Abstract

Introduction: Brain injury is a major cause of death and disability. Computerized Tomography (CT) scanning of the brain is essential for diagnostic screening in need of neurosurgical intervention and also provide information about patient prognosis. Methods: This study is a retrospective study design to assess the comparison of the CT scan scoring system of the head with the prognostic factors of brain injury. Head CT Scans were assessed according to the following Rotterdam and Helsinki CT Scores. All patients were managed according to standard guidelines. Surgery was performed as indicated. The primary objective was mortality assessed using the Glasgow Outcome Scale (GOS) at discharge and 3 months after. Results: Altogether, 60 patients were included. The results of the ROC analysis of the two CT scoring systems, the Helsinki score provided a better sensitivity score> 80% compared to the Rotterdam score, with a positive predictive value of around 74% each for assessing the outcome (GOS) of brain injury patients Conclusions: The Helsinki score scoring system provides a better sensitivity compared to the Rotterdam score

Abstract

Introduction: The clinical usage of the ureteral stent caused several stent-related symptoms (SRSs), including lower urinary tract symptoms, pain, general health, work performance, sexual matter and additional problems. This study aims to evaluate the effectiveness between tamsulosin and solifenacin combined with tamsulosin for SRSs.Methods: This double blind randomized controlled trial used 50 patients. Between September 2020 and February 2021, patients underwent double-J stenting after retrograde ureteroscopy were analyzed. All patients would be randomized in a 1:1 ratio in a 1:1 ratio to receive either tamsulosin 0,4 mg (Group A: 25 participants) or tamsulosin 0,4 mg & solifenacin 5 mg combination (group B: 25 participants).We used the Ureteral Symptoms Score Questionnaire (USSQ) as an outcome measure at 1s, 2nd, 3rd and 4th weeks after stent insertion. Results: Group A was found more in women, while in group B was more in men. The highest age in group A and group B was 50-60 years old (36% and 64%). The location of stenting in group A was found more in left (60%), while the group B in the right (52%). Both groups had more patients who were normal nutritional status (60%). There was an improvement in pain and work performance after treatment in both groups, meanwhile group B showed better improvement than group A at all weeks. in addition, there was an improvement in urinary symptoms and sexual matter in group B better than in group A at 2nd to 4th week. Furthermore, group B showed better improvement in general health and additional problem only at 4th week. Conclusions: Combination therapy with tamsulosin and solifenacin improved USSQ score more than the monotherapy group. This implied that combination therapy is optimal for improving SRSs.

Abstract

Introduction: The readmission rate due to heart failure increases every year in South Sulawesi. A Frequently used method for assessing readmission is an echocardiographic examination by measuring ejection fraction. However, recent studies show that the speckle tracking parameters are more sensitive to predict readmission of heart failure patients. In this study, we assess the role of Global Longitudinal Strain (GLS) and Global Circumferential Strain (GCS) parameters on the Echo Heart Failure Score (EHFS) as a 60-days readmission predictor in heart failure patients. Methods: We included 175 patients admitted through the inpatient installation of Integrated Heart Center Dr. Wahidin Sudirohusodo General Hospital. In addition, we reviewed up to 60 days after outpatient prospectively by collecting data through medical records. The correlation between echocardiographic parameters and readmissions was analyzed using the Spearman Rank Correlation. To determine each echocardiographic parameter's cut-off point, sensitivity, and specificity, we use Receiver Operating Characteristic (ROC) curve. Results:GLS and GCS are very sensitive and specific parameters in predicting 60-days readmission with an area under the curve (AUC) value > 0.7. This study shows that adding GLS and GCS parameters to the EHFS increases the predicted value (AUC 0.850 vs. 0.820) and sensitivity to 79%. Conclusion: GLS and GCS parameters additional on EHFS can help predict 60-days readmission of heart failure patients with sensitivity and specificity of 79% and 83%, respectively. The present study shows that with higher GLS and GCS scores on the EHFS, the risk of readmission in heart failure patients will increase.

Abstract

Introduction: Pseudomonas aeruginosa is an opportunistic pathogen and causes of nosocomial infections in hospitals. Due to high antibiotic resistance and the ability to develop new resistance during antibiotic treatment, Pseudomonas aeruginosa infection is difficult to eradicate because the physical treatment becomes difficult and ineffective. This study was conducted to evaluate the antibiotic sensitivity pattern of Pseudomonas aeruginosa strains at two different hospital in Makassar. Methods: This study is a cross-sectional study from March to May 2021. The research samples were taken from the results of culture and antibiotic sensitivity tests conducted at two different hospital, Hasanuddin University Hospital and Dr. Wahidin Sudirohusodo Hospital for the period January 1 to September 30, 2019. A total of 84 samples were cultured and tested for antibiotic sensitivity of Pseudomonas aeruginosa. Results: Antibiotic sensitivity of Pseudomonas aeruginosa was best with aminoglycoside antibiotics, gentamicin (100%) at Hasanuddin University Hospital and amikacin (95.8%) at Dr. Wahidin Sudirohusodo Hospital. At Hasanuddin University Hospital followed by antibiotics amikacin (92.3%) and meropenem (84.6%). At Dr. Wahidin Sudirohusodo Hospital, Pseudomonas aeruginosa also showed good sensitivity to gentamicin (91.5%) and meropenem (77.5%). The sensitivity of Pseudomonas aeruginosa was lowest to piperacillin/tazobactam. Conclusions: This study shows that the level of effectiveness of the antibiotics meropenem, amikacin and gentamicin is high enough and it can be used as a treatment option in Pseudomonas aeruginosa infection. This study can help as a reference to prevent mortality and morbidity associated with Pseudomonas aeruginosa infection.

Abstract

Introduction: IL-1β is one of the proinflammatory cytokines which is commonly found in various diseases, including Chronic Rhinosinusitis (CRS). It has been widely targeted as a therapeutic approach for inflammatory diseases. However, study of IL-1β expression and its correlation with CT-Scan grading scores in CRS is still scarce. Objective: To investigate IL-1β expression based on immunohistochemical examination and analyse its correlation with Lund-Mackay score in CRS with and without polyps. Method: This analytic observational study was done on 50 patients: 25 subjects in the CRS with Nasal Polyps (CRSwNP) group and 25 subjects in the CRS without Nasal Polyps (CRSsNP) group. The uncinate process was taken for immunohistochemical examination of IL-1β. Lund-Mackay score was calculated based on CT-Scan of the paranasal sinus. Result: IL-1β expression in CRSsNP (mean=36.38) was significantly higher than that in the CRSwNP (mean=14.62) with p<0.001. Lund-Mackay score in CRSwNP was higher (mean=35.22) compared to CRSnNP group (mean=15.78) with p<0.001. There was a moderate negative correlation (-0.403) between IL-1β expression and Lund-Mackay score (p<0.05). Conclusion: There was a moderate negative correlation between IL-1β expression and Lund-Mackay score. The IL-1β and the Lund-Mackay score in CRSwNP were lower and higher, respectively, than in CRSsNP

Abstract

Introduction: To observe the relation between Interleukin-1β with exacerbation of Chronic Obstructive Pulmonary Disease (COPD). Methods:  This study was designed for a cross-sectional study. 65 acute exacerbation COPD patients were selected. The serum level of Interleukin-1β was recorded. Results:  Most of the subjects are men (95,4%), moderate smokers (41,5%), normal BMI (61,5%), more than 60 years old (73,8%), have more than 2 exacerbations in a year (67%), severe airway obstruction (41,5%) and mostly in group D (80%). The mean level of Interleukin-1β was 1.63 pg/ml range from 0,009-17,62 pg/ml. Kruskal Wallis analysis test noticed significant differentiation mean serum level of Interleukin-1β in several groups of GOLD. p<0,05. There was no correlation between serum level of IL-1β with the severity of airway obstruction (GOLD) p>0.05. Conclusions: there was no correlation between the severity of airway obstruction with serum level of IL-1β in acute exacerbation of COPD. there was a vicious trend of increasing IL-1β serum level according to the severity of airway obstruction mainly seen in GOLD 2, GOLD 3, and GOLD 4.

Abstract

Introduction. Lung cancer is the type of non-small cell carcinoma (NSCLC) consists of non-squamous and squamous. Non-small cell lung cancer of non squamous types consist of adenocarcinoma and large cell carcinoma. Currently, lung cancer therapy is highly developed of chemotherapeutic agents to targeted therapy especially EGFR-TKI. This study aims to assess the survival rate of NSCLC patients who receive first line chemotherapy and those who recieve EGFR-TKI therapy at Wahidin Sudirohusodo hospital. Methods. This study is a retrospective study between 2017 to 2019 from the medical records of NSCLC patients who receive first-line chemotherapy and thise who recieve EGFR-TKI. Patients with platinum-based chemotherapy and EGFR-TKI with gefitinib therapy 1x250 mg/day or erlotinib 1x150mg/day and or afatinib 1x40 mg/day. Survival rate assessed from start to erect the diagnosis until the patient dies or when the  study is     discontinued. Result. From 239 subject of NSCLC patients consisted of 135 patients who receive first-line chemotherapy, and 104 patients are treate with EGFR-TKI. Based on the characteristics of the patients, most are more than 40 years old (chemotherapy 124 (91.9%) and EGFR-TKI 101 (97.1%) with the male gender that dominates (chemotherapy 98 (72.6%), EGFR-TKI 64 (61.5%). Smoking patients who received first-line chemotherapy are 65.2% and 61.5% of EGFR-TKIs with chemotherapy highest IB (severe IB 28.9%) and for EGFR-TKI (moderate IB are 26.9%). 73.2% of adenocarcinoma histology type with a predominance of stage IV 86.6% (83.7% for chemotherapy and EGFR-TKI 90.4%). Survival rate of patients are 98,7% for 6 months survival, 1 year survival rate is 94.1% and 2 years survival rate of 24.3%. Median survival patients who receiving EGFR-TKI longer than they received first- line chemotherapy (21 months versus 20 months). The 18 months PFS showed that patients treated with EGFR-TKI were 15 months, while patients receiving chemotherapy was 11 months. (P 0.000). Conclusion. Survival rates in NSCLC patients with EGFR-TKI therapy had significantly the highest survival rates compared with all other chemotherapy. Progression was faster in patients with first-line chemotherapy than EGFR-TKI. The factors that most influence the survival rate is type of therapy with p value<0.05.

Abstract

Background: Adenoid hypertrophy is one of the most common disorders in children which may lead to upper airway obstruction. Various modalities to measure airway obstruction in patients with adenoid hypertrophy, including true lateral radiographs, nasoendoscopy, and rhinomanometry are available; however, the results from different studies are still controversial. This study aimed to determine the relationship between the results of rhinomanometry and the true lateral radiographs to the degree of upper airway obstruction in patients with adenoid hypertrophy. Methods: This cross-sectional study included a total of 33 patients with adenoid hypertrophy aged 5-18 years using a purposive sampling technique. Patients diagnosed with adenoid hypertrophy were subjected to a true lateral examination using lateral neck radiographs to measure the degree of airway obstruction. Subsequently, an active anterior rhinomanometry was performed by measuring resistance and nasal airflow and then measuring the degree of airway obstruction. Data analysis was done using Chi-Square test. Results: The rhinomanometric nasal resistance in the inspiratory phase was 0.4159 ± 0.15201 Pa/cm3/s and 0.3694 ± 0.13717 Pa/cm3/s in the expiration phase. The results showed a significant relationship between the true lateral radiographs and both nasal inspiratory (p=0.005) and expiratory resistance (p=0.004). Similarly, the relationship between the true lateral radiographs and nasal airflow on both inspiratory and expiratory rhinomanometry was significant (p<0.05). Conclusion: Rhinomanometric measurements can be used as an additional objective examination in assessing the degree of upper airway obstruction in patients with adenoid hypertrophy prior to surgery.

Abstract

Introduction and importance:  Occurring in only 1% of cases of anorectal malignancy, malignant melanoma generally appears in the fifth and sixth decades of life, with complaints of anorectal bleeding or pain. The prognosis is generally poor. Although anorectal melanoma currently lacks a recommended treatment, surgery alone remains the primary modality treatment, the role of adjuvant therapy is generally minimal, and survival rate improves with early diagnosis. Presentation of case : Here, we report two cases of a rare malignant melanoma in the rectum treated with abdominoperineal resection and local excision.  The first case is a 60-year-old man with a history of defecating bloody stool and the appearance of a lump on the anal opening. The abdominoperineal resection was performed followed by adjuvant chemoradiotherapy.  The second case is a 51-year-old woman with a similar complaint and clinical finding with the first case. The second case was treated with a tumor excision procedure followed by adjuvant chemoradiotherapy. However, the lump was recurrence after 7 months of prior tumor excision. Discussion:    Surgical resection of anorectal melanoma is debatable. While surgical resection is still the most common treatment option, there is no agreement on whether abdominoperineal resection or local excision is the best option. Conclusions:   The abdominoperineal resection may offer a lower recurrence probability than that of tumor excision alone. However, further study is required to provide evidence-based surgical treatment

Abstract

Introduction: Nasopharyngeal carcinoma (NPC) is a non-lymphomatous squamous cell carcinoma in the nasopharyngeal epithelial layer which can be classified into three categories with different prognosis based on histopathological examination. This study aimed to compare platelet-lymphocyte ratio (PLR) in NPC patients before and after chemotherapy based on histopathological type. Method: this cohort study recorded data from medical records. The histopathological type, chemotherapy regimen, clinical stage, and PLR of NPC patients were recorded and compared before and after therapy using paired T-test and Wilcoxon test. The prognostic strength of PLR and the value of the cut-off point was determined by looking at the Area Under Curve (AUC) value using the Receiver Operating Characteristic (ROC) curve method. Results: A total of 44 NPC patients received chemotherapy for three cycles, including 21 patients with NPC type-2 and 23 patients with NPC type-3. There were significant differences in the average PLR before and after undergoing three cycles of chemotherapy in type III NPC (p=0.023). At the same time, there were no statistically significant differences in type-II NPC (p=0.131). The prognostic ability of pre-chemotherapy PLR in assessing disease progression in type II NPC was good (AUC=0.763) with 100.0% sensitivity and 73.68% specificity. In type III NPC, the prognostic ability of PLR was very good (AUC 0.881) with 100% sensitivity value and 76.19% specificity. Conclusion: A significant PLR decrease was obtained after the 3rd cycle of chemotherapy in type III NPC following brexel-cisplatin regimen but not in type II NPC. This is probably due to the use of a cisplatin therapy regimen that is more responsive to type III NPC. In addition, examining PLR value before undergoing chemotherapy can be a predictor in assessing disease progression in type III NPC patients.
