Abstract

Background: Drug-resistant tuberculosis (DR-TB) is an increasingly complex health issue, especially when accompanied by chronic obstructive pulmonary disease (COPD) comorbidity, which can exacerbate lung conditions, affect therapeutic response, and reduce treatment success. This study aims to describe the characteristics of DR-TB patients with COPD comorbidity at RSUD Dr. H. Abdul Moeloek from 2022 to 2024. Method: This study utilized a retrospective descriptive design. Data used were secondary data from patient medical records. Univariate analysis was performed to describe subject characteristics. Results: Among 46 patients, the majority were male (78.3%) aged 41–60 years (50%). RR-TB classification (76.1%) and long-term regimens (78.3%) dominated the findings. Therapeutic outcomes recorded 54.3% cured, 30.4% treatment completed, 8.7% failed, and 6.5% deceased. Discussion: DR-TB patients with COPD are more frequently observed in middle-aged males due to smoking exposure and occupational factors. The majority were classified as RR-TB, consistent with gradual resistance patterns. Long-term regimens were preferred as they are considered safer for lung function. Although COPD aggravates the condition, therapy remains likely to succeed with appropriate drug selection and patient adherence. Conclusion: DR-TB patients with COPD comorbidity are generally middle-aged males with a relatively good proportion of treatment success. Appropriate regimen selection and strict monitoring are required to optimize treatment outcomes.

Abstract

Background: Cardiovascular diseases remain the leading cause of mortality worldwide, with Acute Coronary Syndrome (ACS) as the most threatening manifestation. High-density lipoprotein (HDL) has protective effects through reverse cholesterol transport, as well as anti-inflammatory, antioxidant, and antithrombotic functions. Low HDL levels are suspected to increase the risk of ACS. This study aims to determine the association between HDL levels and the occurrence of ACS at Buleleng General Hospital. Method: This study employed a cross-sectional design involving 108 patients admitted to the ICVCU of Buleleng General Hospital during January–June 2024, comprising ACS and non-ACS groups. Data were obtained from medical records, including patient characteristics and lipid profile results. Statistical analysis was performed using the Chi-square test. Results: Out of 108 patients, 75 (69.4%) were diagnosed with ACS, while 33 (30.6%) were classified as non-ACS. Most ACS patients had low HDL levels (64.0%), whereas the majority of non-ACS patients had normal HDL levels (69.7%). The Chi-square analysis demonstrated a significant association between HDL levels and ACS events (p = 0.001). Discussion: The findings indicate that low HDL levels can increase the risk of developing ACS. This result aligns with existing theories and previous studies highlighting the protective role of HDL in cardiovascular health. Conclusion: HDL levels are significantly associated with ACS occurrence. Low HDL increases the risk of atherosclerosis, while normal or high HDL levels provide a protective effect. Lipid profile evaluation, particularly HDL levels, is essential for individuals at high risk as part of early preventive strategies.

Abstract

Pneumonia komunitas (CAP) yang disertai efusi pleura parapneumonik menimbulkan tantangan manajemen yang signifikan, terutama pada pasien dengan beberapa penyakit penyerta seperti gagal jantung kongestif (CHF), diabetes mellitus tipe 2 (DM), dan insufisiensi hepatobiliari. Seorang pria berusia 42 tahun, petugas keamanan, datang dengan sesak napas progresif, batuk berdahak disertai batuk darah, dan efusi pleura kanan. Pemeriksaan menunjukkan toraks asimetris, tanda-tanda efusi pleura sisi kanan, hepatomegali, asites, dan edema pitting. Temuan laboratorium menunjukkan leukositosis, hipoalbuminemia, hiponatremia, serta gangguan fungsi hati dan ginjal. Pemeriksaan pencitraan mengonfirmasi pneumonia komunitas (CAP) dengan efusi pleura parapneumonik kanan. Ekokardiografi menunjukkan gagal jantung kongestif (CHF) dengan fraksi ejeksi yang menurun (19%) dan trombus intrakardial. Pengobatan meliputi terapi oksigen, antibiotik, diuretik, insulin, dan perawatan suportif. Pasien membaik secara bertahap, menekankan perlunya pendekatan multidisipliner dalam mengelola CAP dengan efusi pleura dan penyakit penyerta.
