Abstract

Nursing documentation is a means of communication between medical personnel aimed at restoring patient health. Documentation is nursing work that can facilitate nursing management, improve continuity of care, coordinate care, and evaluate patient interventions. DThe completeness and suitability of nursing care documentation in accordance with the 3S nursing care process standards (SDKI, SLIKI, SIKI) in several hospitals is still low. Implementing case reflection discussions can help improve a nurse's ability to carry out good and effective planning in improving the quality of nursing. This research is to determine the influence of case reflection discussions (drk) on improving nursing care documentation according to 3s standards (SDKI, SLKI, SIKI). This article was written using a literature study method which was analyzed from several journals related to the topic taken, journals were searched through Proquest, Pubmed, Google Scholar, Garuda, Sinta, which were then selected according to the topic, literature searches were carried out within the publication period journal from 2018 to 2024. The results of the discussion of the analysis of 20 literature reviews with hypothesis testing for each journal show that there is an influence of DRK on the quality of nursing care. DRK has the benefits of increasing knowledge, solving service problems, increasing awareness of quality practices and critical thinking. Nurses' knowledge influences the documentation of nursing care according to 3S standards (SDKI, SLKI, SIKI). One way to increase nurses' knowledge is with Case Reflection Discussions (DRK).

Abstract

Objective: Transformational leadership is known as a leadership approach capable of motivating and inspiring subordinates to achieve higher performance through empowerment and a shared vision. This study aims to examine the effect of transformational leadership intervention on improving nurses' performance. Methods: The research design used is quantitative with a quasi-experimental approach, employing a pretest-posttest with control group design. The research sample consisted of nurses participating in the transformational leadership intervention program, with 15 respondents in the intervention group and 15 respondents in the control group over a specific period. Data collection was carried out by measuring performance before and after the intervention using a nurse performance questionnaire, and the data were then analyzed using univariate tests for respondent characteristics, followed by bivariate tests using the non-parametric Wilcoxon test and the Mann-Whitney U test. Results:The analysis results showed a p-value of 0.001 < 0.05, indicating that the null hypothesis (Ho) was rejected and the alternative hypothesis (Ha) was accepted. The findings reveal a significant effect of transformational leadership intervention on improving nurses' performance. Conclusion: The conclusion of this study suggests that the implementation of transformational leadership can serve as an effective managerial strategy to enhance nurses' performance. The practical implication of this research recommends transformational leadership training and development for nurse managers or head nurses in healthcare facilities.

Abstract

Basic immunization is an important health intervention to prevent dangerous infectious diseases in infants. However, the coverage of basic immunization in Sungai Raya Village, Pinoh Utara Sub-District, Melawi Regency, West Kalimantan Province is still low, at only 67.8%, far below the national target of 95%. The low compliance of mothers in providing basic immunization is influenced by various internal and external factors. This study aims to explore mothers’ experiences in complying with basic immunization for infants aged 0–12 months, as well as to identify the supporting and inhibiting factors of such compliance. This research used a qualitative method with a phenomenological approach. Informants were selected using purposive sampling, namely mothers who have infants aged 0–12 months and who reside in Sungai Raya Village. Data were collected through in-depth interviews using an interview guide, and were then analyzed thematically. The study revealed several main themes, namely: Mothers' Perceptions of Disease Risk and Immunization Benefits; Level of Compliance and Mothers' Experience in Immunization; Barriers in Fulfilling Basic Immunization; Supporting Factors for Mother’s Compliance; and the Influence of Social Environment and Information. Compliance with basic immunization is influenced by a complex interaction between internal factors (knowledge, attitude, beliefs) and external factors (family support, access to health services, culture). Community-based interventions are needed to strengthen health education, improve service access, and involve community leaders to overcome cultural barriers.

Abstract

Aims: This research aims to analyze the effectiveness of I-CARE-based self-efficacy nurses training for the development of organizational commitment at Panti Rapih Hospital Yogyakarta. Methods: The research has been designed as a quasi-experiment one group pretest and posttest. The respondents are 143 nurses, mostly from D3 (associate degree) in Nursing (82.5%), female (88.1%), and young adult age or generation X (48.8%). Most of them have also been working for 6-15 years (51.8%). Ordinal regression discrimination test and SEM Smart PLS.4 with path analysis were also conducted. The data is analyzed by using software SPSS.25 and Wilcoxon pretest and posttest on I-CARE-based self-efficacy training and organizational commitment. Results: I-CARE-based self-efficacy has been found “high” (55.9%). Nurses’ organizational commitment is “high” (77.6%), “average” (18.2%) and “low” (4.2%). It is also found that I-CARE-based self-efficacy does not have a significant relation with organizational commitment, which P value is >0.005. The variables like age, gender and length of working have also been insignificant to the enhancement of organizational commitment, which P value is >0.005. The level of education gives a significant influence to organizational commitment, which P value is 0.02 (<0.005). The enhancement of education level will reinforce organizational commitment. Conclusion: I-CARE-based self-efficacy training is effective in enhancing organizational commitment, but for significant impact, it should be supported by a higher level of education. This research recommends increasing formal education levels to enhance staff organizational commitment.

Abstract

Aims: The aim of this research is to explore the health care function of families of elderly people with hypertension who live their lives alone in Bandung Regency. Methods : This research design uses a qualitative method with an exploratory phenomenological approach. The number of participants was 7 people who conducted a focus group discussion (FGD). Results : The results of this research obtained 5 themes, namely recognizing internal and external health problems based on reflex response, making decisions based on personal needs and complaints of the elderly, caring for oneself by preventing and strengthening positive energy, maintaining environmental health through dividing tasks and joint financial responsibility, and increasing optimization utilization of health services. Conclusions : Research on elderly hypertension families reveals five main themes: understanding hypertension through psychological and physical responses, making decisions based on personal needs, self-prevention and positive energy, dividing environmental health and joint financial responsibility, and optimizing health facility use through PKM and Health Services.

Abstract

Situation, Background, Assessment, Recommendation (SBAR) is a communication tool used to convey information clearly and in a structured manner, especially in the context of health services. The implementation of this assesment scale consideration has not been carried out optimally for nurses in the hospital. This study aims to analyze the validity and reliability of the SBAR-based weighing evaluation instrument from primary nurses and associate nurses. Multivariate validity and reliability test using the Exploratory Factors Analysis (EFA) test. The results aim to validate the instrument content. The numbers in the table show the loading factor values for each statement item which have been sorted from the largest value to the smallest value for each component. Statement items are valid and have an important correlation in the handover implementation instrument if they have a factor loading value of <0.40. Of the 25 statement items, 8 items were invalid, namely item numbers 2,3,4,6,10,12,17,19 while the other 17 items were declared valid and formed 4 components (Component 1; Situation indicator, Component 2; Backgound indicator, Component 3; Assessment indicator, Component 4; Assesment) which were used as indicators for compiling the SBAR-based handover implementation instrument. CFA test results were the relative chi-square value (CMIN/df) shows a fairly good fit with a ratio <5, the comparative fit index (CFI) value indicates marginal fit because it is in the range 0.80<CFI <0.90, The root mean square error (RMSEA) value indicates a reasonable agreement with a value of <0.08, the goodness-of-fit (GoF) value indicates a good fit because it is >0.80. It can be interpreted that based on the results of the CFA analysis it has fulfilled the construct feasibility test so that in general it can be said that the handover evaluation instrument by primary nurses and associate nurses is fit to be used in measuring handover implementation.

Abstract

Literature studies indicate that implementing clinical authority can enhance the quality of nursing care. However, many hospitals still have not fully applied clinical authority aligned with nurses' career paths. This research explores the impact of clinical authority on nurses' performance, both directly and indirectly, through situational leadership in Hospital X. Using a mixed-method approach with a parallel concurrent triangulation model, the study collected quantitative data via questionnaires from 300 nurses using total sampling. Descriptive crosstab analysis revealed no significant differences in perceptions of clinical authority, performance, and situational leadership based on individual characteristics. Structural Equation Modeling showed that clinical authority positively and significantly affects nurse performance. Qualitative data were gathered through focus group discussions with 13 nurses and semi-structured interviews with senior nursing managers. Thematic analysis identified six key themes: positive impacts on performance, the role of situational leadership styles, strategies to optimize nursing services, encountered obstacles, competitive challenges, and motivation to enhance care. The qualitative findings indicated that clinical authority fosters nurses' responsibility, adherence to standards, mutual trust, teamwork, and improved communication and caring attitudes. The corroboration of quantitative and qualitative analyses demonstrated alignment, confirming that clinical authority significantly influences nurse performance. To improve nursing care quality, hospitals are encouraged to implement structured career paths for nurses, ensuring that clinical authority is supported at every level

Abstract

The current patient service paradigm is patient- focused services (Patient Centered Care). Patients and families need accurate information, so caregiver professionals (PPAs) are required to work in an integrated manner in providing services to patients. The problem arises from the many conflicts between interdisciplinary service providers, plus the lack of quality control and control which is a serious problem for hospitals. Case Manager is an important and comprehensive intervention in order to improve quality, cost control, patient safety, integrated care, continuity of service and patient satisfaction. The role of Case Manager is very important and very complex as part of implementing patient-focused services, playing a role in helping to improve interprofessional collaboration and being able to collaborate with other health care teams. of this study was to describe the role of Case Manager and its implementation in supporting Patient Center Care at the hospital. Type of research qualitative with Phenomenological Approach with Main Informant Case Manager and Service Supervisor 27 Informants, data were obtained through in-depth interviews and Focus Group Discussion (FGD). This research identified six main themes: (1) Qualifications and Competencies; (2) SOP for Task Implementation; (3) Document Completeness; (4) Reporting System; (5) Integrated Services; (6) Obstacles to the Implementation of Duties. The role of Case Manager and Implementation at Tobelo Regional Hospital has supported Patient Centered Care and the dominant role is the role of communication and the role of coordination

Abstract

Abstrak Protokol penilaian kinerja perawat berbasis caring merupakan sebuah panduan penilaian kinerja perawat pelaksana yang dikembangkan untuk mengukur kinerja perawat sehari-hari berdasarkan perilaku caring perawat. Penelitian ini bertujuan untuk mengembangkan protokol penilaian kinerja perawat berbasis caring di Rumah Sakit Umum Islam Banyubening Boyolali. Jenis penelitian yang digunakan adalah Action Research, penelitian dilakukan dalam 1 (satu) siklus dari bulan Juli 2023 sampai oktober 2023. Pengambilan sampel dalam penelitian ini menggunakan purposive sampling. Jumlah partisipan dalam penelitian ini sebanyak enam orang, yang terdiri dari satu kepala bidang keperawatan dan lima kepala ruangan. Instrumen yang digunakan dalam penelitian ini adalah pedoman wawancara mendalam dan panduan focus group discussion. Data dianalisis secara kualitatif menggunakan content analys. Hasil penelitian menunjukkan ada 5 tema yang muncul, yaitu: i) Indikator kinerja perawat yang harus dinilai, ii) Manfaat penilaian kinerja untuk evaluasi dan penghargaan , iii) Faktor pendukung dalam pelaksanaan penilaian kinerja, iv) Hambatan dalam pelaksanaan penilaian kinerja, v) Harapan terkait pelaksanaan penilaian kinerja. Penelitian ini menghasilkan suatu protokol penilaian kinerja perawat berbasis caring, yang perlu diterapkan oleh manajerial rumah sakit. Protokol ini direkomendasikan kepada pihak manajerial keperawatan dalam mengukur kinerja perawat secara spesifik, sehingga dapat mengurangi komplain dari pasien terhadap pelayanan rumah sakit dan diharapkan meningkatkan mutu asuhan keperawatan.

Abstract

Kepala ruangan memegang peranan penting dalam keberhasilan pelayanan keperawatan melalui fungsi manajemen,karena berdampak dalam peningkatan mutu pelayanan di rumah sakit. Penelitian ini bertujuan untuk merumuskan Rancangan Model kepemimpinan dan manajemen kepala ruangan berbasis Patricia Benner di ruang rawat inap BLUD RSUD Kotamobagu. Penelitian ini menggunakan desain fenomenologi eksploratif dengan metode focus group discussion dan Action Research dengan metode Workshop Pakar. Partisipan FGD adalah 12 orang kepala ruangan, dan Partisipan saat Workshop pakar adalah 24 orang melalui teknik purposive sampling. Data yang dikumpulkan berupa hasil rekaman, catatan lapangan, Hasil Workshop pakar yang dianalisis dengan mene rapkan teknik Collaizi. Keabsahan data dijamin dengan memenuhi prinsip credibility, dependability, confirmability, dan transferability. Penelitian ini mengidentifikasi enam tema utama: (1) Kompetensi dalam peran organisasi; (2) Fungsi manajemen yang efektif; (3) Peran kepala Ruangan; (4) Perencanaan dan pengarahan ; (5) Pengendalian dan pengawasan; (6) Manajemen Konflik dan Penyelesaian Masalah. Model kepemimpinan dan manajemen kepala ruangan berbasis patricia Benner Theory ini dapat menjadi panduan dalam Menyusun rencana strategi bidang Keperawatan dapat dimodifikasi oleh BLUD RSUD Kotamobagu guna meningkatkan kemampuan leadership dan kemampuan mengelola fungsi manajemen keperawatan kepala ruangan.
