Abstract

The Covid-19 pandemic had a big impact on the world over the past two years., and it is estimated that 1 in 10 people have been infected. The development of prevention and treatment systems can reduce mortality, but a new problem has emerged in the group of survivors. New clinical complaints, Getting COVID-19 again or having ongoing symptoms of COVID-19 for four weeks after the first infection, can affect a wide variety of organ functions. The long-term effects of having COVID-19, like feeling tired, having trouble breathing, chest pain, difficulty thinking, and joint pain, can make life worse. The study was conducted using a qualitative descriptive method using an online questionnaire. The results showed that some residual symptoms that respondents still felt after four weeks since being infected with COVID-19, such as fatigue easily (80%), joint pain (63.3%), insomnia (56.7%), depressive symptoms (53.3%), shortness of breath (50%), impaired concentration and memory (50%), livelihood disorders (43.3%), cough (40%), respiratory disorders (36.7%), weight loss and appetite (33.3%), fever (26.7%). Overall, it was found that 20% of COVID-19 survivors experienced daily activity disruptions assessed using the Modifications of Barthel Index and EQ-5D-5L questionnaires.

Abstract

About 10 million people in Indonesia have been impacted by heart failure (HF), which is a complex clinical syndrome caused by insufficient cardiac output that is unable to meet the body's metabolic needs. The most frequent kind of HF, heart failure with preserved ejection fraction (HFpEF), has the fewest available treatments. Diuretics and SGLT-2 Inhibitors are the first-line treatments for HFpEF. This paper aim to determine the mechanism of sodium-glucose cotransporter-2 (SGLT-2) Inhibitors in HFpEF and the newest recommendation for it, so that it might be utilized as a reference for management in the future. This is a literature review study which conducting by gathering numerous books and journal articles which relevant with the topic. The mechanisms of SGLT-2 Inhibitors in HFpEF are not well understood, but some theories suggest that these medications affect preload, afterload, cardiac remodeling, blood pressure, arterial stiffness, and ketone body formation. They can also stimulate erythropoiesis, inhibit the myocardial Na+/H+ exchanger (NHE), and have anti-oxidant and anti-inflammatory effects, so they are no longer just seen as anti-diabetic medications. Recently, Perhimpunan Dokter Spesialis Kardiovaskular Indonesia (PERKI) published a Heart Failure guidelines and classify SGLT-2 Inhibitors as a class IB treatment for HFpEF.

Abstract

Being a member of the Tim Bantuan Medis (TBM) is required not only to have excellent health but also to have optimal physical fitness, bearing in mind that Indonesia is a disaster-prone area where disasters can occur anytime and anywhere. The research was conducted on members of the TBM of Fakultas Kedokteran dan Ilmu Kesehatan (FKIK) Warmadewa, aiming to compare the step exercise (SE) method to the resting pulse rate and recovery pulse rate in TBM students. The research was conducted at the Physiology Laboratory of FKIK Warmadewa, with ethical clearance Number: 323/Unwar/FKIK/EC-KEPK/IV/2023. This study used 71 samples of TBM students who were divided into 3 groups using a simple random method. P1 group did SE at a frequency of 100 x/minute for 216 seconds, group P2 did SE at a frequency of 110 x/minute for 196 seconds, group P3 did SE at a frequency of 120 x/minute for 180 seconds, exercises were carried out 3 times per week, during 4 weeks. All subjects were tested for cardiorespiratory fitness with the Harvard Step Test and compared the pre and post exercise results. Data analysis was performed using the paired t test method to see a comparison of the step exercise method on resting pulse and recovery pulse rate. Abstract

Introduction: Old age is a period that indicates a process of physical, mental, and social deterioration over time. There are several things that concern health in old age, one of which is functional status which is a major health problem in the elderly. A person's ability to perform daily activities can be assessed by their functional capacity. Physical functional capacity can be measured using the six-minute walk test (6MWT). There have been many studies that discuss the 6MWT, but none have specifically looked for the relationship between physical functional capacity and sleep quality in the elderly. Purpose: This study was conducted with the aim of knowing the relationship between physical functional capacity and sleep quality in the elderly at Gotong Royong Hospital. Methods: This study used an analytic observational research design with a cross sectional approach. Data were collected from demographic questionnaires, Pittsburgh Sleep Quality Index (PSQI), and direct 6MWT measurements. Data analysis in this study used univariate and bivariate analysis. Results: In this study, the results obtained p = 0.000 (p < 0.05) indicate that there is a significant relationship between physical functional capacity and the quality of sleep of the elderly at Gotong Royong Hospital. Conclusion: There is a relationship between physical functional capacity and elderly sleep quality at Gotong Royong Surabaya Hospital.

Abstract

Hashimoto thyroiditis (AIT) is a common cause of hypothyroidism that can occur in childhood and adolescence (ages six to sixteen years) and often occurs in women. AIT can be determined by increases in TSH with normal thyroid hormone serum levels. The diagnosis of AIT based on clinical symptoms of hypothyroid, biochemical results, and ultrasound images. Treatment is carried out by administering levothyroxine at an initial dose of 1 – 2 g/weight/day. This case report aims to show that AIT can show a wide range of symptoms and effects from the treatment that given to patient.

Abstract

Background of the Problem: In 2000, deaths caused by air pollution in the world reached 57,000 people per year and it is estimated that over the next 20 years this figure will increase to close to 14% or 0.7 per year.  According to Calvin, 2020, in Gold Mining, incidents of Silica exposure among workers aged 45-55 years out of 2255 people, it was found that 1592 (71%) people died. From the number above, there were 1296 people (81%) suffering from silicosis and pulmonary Tuberculosis (TB). Chemical dust (Silica) causes lung disorders, which is one of the Occupational Lung Diseases. The above process has a danger to the environment and workers. According to BPJS Employment 2022, the number of cases of KK (working accidents) and PAK (occupational diseases) in 2021 is 234,370 cases.Method of Research:       Population of 100 workers aged 20 – 61 years, Natural Stone Processors located in Tulungagung, East Java, Indonesia. Workers inspection: (1) Interview and physical examination of workers, (2) Supporting examination: Spirometry and Thorax photos, (3) Work Environment Testing consisting of: Work climate, dust, gas (NO2, CO2).Results: Worker habits: smoking as much as 55%, PPE (Personal Protective Equipment) used by the majority of cloth masks 71%, chemical dust (Silica) highest 4.6995 mg/m3, Climate in the workplace (temperature >32°C). These conditions can influence the occurrence of occupational lung disorders. Lung examination of Natural Stone processing workers in Tulungagung showed lung function disorders in 61 people (61%) and thorax photo examination showed abnormalities in 11 people (11%). WorkingPeriod has an influence on Occupational Lung Disease with a significance value of 0.03 (p-value < 0.05), with a confidence level of 95%.Conclussion: Working Period influences Occupational Lung Disease in workers in the production department of the Natural Stone Processing Company, in Tulungagung (1). Other factors that are suspected to influence the health of workers include: Occupational Lung Disease, namely smoking habits, not using the correct PPE, a dusty work environment (chemical substances) and a workplace climate with temperatures above the recommended values (2).Keywords: PAK, KK, APD, Silica, Working Period, Lung Disease due to work

Abstract

Glaucoma is a crucial ocular health issue that warrants meticulous attention according to its status as the leading cause of irreversible visual impairment on a global scale. The incidence of this illness has been on a steady rise, notably within the Asian region. Blood pressure impacts both intraocular pressure (IOP) and ocular perfusion pressure (OPP), which is the pressure responsible for facilitating blood flow to the eyeball. Hypertension is considered a contributing factor in the development of glaucoma. The Renin-Angiotensin-Aldosterone System (RAAS) plays a crucial role in the pathophysiology of hypertension and also contributes to the pathogenesis of glaucoma. The presence of Renin-angiotensin-aldosterone system (RAAS) inhibitors medications, such as angiotensin-converting enzyme inhibitors (ACE-I) and angiotensin receptor blockers (ARB) have demonstrated efficacy in reducing intraocular pressure (IOP) and reducing ganglion cell apoptosis. Consequently, these pharmacological agents present a viable therapeutic approach to patients afflicted with hypertension and glaucoma.

Abstract

When the availability of iron is insufficient to meet the needs of the body with or without anemia, it is classified as iron deficiency characterized by a serum ferritin concentration of less than 100 ng/ml or a 100-299 ng/mL with a transferrin saturation (TSAT) less than 20% in patients with heart failure. Reduced intake, low iron absorption due to increased hepcidin secretion, iron secretion in the reticuloendothelial system as a result of inflammation, and blood loss from the gastrointestinal system are mechanisms believed to cause iron deficiency (ID). With 25–42% of Heart Failure (HF) patients having ID when anaemia is absent, ID is frequent regardless of the presence of anaemia in HF. ID management in HF requires a shift in the therapeutic focus from dealing with anemia to iron supplementation, both oral and intravenous (IV). Current guidelines also support the use of IV iron. New drugs also focus on iron metabolism, which these drugs are still in development.

Abstract

Syndactyly is interdigiti soft tissue and/or osseous fusion or adhesion and may affect hand and/or foot. Syndactyly is caused by congenital or acquired. Acquired syndactyly is a rare disease that may caused by trauma, inflammation, or infection around the finger. This abnormality may affect aesthetic and functional impairment, so that surgery procedure is needed to syndactyly release. The aim of this paper is to report a rare case of simple incomplete acquired syndactyly post-operative in a man aged 53 year old. Patient came to surgery clinic because adhesion of his third and fourth left hand fingers since 21 years ago. The abnormality was disturbing his work as a baker currently. He had trauma 21 years ago when he worked as construction labour that cause swelling in his left palm, so he went to hospital and got needle aspiration. After needle aspiration, he experienced worse symptoms like recurrency swelling, rednesss, itchy, and pain then got surgery procedure that did not affect the fingers area directly. After surgery, the symptoms got improved, but he found adhesion between his third and fourth fingers days later after surgery. From currently physical examination, there was left hand interdigiti adhesion between 1/3 proximal thirth and forth fingers and from palpation there was no adhesion between the finger bone. He was diagnosed simple incomplete acquired syndactyly third and fourth digiti manus sinistra. The patient got surgery procedure with z-plasty technique to syndactyly release and hospitalized for 3 days. He got Cefixime, Ketoprofen, and Mefenamic Acid when he went home. The patient came to hospital to get follow-up 6, 10, and 17 days after surgery. Keywords: Simple incomplete acquired syndactyly; post-operative; case report.

Abstract

BACKGROUND: Indonesia is the largest archipelagic country in the world across the equator, located between two continents, two oceans, and located between three plates of the earth. This condition causes the risk of natural disasters such as floods. Tamping is a hamlet that often experiences flooding in Gresik Regency caused by the overflow of the Lamong tributary. This causes disruption in various sectors, especially the economy in the village, causing stress. AIM: Researchers want to know whether there is a correlation between community preparedness and coping mechanisms in dealing with floods. METHODS: The research design used was observational analytic with a cross sectional research design. In this study, the sample size was 59 people who were housewives in Tamping hamlet. The research data was taken through a questionnaire. RESULTS: Based on the results of data analysis using Spearman's test regarding the preparedness of housewives with coping mechanisms, the value of r=0.502 and significance value of p=0.001. CONCLUSION: There is a relationship between the preparedness of housewives in dealing with floods and coping mechanisms in Tamping Hemlet.
