Abstract

Abstract

Abstract

In 2021, the Maternal Mortality Ratio (MMR) in Lampung Province increased significantly, rising from 115 to 187 cases compared to 2020. Bandar Lampung emerged as one of the areas with the fourth highest MMR, recording 15 cases (8.02%). This study aims to identify determinants of childbirth complications among mothers in Bandar Lampung in 2024. This type of research is analytical-descriptive, with random sampling, so that samples are obtained from 432 post-natal mothers. This research method uses interviews and observations of post-natal mothers analyzed through a quantitative cross-sectional approach with a random sampling of 432 participants. Results indicate a correlation between birth spacing (p=0.015, OR=2.725), history of pregnancy complications (p=0.000, OR=2.049), and parity (p=0.011, OR=1.764) with childbirth complications. Recommendations include improving healthcare services at the primary level and educating pregnant women and families on risk factors to reduce maternal mortality.

Abstract

Breast milk is the gold standard baby food, breast milk is proven to have advantages that cannot be replaced by any food and drink, because breast milk contains the most appropriate, complete nutrients, and always adjusts to the needs of babies at any time. The gold standard of infant food begins with IMD, followed by exclusive breastfeeding for 6 (six) months (Munzia, 2013).Only 3.7% of infants in Indonesia are breastfed within the first 1 hour after birth. Breastfeeding is recognized as one of the most powerful influences on child survival, growth, and development. Exclusive breastfeeding can reduce the mortality rate of children under five by about 20%. Early initiation of breastfeeding will also greatly help achieve MDGs goal number four of reducing child mortality, as early breastfeeding within the first hour after delivery will reduce newborn mortality. The failure of mothers to breastfeed their babies until six months of age is actually only one problem, which is that mothers do not fully understand the correct way to breastfeed, including techniques and ways to obtain breast milk, especially when they have to work. Understanding IMD and exclusive breastfeeding is a very important issue. IMD and exclusive breastfeeding are possible if individuals, families, health workers and communities understand the meaning, benefits and goals of IMD and exclusive breastfeeding (Munzia, 2013).The results of the 2018 Basic Health Research stated that the exclusive breastfeeding rate for 6-month-old babies in Lampung Province only reached 37.3% of the target of 60%. Meanwhile, in West Tulang Bawang District, the achievement of exclusive breastfeeding was 29.1% of the target of 60%, the achievement of IMD 1 hour 1.8% and IMD 1 hour 39.6% which should be done in all newborns (Riskesdas, 2018).The purpose of the study was to determine the application of the Breastfeeding Model in pregnant women, delivery mothers, and postpartum breastfeeding mothers in West TulangBawang Regency in 2023. This research method uses quasi experiment with pretest-posttest group design. The intervention provided is assistance to pregnant women, laboring women and postpartum breastfeeding mothers who are willing to be assisted and in accordance with the inclusion and exclusion criteria. Before providing assistance, health cadres were trained and provided with knowledge about breastfeeding by the research team in accordance with the research design that had been prepared. The breastfeeding preparation assistance process for pregnant women was carried out during mother's class or according to the free time of pregnant women. The IMD and breastfeeding preparation assistance process was carried out at the place where the mother gave birth for approximately 120 minutes until the end of the postpartum period. The mentoring process is carried out for approximately 6 months by applying the breastfeeding model that has been obtained from the first year of research with a research instrument in the form of a Pocket Book that adapts to the conditions of the respondents. Furthermore, the alternative methods of mentoring will be outlined in a guidebook for pregnant women mentoring models. The population in this study were all pregnant women, laboring mothers, and postpartum breastfeeding mothers in the work area of the West Tulang Bawang District Health Office in 2023. The sample of this study were 30 pregnant women, 30 mothers in labor and 30 postpartum breastfeeding mothers at the Puskesmas in the working area of the West Tulang Bawang District Health Office in 2023 with a total of 90 people. This research is planned to be published in Scientific Journals and Accredited Proceedings, become teaching materials and will be registered as Intellectual Property Rights (IPR). The Technology Readiness Level (TKT) of this research is at level 3 with readiness at this stage at level 2-3.

Abstract

The E-Kescatin application was developed as an innovation in premarital health services to support stunting prevention efforts through a digital approach. However, the effectiveness of utilization of this application still shows variations between regions and is not optimal at the implementation level. This study aims to explore the contextual factors that influence the effectiveness of the E-Kescatin application utilization in Pringsewu District, Lampung Province. The research used a qualitative approach with an exploratory study design. Informants were selected purposively, consisting of midwives, brides-to-be, Health Office officers, and MCH program managers. Data collection was conducted through in-depth interviews and focus group discussions. Data analysis was done thematically with an interpretative approach. The results identified five main themes that influence the effectiveness of app utilization, namely: (1) accessibility of technology and digital infrastructure; (2) digital competence of health workers; (3) perceived benefits from the user side; (4) policy and regulatory support at the local level; and (5) cross-sector integration in the implementation of premarital services. It is concluded that the effectiveness of E-Kescatin application utilization is influenced by contextual factors that include technical, social, and institutional aspects. Therefore, improving the success of application implementation requires a collaborative and adaptive approach to local conditions. The findings of this study provide a significant contribution to strengthening digital transformation policies in the field of reproductive health.

(Riskesdas, 2018). The aim of the research is to determine the influence of assistance to mothers in labor & birth companions on the success of IMD in the PMB in the West Tulang Bawang Regency Service Work Area in 2022. This research uses a quasi experiment with control group design. The population in this study were all birth attendants and birth mothers in the work area of the West Tulang Bawang District Health Service in 2022. The sample for this study was birth attendants and birth mothers in the work area of the West Tulang Bawang District Health Service in 2022, totaling 96 people. Data was collected by means of documentation studies, interviews and observations. Data is processed and analyzed with descriptive and analytical statistics. The results of data processing show that the average IMD success in the intervention group was in the second 30 minutes, while for the control group there were 7 respondents who failed IMD and the rest succeeded IMD in the third 30 minutes. On average, respondents who were given assistance 3 times or more were successful in carrying out IMD in the first 30 minutes. Those who were given assistance 1-2 times managed to carry out the IMD in the second 30 minutes. Meanwhile, respondents in the control group (no assistance) on average succeeded in carrying out IMD in the 3rd 30 minutes. The results of statistical tests obtained a value of p=0.002, it can be concluded that there is a significant influence if assistance is provided by birth attendants on the success of IMD in mothers giving birth.

Abstract

Breast milk is the gold standard for baby food; it has been scientifically established to provide benefits that no other food or beverage can match since it includes the most suitable, complete nutrients and constantly adapts to the baby's changing demands. The gold standard for infant nutrition starts with an Early nursing Initiation and continues with six (six) months of exclusive nursing (Munzia, 2013). Only 3.7% of infants in Indonesia are nursed within the first hour of their lives, according to research. There is only one issue with moms not continuing to breastfeed their children until they are six months old: they don't completely understand how to breastfeed well, including procedures and how to obtain breast milk, especially if they also have to work. According to the 2021 Basic Health Research (Riskesdas) findings, the province of Lampung achieves an early breastfeeding initiation rate of 80.4% and an exclusive breastfeeding rate of 73.6% for babies who are 6 months old. Compared to West Tulang Bawang Regency, where the success rate for exclusive breastfeeding was 29.1% rather than the desired 60%, the success rate for early breastfeeding initiation was 1.8% rather than the recommended 39.6%. Early breastfeeding is practiced for all newborns (Riskesdas, 2021).The goal of the study was to ascertain how family support affected the effectiveness of early breastfeeding initiation in 2022 at the PMB Work Area of the West Tulang Bawang Regency Service. In this study, a quasi-experiment with a control group design was used. All women and birth attendants in the West Tulang Bawang District Health Office's service area in 2022 made up the study's population. The 96 birth moms and delivery attendants who worked at the West Tulang Bawang District Health Office in 2022 were the study's sample. Observation, interviews, and documentation studies are used to acquire data. Descriptive and analytical statistics are used to analyse and analyze data. The analysis of the data reveals that respondents who had family assistance were able to initiate early breastfeeding within the first 30 minutes. In contrast, those who had family assistance on average were able to initiate early breastfeeding in the third 30 minutes. According to the statistical test results, p = 0.014, it may be inferred that the effectiveness of early breastfeeding initiation in new moms is significantly influenced by the family's support.Researchers in scientific development may offer the following advice: having a partner present during labor and delivery will have a really exceptional impact, especially on the mother's psychologically. Giving this care will mark the start of a successful nursing relationship, and vice versa. In an effort to increase the coverage of exclusive breast milk and achieve mother and infant health in Tulang Bawang Barat, the results of this research can be continuously improved to provide more pertinent information and learning resources about early breastfeeding initiation. These improvements will also be very helpful for future scientific advancement.

Abstract

The current maternal and child health problem that is the focus of government programs is stunting. Stunting is a condition of failure to thrive in children under five. The incidence of stunting in Indonesia in 2021 will still be at 24.4% (RI, 2020). Many factors are related to the incidence of stunting.   One of them is exclusive breastfeedingg for babies 0-6 months. Exclusive breast milk is needed by babies because it is very important for the baby's growth and development (Novianita et al., 2022). Baby who receive exclusive breast milk have a lower risk of stunting (Arsin & Syafar, 2020).Exclusive breastfeeding has been programmed by the government since 2012, but success in achieving exclusive breastfeeding has been slow. In 2021, the achievement of exclusive breast milk will only reach 71.58% for Indonesia, and specifically for Lampung province, exclusive breast milk will reach 74.93% in 2021 (Statistik, 2023) one way is by providing counseling to pregnant women in pregnancy classes. However, this outreach is only limited to the KIA section. After the baby is born and so on, mothers rarely contact MCH to consult on issues regarding breast milk after the baby is born. Mothers and babies have a lot of contact with health workers at community health centers just to get basic immunization injections.  As a result, there is failure to provide exclusive breastfeeding to babies due to problems that are not resolved during breastfeeding. Ideally, there should be collaboration between professional sections from the immunization section and the MCH section so that they can refer mothers and babies who experience problems related to breast milk (little breast milk, sore nipples and so on).Purpose :  of this research is to determine the effect of implementing interprofessional collaboration practice on the achievement of exclusive breastfeeding. Methods:  The design of this research is a quasi-experiment with a one group pretest and posttest design.
