Abstract

Sustainable tourism can contribute to regional development when environmental responsibility, community participation, hospitality performance, and economic benefits are managed within an integrated framework. This study develops and empirically examines a Hospitality-Based Sustainable Tourism Model that explains how Sustainable Tourism Practices contribute to Regional Development Performance through Hospitality Competitiveness in selected tourism destinations in North Sumatra, Indonesia. The study employed a quantitative cross-sectional design involving tourism stakeholders representing hospitality businesses, tourism operators, government agencies, local communities, and tourism organizations. Data were collected using a structured questionnaire. The measurement and structural models were evaluated through instrument validity and reliability assessment, exploratory factor analysis, confirmatory factor analysis, structural equation modeling, and mediation analysis using a bootstrapping procedure. The findings confirm that Sustainable Tourism Practices strengthen Hospitality Competitiveness and contribute positively to Regional Development Performance. Hospitality Competitiveness also enhances Regional Development Performance and partially mediates the relationship between sustainable tourism practices and regional development. These results indicate that sustainability-oriented tourism generates stronger regional benefits when environmental stewardship, community involvement, cultural preservation, and responsible governance are translated into competitive hospitality services, workforce capability, innovation, destination image, and visitor value. Tourism sustainability and hospitality competitiveness should be managed as interconnected regional development priorities rather than as separate policy agendas. This study integrates sustainable tourism, hospitality competitiveness, and regional development into a unified empirical model, highlighting hospitality competitiveness as an important mechanism through which sustainability practices are translated into broader regional development outcomes.

Abstract

The existence of customary law is constitutionally guaranteed under Article 18B, paragraph (2), of the 1945 Constitution. However, within the normative framework of customary inheritance, Lampung practices remain entangled in the rigidity of a discriminatory patrilineal system. This system leads to the absolute centralization of hereditary estates to the eldest son (anak tuha), creating a normative antinomy to the constitutional principle of equality. This study employs a doctrinal approach to analyze the constitutional and structural defects within Lampung customary inheritance law, dissect the normative​ factors driving gender inequality, and formulate a new theoretical framework for asset distribution based on substantive justice. Employing statutory, conceptual, and philosophical approaches, this study analyzes regional customary codifications in light of national constitutional mandates. The results indicate a profound normative disharmony between the constitutional ideal of equality and the rigid unwritten norms of male primogeniture. To resolve this conflict, this study offers a theoretical breakthrough by normatively reinterpreting the life philosophy of Piil Pesenggiri. This philosophy is reconstructed through Satjipto Rahardjo’s progressive law paradigm as an inclusive moral-legal norm to protect material rights, rather than a textual tool to legitimize the centralization of wealth. As a conceptual output, this formulation produces a dynamic “rights-dichotomy model” and proposes a normative institutional framework for a Customary Mediation Body. This proposed model harmonizes Lampung customary inheritance within the national legal pluralism system, offering a structured conceptual recommendation that ensures legal protection for both daughters and non-successor sons.

Abstract

Background: Anemia remains an important health concern among adolescent girls and young women. Reproductive Health Gymnastics (RHG) may offer a complementary physical-activity approach, but evidence regarding its short-term association with hemoglobin (Hb) concentration is limited. Objective: To evaluate the short-term effectiveness of RHG in increasing Hb concentration among female midwifery students at Bhakti Asih Polytechnic Purwakarta. Methods: This quasi-experimental pretest-posttest control-group study included 64 first-year female midwifery students selected by purposive sampling and analyzed in an RHG group (n = 32) and a control group (n = 32). The RHG group participated in a 12-day structured program comprising aerobic movements, stretching, core-muscle strengthening, breathing exercises, and pelvic-rocking movements delivered with standardized video guidance and monitored using a compliance logbook; the control group continued routine activities. Capillary Hb was measured before and after the intervention using an Easy Touch GCHb digital hemoglobin meter. Because Hb data were non-normally distributed, within-group changes were analyzed using the Wilcoxon signed-rank test and between-group differences in Hb change (posttest minus pretest) using the Mann-Whitney U test (two-sided alpha = 0.05). The study record did not retain the original nonrandom allocation mechanism, a priori sample-size calculation, or complete intervention-dose and Hb-standardization details; these limitations are reported explicitly rather than reconstructed retrospectively. Results: The reported mean Hb increase was 1.38 g/dL in the RHG group and 0.14 g/dL in the control group, corresponding to an unadjusted arithmetic difference in mean change of 1.24 g/dL. The intervention group showed a significant pretest-posttest change (Wilcoxon Z = -4.953; p < 0.001; rank-based r = 0.88). The between-group comparison of Hb change was reported as significant (Mann-Whitney U test, p < 0.05). However, the archived study record does not retain group-specific absolute pretest and posttest Hb summaries, SD/IQR or range values, the control-group Wilcoxon output, the Mann-Whitney U statistic and exact p-value, or 95% confidence intervals. These unavailable statistics were not reconstructed, and the precision and clinical significance of the comparative effect therefore remain incompletely quantified. Conclusion: In this single-institution sample of first-year female midwifery students, participation in a 12-day RHG program was associated with a greater short-term increase in measured Hb concentration than routine activities. The finding should be interpreted cautiously given the quasi-experimental design, short follow-up, and incomplete control of potential confounding factors.

Abstract

This study examines the implications of administrative position equalization for the long-term career development of functional civil servants, particularly the promotion pathway from Senior Expert Functional Positions (Ahli Madya) to Principal Expert Functional Positions (Ahli Utama). Unlike previous studies that primarily focused on administrative implementation, this research evaluates changes in career opportunity structures resulting from shifts in the composition of Indonesia's civil service positions following bureaucratic simplification. The study employs a qualitative descriptive approach combining normative-juridical analysis with secondary data analysis using the National Civil Service Agency's civil service statistics (2021 and 2025), relevant regulations on bureaucratic simplification and functional position management, and a case study of the position map at the Agency's headquarters. The findings indicate that the substantial increase in Ahli Madya positions has not been accompanied by a proportional expansion of Ahli Utama formations. This imbalance narrows promotion opportunities and provides strong evidence of a structural career bottleneck for officials appointed through position equalization. The findings suggest that the success of bureaucratic simplification should be evaluated not only by organizational streamlining but also by the sustainability of career development within the merit system. The study contributes to the literature by integrating national civil service workforce composition, institutional arrangements governing functional positions, and organizational position mapping to explain how bureaucratic reform reshapes career opportunity structures. It recommends revising Ahli Utama workforce planning to better align with the growing population of Ahli Madya officials.

Abstract

This cross-sectional correlational study examined the association between smartphone addiction and learning motivation among adolescents attending Pasraman Tirta Bhuana, a nonformal Hindu religious education institution in Bekasi, Indonesia. The accessible population comprised 223 students aged 12-18 years, and 205 complete responses were included in the analysis (91.9% of the population). Smartphone-addiction symptoms were measured using the 21-item Indonesian modification of the Smartphone Addiction Scale, while learning motivation was assessed with a 24-item questionnaire organized around intrinsic motivation, extrinsic motivation, and amotivation. Preliminary instrument evaluation yielded Cronbach's alpha coefficients of .842 and .883, respectively. After residual normality, linearity, and homoscedasticity checks, simple linear regression showed a significant inverse association between smartphone-addiction and learning-motivation scores (B = −0.334; R = 0.525; R² = 0.276; p < 0.05). Thus, higher smartphone-addiction scores were associated with lower learning motivation, and the model accounted for 27.6% of the observed variance. Self-Determination Theory provides a plausible interpretive framework, although autonomy, competence, and relatedness were not measured directly. The findings support context-sensitive digital self-regulation strategies and longitudinal research rather than causal conclusions from cross-sectional data.

Abstract

Public Service Malls (PSMs) are designed to integrate services from multiple agencies within a single service environment, yet physical co-location does not necessarily ensure organizational integration. This study examines how structural arrangements shape coordination and service integration in the Payakumbuh City Public Service Mall and provides an empirical diagnosis of organizational fragmentation underlying its operation. Using a qualitative single-case study design, data were collected through an in-depth semi-structured interview with the Coordinator of the Integrated Service Substance Sector of the Payakumbuh City Investment and One-Stop Integrated Service Office (DPMPTSP), non-participant observation, and analysis of organizational and cooperation documents. Data were analyzed through thematic coding across structural dimensions, including division of labor, authority distribution, coordination, formalization, communication, technological integration, human resources, and institutional support, supported by source triangulation. The findings indicate that the Payakumbuh City PSM has successfully improved physical accessibility through shared facilities, common front-office arrangements, and municipal support. However, participating agencies continue to retain their own mandates, procedures, personnel arrangements, and decision-making authority. DPMPTSP performs a coordinating role but lacks sufficient formal authority to harmonize substantive service procedures across agencies. As a result, coordination remains partly reactive, digital integration is limited, and staff rotation affects service continuity. The study concludes that the Payakumbuh City PSM demonstrates physical integration without equivalent organizational integration. Its contribution is not the development of a universal organizational-design model, but a case-based structural diagnosis showing how fragmented authority, agency-specific formalization, and limited integrative mechanisms constrain one-stop public service arrangements. The findings highlight that effective public service integration requires not only physical co-location but also aligned authority, procedures, technology, and human resources across institutional boundaries.

Abstract

Background: Uterine fibroids and ovarian cysts are common gynecologic disorders that may require surgical management and can be associated with clinically significant intraoperative bleeding. Temporary uterine tourniquet placement reduces uterine perfusion during surgery and may facilitate hemostasis. Case Presentation: A 40-year-old woman with abdominal pain and a five-year history of heavy, irregular menstruation underwent combined myomectomy and right ovarian cystectomy. Ultrasonography showed an 8.5 x 7.6 x 8.2 cm mixed-echo intrauterine mass and a 6.8 x 6.9 cm hypoechoic mass; preoperative hemoglobin was 10.0 g/dL. Intraoperatively, multiple uterine fibroids, adenomyosis, and a right ovarian cyst with adhesions were identified. A Foley-catheter tourniquet was applied around the lower uterus for 30 minutes, with temporary release for hemostatic assessment. Estimated blood loss was 100 mL, no transfusion was required, and hemoglobin two hours after surgery was 9.1 g/dL. Histopathology confirmed uterine leiomyoma, adenomyosis, and an ovarian endometrioma. The patient was discharged in stable condition on postoperative day 3. Conclusion: In this case, the temporary tourniquet technique provided effective intraoperative bleeding control during combined conservative gynecologic surgery. Further studies are needed to assess long-term reproductive outcomes and its role in combined procedures.

Abstract

Background: Access to maternal health services in archipelagic regions remains challenging because geographic barriers can create systemic delays in care. Balobaloang Caddi Island, Pangkajene and Islands Regency, South Sulawesi, is served only by a village health post with limited basic emergency obstetric capacity, while the nearest primary healthcare facility with a sea ambulance is located on another island. Prolonged obstructed labor without timely intervention may result in severe perinatal complications, including intrauterine fetal death (IUFD). Case Presentation: A 33-year-old primigravida (G1P0A0) was referred from Balobaloang Caddi Island for obstructed labor after full cervical dilation had been present since 00:00 Central Indonesia Time without descent of the fetal head. Fetal heart tones were audible before departure but became undetectable during transfer. Referral by sea required approximately 14–20 hours depending on the vessel and weather. The patient arrived with IUFD. A maternity waiting home was available in the regency capital, but the patient had declined to use it for an unknown reason. Conclusion: This case illustrates all components of the Three Delays model, intensified by extreme geographic barriers and limited transport infrastructure. Strengthened antenatal risk assessment, inter-island referral systems, and effective use of maternity waiting homes are essential to reduce preventable perinatal loss in remote island communities.

Abstract

Uterus didelphys is an uncommon Müllerian duct anomaly that may be overlooked when one hemiuterus is gravid and the contralateral hemiuterus is mistaken for a leiomyoma. We report a 25-year-old G5P2A2 woman who presented with one week of vaginal bleeding, reduced fetal movement, lower abdominal pain, and absent fetal heart activity. A prior two-dimensional ultrasound had described a uterine myoma, and no three-dimensional ultrasound or magnetic resonance imaging had been performed. At emergency cesarean delivery, two distinct uterine bodies were identified, confirming uterus didelphys; the clinical record also documented cervical duplication. A 1300-g female fetus was delivered without signs of life, and no corrective metroplasty was performed. This case emphasizes that two-dimensional ultrasonography may be insufficient when uterine contour is atypical and that definitive characterization requires assessment of both the internal cavity and external fundal contour, preferably with three-dimensional transvaginal ultrasound or magnetic resonance imaging. Although uterus didelphys does not necessarily reduce fertility, it is associated with higher risks of preterm birth, malpresentation, cesarean delivery, and other adverse perinatal outcomes. Preconception anatomical confirmation, early pregnancy surveillance, and individualized counseling are therefore essential in subsequent pregnancies.

Abstract

Background: Mayer–Rokitansky–Küster–Hauser (MRKH) syndrome typically involves Müllerian duct agenesis, whereas its coexistence with fusion anomalies such as uterus didelphys, vaginal agenesis, and cervical atresia is exceptionally rare. This case highlights the diagnostic challenge created by discordance between preoperative ultrasonography and intraoperative findings in a complex Müllerian anomaly. Case: A 26-year-old woman presented with acute-on-chronic lower abdominal pain and primary amenorrhea. Examination showed a normal vulva and a blind vaginal pouch without a visible cervix or external cervical os. Ultrasonography suggested a complex left adnexal cyst and a hypoplastic uterus, whereas magnetic resonance imaging showed a 5-cm left dermoid cyst and a retroverted uterus. Cyclic molimina suggested functional but obstructed endometrium. Intraoperative exploration revealed uterus didelphys with vaginal agenesis and total cervical atresia. A left salpingo-oophorectomy was performed for the dermoid cyst. Discussion: The case demonstrates the limitations of ultrasonography for detailed mapping of complex Müllerian duct anomalies, particularly when adnexal masses obscure pelvic anatomy. The combination of a blind vaginal pouch and cyclic pain indicates complete outflow tract obstruction and creates major reproductive and reconstructive challenges. Conclusion: Careful physical examination, advanced imaging, and intraoperative confirmation are essential when imaging findings are discordant with clinical symptoms. Precise anatomical mapping is critical for fertility counseling and planning future reconstructive treatment.
