Abstract

Nurses often face high workloads and significant levels of stress due to the demands of complex work and high pressure. The academic supervision model is important because it can help nurses manage workload, improve skills and knowledge, and reduce stress levels. Effective supervision is expected to improve the quality of health services, job satisfaction, and have a positive impact on patients and the health system as a whole. This study aims to determine the influence of academic model supervision on the workload and stress of nurses in hospitals. This study uses a literature review method from research articles in the Scopus and Google Scholar databases published in 2014-2023 with the keywords "Supervision", "Academic Model", "Nurse Workload", and "Nurse Stress". Based on the analysis of 22 research articles, it was found that the supervision of the academic model had a positive impact on nurse performance, job satisfaction, work motivation, quality of nursing care, reduction of stress levels, and improvement of overall health service quality. Academic model supervision can help nurses improve skills and knowledge, improve services provided, develop professionalism, increase independence, improve nursing care, reduce workload, improve nurse performance, provide good emotional support, and continuous learning, and reduce the level of stress experienced by nurses. The implications of this research suggest that implementing academic model supervision in hospitals can lead to better nurse outcomes, including enhanced job satisfaction, reduced stress, and improved quality of care. Consequently, this can result in higher patient satisfaction and a more efficient and effective health care system overall.

Abstract

The WHO recommends that exclusive breastfeeding for six months must be carried out worldwide because it has benefits for both mother and baby, and is the baby's right to get breast milk. This study aims to determine whether there is a relationship between maternal breast care in the postpartum period and exclusive breastfeeding at the Ulu Moro'o Health Center, West Nias. This research is a quantitative study using an analytic design with a cross-sectional approach. The number of 60 respondents is based on the Slovin formula, with a total sampling in December 2021. Data collection uses a questionnaire about characteristics and breast care. The results showed that the majority (90%) were female reproductive age 20-35 years old, household work (91.7%), multiparous (61.7%), normal delivery (90%), and breastfeeding history was not the first experience (78.3%). The results of the chi-square test showed that there was no relationship between breast care (massage p= 0.467; warm or cold compresses p= 0.601) of mothers during the postpartum period with exclusive breastfeeding, so it was concluded that breastfeeding problems could not only be overcome by breast care such as massage or breastfeeding. However, it requires the cooperation of health workers in making skin-to-skin contact and IMD which is the first step in maintaining breastfeeding. The Puskesmas is expected to provide alternatives or other methods of breast care as well as education at least twice during pregnancy to mothers according to government regulations and intensive assistance to mothers in carrying out breast care education.

Abstract

The decision-making process regarding whether to breastfeed or not among mothers living with HIV is complex and influenced by multiple factors, including personal experiences, level of knowledge, cultural values and norms, and support from healthcare professionals. Mothers who choose to breastfeed often face ongoing anxiety related to potential risks and concerns about their infant’s health. Conversely, mothers who decide not to breastfeed may experience emotional conflicts, such as feelings of guilt and a diminished sense of confidence in fulfilling their maternal role. This study aims to explore the breastfeeding experiences of HIV-positive mothers with their infants using a qualitative phenomenological approach. Eight participants were selected through snowball and interviewed in depth using a semi-structured guide, and the data were analyzed thematically; the findings revealed five main themes, namely considerations in decision-making regarding infant feeding influenced by economic factors, recommendations from health professionals, and family support; mothers’ emotional reactions from the moment of HIV diagnosis characterized by shock, fear, confusion, mixed feelings during breastfeeding, and sadness or guilt for those who did not breastfeed; efforts to prevent HIV transmission by adhering to antiretroviral therapy (ART), maintaining hygiene of the breast and feeding equipment, practicing exclusive breastfeeding without mixing formula, and avoiding breastfeeding when nipples were injured; challenges and obstacles including social stigma, the financial burden of formula milk, lack of family support, and inconsistent medical information; and finally, support, information, and expectations, where mothers gained strength through regular consultations with healthcare providers, encouragement from husbands, and peer support from other HIV-positive mothers, while expressing hopes for more intensive assistance, continuous education, and more humanistic health services; this study concludes that infant feeding decisions among HIV-positive mothers are not merely medical issues but are shaped by the complex interaction of psychological, social, economic, and environmental support factors.
