Abstract

Objective: To examine the intervertebral disc characteristic on magnetic resonance imaging (MRI) in lumbar herniated disc (LHD) patients with progressive neurological deficit.Methods: Patients were collected retrospectively from Dr. Hasan Sadikin General Hospital Database from 2011–2013 with LHD, had neurological deficit such as radiculopathy and cauda equine syndrome for less than four weeks with a positive sign confirmed by neurological examination and confirmatory with MRI examination.Results: A total of 14 patients with lumbar herniated disc disease (10 males, 4 females) suffered from progressive neurological deficit with an average age of (52.07±10.9) years old. Early disc height was 9.38±0.5 mm and progressive neurological deficit state disc height was 4.03±0.53 mm, which were significantly different statisticaly (p<0.01). Symptoms of radiculopathy were seen in 11 patients and cauda equine syndrome in three patients. Modic changes grade 1 was found in five patients, grade 2 in eight patients,grade 3 in one patient, Pfirmman grade 2 in eleven patients and grade 3 in three patients. Thecal sac compression 1/3 compression was seen in four patients and 2/3 compression in ten patients.Conclusions: Neurosurgeon should raise concerns on the characteristic changes of intervertebral disc in magnetic resonance imaging examination to avoid further neural injury in lumbar herniated disc patients.Keywords: Lumbar herniated disc, magnetic resonance imaging, progressive neurological deficit DOI: 10.15850/ijihs.v5n2.1042

Abstract

Objective: To examine the correlation between physical activity using the Global Physical Activity Questionnaire (GPAQ), anthropometry (body mass index/BMI), and forced expiratory volume in one second (FEV1) value with six-minute walk distance in healthy community-dwelling elderly.Methods: A cross-sectional design study with consecutive sampling was conducted. This study included sixty nine participants (30 males and 39 females), aged ≥60 years who lives at Batununggal region in Bandung, Indonesia. The data was analyzed statistically by using normality, correlation, and multiple regresion tests. Results: The results showed that there was a correlation between six-minute walk distance and BMI in females (p=0.006), and FEV1 value in males (p=0.010). No significant correlation was found between GPAQ value and six-minute walk distance in females (p=0.4074) and in males (p=0.0926). Conclusions: There was a correlation between anthropometry and FEV1 value with six-minute walk distance in healthy elderly. There was no significant correlation between physical activity and six-minute walk distance. Abstract

Objective: To compare the effects of foot muscle strengthening exercises with and without elastic band to the dynamic balance function in children aged 8–12 years with flexible flatfeet.Methods: Forty-one children (aged 8–12 years) with flexible flatfeet and dynamic balance problem were randomly allocated into either the intervention or control group. Both groups received thrice weekly training for 6 weeks. The foot strengthening exercise regimens consisted of pronation, supination, short foot, and tiptoeing using elastic band for the intervention group and without elastic band for the control group. Star Excursion Balance Test (SEBT) was tested before and after 6 weeks of training. Results: A statistically significant difference of normalized SEBT scores was found for both groups in both legs (p=0.00, p<0.05). The intervention group was significantly better than the control group for both right and left legs (p=0.00, p<0.05). Conclusions: Foot muscle strengthening either with or without elastic band improves dynamic balance in children aged 8–12 years with flexible flatfeet. Abstract

Objective: To determine the level of Selenium (Se) in peripheral and intracranial serum in pediatric patients with intracranial tumor. Selenium has chemopreventive potentials and acts as neuromodulator.Methods: This study was conducted on 13 pediatric patients with intracranial tumors who were treated in Dr. Hasan Sadikin Hospital, Bandung in the period of February 2014 to February 2015. Samples were taken from peripheral and intracranial serum. The results were analyzed with independent t-test and Pearson correlation test. Significance is defined as p≤0.05 with 95% confidence interval.Results: The results showed that the average Se concentration in peripheral serum was significantly higher than the Se concentration in intracranial serum (95.92±20.95 μg/L and 66.62±22.37 μg/L, respectively). After classifying the subjects into groups based on sex, age, tumor location, and grades, the difference between Se concentrations were still statistically significant (p≤0.05), with the exception of the supratentorial group (p=0.0053). Pearson correlation test showed very-low to medium strength correlations between peripheral and intracranial serum Se concentration in all groups (r=0.16–0.59, p>0.05).Conclusions: A significant difference is seen between the peripheral and intracranial serum Se concentration means in pediatric patients with intracranial tumors, with higher concentrations observed in the peripheral serum. Abstract

Objective: To evaluate the cytotoxic effects of ethanol extract from marine sponge Stylissa carteri in both parental and paclitaxel-resistant HeLa cervical cancer cells. Methods: This was an experimental in-vitro study subjected ethanol extract from Stylissa carteri obtained from Pramuka Island, Kepulauan Seribu National Park, Jakarta. Both parental and paclitaxel-resistant HeLa cells were treated with ethanol extract followed by microscopic observation and MTT assay. The IC50 and drug curves were analyzed using four-parametric logistic regression by SigmaPlot for Windows version 12.0 from Systat Software Inc., USA.Results: Ethanol extract from Stylissa carteri triggered cell death-associated morphological changes in parental Hela cells. It demonstrated cytotoxic activities with an IC50 value of 1 ppm. Importantly, this extract also triggered cell death in paclitaxel-resistant HeLa cells. The IC50 of ethanol extract of Stylissa carteri was 4 ppm in paclitaxel-resistant HeLa cells.Conclusions: There is a potential cytotoxic activity of Stylissa carteri that is not only confirmed in parental HeLa cells but also in paclitaxel-resistant HeLa cells.Keywords: Cervical cancer, paclitaxel resistance, HeLa cells, cytotoxicity assay, Stylissa carteri DOI: 10.15850/ijihs.v6n2.1378

Abstract

Objective: To determine the prevalence of Mycoplasma genitalium (M. genitalium) infection in HIV positive patients by PCR examination in Teratai Clinic of Dr. Hasan Sadikin General Hospital, Bandung in order to reduce sexually transmitted diseases, especially M. genitalium infection in HIV positive patients.Methods: This study was a cross-sectional study with consecutive sampling methods. Eighty one HIV positive patients attending the Teratai Clinic of Dr. Hasan Sadikin General Hospital, Bandung were recruited to be the subjects of this study. All subjects underwent history taking, physical examination, and PCR examination for M. genitalium. Specimens were taken from cervical smear in females and first void urine in male.Results: The prevalence of M. genitalium based on the PCR examination in HIV positive patients attended to Teratai Clinic Dr. Hasan Sadikin General Hospital Bandung was 9%. Abstract

Objective: To analyze the effectiveness of phosphorus 32 (32P) application compared to 10 mg/cc triamcinolone acetonide (TA) intralesional injection in keloid.Methods: A single blind (evaluator blind) randomized clinical trial on 52 keloids was conducted during the period of May – August 2014 in Tumor and Skin Surgery Clinic, Department of Dermatovenereology and Department of Nuclear Medicine and Molecular Imaging, Dr. Hasan Sadikin General Hospital Bandung.Results: Lesions were divided into two groups with each group consisted of 26 lesions. Group A was treated with 32P application whereas group B was treated with 10 mg/cc TA intralesional injection. Flattening of the lesion of more than 50% was higher in the group treated with 32P application (76.9%) compared to the group that was treated with 10 mg/cc TA intralesional injection (57.7%) on the 8th week, but the difference was not statistically significant (p>0.05).Conclusions: Based on the statistical analysis, the application of 32P was as effective as the intralesional injection of 10 mg/cc TA for keloid lesions.Keywords: Flattening, keloid, phosphorus 32, triamcinolone acetonide DOI: 10.15850/ijihs.v4n1.683

Abstract

Objective: To analyze the correlation between nutrition knowledge, vitamin C intake, and nutritional status on blood pressure among elderly with hypertension.Methods: This was a cross-sectional study with purposive random sampling which was conducted from April to May 2017. This study included one hundred twenty-five elderly subjects with hypertension who met the inclusion criteria (17 males and 108 females), aged ≥60 years, who lived in the work area of Juwiring Public Health Center in Klaten district, Central Java, Indonesia. The data were analyzed statistically by using normality, correlation, and multiple linear regression tests.  Results: The results of the correlation test showed that there was a correlation between nutrition knowledge (p=0.011), vitamin C intake (p=0.012), nutritional status (p=0.048), and blood pressure hypertension in elderly based on the results of multiple linear regression. All data were analyzed statistically using Spearman correlation test to examine the distributed data. A significant correlation was found between nutrition knowledge, vitamin C intake, and nutritional status (p<0.05) using multiple linear regression (p<0.05). A normality test using the Kolmogorov-Smirnov test resulted in normal residue with p-value 0.000. The results showed that the regression analysis could be used to predict the blood pressure.Conclusion: There was a correlation between nutrition knowledge and blood pressure of hypertension in elderly. There was a correlation between vitamin C intake and a blood pressure of hypertension in elderly. Abstract

Objective: To study the transformation point of meningeal and periosteal dural at the porus of internal acoustic canal (IAC) in order to verify the different thickness of meningeal and periosteal dura in vestibular schwannomas (VS). Methods: Three IAC cadaver specimens and ten samples of VS patients from porus were obtained and analyzed. Samples were stained by using Masson trichrome technique after cutting in 6 micron of thickness. The samples were then observed under light microscopes to understand the meninges pattern in the IAC. Results: The meningeal dura is becoming thin at the porus and disappears at the meatal portion to form epineurium. However, the periosteal dura is lining continuously to the fundus. In VS, the meningeal dura becomes thick and forms a pseudo-capsule in the middle of meatus, known as perineurium. The residual nerve filament was compressed by the tumor parenchyma. Between the tumor and nerve interface, three or more perineureal layers are seen. The perineurium in the cisternal portion was consistently loose and forms the tumor and arachnoid nerve interface. Almost all the nerve filaments are displaced to the tumor periphery near the pseudocapsule. In contrast, the periosteal dural of VS is becoming thin and disappear nearby the middle of meatal portion. This changing site establishes “meningo-periosteal ring” of VS because of the encircling nearby the porus. Conclusions: In IAC, the meningeal dural becomes thin. The periosteal dura is lining continuously to the fundus. In VS, the meningeal dura becomes thick, joins perineurium and forms pesudocapsule near the porus, but the periosteal dura disappeared. Abstract

Objective: To assess the safety and immunogenicity of Haemophilus influenzae type b/polyribosylribitol phosphate-Tetanus (Hib/PRP-T) liquid vaccine in healthy adults.Methods: An open label prospective intervention phase I study was conducted in Dr. Hasan Sadikin General Hospital from November to December 2010. Healthy adults aged 18−40 were eligible to participate. Participants received one dose of Hib/PRP-T liquid vaccine. Blood samples were taken before, 4 days and 1 month after vaccination. For a 28-day period following vaccination, solicited adverse events were collected in the subjects’ diary and assessed afterward. Results: Neither local reactions nor immediate systemic events were observed during a 30-minute period after immunization. There were no serious local or systemic reactions in this study. All of local and systemic reactions observed were slight, transient, self-limiting in time, without lasting for more than 72 hrs. after the administration of the vaccine, and resolved without any medical intervention. Hematologic and biochemical indices before and 4 days after vaccination showed in normal limits. All subjects (100%) reached protective levels of antibodies (seroprotectivity) against Hib. All subjects demonstrated antibodies performing high bactericidal activities 1 month after immunization. Conclusions: This study demonstrated that liquid Hib/PRP-T vaccine is highly immunogenic and have a beneficial safety when administered to healthy adults.
