Abstract

Background: The prevalence of vitamin D deficiency in both developed and developing countries are 30% - 75% with increased risk reported among women, elderly, obese individuals and those with low sun exposure. Fisherman has been acknowledged as an occupation with high sun exposure. However, a previous study showed that 4 in 5 fishermen were vitamin D deficient. The objective of this study is to investigate vitamin D intake and sun exposure among fishermen. Method: This cross-sectional study recruited fishermen residing in the working area of several public health centres in Mataram and West Lombok districts. The subjects were classified as fishermen and non-fishermen. Vitamin D intake and sun exposure were the the main variables. Results: One in two fishermen and three in four non-fishermen exhibited low intake of vitamin D. Fish intake in the fisherman and the non-fisherman groups were 937 gram per week and 652 gram per week, respectively. The most consumed fish was tuna. Fish intake of 937 gram per week was equivalent to ~12 µg vitamin D intake and 23.6 nmol/L vitamin D concentration. The majority of subjects were unable to achieve the optimal UVB radiation. Conclusion: Insufficient vitamin D intake and limited sun exposure are prevalent in fisherman and non-fisherman groups.

Abstract

Background: Patients suffering from coronary heart disease can experience psychological changes as well as quality of life. This study aims to determine the quality of life of patients with coronary heart disease post heart attack. Methods: A total of 30 patients with coronary heart disease after diagnosed STEMI, NSTEMI, or unstable angina for less than one year and had never been diagnosed with a mental disorder. Quality of life was measured by the Indonesian version of the SF-36 Health Survey instrument. Unpaired t-test and Mann Whitney test was conducted to analyze the differences the quality of life based on gender. Results: As many as 36.7% of subjects stated that the current condition was somewhat worse than one year ago. Subjects who stated the same or better were 56.6%. Men have a higher quality of life than women, although it is not statistically significant (p> 0.05). Emotional well-being is the dimension that has the highest average (75.1 ± 17.3) in both the male group (74.8 ± 17.7) and the female group (77.3 ± 16.7). Role limitations due to emotional problems have the lowest score (18.8 ± 33.5) followed by role limitations due to physical problems (20.0 ± 29.7). Conclusion: Patients with coronary heart disease after the attack have the perception that there are limited roles both due to emotional and physical problems. There was no significant difference between the quality of life of coronary heart disease sufferers after the attack of the male and female groups.

Abstract

Background: Determining the time of death (post-mortem interval) is very important in forensic investigations. Blowflies growth is the main and most accurate indicators time of death to estimation PMI. The growth of flies can be affected by various factors including climate, geography, and medicines contained in the body. The aim of this study was to examine the effect of giving variations barbiturate (sodium phenobarbitone) lethal dose to the length of Calliphoridae larvae to predicting PMI. Methods: This study is an experimental study with post-test only control group design, the data were analyzed using multivariate linear regression analysis test. This study consisted of 1 control group (neck dislocation) and 3 control groups of phenobarbital (750 mg/kgBW, 900 mg/kgBW and 1050 mg/kgBW). All group are put into a cage containing Chrysomya sp. The measurements of larva length are carried out every 12 hours by taking the largest larvae size in each rat in the study group. Result: The results shown that phenobarbital was the most influential factor in the larval length of Chrysomya sp. (p <0.05, R = -0.64) compared to temperature and humidity. Mann Whitney test results (p <0.05) shown the significant difference in Chrysomya sp. larvae length in each study group. Conclusions: Variation lethal dosage of barbiturates (sodium phenobarbital) affects the growth of Chrysomya sp larvae length.

Abstract

Mercury exposure of pregnant women comes from various sources including pollution from gold mining, from the consumption of fish and equipment in the neighborhood where they live. Intoxication that occurs as a result of exposure is determined by the type of mercury, route, dose, duration of exposure and age of the host. Mercury exposure in a pregnant woman not only causes intoxication for her but also affects the fetus . The fetus is a developmental phase that is very susceptible to mercury exposure of all kinds from its mother. Various effects that can arise from this exposure on the fetus include stillbirth, low birth weight and congenital abnormalities.

Abstract

The physiological function of the immune system is to defend against infectious microbes; even non-infectious foreign substances and damaged cell products that can elicit an immune response. The response from exercise, both acute and chronic, will have an effect on the components of the immune system. The immune response that occurs depends on the intensity and duration of exercise. Moderate intensity exercise will stimulate various parameters related to cellular immunity so that it can reduce the risk of infection, while high intensity exercise will result in a reduction in the same parameters, which will increase the risk of infectious diseases. The response from exercise or doing regular physical activity can affect immune cells, such as: neutrophils, antigen precenting cells (APC), natural killer cells (NK cells), and lymphocytes as well as cytokine production.

Abstract

Scabies is an infected skin disease caused by infestation and sensitization of Sarcoptes scabiei var. Hominis and its products with nocturnal pruritus as symptom. At 2017, scabies was included to Neglected Tropical Diseases (NTD). This disease related to low economic state, low educational level, poor hygiene, linited access to clean water, dense settlement, wrong diagnosis and treatment. Scabies can easily transmitted both directly and indirectly. Scabies was neglected by people because people thought thah this disease wasn’t a life-threatening disease, however this disease can lead to some complications such as impetigo, celullitis, abscesses, even sepsis, in the case of S. pyogenes, post-streptococcal glomerulonephritis and acute rheumatic fever can happen. In resource-poor countries, outbreaks of acute poststreptococcal glomerulonephritis coincide with outbreaks of scabies and may contribute to development of chronic kidney disease and subsequent renal failure. Patient came with complaints itching between fingers on both hands and foots, wrist, back, and crotch especially at night since four months ago. He has poor personal hygiene. The patients was diagnosed with scabies, then he got treatment with family medicine approach. Medicine treatment he got was Permethrine cream 5%, Cetirizine tablet, and mixed cream of Fusidat Acid and Desoximetason. The family who lived together with patient also got treatment and educated about the disease.
