Abstract

Since the opening of forensic services at RSUP dr. Soeradji Tirtonegoro Klaten since 2014, many forensic cases were handled. Based on the type of case, in the death cases, there are traffic accidents, crime, and other deaths. While in life cases, there are sexual violence, physical violence, and psychiatry. Research on basic characteristic patients’ are very important to do, especially for the police, government, hospital, and community. The purpose of this study was to determine the basic characteristic patients’ of Visum et Repertum in RSUP dr. Soeradji Tirtonegoro Klaten 2014-2016. This research was a cross-sectional study, with a total sampling method. The data were taken from Visum et Repertum during    2014 - March 2016. There were 242 life cases (124 women, 118 men), and 23 death cases (6 women, 17 men). Most patients were productive age (22-40 years old), with 33.5% life cases and 30.4% death cases. The highest number of life cases was physical abuse (109 cases; 45%), then all of death cases was accident. The highest profession of patients were students (30.6% in life cases), while in the death cases were private (43.5%). Head trauma was the most common injury in patients (45.9% life cases and 82.6% death cases). Most of the perpetrators were unknown people (77.7% life cases, as well as 82.6% of death cases). From both cases, the patients were from productive age, distribution of fatal wound was in the head, and the prepetators were unknown. These data are very important in educational process of society.

Abstract

Ethanol extract of Gratophyllum pictum L (G. pictum L). leaves contain phytochemical compounds such as alkaloids, tannins and flavonoids, which have antibacterial activity against Staphylococcus aureus. This study aims to prove the effectiveness of  ethanol extract of G. pictum L. to reduce the level of TNF-α and NO in Swiss mice infected with S. aureus.The study treated 30 Swiss mice divided into 5 groups, consist of: K (-) normal mice ; K (+) mice were infected with S. aureus ; P1, P2, P3 were infected with S. aureus and given the ethanol extract of G. pictum leaves,75,150, and 300 mg/kgBW/day for 7 days . Mean levels of TNF-α of group K(-), K (+), P1, P2, P3 were 300.29; 550.29; 390.34; 517.67; 327.55 pg / mL respectively. One Way ANOVA test showed that there were significant differences between all treatment groups (p = 0.001), LSD Post Hoc test revealed P1 and P3 groups different significantly with K (+) with p= 0.013 and p = 0.001. Mean levels of NO of group K (-), K (+), P1, P2, P3 were 1.32; 3.56; 2.50; 2.64; and 1.56 μM/mL respectively. Kruskal WallisTest showed that the levels of NO not significantly different, although NO levels at 300 mg/kgBW/day similar with to the levels of NO group K(-). Spearman correlation test showed that there were correlations between TNF-α and NO with the variation results on each treatment groups. Ethanol extract of G. pictum leaves at 300 mg/kgBW/day for 7 days is regarded as the effective dose to reduce TNF-α and NO levels in Swiss mice infected by S. aureus.

Abstract

 The Eurycoma longifoliais bitter therefore, to address this problem, it is prepared in a capsule form. The capsule dosage form  must has  the parameters of good quality, thus expected to increase the potential use of biodiversity and the interest of the community in consuming the medicine from the natural materials. The purpose of this research is to get the capsule product of ethanol extract of the Eurycoma longifolia root of quality through the test of total plate value and yeast yeast contamination, the determination of the metal content of Pb, Hg, Cd and the level of flavonoids released in the capsule dissolution test of ethanol extract Eurycoma longifolia root. The result of this research shows that the dissolution efficiency (DE120) capability of ethanol extract of Eurycoma longifolia root is 81,04±2,38%. Test of heavy metal content in capsule of  of ethanol extract of Eurycoma longifolia root found level of Pb equal to 4,132 ppm and not detected content of Cd and Hg in capsule. This indicates that the capsule meets the requirements set by BPOM RI. Tests for total plate numbers (ALT) and yeast capsule ethanol extract of the E. longifolia root were each obtained data of 70 x 102 colonies / g and 4 x 102 colonies / g. It also shows that the capsules meet the requirements by BPOM RI.

Abstract

Endophytic fungi are microorganisms reside in the living tissues of the host plant. This fungi can contribute to provide protection to the host from the infection caused by another microorganism. The one reasonable strategy to discover a new antibacterial agent from endophytic fungi are from the plant which lived in special condition such Rhizophora mucronata. The aim of this research is to determine antibacterial activity of the endophytic fungi extracts assosiated with Rhizophora mucronata from Sagara anakan. The isolated endophytic fungi was identified as Neopestalotiopsis sp. and Peniophora lycii using molecular analysis method. The antibacterial activity was carried out by using microdillution method. The antibacterial properties from mycelium extracts showed moderate to low antibacterial activity with Minimum Inhibitory Consentration (MIC) values of 125-500 µg mL-1 against Eshcerichia coli ATCC 25922 and Staphylococcus aureus ATCC 25923 bacteria. The n-hexane extract of both endophytic fungi shows the strongest antibacterial activity against Escherichia coli with MIC values of 125 µg mL-1

Abstract

Pendahuluan: Stroke yang berhubungan dengan fibrilasi atrium dan kardioemboli cenderung bermanifestasi lebih berat, berisiko tinggi berulang, serta mortalitasnya lebih tinggi. Pemberian antikoagulan lebih dianjurkan pada stroke iskemik dengan fibrilasi atrium sebagai upaya pencegahan, baik primer maupun sekunder. Skor stratifikasi CHA2DS2-VASc digunakan untuk menseleksi pasien pada risiko terkena stroke iskemik untuk pemilihan antitrombotik yang tepat, pada risiko untuk terjadinya perdarahan skor HAS BLED digunakan sehingga dapat mengurangi kekhawatiran dokter dalam menggunakan antikoagulan. Penelitian sebelumnya sangat terbatas sehingga diperlukan penelitian yang lebih mendukung pada penelitian-penelitian sebelumnya.Tujuan: untuk mengetahui penggunaan antitrombotik berdasarkan pada skor CHA2DS2-VASc dan skor HAS BLED terhadap pola pengobatan pada pasien stroke dengan fibrilasi atriumMetode: penelitian ini merupakan studi deskriptif dengan rancangan potong lintang (cross sectional). Sampel diperoleh dari stroke registry  dan rekam medis dari pasien di Poli Saraf  Rumah Sakit Bethesda Yogyakarta pada tahun 2009-2016. Data dianalisis menggunakan analisis univariat.Hasil: data diperoleh dari 77 pasien stroke iskemik dengan fibrilasi atrium yang terdiri dari 33 laki-laki (42.8%)  dan 44 perempuan (57.1%), proporsi terbesar ada pada pasien berusia >70 tahun (33.3%). Hasil analisis univariat menunjukkan bahwa penggunaan antiplatelet ada pada 71 (92.2%) pasien, penggunan antikoagulan 6 (7.7%) pasien, serta nilai skor CHA2DS2-VASc 0=3 (3.8%), 1=18 (22.5%), ≥2=56 (70.1%), pada skor HAS BLED 0=7 (8.8%), 1=27 (33.8%), 2=29 (36.3%), 3=13 (16.3%), 4=1 (1.3%)Kesimpulan   : skor CHA2DS2-VASc dan skor HAS BLED belum mempengaruhi penggunaan antitrombotik pada pasien stroke iskemik dengan fibrilasi atrium di RS Bethesda YagyakartaKata Kunci    : Stroke iskemik, skor CHA2DS2-VASc, skor HAS BLED, Fibrilasi atrium.

Abstract

Stroke murupakan keadaan gawat darurat yang memerlukan penanganan dengan cepat dan tepat di IGD, namun kenyataannya sering dilaporkan kejadian medication error (ME) di IGD. Keterbatasan waktu, kondisi klinis pasien dan kondisi Instalasi Gawat Darurat (IGD) merupakan faktor yang diduga berkaitan dengan peningkatan kejadian ME di IGD. Belum diketahui dengan pasti berapa kejadian ME pasien stroke di IGD rumah sakit X dengan keterbatasan sarana prasarana. Penelitian ini bertujuan menentukan gambaran kejadian ME di IGD RS daerah dengan keterbatasan sarana dan prasarana di rumah sakit X.Telah dilakukan penelitian observasional deskriptif dengan pengambilan data pasien secara prospektif pada bulan Desember 2014 – April 2015 di IGD Rumah Sakit X di Yogyakarta. Didapatkan sebanyak 106 pasien dengan kriteria inklusi subjek adalah pasien dengan diagnosis stroke non hemoragik maupun stroke hemoragik, baik dengan penyakit penyerta maupun tanpa penyakit penyerta. Dirawat dalam ruangan IGD di Rumah Sakit X di Yogyakarta pada periode penelitian yaitu selama Desember 2014 – April 2015 dan bersedia terlibat sebagai responden dalam penelitian. Data obat dan riwayat pengobatan diambil dari catatan rekam medis. Peneliti melakukan wawancara dengan pasien atau keluarga pasien untuk melengkapi data riwayat pengobatan. Dari data tersebut dilakukan kajian terhadap kemungkinan terjadinya ME pada fase prescribing dan administrasi (cara pemberian) berdasarkan PERDOSSI. Pengolahan data dilakukan dengan analisa monovariat untuk mengetahui gambaran karakteristik umum pasien (gambaran demografi), karakteristik klinik pasien serta gambaran kejadian, distribusi dan fase ME. Data gambaran kejadian, distribusi dan fase ME disajikan secara deskriptif (persentase).Hasil dari penelitian ini adalah jumlah pasien yang berusia lebih dari 60 tahun  sebanyak 67 pasien (63,2%). Sebanyak 85,8% pasien didiagnosis  stroke non hemoragik dan sisanya sebanyak 14,2%  didiagnosis stroke hemoragik. ME terjadi pada 104 pasien (98,1%). Distribusi kejadian ME pada fase prescribing dan administrasi per pasien adalah 28 pasien mengalami satu kejadian ME, 48 pasien mengalami dua kejadian ME, Sebanyak 20 pasien yang mengalami tiga kejadian ME, 8 pasien mengalami empat kejadian ME. Total kejadian medication error yang terjadi sebanyak 216 dari 106 pasien dengan nilai rata-rata 2 kejadian error per pasien. Medication error di fase prescribing sebanyak 98,11% dan medication error di fase administrasi sebanyak 61,32%.Kata kunci : Stroke, geriatri, medication error, prescribing, administrasi.

Abstract

The suji leaf are generally used by the community as a gargle to remove plaque. Toothpaste is one form of dosage suitable for oral health with triclosan active ingredients, but triklosan is corrosive, resistant to bacteria, and decrease the hormonal system, especially the thyroid hormone. The aim of this research is to know the effect of the addition of variation of the concentration of combination of triclosan-ethanol extracts of suji leaf (T-EESL) on toothpaste preparation to the chemical characteristics, antibacterial activity of Streptococcus mutans. The ethanol extract of 70% of suji leaf was obtained by maceration method, then made toothpaste preparation with variation of T-EESL combination of FI (1:0), FII (0,5:0,5), FIII (0:1). Toothpaste was tested chemical physics characteristics (organoleptic, viscosity, pH, and foam power), antibacterial activity test. The test results are analyzed descriptively. Based on the results of the research obtained it can be concluded that the results of testing the physical physics of chemistry showed the results that all toothpaste formulas meet the physical characteristics of chemical toothpaste. T-EESL toothpaste (0.5:0,5) (FII) has strong antibacterial activity against Streptococcus mutans so it can be used as an alternative to the existing dosage preparation in the market with triclosan active substance.

Abstract

The present study tried to investigate potency of L. sarmentosa root extract on quality and viability of mice sperm.This study used 25 Swiss male mice were randomly divided into five experimental groups of 5. The ethanolic extract of L. Sarmentosa (100, 200, 400 mg/kg b.w/days), control and X-gra were administered orally for 14 days. The animals were evaluated for sperma quality (sperm count and sperm motility) and viability at 14 days treatment. The results showed that administration of L.sarmentosa roots extract significantly increasing the sperm count (970x106cell/mL) at 400 mg/kg b.w/days (P 0.0040). The extract dose of 200 mg/Kg b.w and 400 mg/Kg b.w had the highest motility values (95%). Viability of sperm are 83.8%; 96.0%; 87.8% respectively at a dose of 100 mg/kg b.w, 200 mg/kg b.w and 400 mg/kg b.w. Present finding oral administration of L. sarmentosa roots extract has a potency of the quality and viability of mice sperm.

Abstract

Centella asiatica is a kind of herbs which contains antioxidant that be trusted to overcome of various skin problems such as keloids, cellulite, and black stains. One of the additional ingredients used to make the transparent soap is sucrose or frequently called white sugar. In the process of making the transparent soap, white sugar is served to help the transparency of soap. The purpose of this research is to find out the effect of increasing the sucrose toward the degree of the transparency of the soap, stability of physical properties before and after storage for eight weeks and find out the skin response towards the soap by doing irritability test towards some respondents. Pegagan extract transparent soap is varied with the concentration of sucrose 11 %, 13%, and 15 %. The soap which is produced is conducted with a series of tests of physical characteristics include transparency, pH, water content, and the stability of the foam. The results showed that the addition of sucrose affects the level of transparency and soapy water levels and no effect on pH and foam stability. Irritation test of the respondents showed most respondents did not become irritated. Abstract

Glimepiride is a third generation sulphonylurea antidiabetic drug. Glimepiride is poorly water soluble drug that may cause poor dissolution and unpredicted bioavailability. Self nanoemulsifying drug delivery systems (SNEDDS) have become a popular formulation option as nanocarriers for poorly water-soluble drugs. The objective of this research was to develop SNEDDS formulation of glimepiride to improve oral dissolution and bioavailability. Glimepiride SNEDDS  was formulated using oleic acid as oil phase, tween 80 as surfactant, and transcutol as co-surfactant due to their higher solubilization effect. The formulated SNEDDS were evaluated for % transmittance, dispersibility, thermodynamic stability, dissolution, globule size and morphology analysis. The results showed that the glimepiride SNEDDS was rapidly formed clear emulsion and stabile based on thermodynamic test. Transmission electron microscopy demonstrated the spherical droplets morphology in nanometer range. The globule average diameter size was 45 nm. The SNEDDS formulation significantly increase dissolution of glimepiride compared with pure drug.
