xplore, and Web of Science from 2015 to 2025. The inclusion criteria consist of studies on psychosocial therapies for adult individuals with mental health disorders. Ten actual papers from an anticipated forty served as the foundation for thematic analysis of the material. Results: User-friendly interfaces, empathy-driven chatbots,gamification, AI personalization, motivational reminders, and virtual communities contribute to enhanced therapy adherence (15–30%), increased session frequency (20–30%), elevated patient satisfaction (75–88%), and reduced service dropout (10–15%). The challenges encompass inadequate digital literacy, technological costs, data privacy concerns, cultural resistance, and insufficient infrastructure. Research gaps encompass the lack of studies on vulnerable populations, long-term effects, cultural differences, and significant disruptions. Conclusion: The psychosocial approach in telehealth effectively enhances patient engagement by addressing emotional, cognitive, and social needs, hence fostering community health empowerment. However, to optimize its efficacy, one must address implementation challenges and research gaps. Keywords: Psychosocial Approach; Patient Engagement; Telehealth.

Abstract

Background: Diabetes mellitus is a chronic disease with an increasing number of sufferers each year. Diabetes without comprehensive treatment not only affects physical health but also affects the mental health of sufferers, including diabetes distress. Diabetes distress can affect the quality of life of sufferers if not treated properly. Education on self-care management and social support, especially from family, can improve patients' clinical outcomes, psychosocial well-being, and decision-making abilities. The role of family support as a support system and collaborator in healthcare contributes to increased self-efficacy in self-care and can reduce distress levels for diabetes patients. Purpose: To determine the relationship between family support and distress levels in patients with type 2 diabetes mellitus. Method: This study was conducted in July 2025 using descriptive quantitative research. The respondents in this study consisted of 30 type 2 diabetes mellitus patients who visited the Lesteri Mulyo 3 Health Center in Dukuh Joholor Triyagan Mojolaban Sukoharjo, Central Java. The technique used in this study was accidental sampling, and the instruments used were a demographic data questionnaire, a diabetes distress scale questionnaire, and a Hensarling Diabetes Family Support Scale (HDFSS) questionnaire. Data analysis in this study used the Chi-Square test. Results: Most respondents received good family support. Based on the results of the family support relationship analysis test, a p-value of 0.01 was obtained. A significance value of less than 0.05 means that the research hypothesis states that there is a significant relationship between family support and the level of distress in diabetes patients. Conclusion: Family support significantly affects the level of distress in patients with type 2 diabetes mellitus. Keywords: Diabetes Distress; Family Support; Type 2 Diabetes Mellitus.

Abstract

Background: Patient safety is a global health priority that is closely related to the quality of nursing care. Nurse behavior is a major determining factor in preventing Patient Safety Incidents (PSIs) in hospitals. Purpose: To identify and analyze factors that influence nurse behavior in implementing patient safety in hospitals. Method: The research design was a Systematic Literature Review (SLR) following the PRISMA guidelines. Articles were searched for in the Wiley Online Library, PubMed, and ScienceDirect databases for publications from 2015 to 2025. From the screening results, 11 articles that met the inclusion criteria were selected. Results: The analysis revealed four main themes that influence nurses' safety behavior: (1) Organizational culture and teamwork; (2) Work environment conditions (night shifts and workload); (3) Psychological factors (stress and burnout); and (4) Effectiveness of interprofessional communication and situational awareness. A strong safety culture was found to increase nurses' courage to speak up (voice behavior), while work stress and physiological fatigue were major barriers to compliance with safety principles. Conclusion: Nurses' patient safety behavior is influenced by the interaction between systemic management support and individual psychological well-being. Improving patient safety requires a comprehensive approach that focuses not only on technical training but also on improving shift management and creating a non-punitive work environment. Abstract

Background: Smoking behaviour among adolescent girls is a growing public health concern, influenced by peers, parental guidance, and social environment. These factors play a critical role in shaping norms and smoking habits. Purpose: To determine the factors related to smoking behavior among adolescent girls. Methods: A cross-sectional design was used with a total population of 76 female adolescents. Data were collected using a structured online questionnaire via Google Forms, measuring smoking behaviour and the influence of peers, parents, and social environment. Univariable analysis described respondent characteristics, while bivariate analysis using the Chi-square test examined the association between variables, with a significance level of p < 0.05. Results: The study found that 53.9% of respondents were active smokers. Bivariate analysis indicated that peer influence (p = 0.031) and social environment (p = 0.009) were significantly associated with smoking behaviour, whereas parental influence was not statistically significant (p = 0.131). Adolescents with influential peers and a permissive social environment were more likely to smoke. Conclusion: Smoking behaviour among female adolescents is significantly influenced by peers and social environment, while parental influence shows no significant association in this age group. Preventive interventions should focus on strengthening positive social influences and controlling environmental factors to reduce the risk of smoking among adolescent girls.
