Abstract

The Province of the Special Region of Yogyakarta ranks third with the highest number of cases of Diabetes Mellitus (DM) nationally. One of the complications of DM, a severe health problem, is diabetic ulcers. Diabetic ulcer care and Covid-19 prevention are strictly associated with a high risk of transmission, a poor prognosis, and a high mortality rate when infected with Covid-19 are needed during a pandemic situation, one of which is to increase the role of the family (caregiver). The purpose of the activity is to optimize the role of caregivers in the simple care of diabetic wounds at home (stay at home) during the Covid-19 period. Comprehensive education about DM management in general and diabetic wounds in particular to families (caregivers), which is carried out using a direct method in conjunction with homecare services for simple treatment of diabetic ulcers while still implementing the Covid-19 health protocol. Providing good benefits to the family (caregiver) as evidenced by an increase in the average score of knowledge and also an increase in the average score of self-perception of ability in performing simple diabetic wound care. The development of long-term care for chronic patients can continue to be carried out by involving the active role of the family (caregiver) as the closest support system for patients. Education to the family directly increases the knowledge and confidence of the family in performing simple diabetic wound care. This program can be an alternative to optimizing infection prevention and the healing process of diabetic wounds.

Abstract

The Special Region of Yogyakarta has the highest proportion of elderly population in Indonesia, accompanied by a very high prevalence of various chronic diseases. COVID-19 positive patients who have comorbidities such as diabetes mellitus (DM) pneumonia, kidney failure and hypertension have a poor prognosis and a high mortality rate. Health service assistance is needed that is long-term care directly in the community to reach and maintain and improve conditions, prevent further severity and complications during the COVID-19 pandemic. The Long Interdisciplinary Community Care Program at 'Aisyiyah University Yogyakarta (PoLICE_Unisa) provides information on the need to improve long-term care services specifically based on the principles of patient centered care in the community. (1) forming and optimizing village health cadres that are integrated with the PoLICE_Unisa team, (2) screening educational needs in the community (3) comprehensive education about chronic disease management. The evaluation was carried out by evaluating the condition of the community individually, such as the absence of complications and the severity of the disease, both objectively and subjectively. after going through general health screening, in the end there were six patients/families who were identified as having chronic disease problems; stroke, diabetes mellitus and hypertension who are then willing to be directly involved in this activity. The involvement of all components, namely health cadres, families and patients as well as teams consisting of several professions, has provided integration and collaboration that can be continuously improved to improve the level of health for people in need. this activity provides things that are felt to be meaningful for the health status of families/patients directly, especially in the management of chronic diseases.

Abstract

The Family Program for Healthy Physically-Mentally and Empowered Elderly (Klasikal-ya) in the management of household organic waste at the Aster Elderly Posyandu, Bantul, Yogyakarta is a Community Service activity aimed at maintaining and improving the physical and mental health of the elderly with physical and social activities in the form of managing household organic/kitchen waste using a composter for leftover kitchen waste (LOSIDA) which produces compost which is then used to grow productive home garden plants, of course adjusted to the functional capacity of the elderly participants. This program is integrated with TiM at the University (Universitas 'Aisyiyah Yogyakarta) which targets the elderly as the main focus by involving the active role of competent students and educators (lecturers). The implementation of activities begins with socialization to introduce the program for healthy physically and mentally and productive elderly; Training, carried out with counseling on healthy activities for the elderly in managing household organic waste using the LOSIDA method. Application of Technology with screening and activities to improve physical and mental health, making and implementing the LOSIDA composter; Mentoring and Evaluation; Monitoring the physical and mental health of the elderly, and utilizing compost from activities for home planting activities as a healthy physical and mental activity for the elderly. Sustainability of the Program; carried out with periodic communication/consultation channels. The benefits of this program are specifically to increase the physical activity of the elderly according to their functional capacity which will have a positive effect on the mental health of the elderly. Increasing community participation, especially the elderly, in dealing with organic waste which can then be used productively through useful home yard plants
