Abstract

The study aimed to investigate the short-term outcome and associated factors among acute ischemic stroke patients given rTPA (Recombinant tissue plasminogen activator).  This is a hospital-based cross-sectional retrospective study among 107 acute stroke patients given rTPA for a span of 3 years.  A favorable outcome is defined as the modified Rankin Scale (MRS) 0 to 2, while a score > 2 is defined as a poor outcome.  Mortality and adverse events were also documented. The majority (57%) had favorable outcomes on discharge. Initial stroke severity, stroke etiology, and presence of symptomatic Intracerebral hemorrhage (ICH) were significant factors for discharge outcome.  There was no significance as to age group and as to needle time of 0-3 hours versus 3-4.5 hours. The symptomatic ICH rate is 5.6%.  The overall mortality rate is 15.88% (47% due to malignant infarction, 17.6% due to symptomatic intracerebral hemorrhage (sICH), 23.52% due to basilar artery occlusion (BAO), and 11.7% due to medical causes). With a dose of 0.6mg/kg, our data indicate favorable discharge outcomes of thrombolysis.  A high baseline NIHSS, cardioembolic stroke etiology, and the presence of sICH are seen in patients with poor outcomes.  The rate of sICH is consistent with other literature, however with high mortality (100%).  Overall mortality is high primarily due to malignant infarct.   Institutions must be capacitated to address outcomes and complications of thrombolysis.

Abstract

Patient service satisfaction is an important factor for hospitals in maintaining their reputation and being trusted by the community. The violation of expectations between nurses and patients is an obstacle to service satisfaction and affects the hospital's reputation in the community. This study aims to find out the form of violation of nurses' expectations of patients and how the public relations strategy in carrying out its role manages the violation of expectations. Descriptive qualitative research case study. Data collection techniques with interviews, using the purpose sampling method. Data analysis was carried out using the data matching pattern technique, namely the hypothesis between the research results and the theory used, and then conclusions were drawn. Data validation with triangulation of sources and data. This study succeeded in describing the form of violation of nurses' expectations of patients by drawing conclusions from field data with the theory of assumptions of violation of expectations from Judee Burgoon. Nurse communication when providing health services raises violations of patient expectations by being slow to respond to complaints, unfriendly and impolite, and public relations strategies manage violations of expectations according to Dozier and Broom's theory, namely as expert advisors, communication facilitators, and problem solvers. Forms of violations of nurses' expectations of patients: Nurses are less alert to address patient complaints. Nurses use body language that is less polite in communication. Nurses lack skills and knowledge in care services to patients. The public relations strategy in carrying out the role to overcome the form of violation of expectations is to provide input to management as a bridge between the two parties and carry out ongoing training to improve nurses' communication skills. The results of the study will minimize the occurrence of violations of expectations and increase service satisfaction, which affects public trust.

Abstract

Malnutrition remains a significant global challenge, especially in developing countries. This scoping review aimed to identify community-based strategies for malnutrition in children under five. The scoping review conducted herein adhered to the methodologies outlined in conjunction with the guidelines specified in the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) extension for the scoping Reviews checklist. This study used three databases (PubMed, ProQuest, and Scopus) and included studies published between 2014-2024, with the search keywords "Intervention" or "Community-Based Intervention," “Children,” and “Malnutrition.” Results: Out of the 4846 identified articles, 11 studies were selected based on strict eligibility criteria. The results show that education, empowerment, supplementation, screening and monitoring, and other community-based interventions (home visits and meeting groups) contribute to malnutrition. The importance of community-based innovation in overcoming malnutrition in children can improve children's nutritional and health status. Program development includes increasing nutrition education, empowerment, providing adequate supplements, effective screening and monitoring to ensure the sustainability and effectiveness of community-based nutrition interventions.

Abstract

Nurses are health workers who are at high risk of experiencing occupational diseases and accidents. The purpose of this study was to obtain an overview of the experiences of nurses in coastal and border areas who have experienced occupational diseases or accidents. This study used a qualitative design with a phenomenological method. Participants were selected using purposive sampling techniques of 9 nurses from different rooms. Data were collected using in-depth interviews related to experiences. 7 themes were identified in this study, namely: 1)Implementing Occupational Health and Safety is Important to Maintain the Safety of Yourself and Others. 2) Nurses experience more than one type of occupational disease or accident during work. 3) Unsafe acts and conditions are the causes of occupational diseases and accidents. 4) Nurses’ initial response when experiencing occupational diseases or accidents. 5) Changes felt by nurses after experiencing occupational diseases and accidents. 6) Efforts to prevent occupational diseases and accidents by nurses and hospital management. 7) Nurses need Hospital Management support to maintain their Occupational Health and Safety. Conclusion: occupational diseases and accidents can be caused by the negligence of nurses themselves, co-workers, patients, and patient companions, the lack of managerial roles, and an unsafe environment. Experience of work accidents causes physical and psychological discomfort in nurses which can be temporary or long-term. Therefore, support from all parties is needed to maintain Occupational Health and Safety for nurses.

Abstract

The quality of nursing services is the main indicator of the quality of health services and is one of the determining factors for the image of health service institutions in the eyes of the community. The quality of nursing services has a relationship with patient safety. The relationship can influence the behavior of the person in doing something including the implementation of nursing and patient safety in communicating effectively; one of them is when the nurse does the handover. This study aims to evaluate the ability of nurses to carry out effective communication using the SBAR (Situation, Background, Assessment, Recommendation) method in Inpatient Installations in special hospitals. For this research can be done based on EC no. IP.03.01/D.XXV.2.3/8487/2024. The research method used was observation and questionnaires covering SBAR items, with selected respondents based on the criteria: age, education, and working period. Sixty-one nurses were taken as a sample. The results of the analysis indicated that the nurses' communication skills in the category were very good, with the highest scores in the aspects of Situation (97%) and Background (96%), while Assessment (75%) and Recommendation (95%) indicated an opportunity for improvement. The study also found that effective communication is critical for patient safety, with potential for improvement through additional training and interdisciplinary collaboration. These findings assert the importance of applying the SBAR method to improve communication within the medical team, which in turn can contribute to the improvement of patient safety and satisfaction.

Abstract

Premenstrual syndrome is a prevalent health issue, especially among female medical school students, affecting their academic and professional performance. The study aimed to assess the extent of PMS experienced by female medical school students and identify factors that can be associated with the severity of PMS symptoms. A descriptive, cross-sectional design utilizing a quantitative approach was adopted from October 8th, 2022, to March 30th, 2023. Simple random sampling was used to select a sample of 185 students from Mosul University Medical Colleges, with 51 students from the Nursing College, 44 from the Medicine College, 47 from the Pharmacy College, and 43 from the Dentistry College. Data were collected using a structured questionnaire and Premenstrual Syndrome Scale. The sample consisted of female students whose mean age was 22.6 ± 4 years and whose body mass index was 22.8 ± 4.4. Of the total sample, 97 participants (52.4%) reported not participating in physical activity. Furthermore, 79 students (42.7%) exhibited moderate symptoms of premenstrual syndrome (PMS), while 27.6% reported severe symptoms of PMS, assessed using the premenstrual scale. In conclusion, the study found a significant statistical difference between cycle duration, duration of menses, regularity of period, amount of bleeding, duration of premenstrual symptoms, and familial history of PMS concerning the severity of PMS symptoms; further studies could explore the potential role of dietary and lifestyle modifications in the management of PMS symptoms and develop effective interventions to address its impact on female students.

Abstract

The rising number of older adults in Indonesia, particularly those living with non-communicable diseases (NCDs) like diabetes mellitus (DM), presents significant challenges in enhancing their quality of life. This study investigates the impact of combining autogenic exercise with Al-Quran murottal on the quality of life among the elderly with DM. Employing a quasi-experimental design with a pre- and post-test approach, the research involved 89 elderly participants who met the inclusion criteria. These participants were divided into two groups: an intervention group, which received regular sessions of autogenic training combined with Al-Quran murottal, and a control group, which practiced self-directed internalization. The WHOQOL-OLD instrument was used to assess quality of life, with data analyzed using paired t-test and independent t-test. The findings revealed a significant improvement in the quality of life for the intervention group, as reflected by an increase in the average score from 49.42 to 74.72 (p < 0.05). Conversely, the control group experienced a minor, statistically insignificant decline in quality of life. These results demonstrate that integrating physical and spiritual elements, such as autogenic exercises and Al-Quran murottal, effectively enhances the quality of life for older adults with DM, offering a holistic approach to improving their well-being.

Abstract

Children under five years old are the golden age for a child to acquire excellent skills such as perception, interaction, speaking, etc., and therefore, developmental screening at this age to identify developmental problems in children early and provide treatment. Families, especially caregivers, provide the care needed by children to develop in the early years, so they need support to provide stimulation. The problem of gross motor development at an early age is the greatest and the older the age, the more advanced developmental problems such as fine motor skills, walking. The purpose of this study was to determine the effect of educational video stimulation of toddler development on increasing developmental milestones. This research method uses a quasi-experimental design, one group pre-test post-test. This research was conducted at the Posyandu Balita Sahabat Masyarakat, Kusumadilagan, RT 04 / RW 12, Joyosuran, Pasarkliwon, Surakarta. The sample in this study was 20 pairs of mothers and children with purposive sampling techniques. The research instrument was in the form of a pre-screening development questionnaire (KPSP). The results of the study obtained a significance value of 0.002 from the pre-test and post-test values, so it can be concluded that there is an effect of providing educational videos on toddler development stimulation on increasing development milestones.

Abstract

Stress conditions in children with chronic disease affect psychoneuroimmunological markers. One of the mind-body modalities commonly used in pediatric chronic disease is music therapy. This systematic aim was evaluating the effect of music therapy on psychoneuroimmunological markers among children with chronic disease. Articles published between March 31, 2014, and April 1, 2023, were retrieved from five databases: Scopus, PubMed, Web of Science, EBSCOhost, and ProQuest. Rayyan, an AI-powered tool for systematic literature reviews, facilitated paper selection. Studies were screened based on pre-defined inclusion and exclusion criteria. The PRISMA checklist guided selection, and the JBI Critical Appraisal for quality assessment. Data analysis employed a descriptive approach. A total of 15 articles published between 2014 and 2023 were included in this review. 10 (66,7%) of 15 studies assessed the effectiveness of music therapy on psychological and physiological responses. Meanwhile, five (33,3%) studies assessed the effectiveness of music therapy on neuroimmunological markers. Among the five studies, one did not exhibit a statistically significant impact. In contrast, the remaining four studies displayed a significant impact of music therapy on neuroimmunological markers. The most studies assessed the impact of music therapy on psychological and physiological responses, but only a small proportion assessed the effectiveness of music therapy on neuroimmunological markers. Music therapy can be considered a safe and generally well-accepted intervention in pediatric health care to manage psychoneuroimmunological markers.

Abstract

Hemodialysis is a frequent treatment for patients with chronic kidney disease (CKD), which causes anxiety. The level of anxiety is determined by various factors, including age, knowledge, environment, duration of hemodialysis, and family support. The study aims to investigate the elements that influence patients' anxiety about hemodialysis during the COVID-19 epidemic. This cross-sectional study included 54 patients on hemodialysis at M. Yunus Hospital in Bengkulu. The sample was collected using the purposive sampling technique. Anxiety was measured using a collection of questionnaires, including the Gutman and Likert scales. Data indicated that a majority of patients were females 28 (52.0%), most of them had a diploma education 21 (38.9%), housewives dominated the job 14 (26.0%), more patients were married 26 (48.0%), and the majority of them were Muslim 51 (94.4%). The Pearson chi-square test showed that knowledge and family support influenced patients’ anxiety with a p-value of 0.026 and 0.000, respectively. The level of information and family support had an impact on the anxiety of CKD patients undergoing hemodialysis during the Covid-19 epidemic. Research on enhancing knowledge of disease signs, treatment methods, and the value of morning exercise for hemodialysis patients during the COVID-19 pandemic is urgently needed
