Abstract

The coronavirus (COVID-19) pandemic has presented a significant public health threat, posing an immediate risk to the health of the global population and creating far-reaching long-term consequences. The main disorder that is often caused by COVID-19 is the respiratory system, so smoking and cigarette use can be categorized as people who have a high risk of developing severe lung infections. Smoking is associated with a higher chance of developing a critical illness and a higher chance of dying from the COVID-19 disease. This study aims to analyze the effect of smoking history on the severity of COVID-19. This study uses a systematic review and meta-analysis design. Screening articles using the PRISMA method, from 2,014 articles, 9 articles were obtained that met the restriction criteria. Based on the results of a systematic review that has been carried out on the influence of a history of smoking on the severity of COVID-19, it shows that a history of smoking can develop a critical illness that requires mechanical ventilation, developing the classic triad of symptoms that indicate a diagnosis of COVID-19 (fever, persistent cough and shortness of breath). , increased burden of COVID-19 symptoms, ICU admission, need for intubation, increased frequency of ACE2 genotype and had a higher risk of death. Active smoking and smoking history significantly increased the severity and mortality of COVID-19 (OR= 2.59; p= 0.006).

Abstract

Prevention of catheter-associated urinary tract infections (CAUTI) can be done with simple initiatives such as catheter insertion and care can contribute to preventing infection, chlorhexidine is one of the commonly used antiseptic agents due to its cationic molecular nature with various antimicrobial activities, however, until now the choice of the concentration of the use of chlorhexidine is still a polemic in its use. Therefore, the aim of this review is to evaluate the use of chlorhexidine before urinary catheter insertion and treatment in preventing CAUTI. Manual reference searches were carried out using the scoping review method, through the PubMED, DOAJ, Science direct, EBSCO, ProQuest, Garuda, and Gray Literature databases (google scholar). The study was included based on PCC criteria (Population: patients with urinary catheters, Concept: Chlorhexidine use, Context: urinary catheter insertion and care), published from 2011-2021. A total of 171 studies were screened, the remaining eleven articles that met the inclusion criteria and were included in this review. Based on the results of the study found the use of Chlorhexidine with various concentrations, namely 0.05%, 0.1%, 0.5%, 0.9%, 2%, and 4% with various intervention uses, namely, SRV (sustained-release varnishes), periurethral cleaning before catheterization , cleaning urine bag, and bathing Chlorhexidine. The application of the use of chlorhexidine with a concentration of 0.5% - 4% is one of the strategies to prevent CAUTI at the time of insertion and treatment of urinary catheters, especially in the intensive care room.

Abstract

Administration of Dexamethasone in patients with diabetes needs attention because corticosteroids can increase blood glucose concentrations. Therefore, this study aims to determine the effectiveness of using Dexamethasone in patients with severe COVID-19 and how to regulate blood glucose in patients with severe COVID-19 in patients with comorbid Diabetes Mellitus. Inclusion criteria for severe COVID-19 patients with comorbid Diabetes Mellitus who use Dexamethasone and therapy groups and receive insulin therapy with or without OAD in inpatient installations. The sample data obtained from the patient's medical records were 27 patients who met the inclusion and exclusion criteria consisting of 70% male and 30% female. Dexamethasone dose of 1 x 6 mg iv mostly uses combination therapy A, the duration of administration is 8 days, and has an average length of stay of 13 days. The shortest average length of hospitalization was 12 days for patients receiving dexamethasone with therapy C for 10 days, while therapy B for 5 days had an average hospitalization of 14 days. Administration of Dexamethasone dose of 1 x 6 mg iv is effective in severe COVID-19 patients with comorbid Diabetes Mellitus based on the length of hospitalization, respectively 13 days, 14 days, and 12 days with an improvement in the clinical condition of oxygen saturation by 91%, 100%, 100%; RT-PCR/Rapid Ag became negative 74%, 100%, 100%; improvement of chest x-ray results, namely 83%, 33%, 100%. The effect of glucocorticoid dexamethasone in comorbid Diabetes Mellitus patients is regulated by administering long-acting insulin, rapid-acting insulin, and oral anti-diabetic.

Abstract

Diarrhea is still a significant problem for people around the world. Diarrhea, a water-borne disease, can be caused by one type of parasite, namely intestinal protozoa. Intestinal protozoa can contaminate water sanitation if their hygiene is not maintained properly. This study’s purpose was to comparison of protozoal intestinal contamination of dug wells with hand pump wells based on physical conditions in Jelbuk District, Jember Regency. This study is an observational analytic study with a cross-sectional design. This study used primary data on the physical condition of dug wells and hand pump wells. The sedimentation method obtained primary data on intestinal protozoa contamination by microscopic detection. This study obtained 36 well water samples consisting of 30 dug wells and 6 hand pump wells. Entamoeba histolytica found in 4 dug wells (11.1%) is the type of intestinal protozoa that contaminates. This study concludes that there is a difference in contamination between dug wells and hand-pumped wells because the intestinal protozoan type Entamoeba histolytica is only found in dug well, but statistically, there is no difference in contamination between dug wells and hand-pumped wells.

Abstract

Background: 80% of women describe labour pain as very severe and unbearable pain. Physiologically, labour pain will arise longer and more frequently, this will make the mother feel restless and anxious which causes an increase in excessive catecholamines and steroid hormones and causes uterine smooth muscle tension, making pain stimulation more intense. Lavender aromatherapy contains linalool, linalyl acetate and 1,8- cincole which can relax and quickly relax tension in muscles. Objective: To determine the effect of lavender aromatherapy on the level of labour pain in the active phase of the first stage of labour at RSIA Asy-Syfa Sangatta. Methods: This type of research is pre-experimental with one group pretest-posttest design by purposive sampling. The respondents in this study were 26 mothers who gave birth at RSIA Asy-Syfa Sangatta. The instrument uses a numerical rating scale (NRS) observation sheet and lavender aromatherapy SOP. Data were analyzed by Wilcoxon Signed Rank Test. Results: Of 26 respondents, most experienced severe pain as many as 21 people (80.8%) and after giving lavender aromatherapy 23 people (88.5%) experienced severe pain, 3 people (11.5) experienced severe pain. Conclusion: From this study, it was obtained a value of < 0.001 which means that there is an effect of lavender aromatherapy on the level of labour pain in the active phase of the first stage of RSIA Asy-syfa Sangatta.
