Abstract

Chronic kidney disease (CKD) is a progressive chronic condition that requires long-term hemodialysis and is frequently associated with fatigue, which can negatively affect patients' physical functioning and quality of life. Qigong exercise delivered through smartphone-based instructional videos may provide an accessible intervention to help reduce fatigue among patients undergoing hemodialysis. This study aimed to determine the effect of a Qigong-based mHealth intervention on fatigue among patients with CKD undergoing hemodialysis. A quantitative pre-experimental study was conducted using a one-group pretest-posttest design. The study involved 30 patients undergoing routine hemodialysis at Bhayangkara Setukpa Lemdiklat Polri Hospital, Sukabumi City, who were selected using purposive sampling. Fatigue was measured using a modified Functional Assessment of Chronic Illness Therapy–Fatigue (FACIT-F) questionnaire consisting of 25 items, which had been tested for validity and reliability. Participants received Qigong exercise through smartphone-based instructional videos three times per week. Data were analyzed using a paired-samples t-test. The results showed that fatigue scores decreased from 55.67 before the intervention to 44.63 after the intervention, with a statistically significant difference (p < 0.001). It was concluded that mHealth based Qigong exercise significantly reduced fatigue among patients with CKD undergoing hemodialysis.

Abstract

Exclusive breastfeeding and appropriate family practices are essential components of stunting prevention. Digital health interventions may provide an accessible approach to strengthening families’ knowledge, attitudes, and practices related to child nutrition and care. This study aimed to determine the effect of the Digital Smart-Parenting intervention on families’ knowledge, attitudes, and practices regarding exclusive breastfeeding and stunting prevention. A pre-experimental one-group pretest–posttest design was conducted among 50 family members of infants or children under five years of age in nine subdistricts of Sukabumi Regency, Indonesia. Participants were selected using purposive sampling and used the Digital Smart-Parenting application for one month. Knowledge, attitude, and practice scores were assessed before and after the intervention and analyzed using paired-samples t-tests. After the intervention, knowledge scores increased with a mean difference of 0.58 (95% CI: 0.23–0.92; p = 0.001; Cohen’s d = 0.48). Attitude scores increased with a mean difference of 1.86 (95% CI: 0.81–2.90; p < 0.001; Cohen’s d = 0.50). Practice scores increased with a mean difference of 2.16 (95% CI: 1.06–3.25; t(49) = 3.9; p < 0.001; Cohen’s d = 0.55). Digital Smart-Parenting was associated with significant improvements in families’ knowledge, attitudes, and practices related to exclusive breastfeeding and stunting prevention, with moderate effect sizes.

Abstract

Previous studies in Indonesia have reported a relatively high prevalence of depressive symptoms, with variations observed based on characteristics and timeframes; consequently, the relationship between these characteristics and depressive symptoms may differ across survey periods. This study aims to analyze the relationship between gender, age, marital status, and depressive symptoms using data from waves 4 and 5 of the Indonesia Family Life Survey (IFLS). Method: This cross-sectional study used data from IFLS-4 (2007) and IFLS-5 (2014). A total of 29,965 respondents from IFLS-4 and 31,441 respondents from IFLS-5 were included in the analysis. Depressive symptoms were categorized as normal or having depressive symptoms. The datasets were merged and analyzed using Stata. Chi-square tests were used for bivariate analysis of categorical variables, and binary logistic regression was used for independent variables with multiple categories. In IFLS-4, sex/gender (p = 0.458), age (p = 0.126), and marital status (p = 0.703) were not significantly associated with depressive symptoms. In contrast, all three variables were significantly associated with depressive symptoms in IFLS-5 (p < 0.001). In IFLS-5, females represented a higher proportion of respondents with depressive symptoms than males (55.23% vs. 44.77%), respondents aged 14–20 years represented a higher proportion of those with depressive symptoms than those without symptoms (14.29% vs. 10.67%), and respondents who had never married were more frequently represented among those with depressive symptoms (25.63% vs. 18.18%). The discrepancy between the two survey waves is an important finding.

Abstract

The management of Human Immunodeficiency Virus (HIV) remains suboptimal, particularly in terms of treatment adherence and quality of life, calling for community-based solutions to strengthen social support for people living with HIV. This study aimed to develop the PEER-CARE Model to improve treatment adherence and quality of life among people living with HIV within the community. This study employed a one-group pre-test and post-test design involving 15 people living with HIV in Region IV of Sukabumi Regency, selected using purposive sampling. Data were collected using the MMAS-8, WHOQOL-HIV BREF, and MSPSS, which have demonstrated satisfactory validity and reliability in previous validation studies. Data were analysed using a paired-samples t-test, with effect size calculated using Cohen’s d. There was a statistically significant increase in treatment adherence, quality of life and perceived social support following the intervention (p<0.05). The PEER-CARE Model had a significant effect on treatment adherence and quality of life among people living with HIV.

Abstract

Tuberculosis (TB) patients living in disaster-prone areas may experience interconnected food, nutritional, socioeconomic, and healthcare vulnerabilities that can be exacerbated by disasters. However, an instrument that comprehensively assesses food and nutrition vulnerability in this population remains limited. This study aimed to develop and validate the multidimensional Food Security-Nutrition Vulnerability (FOSENUVU) instrument for assessing food and nutrition vulnerability among TB patients living in disaster-prone areas. A methodological study was conducted through sequential instrument development and psychometric validation. Initial item development involved a literature review, healthcare worker interviews, item construction, and expert assessment. Content validity was evaluated by five experts using the Content Validity Index. The resulting 30-item instrument was administered to 300 adult pulmonary TB patients living in disaster-prone areas of Sukabumi Regency, Indonesia, selected using purposive sampling. Validity analysis was carried out in stages: content validity was assessed by five experts using the Content Validity Index (CVI), followed by item validity testing using corrected item-total correlation. Furthermore, construct validity was analysed using Exploratory Factor Analysis (EFA) and Confirmatory Factor Analysis (CFA), whilst convergent validity was assessed using Average Variance Extracted (AVE) and Composite Reliability (CR). Expert assessment demonstrated 90% agreement, supporting the content validity of the instrument. The 30-item FOSENUVU showed strong internal consistency, with Cronbach’s alpha of 0.979 overall and 0.872–0.894 across six conceptual domains. EFA identified a dominant single-factor structure explaining 60.5% of the total variance, with factor loadings ranging from 0.709 to 0.825. CFA confirmed the one-factor model with excellent fit (χ²/df=1.046, CFI=0.997, TLI=0.997, RMSEA=0.012, and SRMR=0.022). Convergent validity was supported by an AVE of 0.605 and composite reliability of 0.979. All 30 items were retained. FOSENUVU demonstrated strong content validity, internal consistency, structural validity, and convergent validity. The final 30-item instrument provides a multidimensional yet integrated measure of food and nutrition vulnerability among TB patients in disaster-prone areas.

Abstract

Stunting is a chronic nutritional problem caused by prolonged inadequate nutritional intake that can impair children's growth and development. One complementary intervention that can be applied in family nursing care is Tui Na massage to help improve children's appetite. To analyze family nursing care for children with stunting through the implementation of Family-Centered Nursing using the Tui Na massage intervention. This study employed a case study design involving two families with stunted children in Cempaka Putih Barat Village, Cempaka Putih District, Central Jakarta. Family nursing care was conducted over 8 days, with the Tui Na massage intervention provided for 5 consecutive days. Data were collected through interviews, observation, physical examination, and documentation. After five days of Tui Na massage, both children showed an increase in appetite, as indicated by eating three times a day and being able to finish ¾–1 serving of food, as well as an improvement in family health care practices. The implementation of Family-Centered Nursing with the Tui Na massage intervention was beneficial in improving the appetite of children with stunting, as well as enhancing family knowledge, skills, and the family's health care function in providing independent child care.

Abstract

The implementation of electronic prescribing (e-prescribing) integrated with electronic medical records (EMR) represents a key innovation in enhancing patient safety and healthcare service quality. This study explores healthcare professionals' perceptions of e-prescribing utilization and its impact on the prescribing and dispensing phases. This study employed a qualitative approach with a phenomenological design. Data were collected through in-depth interviews with nine informants, comprising medical specialists, general practitioners, pharmacists, pharmacy technicians, and IT staff. Data analysis was conducted using a thematic analysis approach assisted by NVivo 12 software. The findings indicated five primary themes: strengths of e-prescribing, weaknesses of e-prescribing, impacts on medication errors, patient care and satisfaction, and recommendations for system development. During the prescribing phase, e-prescribing enhanced the clarity of medication information, improved ease of use, and reduced prescribing errors. However, limitations persist, including sub-optimal usability, the absence of prescription templates, and insufficient system validation, which lead to residual errors. In the dispensing phase, e-prescribing implementation has not fully improved service efficiency, as evidenced by prolonged waiting times and a tendency for patients to fulfill prescriptions outside the healthcare facility.  This study concludes that while e-prescribing plays a pivotal role in enhancing prescribing quality, it has yet to optimize efficiency in the dispensing phase. Therefore, it is imperative to develop a more integrated, responsive, and user-centric system.

Abstract

Human Immunodeficiency Virus (HIV) is a chronic disease that affects not only physical health but also the psychological well-being of individuals living with HIV/AIDS. One psychological response experienced by people living with HIV/AIDS (PLWHA) is the process of self-acceptance, which can be understood through the Kübler-Ross Theory. Assessment of self-acceptance is an important component of nursing care because patients may demonstrate different emotional responses and therefore require individualized nursing approaches. To describe the implementation of nursing care for patients with HIV/AIDS, with a focus on assessing self-acceptance based on the Kübler-Ross Theory. This study employed a case study design involving two female patients with HIV/AIDS receiving care at the Teratai Clinic of Persahabatan Hospital. Data were collected through in-depth interviews, observation, physical examinations, documentation review, and psychological assessment based on the five stages of the Kübler-Ross Theory. The data were analyzed descriptively by comparing the self-acceptance processes of the two patients. The assessment revealed differences in the self-acceptance processes of the two patients. Although both patients had been receiving antiretroviral (ARV) therapy for several years, they demonstrated different psychological responses based on the Kübler-Ross Theory. Mrs. MK exhibited characteristics of several stages of the Kübler-Ross Theory, with depression being the predominant response, characterized by feelings of guilt, regret, worthlessness, and persistent sadness. In contrast, Mrs. SH demonstrated characteristics of acceptance, as indicated by her ability to accept her condition as a person living with HIV, maintain adherence to ARV therapy, have positive life goals, and actively participate in a Peer Support Group (KDS). These findings indicate that the process of self-acceptance is individual and dynamic and is not solely determined by the length of time a person has been living with HIV/AIDS. Assessment of self-acceptance based on the Kübler-Ross Theory helps nurses identify the stage of psychological adaptation in patients with HIV/AIDS and provides a basis for developing individualized, holistic, and patient-centered nursing care.
