Abstract

Stress is one of the factors that affect mental health and athletic performance. In student athletes, the demands of intensive training and academics can increase the risk of severe stress. This study aims to analyze the relationship between training intensity and stress levels in athletes at the North Sumatra Student Sports Education and Training Center. The study uses a quantitative approach with a descriptive analytical design and cross-sectional method. The study population consisted of 142 active athletes, with purposive sampling technique. The sample size was determined using the Slovin formula with a 5% error rate, resulting in 90 respondents. Data were collected using the Perceived Stress Scale (PSS-10) and International Physical Activity Questionnaire-Short Form (IPAQ-SF) questionnaires. The instruments were tested for validity through item-total correlation (all items were valid) and reliability using Cronbach's Alpha (>0.70), which showed good internal consistency. The results showed that most athletes were involved in high-intensity training and experienced high levels of stress, both in cardiorespiratory sports (56.8% high-intensity training; 73% high stress levels) and martial arts (67.9% high-intensity training; 75.5% high stress levels). Statistical analysis shows a significant relationship between training intensity and stress levels, with odds ratios (OR) of 6.42 for cardiorespiratory athletes and OR 8.58 for martial artists. These findings emphasize the importance of managing training intensity, monitoring stress levels, and implementing coping strategies to maintain athletes' mental health and performance.

Abstract

The maternal mortality rate (MMR) in Indonesia remains high at 305 per 100,000 live births, far from the national target of 183 per 100,000 live births by 2024. Pregnancy complications such as preeclampsia, gestational diabetes, and preterm labor are the main causes of high maternal morbidity and mortality. One contributing factor is the delay in early detection due to limited access and resources of health workers in primary care facilities. This research aims to develop Gestasi Care , a web-based digital application designed to support early detection of pregnancy complications, monitor antenatal care (ANC) schedules, and provide education to pregnant women. Method: The research method used was Research and Development (R&D) with a waterfall System Development Life Cycle (SDLC) approach. The research stages included needs analysis, prototype design, initial data collection through a survey at the Sidodadi Community Health Center, and initial application trials with midwives and doctors. A prototype application has been developed with key features including pregnancy risk screening (low, medium, and high categories), ANC and ultrasound schedule reminders, digital medical records, teleconsultation, and trimester-based education. The application is currently undergoing content validation and preparation for a limited field trial to assess feasibility and user acceptance. Gestation Care has the potential to be an innovative digital-based solution to strengthen maternal health services in primary care facilities, increase the effectiveness of early detection of pregnancy complications, and support efforts to reduce maternal mortality in Indonesia.

Abstract

The main problems faced by hospitals in Indonesia from an internal perspective are the lack of human resource commitment, inconsistent organizational systems and leadership, differing perceptions regarding service quality, limited funds, and a lack of work culture. The purpose of this study is to identify the relationship between job satisfaction dimensions and organizational commitment dimensions and positive work behavior of employes at Kudungga Sangatta Regional General Hospital. The type of research used an observational cross-sectional quantitative research method, conducted over 3 months. The total sample size was 158 employes. The population in this study consisted of all employes in the support services department across 12 (twelve) installations or units at Kudungga Sangatta Hospital. Data collection techniques and research instruments are interviews and questionnaires. Data analysis is done using statistical tests, namely: Chi-Square test with a significance level of α= 0.05, and multiple logistic regression test. The conclusion from the 7 dimensions of independent variables that are related to positive work behavior of employes is job characteristics (p=0.000), rewards (p=0.000), work environment (p=0.000), and relationships with management (p=0.010), while those that are not related to positive work behavior of employes are affective commitment (p=0.518), continuance commitment (p=1.000), normative commitment (p=0.861), and the most dominant influence on positive work behavior of employes at Kudungga Sangatta Regional General Hospital is the rewards dimension with an Odd Ratio value of 7.338. It is recommended that efforts to improve the quality of hospital services by increasing employe job satisfaction include proportional recognition and attention, managing job characteristics to align with employe competencies and capacities, and creating a good work environment that can motivate employes and increase the intensity of management communication by involving employes in formal activities or joint events outside of office hours.

Abstract

End-stage chronic kidney disease requires hemodialysis with vascular access via an arteriovenous fistula (AVF). AVF cannulation is often painful. Cryotherapy, a cold therapy that can reduce pain through gate control theory, is used. Objective to assess the effectiveness of cryotherapy in reducing pain during arteriovenous fistula (AVF) puncture in hemodialysis patients. This study applied an Evidence-Based Nursing Practice approach, including a literature search through PubMed and ProQuest databases. The primary reference used was a systematic review and meta-analysis entitled "The Effect of Cryotherapy on Pain Associated with Arteriovenous Fistula Puncture in Hemodialysis Patients." This study used a single-group pre-test and post-test design. The population consisted of hemodialysis patients with AVFs who met the inclusion criteria, patients undergoing regular hemodialysis, using AVF access, and who had been using AVF access for more than three months. Exclusion criteria included patients who had taken pain medication within three hours before hemodialysis. A total of 19 participants were included. The intervention involved applying ice gel compresses to the cannulation area for 10 minutes before the procedure. Pain levels were measured using a Visual Analogue Scale before and after the intervention. Data were analyzed using paired t-tests and Chi-square tests. Cryotherapy significantly reduced AVF cannulation pain, with a p-value of 0.001. There was a difference in pain levels before and after the cryotherapy intervention.

Abstract

Basal cell carcinoma (BCC) is the most prevalent type of nonmelanoma skin cancer, primarily associated with chronic ultraviolet exposure and typically affecting elderly individuals. This case report aims to describe the clinicopathological features and management of BCC in an 81-year-old woman who presented with a hyperpigmented nodule on the left mandibular region. A comprehensive approach was undertaken, including clinical assessment, dermoscopic evaluation, and histopathological examination. The lesion was surgically excised, and the resulting defect was reconstructed using a rhomboid flap. This procedure achieved complete tumor clearance and successful defect closure, with favorable aesthetic and functional outcomes and no postoperative complications. This case highlights the effectiveness of rhomboid flap reconstruction in restoring tissue intergrity following BCC excision, emphasizes the improtance of early diagnosis and histopathology confirmation, and highlights the role of surgical technique in optimizing patient prognosis. Furthermore, the findings contribute to the body of evidence supporting the rhomboid flap as a reliable option for facial reconstruction following oncologic surgery, enhancing postoperati ve recovery and quality of life for patients undergoing facial oncologic surgery.

Abstract

Patient revisit intention is an important indicator for evaluating hospital service performance, as it reflects patient satisfaction and loyalty. This study aims to analyze the effect of service quality, tariffs, and digital marketing on patient revisit intentions at Bhayangkara Kediri Hospital in 2025, both partially and simultaneously. This research employs a quantitative approach with an observational analytic design and a cross-sectional method. The study population included all outpatient patients who revisited the hospital from June to August 2024, totaling 18,317 patients, and a sample of 110 respondents was selected using purposive sampling based on inclusion criteria, calculated with Slovin’s formula and supported by a power test. Data were collected using structured questionnaires that had been tested for validity and reliability, and subsequently analyzed using multiple linear regression. The results indicate that, partially, service quality (X₁) has a positive and significant effect on patient revisit intention (β = 0.147; t = 3.297; p = 0.001), as do tariffs (X₂) (β = 0.173; t = 2.453; p = 0.016) and digital marketing (X₃) (β = 0.299; t = 4.158; p < 0.001). Simultaneously, all three independent variables significantly affect patient revisit intention (F = 38.336; p < 0.001), with a coefficient of determination (R²) of 0.520, indicating that 52% of the variation in revisit intention is explained by the study variables, while 48% is influenced by other factors. These findings emphasize that improving service quality and optimizing digital marketing are the main factors in strengthening patient loyalty, while tariffs act as a supporting variable in patients’ decision to revisit.

Abstract

Sepsis is a life-threatening condition characterized by organ dysfunction resulting from a dysregulated immune response to infection. Sepsis is the leading cause of death from infection globally. Blood Urea Nitrogen (BUN) serves as a marker of kidney damage, and albumin is an indicator of fluid homeostasis. The BUN/albumin ratio can be used to predict mortality in infectious diseases, making it a widely studied and effective prognostic marker for mortality in sepsis patients. Analyzing the Blood Urea Nitrogen (BUN)/albumin ratio as a predictor of clinical outcome in sepsis patients. This retrospective cohort study used secondary data from laboratory results and electronic medical records of adult sepsis patients treated in intensive care units at RSUP Dr. Hasan Sadikin Bandung from January 2022 to December 2023. Patients were identified using ICD-10 code A41.9 and a Sequential Organ Failure Assessment (SOFA) score ≥2. Blood urea nitrogen and serum albumin levels were obtained from the first laboratory examination within 48 hours after ICU admission. A total of 139 patients who met the inclusion criteria were included. Statistical analysis was performed, with significance set at p<0.05. The subjects of this study included 139 sepsis patients, with a significant difference in the BUN/Albumin ratio between those who died and those who survived. The median BUN/albumin ratio was significantly higher in sepsis patients who died (16.58) compared to those who survived (9.16), p < 0.001. The analysis of the Receiver Operating Characteristic (ROC) curve showed an Area Under the Curve (AUC) of 0.814, indicating good predictive ability for mortality. The BUN/Albumin ratio with a cutoff of ≥10.1617 had a sensitivity and specificity of 73 and 71.8%, respectively. The Relative Risk in this study was 1.77, indicating that sepsis patients with a BUN/albumin ratio of ≥10.1617 had a 1.77 times higher risk of death compared to those with a BUN/albumin ratio of <10.1617. The BUN/albumin ratio is a significant predictor of mortality for determining clinical outcomes in sepsis patients. A higher BUN/albumin ratio is associated with increased mortality in sepsis patients, therefore the BUN/albumin ratio is useful as a prognostic marker of mortality in sepsis patients in clinical practice.
