Abstract

Disaster emergency response is a rapid action to mitigate the impact of a disaster. Therefore, community emergency response is crucial in reducing risk. This requires knowledge and training to enable the community to act appropriately and effectively when a disaster occurs. To determine the relationship between community knowledge and emergency response attitudes regarding the Mount Agung eruption in Tunas Sari Hamlet, Karangasem Regency. This study used a quantitative, correlative analytical approach with a cross-sectional approach, with a sample of 350 respondents selected using a purposive sampling technique. Data collection used a knowledge questionnaire and an attitude questionnaire. The questionnaire for the variables of the level of knowledge and attitudes of the community towards evacuation routes was compiled by researchers based on relevant theories and indicators, then a content validity test (face validity) was carried out through expert judgment by two expert lecturers. Data analysis was descriptive. The Kolmogorov-Smirnov normality test indicated that the data were not normally distributed, therefore the Spearman's Rho test was used. The majority of respondents had good knowledge (67.1%) and moderate emergency response attitudes (64.6%). The Spearman's Rho test showed a significant relationship between knowledge and emergency response attitudes (p<0.001) with a correlation coefficient of r=0.326, indicating a positive relationship with weak strength. Increased disaster education, training, and routine simulations are recommended to strengthen the community's emergency response skills.

Abstract

Surgical procedures can cause stress and anxiety in most preoperative patients. Spiritual therapy and relaxation therapy can be alternative therapy to reduce preoperative anxiety. This study aims to critically analyze and compare scientific evidence regarding the effectiveness of spiritual therapy and relaxation on preoperative patient anxiety levels. The method used in this study was a scoping review of research articles with a randomized controlled trial (RCT) design. Article quality assessment was carried out using the Critical Appraisal Skills Program (CASP) instrument. The reviewed articles focus on spiritual therapy interventions (prayer, hypnosis, healing touch) and relaxation in preoperative patients. Based on the results obtained, it was found that most articles showed that spiritual therapy and relaxation were effective in significantly reducing preoperative anxiety levels. Therefore, it can be concluded that spiritual therapy and relaxation are proven effective in reducing preoperative patient anxiety. This integration can be recommended to improve the quality of care and patient safety.

Abstract

Transforming growth factor-beta (TGF-β) regulates tendon repair, but dysregulated expression contributes to fibrosis. Propolis contains flavonoids and phenolics with immunomodulatory activity and may influence TGF-β signaling. This study evaluated intralesional propolis on TGF-β expression in a Wistar rat partial Achilles rupture model. A true experimental post-test only control group design was used. Male Wistar rats underwent standardized partial Achilles transection (30–40% thickness) and were randomized to: 50% propolis 0.1 mL intralesional (n=9), 0.9% NaCl 0.1 mL (n=9), or no injection (n=9). On day 35, tendons were harvested for immunohistochemistry using an IHC World–based scoring (H-score 0–300) assessed in ≥5 high-power fields by two independent raters. Interobserver agreement was moderate (κ=0.444, p<0.001). TGF-β H-score categories differed significantly among groups (Kruskal–Wallis p<0.001). The propolis group shifted toward higher expression: 88.9% (8/9) were 101–200 and 11.1% (1/9) were 51–100, whereas both comparator groups clustered at 0–50 (44.4%) and 51–100 (55.6%), with none ≥101. Intralesional 50% propolis increased TGF-β immunohistochemical expression at day 35 after partial Achilles rupture, suggesting enhanced regenerative signaling during tendon healing. Further studies should assess dose–response effects and biomechanical outcomes in vivo.

Abstract

Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy, predominantly presenting with sensory symptoms such as numbness and tingling. Conservative therapy with oral mecobalamin is widely used; however, the onset of improvement is often slow. Perineural injection of mecobalamin may provide a faster local effect, but clinical evidence remains limited. This study aimed to evaluate the effectiveness of combined perineural injection and oral mecobalamin compared with oral mecobalamin alone on sensory symptom improvement in CTS patients. This was a comparative interventional study conducted in patients with mild to moderate CTS at Dr. Mohammad Hoesin General Hospital, Palembang. Subjects were divided into two groups: a combination therapy group (perineural injection plus oral mecobalamin) and an oral mecobalamin group. Sensory symptoms were assessed using the Visual Analog Scale (VAS) at baseline, week 2, and week 4. Between-group and within-group comparisons were analyzed using t-tests, while mixed ANOVA was used to evaluate the effects of time and treatment group. Multivariate analysis was performed to identify factors independently associated with sensory symptom improvement. A total of 30 CTS patients were included in this study. There were no significant differences in baseline demographic and clinical characteristics between groups. Mixed ANOVA analysis demonstrated a significant time effect (p < 0.001) and a significant time group interaction, indicating a faster pattern of sensory symptom improvement (VAS score reduction) in the combination group. Multivariate analysis showed that combination therapy, sex, and occupation were independently associated with sensory symptom improvement, with occupation being the most dominant factor. The combination of perineural injection and oral mecobalamin is more effective in accelerating and enhancing sensory symptom improvement compared with oral mecobalamin therapy alone in patients with mild to moderate CTS.

Abstract

Hyperglycemia is a condition characterized by elevated blood sugar levels, which can trigger oxidative stress and cause cellular damage, including to skeletal muscle cells. The antioxidant content in rambutan leaf extract can lower blood sugar levels. This study aims to determine the histological features of skeletal muscle of Wistar rats given rambutan leaf extract after alloxan induction. Method: This study used a laboratory experimental design with a post-test only control group design. A total of 20 Wistar rats were randomly divided into four groups: a negative control group, a positive control group, and treatment groups administered rambutan leaf extract at doses of 25 mg/kg body weight and 50 mg/kg body weight. Data were collected through blood glucose level measurements, examination of histopathological specimens of skeletal muscle stained with hematoxylin and eosin (H&E), and measurements of muscle fiber diameter. The data were analyzed descriptively by comparing histological findings, blood glucose levels, and mean muscle fiber diameters across groups. Group A experienced a decrease in the average diameter of muscle fibers to 49.06 mm (longitudinal) and 70.45 mm (transverse) accompanied by morphological changes in the form of wavy skeletal muscle fibers. Group B showed improvements in muscle fiber structure with an average diameter approaching normal, namely 59.35 mm (longitudinal) and 91.35 mm (transverse). Group C saw an increase in the average diameter of muscle fibers to 100.97 mm (longitudinal) and 130.21 mm (transverse) indicating a tissue protective effect. Group D showed an average diameter of muscle fibers of 29.47 mm (longitudinal) and 54.34 mm (transverse) accompanied by wavy skeletal muscle fibers. Administration of rambutan leaf extract at a dose of 25 mg/kgBW to Wistar rats increased the average diameter of skeletal muscle fibers and maintained the basic structure of skeletal muscle tissue. Administration of rambutan leaf extract at a dose of 50 mg/kgBW showed wavy skeletal muscle fibers and a decrease in the average diameter of skeletal muscle fibers.

Abstract

Herpes zoster (HZ) is caused by the reactivation of latent varicella-zoster virus and most commonly affects the thoracic and trigeminal dermatomes. Involvement of the sacral dermatomes, particularly with perianal extension, is rare and can mimic various anorectal disorders, leading to delayed diagnosis in elderly patients with comorbidities. In this case report, herpes zoster in the perianal area significantly disrupted the patient's daily activities, requiring prompt diagnosis and management. A 78-year-old female presented with blisters accompanied by pain, burning, and tingling in the left buttock area extending to the anus. The dermatologic findings included multiple vesicles to bullae on an erythematous base containing clear fluid in a zosteriform arrangement, discrete to confluent, crusted form. A Tzanck test revealed multinucleated giant cells. The primary therapy consisted of oral acyclovir and mupirocin ointment. Herpes zoster involving the sacral to perianal dermatomes (S2-S4) can cause various functional complications, including neuritis characterized by sensory and motor deficits, which can affect the patient's daily activities. Antiviral treatment within the first 72 hours can help minimize complications. Pain should be controlled to prevent worsening the patient's quality of life. Early diagnosis and treatment, along with vaccination, can reduce morbidity and prevent complications, especially disruption of daily activities in patients with perianal HZ.

Abstract

Adolescents experience a transition from childhood dependence to adult independence, which is often accompanied by a search for identity, exploration of the social environment, and increased curiosity about new things, including risky behavior, namely drug abuse. Based on data obtained at the preliminary study stage at the BNN Banjarbaru City, two junior high schools were found with the most significant number of students who had ever abused drugs. This study aims to determine the description of drug abuse and determine the relationship between respondent characteristics and drug abuse in adolescents. This study employed a quantitative descriptive approach using a cross-sectional design conducted at SMP “X” in Banjarbaru City.  Results: The Simple Random Sampling technique was used, and 70 students were obtained as respondents. The instruments used were demographic questionnaires and ASSIST Young 10-14 and ASSIST Young 15-17. Data analysis was carried out univariately and bivariately (chi-square). There was a significant relationship between age and tobacco (p-value = .036) and alcohol ((p-value = .040). There was no relationship between gender and tobacco (p-value = 0.153) and alcohol ((p-value = 0.363). Prevention and intervention programs should pay special attention to the dangers of tobacco/vaping and alcohol, target adolescents to reduce moderate and high risks, and prevent initiation of use in non-risk groups.

Abstract

The pharmaceutical world is also revisiting traditional medicine. The use of plants or natural ingredients as medicine is known as traditional medicine. Traditional medicine, or herbal medicine, refers to ingredients or concoctions derived from plants, animals, minerals, or a combination of these ingredients. This poses a challenge in efforts to prevent and control malaria in Jayapura. The objective of this study is to examine medicinal plants used in traditional malaria treatment in the Harapan Community Health Center Working Area. This study is a qualitative study with a phenomenological approach. The techniques used are in-depth interviews and observation. The informants selected are those who have had malaria, malaria sufferers, and families of sufferers. a total of 41 respondents with malaria. In this study on the use of traditional medicine for malaria treatment, the sample size was originally planned to be 100 respondents, but only 41 malaria respondents were obtained. This was due to the limited time frame of the study, which ended in July 2025. All respondents were malaria patients who visited the health center, as well as long-term patients who were visited directly at their homes in order to meet the required sample size. The types of medicinal plants used by the community in the Harapan Community Health Center working area are papaya, yellow string.

Abstract

Creatine supplementation is a promising neuroprotective strategy during the perinatal period due to its role in energy metabolism and cellular defense. Preclinical studies have explored its potential to mitigate brain injuries caused by hypoxia, anoxia, and metabolic disturbances. However, variability in study designs and administration protocols necessitates a systematic review of existing evidence. To evaluate the effects of maternal creatine supplementation on neuromuscular development in preclinical studies. A PRISMA-guided review searched Scopus, PubMed, ProQuest, and Cochrane databases (from 2000 up to November 25, 2024) for studies on creatine's effects on brain biomarkers, histopathology, and pathophysiology. Studies using creatine or its derivatives as treatment and reporting experimental outcomes with control comparisons were included. An initial database search identified 346 records. Data were synthesized narratively due to models, outcomes, and dosage heterogeneity. Sixteen studies (2000–2023) involving rodents, sheep, pigs, and cultured fetal tissue were identified. Administration methods included oral, intravenous, intraperitoneal, and ex vivo approaches, with treatment primarily during mid-pregnancy to delivery. Fifteen studies reported significant neuroprotective effects, including biomarker improvements (13 studies) and histopathological changes (3 studies), with no adverse effects noted. Creatine shows potential benefits for fetal neurologic and muscular development, but further research is needed to ensure safe translational application.

Abstract

Exercise has a broad impact on health through various physiological mechanisms, one of which is through the regulation of interleukin-6 (IL-6). IL-6 is a pleiotropic cytokine that plays a role in immune responses, inflammation, and energy metabolism regulation, and is produced not only by immune cells but also by skeletal muscle as myokine during physical activity. This article aims to examine the physiological adaptations of exercise to IL-6 production and function and its relationship to energy metabolism, mitochondrial function, and inflammatory response. The research method employed was a literature review utilizing a descriptive approach. In acute exercise, muscle contraction triggers a rapid and temporary increase in IL-6 through mechanisms involving changes in calcium homeostasis, activation of AMP-activated protein kinase (AMPK) due to decreased energy reserves, oxidative stress, and epigenetic regulation. This study was conducted using a descriptive literature review approach. Relevant primary research articles and review papers were systematically identified through electronic databases, the selected literature was analyzed and synthesized to explore the mechanisms underlying exercise-induced changes in Interleukin-6 (IL-6) expression and signaling, with particular emphasis on acute and chronic exercise adaptations, energy metabolism, mitochondrial function, and inflammatory responses. The increase in IL-6 during this phase plays a role in maintaining energy homeostasis by increasing glucose transport, fatty acid oxidation, and modulating the immune response. Conversely, regular exercise induces long-term adaptations in the form of increased mitochondrial number and function, increased antioxidant capacity, and increased muscle sensitivity to IL-6 through the classical signaling pathway. These adaptations enhance IL-6 signaling efficiency, enabling metabolic and anti-inflammatory responses to be achieved with lower IL-6 production. Thus, regular exercise plays a crucial role in reducing systemic inflammation, improving energy metabolism, and supporting health and the prevention of chronic diseases.
