Abstract

The incidence of malaria in Indonesia is about two million cases annually. Dihydroartemisinin-piperaquine (DHP) is the first line therapy recommended for uncomplicated malaria treatment, whereas DHP is still fully imported. The generic DHP tablet formulation has the potential to become the first of DHP drug which is locally produced. This study is aimed to formulate generic DHP film coated tablets for antimalaria drug. Tablets were compressed with the combination of wet granulation for piperaquine phosphate (PQP) and direct compression method for DHA and coated with a moisture barier coating material. The parameters to evaluate the quality of DHP tablets are physical properties, assay, and dissolution test. DHA and PQP assay were performed by HPLC method. The dissolution testing was conducted by in house method using HCl 0.1 N medium. The result shows physical properties of filmcoated tablets meet the requirement, i.e. uniform weight, 7.0-8.5 kp hardness, 0.02% friability and 3 minute 22 seconds disintegration. The assay to determine DHA in tablet was 95.17% and PQP was 97.05%. The result of dissolution testing shows the content of DHA and PQP in the tablet were 113.51% and 96.55%, respesctively. The formulation which is developed meets the general requirement of API in tablet 90–110% and dissolution requirement >75%.

Abstract

Influenza viruses are classified into subtypes based on two surface antigens known as hemagglutinin (HA) and neuraminidase (NA). Antigenic changes of influenza virus can cause resistance to antiviral drugs which is already limited in variation. Resistance to the drugs used recently are due to antigenic drift by point mutation in a single amino acid at position 275 (H275Y). The purpose of this research is to identify the presence of influenza virus A (H1N1pdm09) that were resistant to oseltamivir from cases of severe acute respiratory infection (SARI)in Indonesia in 2014 by using a rapid detection test. Detection of oseltamivir resistance in NA gene is to identify the single nucleotide polimorphism (SNP) at position 275 (H275Y) using the real-time RT-PCR method from clinical specimens SARI case. A total of 870 specimens from six sentinel hospitals were collected and 15 of them positive H1N1pdm09. Of the 15 clinical specimens, H1N1pdm09 virus strains that have mutations H275Y were not found. Based on this finding, it can be concluded that during the year 2014, there is no influenza virus A (H1N1pdm09) resistant to oseltamivir from SARI cases specimen in six sentinel hospitals in Indonesia.

Abstract

Tetrofosmin radiopharmaceutical compound that is being developed by PTRR BATAN for use as myocardial perfusion imaging agent and cancer diagnostic agent in nuclear medicine. The aim of this study is observe the stability of this liquid kits after stored in various condition to determine its expire date various condition. The stability test was done of 1 hour, 3 hours, 5 hours and 7 hours after labeling. The radiochemical was determined its radiochemical purity has to be ≥ 90 %. Analysis of radiochemical purity was carried out by separation method which using Sep-Pak C18 cartride. Storage condition of tetrofosmin kit was carried out at various temperatures as in the deep freezer (-800C), freezer (-180C), refrigerator (2-60C) and cool box (2-60C). The result showed that the liquid tetrofosmin kits stored in deep freezer, freezer, refrigerator were stable up to 23 months, 8 weeks and 4 days respectively. Simulation of stability test after stored in a cool box was done by observing the temperature of cool box, and the result showed that the temperature in the cool box was constant as in refrigerator for up to 24 hours. Tetrofosmin which has been labeled with technetium-99m can with stand up 7 hours. It is concluded that expiry date of liquid tetrofosmin kit related with storage temperature, the lower the temperature the longer the expiry date of the kits.

Abstract

Stroke is a major cause of mortality and morbidity worldwide. Patients with stroke are susceptible to medical complications, especially infections. This study aim to evaluate antibiotic by stroke inpatients hospitalized in RSUD Koja (KJS and BPJS period) with Gyssens methods. The study design is a retrospective cross-sectional. The sample is consisted of 112 medical records from KJS period (July 2013-December 2013) and 74 medical records from BPJS period (January 2014-June 2014) taken by total sampling. The use of antibiotic were analyzed using Chi Square and logistic regression multivariate. The percentage of antibiotic use was 23,11%, mostly were ceftriaxon 33,3%, ceftizozim 7,6% and amoxicillin – clavulanic acid 7,6%. Length of stay more than 7 days was 77,96%. The most common route of antibiotic administration was parenteral (68,67%). Patients that were given antibiotics were,among others,diagnosal by bronchopneumonia (29,33%), pulmonary tuberculosis and 17,6% and urinary tract infection (8,7%). The clinical outcome showed that 69,3% of 186 patients were recovered after antibiotic were given to treat their infections. Gyssen evaluation method showed that rational antibiotic used on KJS period was 77,4% and BPJS periods was 81,3%. There were correlations between rational use at antibiotic and the route of administration, between clinical outcome anduse at diagnosis and route of administration. The conclusion of this study according to Gyssen method is the rational antibiotic influence the clinical outcome (p < 0,05)

Abstract

Pharmacy is a place where a pharmacist provides pharmaceutical services. To improve services, it is necessary to carry out an evaluation of patients’s or consumers’ satisfaction on pharmaceutical services.This research aimed to assess the satisfaction of the patients covered by national health insurance programme (JKN) and to find out the relationship of patients’ characteristics and their satisfaction. This research used a cross sectional survey design, conducted for patients visiting pharmacy to redeem the prescribed drug in Tangerang Selatan city, Serang district, Bekasi city, Bogor district, Yogyakarta city, Bantul district, Solo city, and Sragen district. The sample was 152 adult JKN patients chosen by accidental sampling. Data were collected using self assessment questionnaire analyzed using chi square analysis. This study found that the highest level of dimension of pharmaceutical services satisfaction was empathy and the lowest one was assurance, in term of the availability of drugs and the number of drugs that was covered by JKN scheme. Among 5 characteristics (i.e: age, sex, marriage status, education, and occupation), only occupation variable that had significant correlation with satisfaction. The pharmacy management should communicate with the JKN provider regarding the kinds of drugs covered and concern with the drugs availability in the pharmacy.

Abstract

To determine the use of herbs as complementary therapy in the practice of herbal medicine doctor , performed a cross sectional study , non-intervention in patients with doctors' complementary - alternative herbal medicine in hospitals , health centers, clinic of physician networks Indonesia. Using medical records and entry program on the website Badan Litbangkes, 10 months evaluated, analyzed descriptively using SPSS software version 19.0.Obtainable 72 patients tumor with 129 visits, varying between 1-4 each patient. The most commonly tumor found is breast (32%). Of the 71 patients with tumors, 80.3% receiving herbal, 14.1% received conventional and herbal, 2.8 % received conventional, herbal and traditional health , and 1.4 % received herbal and traditional health. Conventional included chemotherapy, analgesics/antiinflammatory, antibiotic, stomach medicine, tranexamat acids, vitamins, hormonal drugs. Vitamins most used, followed by analgesic/anti inflammatory. Herbal therapy (potion) most often given white turmeric and pearl grass. Herbs with the same components supplied by 8 different doctorspearl grass, white turmeric,bidara upas.There are 51,4% patients came with good quality of life, 40% moderate and 8.6% bad. After receiving 3 modality therapy, there are 79,6% patients with improved quality of life and 20,4% status quo.

Abstract

Treatments in elderly patients are very complex and usually multiphatology thus causing an increase in number of polypharmacy for every clinical condition. The presence of hypertension, diabetes, and/or dyslipidemic will increase the risk of Adverse Drug Reaction (ADR) because of polypharmacy and long term of treatments. This study aimed to assess the effect of elderly age on the incidence of ADR in patients with hypertension, diabetes, and/or dyslipidemia at Puskesmas Pancoran Mas, Beji, and Tanah Baru in Depok. The design of the study is cohort study. The study was conducted at January-June 2014. Sixty two elderly patients were collected as cohort group and 62 non-elderly patients as control group. Subjects were monitored every week for a month. Clinical Manifestation of ADR event was an evaluation result of the recording complaints experienced by the subject using Naranjo scale. Clinical manifestation of ADR events obtained from both group were analyzed using Chi-square and Logistic Regression Test. This study found that 30,6% patients experienced ADR events with 39 times of accurrences. The most common clinical manifestation was dry cough related to the use of captopril (56,3%). The risk factor of ADR event in elderly age is 3,577 times greater than non-elderly age.

Abstract

The endophytic fungi Cladosporium oxysporum was isolated from medicinal plants Alyxia reinwardtii. The antimicrobial activity of fractions of ethyl acetate extract and also antioxidant and antimicrobial activities of fraction 7 of ethyl acetate extract of (AR-7) were determined in this study. Antimicrobial activity was determined by TLC bioautography and disc diffusion methods. Antioxidant activity was determined by TLC DPPH autography assay. The fractions of ethyl acetate extract of C. oxysporum at dose of 100 mg/ml were active as antimicrobial against S. aureus, E. coli and C. albicans in disc diffusion method test. Fraction AR-7 was active as antioxidant in TLC-DPPH autography assay. Fraction AR-7 was active as antimicrobial against S. aureus, but was not active against another tested microoorganisms in TLC-bioautography
