Abstract

Proper diagnosis is the mainstay of comprehensive cancer management. However, this process is not straightforward. Some types of cancers, such as intra-abdominal cancer, are anatomically challenging to sample. Even after proper resection, the pathological diagnosis may be uncertain and requires further immunohistochemical processing. We present a rare case in which a patient with adenocarcinoma of the colon was diagnosed, surgically resected, and received chemotherapy, in which the postoperative specimen was also diagnosed as adenocarcinoma. However, after five years, he was diagnosed with plasmacytoma of the right iliac wing. Here, we discuss the possibility of unrelated secondary cancer or chemotherapy-induced malignancy.

Abstract

Tuberculoma infection in Papua is still quite high. Tuberculoma is a focus of caseous tubercles found in the brain parenchyma caused by the bacteria Mycobacterium tuberculosis. This study aims to report the outcomes of tuberculosis patients who underwent craniotomy resection of large tuberculoma lesions and continued with therapy using anti-tuberculosis drugs (OAT). There are challenges in the radiological and clinical diagnostic approaches to tuberculoma lesions in these patients who have no history of previous pulmonary tuberculosis infection. A definite diagnosis of tuberculoma is obtained from the establishment of anatomical pathology diagnostics so that the patient is then given FDC category I of anti-tuberculosis drugs for up to 9 months. The patient showed clinical improvement and brain tissue repair after surgery as evidenced by neuroimaging evaluation 7 months after surgery. Management of tuberculosis in Papua requires an educational focus on increasing the synergy of promotive and preventive aspects and finding new cases of tuberculoma to be treated as early as possible.

Abstract

Introduction: Mental well-being is a significant concern for the aging population. With the increasing prevalence of the elderly, identifying effective strategies to improve their mental well-being is crucial. Objectives: This systematic review aims to assess the impact of nutritional choices on the mental well-being of older adults, synthesizing evidence on how diet influences mental health outcomes in this demographic. Materials and Methods: Following PRISMA guidelines, peer-reviewed studies from 2020 to 2023 were sourced from PubMed. The review focused on randomized controlled trials (RCTs) involving elderly participants (65 years and older) and examined the effects of specific nutritional choices on mental well-being outcomes. Results: From an initial pool of 394 studies, 5 were included in the qualitative synthesis. Key findings highlight the positive impact of diets rich in omega-3, omega-6, polyphenols, vitamin E, carotenoids, and probiotics on cognitive function, stress reduction, and overall mental well-being. Conclusion: This review underscores the significant role of nutrition in enhancing mental health among older adults. Effective dietary interventions can play a critical role in mitigating mental health disorders in the elderly, highlighting the need for comprehensive dietary guidelines tailored to this vulnerable population.

Abstract

Introduction: Corneal perforation is an ophthalmic emergency and requires immediate medical and surgical treatment. The most common causes of corneal perforation are infection, inflammation, or trauma. Case Report: A 9-year-old boy came with complaints pain in his right eye for 2 days of BATH, he had history of hit by a sharp tip wire. Visual acuity on the right eye was 6/60 PH 6/30, revealed ciliary injection, corneal rupture with the avulsion, size 6x3x1mm, stromal oedema, iris prolapse, shallow anterior chamber, eye pressure was n-1/p. Patient was diagnosed with corneal rupture and iris prolapse. Patient was treated by penetrating keratoplasty. The last condition of the graft is quite good and the visual acuity was 6/15. Discussion: Perforation cause by the traumatic more than 3 mm can be treated by the penetrating keratoplasty as an emergency situation. Tissue adhesives, such as cyanoacrylate glue, amniotic membrane multilayer transplantation, conjunctival flap, or patch graft, can be used to treat small corneal defects. The success of keratoplasty is measured by corneal clarity and functional refractive outcomes. Conclusion: The success rate of emergency keratoplasty in corneal perforations reported by various studies and one of these case reports reinforces the importance of eye banking in corneal graft supplies.

Abstract

Homocysteine is a non-essential amino acid produced in the biosynthesis pathway between methionine and cysteine. Its levels tend to rise with age due to physiological, lifestyle, and nutritional factors. Elevated homocysteine can induce oxidative stress, inflammation, and endothelial dysfunction, which are critical in the development of cerebrovascular diseases. These mechanisms involve increased free radicals, decreased antioxidant enzyme activity, and enhanced inflammatory responses. Specifically, elevated homocysteine heightens oxidative stress and inflammation by increasing pro-inflammatory cytokines and decreasing anti-inflammatory cytokines, leading to endothelial cell damage and apoptosis. Supplementation with vitamins B6, B9, and B12 has been shown to lower homocysteine levels and reduce the risk of ischemic stroke. However, there is currently no established guideline for the appropriate dosing of these vitamins.

Abstract

Background: The Central Statistics Agency released data showing that Indonesians aged ? 65 have increased by 25%, from 25 million in 2019 to 80 million in 2050. The growing elderly population can lead to health issues in the elderly, such as an increased risk of obesity. This health problem needs attention. Body Mass Index (BMI) is the most commonly used method to assess obesity, but it has shortcomings compared to skinfold calipers, which assess obesity based on fat mass. Objective: To determine the relationship between the level of physical activity divided into rare, occasional, and frequent categories and the occurrence of obesity measured by skinfold calipers in the elderly. Abstract

Introduction: High blood sugar levels can lead to hypercholesterolemia, a condition where cholesterol levels in the blood exceed normal values. The active compounds in Centella asiatica (pegagan) have the potential to act as antidiabetic agents by stimulating insulin production by pancreatic beta cells. Objective: This study aims to determine the effect of Centella asiatica extract on lowering fasting blood glucose (FBG) levels in hypercholesterolemia rat models. Methods: Thirty-two test animals were divided into 4 groups: normal control, negative control, positive control (Simvastatin 1.8 mg/kgBW/day), and treatment (Centella asiatica extract 500 mg/kgBW/day). Hypercholesterolemia was induced using 2% cholesterol and 0.2% cholic acid. FBG measurement was performed after 5 weeks of administration of Centella asiatica extract and Simvastatin. Data analysis used one-way ANOVA and post hoc Bonferroni tests. Results: The mean FBG (mg/dL) in P1=70.85±1.89; P2=151.82±2.91; P3=92.34±1.86; and P4=83.89±2.17. Abstract

Introduction: Ischemic strokes making up over three quarters of all cases are a major global cause of disability and mortality. Survivors face a 15% risk of recurrence within two years, with recurrent strokes often resulting in more severe neurological damage and increasing the risk of death and long-term disability. Methods: This retrospective cross-sectional study analyzed medical records from Gotong Royong Hospital in Surabaya from January 2023 to June 2024, selecting patients with acute ischemic stroke and a history of prior strokes. The data were descriptively analyzed and further examined using bivariate analysis to identify correlations with recurrent stroke episodes, followed by a multivariate regression analysis. Results: The study recruited 87 participants, predominantly male (64.4%), with an average age of 63.2 years and varying medical histories, including hypertension (79.3%), diabetes mellitus (46%), and dyslipidemia (31%). Abstract

This article highlights the diagnostic complexity of Henoch-Schönlein Purpura (HSP) in pediatric patients, particularly when abdominal pain mimicking acute appendicitis. The objective is to emphasize the importance of comprehensive clinical evaluation to prevent misdiagnosis. Case: A 5-year-old boy with abdominal pain, nausea, and joint pain. Two days before symptom onset, he developed palpable purpura. He was diagnosed with appendicitis and underwent an appendectomy, which confirmed an inflamed appendix. Postoperatively, dermatological consultation diagnosed HSP. The patient received intravenous methylprednisolone and topical therapies, leading to symptom resolution. HSP commonly affects children and can present with gastrointestinal symptoms that mimic appendicitis. A thorough examination must be conducted to ensure that there are no other causes of abdominal pain. Conclusion: Henoch-Schönlein purpura (HSP) and appendicitis can occur simultaneously, although this is rare. A comprehensive evaluation is essential to confirm a diagnosis.

Abstract

Stunting is a children growth and development disorder caused by complex interaction of poor nutrition, infection, socio-economic factors, and the absence of adequate environmental stimuli. Stunting incidence in Indonesia is still high, including in Jlegong Village, Keling District, Jepara Regency, Central Java Province, Indonesia. Stunting will impact children’s quality of life, intellectual abilities, health, and productivity which affect the country's economy. The risk factors of stunting are health services, inadequate nutrition, environmental, genetics, and a history of chronic disease. Research about risk factors of stunting has never been carried out in Jlegong village. The aim of this study is to identify risk factors of stunting of toddlers in Jlegong village. The study design was an observational cross-sectional using total sampling. Bivariate analysis was calculated with Chi-square test and multivariate with Fisher's Exact Test. The result showed significant association between birth length with stunting (p-value = 0.001, OR = 19.33). Toddlers who had short birth length (< 50 cm) had 19.33 times higher risk of stunting than toddlers with normal birth length (≥ 50 cm). There was no significant association between birth weight, exclusive breastfeeding, maternal height, frequency of antenatal care visit, the latest level of education of parents with stunting.
