Abstract

practice. Antibiotics are frequently prescribed to support surgical procedures, manage acute inflammation, treat infections, and prevent systemic complications. However, prescriptions are not always based on clinical guidelines or rational considerations. This study aimed to evaluate the rationality of antibiotic use among dental patients at Intan Husada Hospital, Garut, using the Gyssens method. A descriptive quantitative design was applied by reviewing medical records of patients who received antibiotics in December 2024. The Gyssens method classifies prescriptions from category 0 (rational) to category VI (not evaluable due to incomplete data). Out of 58 patients, none of the prescriptions were categorized as 0, indicating 0% rational use. Most prescriptions (89.66%) fell into category IVC, reflecting a lack of consideration for cheaper but equally effective generic alternatives. The remaining 10.34% were classified as IIIB due to prolonged duration of therapy. These findings highlight that irrationality was not primarily related to clinical indications but rather to cost-efficiency aspects. Previous studies confirmed that generic antibiotics, including amoxicillin, are therapeutically equivalent to branded drugs. Strengthening education and implementing policies that encourage the use of generic antibiotics are essential strategies to promote more rational therapy in dental healthcare services

Abstract

Organizational performance (one of which is a pharmacy) can be assessed through various approaches, one of which is the Balance Scorecard (BSC). Measuring pharmacy performance is a crucial aspect of pharmacy growth and development. This study aims to evaluate the performance of Cigadung Pharmacy based on four Balance Scorecard perspectives: financial, customer, internal business processes, and learning and growth. This research is a non-experimental case study with a descriptive approach. A sample of 95 customers was obtained through Purposive Sampling. The results indicate that from a financial perspective, the Net Profit Margin (NPM) is 8%, Return on Investment (ROI) is 47%, and Return on Equity (ROE) is 26%. The customer perspective indicates a satisfactory level of customer satisfaction. The internal business process perspective indicates that the waiting time for prescription services meets the Ministry of Health Regulation standards. The learning and growth perspective also shows quite good results. Conclusion: Cigadung Pharmacy's performance is assessed as good based on the four Balance Scorecard perspectives.Keywords: Pharmacy, Balance Scorecard, Evaluation, Service.

Abstract

The majority of people in Cisantana Village work as livestock farmers, and most of them really enjoy consuming fresh cow's milk. However, most of them are unaware that fresh cow's milk is a food ingredient that contains many bacteria that can cause disease. The purpose of this study was to determine whether there is Escherichia coli contamination in fresh cow's milk from a group of dairy farmers in Cisantana Village and what factors cause this contamination. The method used was colorimetric analysis with the methylene blue reduction test technique. In addition, organoleptic tests, pH, specific gravity, alcohol, and gram staining were also carried out. The samples in this study were fresh cow's milk from 34 farmers. The results showed that of the 34 fresh cow's milk samples, 13 samples tested positive for Escherichia coli contamination. This Escherichia coli contamination was caused by poor barn conditions, equipment, and sanitation.

Abstract

Introduction: Staphylococcus epidermidis is one of the bacteria that causes acne on the skin and can cause opportunistic infections against weak immunity. Method: This method uses cold liquid extraction with maceration technique with 96% ethanol solvent and disc diffusion to determine the activity against bacteria by using distilled water as a negative control and amoxicillin as a positive control. Result: shows inhibitory power at concentrations of 25%, 50% and 75% with inhibition zones of 9.3 mm, 9.16 mm, 7.83 mm. Conclusion. The effective concentration to inhibit the growth of Staphylococcus epidermidis bacteria is 25%, namely 9.3 mm, which is included in the medium category. Abstract

Introduction: Paracetamol syrup is a commonly used oral liquid dosage form, especially for pediatric patients. Its physical and chemical stability is strongly influenced by formulation and storage conditions. This review aims to analyze the formulation, quality evaluation, and stability of paracetamol syrup based on several studies. Literature searches were conducted through Google Scholar, PubMed, and ResearchGate using keywords such as “paracetamol syrup stability,” “accelerated stability test,” and “formulation of paracetamol syrup.” Paracetamol syrup generally contains 125 mg/5 mL of the active ingredient, with propylene glycol and glycerin as cosolvents to enhance solubility and stability. Sweeteners such as sucrose or honey improve taste and maintain viscosity, while flavors increase acceptability for pediatric use. Evaluation results show that most formulations met physicochemical requirements, with pH values between 4.5–5.5, viscosity ranging from 1800–2200 cP, and paracetamol content above 90% of the initial amount. In both accelerated (40°C/75% RH) and long-term (25°C) stability tests, the formulations remained chemically stable with degradation less than 10%. High temperatures or light exposure increased degradation, while storage at room temperature in amber bottles ensured optimal stability. The use of propylene glycol as a cosolvent and citrate buffer as a stabilizing agent provided good physical and chemical stability. All reviewed formulations fulfilled Indonesian Pharmacopoeia requirements, with the best stability achieved when stored at room temperature and protected from light. Abstract

ntroduction: Emulsifiers are important components in pharmaceutical formulations, functioning to stabilize two-phase systems such as oil and water. Until now, synthetic emulsifiers have been widely used due to their effectiveness, but they have limitations in terms of safety and sustainability. Therefore, the use of natural emulsifiers is a potential alternative that is safer and more environmentally friendly. This article aims to review the potential and effectiveness of natural emulsifiers on the stability of various pharmaceutical preparations. Methods: The study was conducted through online literature searches on the Google Scholar, ScienceDirect, PubMed, and ResearchGate databases, covering publications from the last 10 years. The keywords used included “emulsifier” “natural ingredients”, and “natural emulsifiers”. Relevant articles were selected based on their relevance to the topic of pharmaceutical emulsion stability. Results: Based on the results of the review, natural emulsifiers such as polysaccharides (gum arabic, alginate, chitosan), proteins (soy lecithin, whey protein, vegetable protein), and plant saponins can improve the physical, chemical, and thermal stability of emulsion preparations. The mechanism is through the formation of a strong interface layer that prevents coalescence, flocculation, and phase separation. Additionally, some natural emulsifiers also possess additional biological activities such as antioxidant and antimicrobial properties. Conclusion: Natural emulsifiers show great potential as alternatives to synthetic emulsifiers in pharmaceutical formulations. Further research is needed to optimize the combination of natural ingredients, homogenization methods, and the development of sustainable emulsions based on natural ingredients.

Abstract

Introduction: Atorvastatin is a statin drug with low solubility and stability, resulting in limited bioavailability. This review aims to examine various formulation methods, evaluations, and stability tests of atorvastatin tablets. Methods: This study is a literature review discussing different formulation approaches and stability evaluations of atorvastatin tablets with various release systems. Articles were collected from Google Scholar, ScienceDirect, and ResearchGate, then analyzed based on formulation methods, stabilizing excipients, and stability testing techniques. Results: The use of dry granulation with basic excipients, sustained release and extended release systems, as well as cocrystal formation with coformers such as nicotinamide and ascorbic acid, effectively improved the solubility, stability, and bioavailability of atorvastatin. Conclusion: The combination of appropriate formulation strategies and optimized dosage design is key to developing atorvastatin tablets that are stable and therapeutically effective.

Abstract

Introduction: Acne (acne vulgaris) is an inflammatory skin disorder commonly experienced by adolescents and adults, with Propionibacterium acnes being the primary bacterium responsible for the inflammation. Increasing antibiotic resistance and the side effects of synthetic drugs have encouraged the use of herbal plants as a safer antibacterial alternative. Objective: This review aims to identify herbal plants that have antibacterial activity against P. acnes, as well as to evaluate the extraction methods, test methods, and inhibition zone effectiveness reported in studies from 2020–2025. Methods: The article was developed through a literature review using Google Scholar with the keywords “activity test,” “plant,” and “P. acnes.” The data analyzed included plant type, extraction technique, testing method, and antibacterial activity results. Results: Maceration with ethanol is the most common extraction method, while antibacterial tests often use disc diffusion and well diffusion. Plants such as bay leaves (Syzygium polyanthum), starfruit leaves (Averrhoa bilimbi), red betel leaves (Piper crocatum), and a combination of pineapple leaves and green betel leaves showed strong inhibition zones (20–24 mm). Guava leaves also provided high activity, while jasmine flowers and porang frog showed weak activity. Conclusion: This study shows that several herbal plants, such as bay leaves, starfruit leaves, and red betel leaves, have strong antibacterial activity against Propionibacterium acnes, while others showed less effectiveness. These results confirm that certain herbal plants have the potential to be developed as natural antibacterials for acne treatment, although further research is needed to determine their mechanism of action, safety, and optimal formulation.
